双源CT冠状动脉成像与冠状动脉造影评价冠状动脉病变的对比研究

摘    要:目的探讨双源CT冠状动脉成像(DSCTA)对冠状动脉病变的诊疗价值。方法对42例临床疑似冠状动脉病变患者同时作DSCTA和冠状动脉造影检查(CAG),以CAG结果为参考标准,评价DSCTA对冠状动脉狭窄程度及冠状动脉粥样硬化的诊断价值。结果 504个冠状动脉节段中,485个节段可作DSCTA、CAG配对分析。DSCTA对冠状动脉狭窄评估的灵敏度、特异度、阳性预测值、阴性预测值分别为91.03%、92.35%、83.54%、96.02%。DSCTA显示53个非钙化斑块、49个钙化斑块及56个混合型斑块,其中118个斑块的偏心指数〈0.5,40个斑块〉0.5,而CAG难以对斑块进行评价。结论 DSCTA可准确评估冠状动脉狭窄程度和管壁斑块性质。

Abstract:Objective To investigate the value of dual source computed tomography angiography(DSCTA) in diagnosis of coronary diseases.Methods Forty-two cases of suspected coronary diseases underwent DSCTA and coronary angiography(CAG) simultaneously.Based on CAG results,the diagnostic value of DSCTA was assessed in coronary artery stenosis and atherosclerosis.Results Of the 504 segments of coronary artery in 42 patients,485 segments were evaluated with DSCTA and CAG.The sensitivity,specificity,positive and negative predictive values of DSCT were 91.03%,92.35%,83.54%,and 96.02% for coronary artery stenosis,respectively.Additionally,DSCTA other than CAG found 53 non-calcified,49 calcified,and 56 mixed plaques,including 118 plaques with eccentricity index 〈0.5 and 40 with eccentricity index 〉0.5.Conclusion DSCTA can accurately evaluate the severity of coronary artery stenosis and properties of atherosclerotic plaques.
