Extreme eosinophilia with disseminated eosinophilic granulomatous disease is described in a 4-year-old Arabian mare. Clinical signs included weight loss, coughing, jugular distention, and ventral edema. Cutaneous lesions were not observed. Eosinophilic inflammation was observed in cytologic specimens from the respiratory tract, body cavities, and lymph nodes. At necropsy, a 20-cm diameter intrathoracic mass was observed. Smaller nodules were present in the lymph nodes, liver, spleen, adrenal glands, pancreas, and skeletal muscle. Histologically, these masses and nodules were characterized by infiltrates of eosinophils, macrophages, and multinucleated giant cells, reactive fibroplasia; and multifocal eosinophilic coagula. Microscopically, mild eosinophilic infiltrates were observed in sections of stomach, small intestine, colon, and pleura; however, gross lesions were not observed in these tissues at necropsy. The etiology of the extreme eosinophilia and disseminated eosinophilic granulomatous disease in this horse was not determined.

Five horses with a primary surgical lesion of the small (descending) colon were diagnosed with eosinophilic colitis based on visual and histopathological examination. These were evident as visibly striking, hyperaemic, focal lesions of the small colon, with serosal petechiation, oedema and marked thickening of the intestinal wall at the site. Areas of focal necrosis were also evident. The gross appearance of the lesions were considered to be sufficiently severe in all cases to merit resection, due to concerns about intestinal necrosis and septic peritonitis. An inability to fully exteriorise the affected portion of intestine to perform a resection and anastomosis necessitated intraoperative euthanasia of one horse. A total of three horses survived to hospital discharge. Eosinophilic colitis lesions are a rare cause of severe small colon disease, but should be considered in cases with similar visual characteristics.

A 5-year-old Quarter Horse mare was presented to the Michigan State University Equine Surgery Service for evaluation of a right hind metatarsophalangeal joint injury. On presentation, the horse was a grade 4/5 lame. Radiographs and ultrasonography showed peritendinous ossification/mineralisation of soft tissue structures and digital flexor tenosynovitis with profound intracapsular mineralisation. A digital flexor tendon sheath tenoscopy was performed with a plantar annular ligament desmotomy and a manica flexoria ligament tenotomy. During surgery, a significant amount of tan granular material was found within the digital tendon sheath. Approximately 80% of this material was removed, and a sample was sent for histopathology. The horse was diagnosed with eosinophilic granulomatous tenosynovitis. Long-term follow-up revealed improvement with four treatments of intralesional triamcinolone given 2–4 weeks apart. The horse became serviceably sound (grade 1/5) and was beginning light work under the saddle approximately 5 months post-tenoscopy.

Abstract This study describes 26 cats with mosquito bite hypersensitivity. Most were short-haired, free-roaming cats. Typical clinical features were seasonal (summer), symmetric miliary dermatitis on the pinnae. Mosquito bite exposure with Aedes albopictus was performed on the pinnae and the lateral thorax in five cases and three healthy cats. All sites showed weals within 20 min. Papules, which revealed eosinophilic dermatitis, were recognized during a 12–48-h period. The control cats showed only slight and transient erythema after being bitten. Intradermal skin tests with an extract of A. albopictus were performed in three cases, and Prausnitz-Küstner tests under A. albopictus bites were performed in three cases. Both tests showed weals in all cases studied, although papules were not observed. Intradermal skin tests with the control cats, and Prausnitz-Küstner tests with saline and healthy cat serum failed to produce any positive reactions. The findings indicate that mosquito bite hypersensitivity in cats is triggered by a type I hypersensitivity reaction to mosquito antigens. Certain other factors may be involved in the formation of delayed papular reactions. Whether this is a normal or abnormal reaction to mosquito bites is also discussed. [Mashiko N, Takuo I. Cutaneous reactivity to mosquito bites and its antigens in cats. [Mashiko N, Takuo I. Cutaneous reactivity to mosquito bites and its antigens in cats. A 5-year-old female spayed Shetland Sheepdog Mix dog was evaluated for a history of recent seizure activity, progressive hind limb ataxia, polyuria, and polydipsia and no history of gastrointestinal signs. Physical examination findings included conscious proprioceptive deficits, ataxia, and anterior uveitis along with a hypermature cataract in the right eye. Results of a CBC, serum biochemical profile, urinalysis, and computed tomography scan of the brain were unremarkable. Cerebrospinal fluid (CSF) analysis revealed marked eosinophilic pleocytosis and rare organisms consistent with Prototheca spp within neutrophils and macrophages. On postmortem histologic examination, mononuclear inflammation and numerous intralesional algal organisms, similar to those seen on the cytologic preparation of CSF, were found in the brain, eyes, kidneys, and heart. Abnormalities were not detected on gross and histologic examination of the gastrointestinal tract. Cultures of CSF and subdural/olfactory bulb, but not intestinal tract, yielded growth of Prototheca spp, and PCR analysis and DNA sequencing confirmed the organism as Prototheca zopfii genotype 2. We have reported a rare case of disseminated protothecosis that was diagnosed by evaluation of CSF in a dog presented with neurologic signs and no overt enteric disease. Protothecosis should be considered as a rare cause of seizures, even in the absence of obvious enteric signs, and should be included in the differential diagnosis of eosinophilic pleocytosis.

CASE HISTORY: A 2-year-old Standardbred gelding presented with a history of fever over 1 week, anorexia and skin lesions on all four legs. The lesions were associated with severe pruritus and oedema, and there was no response to therapy.

CLINICAL FINDINGS: The horse was in poor body condition, was lethargic and severely pruritic. Skin lesions consisted of diffuse alopecia and crusting of the distal extremities. Initially it was slightly febrile, but subsequently its temperature increased up to 40°C. Ten days after admission it developed profuse watery diarrhoea and the skin lesions progressed. Skin biopsies revealed superficial and deep perivascular dermatitis with lymphoplasmacytic and eosinophilic predominance. Based on the poor prognosis the horse was subject to euthanasia.

PATHOLOGICAL FINDINGS: The most notable lesions included ulcerative gastritis, typhlitis and colitis with prominent oedema of the intestines, marked subcutaneous oedema and severe thickening of the large bile ducts. Histopathology showed marked eosinophilic and lymphoplasmacytic infiltration of various tissues including the skin, gastrointestinal tract, mesenteric lymph nodes, large bile ducts, pancreatic duct and kidney. Immunohistochemistry revealed a clear predominance of CD3-positive cells in the lymphocytic infiltrations.

DIAGNOSIS: Based on the clinical findings and histopathology a diagnosis of multisystemic eosinophilic epitheliotropic disease (MEED) was made.

CLINICAL RELEVANCE: Multisystemic eosinophilic epitheliotropic disease is rare in horses, and usually chronic. In the current case the horse showed an apparently acute onset with high fever and rapid clinical deterioration. A diagnosis of MEED should be considered in horses presenting with weight loss and skin lesions with or without fever. A final diagnosis is based on histological results of biopsy specimens from affected organs.
