小剂量伊那普利治疗肾移植术后红细胞增多症的疗效与副反应观察

目的:观察小剂量伊那普利治疗肾移植术后红细胞增多症的效果及安全性。方法:46例肾移植后红细胞增多症患者随机分为两组:小剂量组(n=29)和常规剂量组(n=17)。分别应用血管紧张素转换酶抑制剂伊那普利小剂量和常规剂量治疗,用药期间观察患者的外周血红细胞数、血红蛋白、红细胞压积和血压的变化及不良反应。结果:服用伊那普利2~3个月,两组患者外周血红细胞数、血红蛋白及红细胞压积降至正常水平,但常规剂量组出现低血压5例,肾功能损害2例,而小剂量组仅2例在用药期间出现低血压,予以伊那普利减量后血压恢复正常。结论:小剂量血管紧张素转换酶抑制剂对肾移植术后红细胞增多症的治疗是安全和有效的。

Therapeutic efficacy of low dose of enalapril in polycythemia following renal transplantation

Objective: To observe the therapeutic effect and safety of low dose of enalapril on erythromatosis following renal transplantation.Methods: Forty-six patients with postrenal transplant polycythemia were randomly divided into two groups: treatment group(n=29) and control group(n=17) received low(5 mg/d) and usual dose(20 mg/d) of enalapril,respectively.RBC count,hemoglobin and hematocrit level,blood pressure,and side effects in all patients were observed during therapy.Results: RBC count,hemoglobin and hematocrit level recovered nomally 2-3 months after treatment in all patients.In control group,hypotension and impairment of renal function occurred in 5 and 2 cases,respectively;in treatment group,only 2 cases were suffered from reversible hypotension.Conclusion: Low dose of enalapril is safe and effective for the treatment of postrenal transplant polycythemia.
