小剂量阿米替林辅助治疗老年不稳定心绞痛的疗效观察

为了验证小剂量阿米替林辅助治疗老年人不稳定心绞痛的疗效。我们选择了288例已确诊的不稳定心绞痛患者，运用抑郁自评量表（SDS）评估他们的抑郁状态，以得分50分以上者157人入组，再把入组病人随机分成治疗组（79例）和对照组（78例）。对照组按照常规不稳定性心绞痛的药物治疗，治疗组则在常规治疗基础上辅加抗抑郁治疗，即每日中午和晚上睡前各服用阿米替林12．5mg，随访12周。通过治疗组与对照组的比较分析，结果发现在用药后治疗组病人的心肌缺血明显改善，用药8周时尤为显著。心绞痛发作次数降低，差异具有统计学意义（P〈0．05）。对不稳定性心绞痛伴抑郁情绪的老年患者进行小剂量抗抑郁药辅助治疗比常规心绞痛治疗无小剂量抗抑郁药辅助的疗效要好。

Curative Effects for Elderly Patients with Unstable Angina Pectoris Enhanced by Assistant Using Small Dosage Amitriptyline

To validate the curative effects of antidepressant on the elderly unstable angina pectoris patients accompanied with emotional disorder by assistant using small dosage amitriptyline. We recruit 288 patients with unstable angina to be assessed by using the approach of self-rating depression scale （SDS）. Total of 157 elderly unsta ble angina pectoris patients with emotional disorder randomly were grouped as treatment group （79 patients） and control group （78 patients）. 79 patients of treatment group received standard routine treatment and plused anti-depression treatment, and 78 patients of control group received standard routine treatment and added placebo. The results showed that myocardial ischemia was improved significantly compared with the control group, and the frequency of angina pectoris attack were lower （P〈0.05）. Consequently we had a conlusions that assistant antidepressant treatment might enhance curative effects for elderly patients with unstable angina pectoris by assistant using small dosage amitriptyline.
