室间隔缺损并肺动脉高压的手术治疗

对４４例先心性室间隔缺损肺动脉高压患者手术治疗的结果分析表明：按体检、心电图、胸片、动脉血气分析、超声心动图及心导管检查结果进行临床分级（４级），可确定手术适应症和估计基预后。主张经右室流出道切口加补片牢固修补室间隔缺损，同时注意纠正合并畸形及完善心肌保护。术后连续监测左房压及肺动脉有助于合理使用呼吸机，血管护张药、正性肌力药和预防低心排综合症的发生。

SURGICAL TREATMENT OF CONGENITAL VENTRICULAR SEPTAL DEFECT WITH PULMONARY HYPERTENSION

Forty-four cases of congenital ventricular septal defect with pulmonary hypertension undergone surgical treatment have been analysed here.We believe that dividing these cases into 4 classes clinically is very helpful for recongnizing indications Of operation and assessing prognosis of postoperative results.On the operating table,an incision through the right ventricular outflow tract is convenient.Myocardial protection is of vital importance and attension should be payed on it.If there were some combined anomalies,simultaneous correction should be performed.We insist the importance of measuring the LAP,PAP consistently,because these figures benefit rational use of the ventilator,vasodilators and positively inotropic agents,and also can be used for prediction of low cardiac output syndome.
