减压阀植入术治疗难治性青光眼的远期疗效观察

目的:观察青光眼减压阀植入手术治疗难治性青光眼的远期疗效。方法:应用青光眼减压阀植入手术治疗19例21眼难治性青光眼,术后随访5~8a,(平均6a),观察其临床疗效及其手术并发症。结果:本组远期成功率为53.3%;眼压由术前的(45.39±13.14)mmHg降至术后的(18.32±6.42)mmHg,差异有统计学意义(P<0.01)。导致手术失败并发症为:引流盘周纤维包裹6眼(28.6%),引流管口堵塞2眼(9.5%),引流管暴露1眼(4.8%),角膜内皮失代偿1眼(4.8%)。结论:应用青光眼减压阀植入手术治疗难治性青光眼是一种相对有效、安全的治疗方法。

Observation on the long -term effect of the ahmed glaucoma valve implantation on the refractory glaucoma

Objective: To observe the long-term effect of ahmed glaucoma valve(AGV) on the refractory glaucoma.Methods:After AGV implantation,a retrospective study of clinical effect and complication was conducted on 21 eyes from 19 patients with refractory glaucoma with follow-up period ranging from 60 to 96 months (mean 72 months).Results:The long-term successful rate was 53.3%.The average IOP decreased from the preoperative(45.39 13.14) mmHg to the postoperative(18.32 6.42) mmHg,respectively,(P<0.01).The complication caused by the unsuccessful operation included encapsulated bleb in 6 eyes(28.6%),tube blockage in 2 eyes(9.5%),exposure of tube in 1 eye(4.8%),corneal de-compensation in 1 eye (4.8%).Conclusion: The AGV implantation surgery may be effective and safe for treating refractory glaucoma.
