| 摘 要： | 目的探讨脑卒中后癫痫发作与脑卒中的类型、病灶部位等因素的关系及其发作机制。方法选择近5年来我院收治的812例脑卒中患者的临床资料,就癫痫发作与脑卒中类型和部位的关系及癫痫发作的时间与脑卒中性质的关系等方面进行回顾性分析。结果全组812例脑卒中患者,继发癫痫者72例(8.87%),蛛网膜下腔出血与脑出血继发癫痫的发病率高于脑梗死(χ2=8.25,P<0.01;χ2=13.97,P<0.01),脑叶与蛛网膜下腔病灶继发癫痫的发病率高于基底节区(χ2=129.93,P<0.01;χ2=50.70,P<0.01),早发型癫痫以出血性脑卒中为主,迟发型癫痫则以脑梗死为主(χ2=18.09,P<0.01;χ2=8.86,P<0.01)。结论脑卒中后癫痫的发病率与脑卒中的类型及病灶部位有关,其发生的早晚与脑卒中的性质有关。
