Objective To measure the effects of dobutamine infusion on fetal oxygenation during isoflurane anaesthesia in pregnant ewes. Study design Prospective randomized experimental study. Animals Seven clinically normal adult pregnant Rambouillet‐Dorset cross ewes with fetuses of 117–122 days gestational age. Methods The ewes were anaesthetized with ketamine (2 mg kg?1) IM, and isoflurane (FE′ISO 2.0%) in oxygen. After instrumentation and stabilization, dobutamine was infused at 4 µg kg?1minute?1 for 60 minutes and 10 µg kg?1minute?1 for 60 minutes in random order, separated by a 20‐minute washout period. Catheters were placed in the maternal and fetal carotid arteries; these were used for continuous blood pressure measurement and intermittent blood sampling. Results Maternal mean systemic carotid arterial pressure was 60 mm Hg prior to dobutamine infusion. After 5 minutes of dobutamine infusion, fetal oxygen saturation increased (p < 0.05) from 0.62 (0.17–0.71, minimum–maximum) to 0.72 (0.28–0.78) at a dose of 4 µg kg?1minute?1 and to 0.70 (0.20–0.73) at a dose of 10 µg kg?1minute?1. These increases were maintained during the infusion and were not significantly different between doses. Maternal oxygen saturation remained constant at 1.0 before and during all infusions. Although maternal heart rate and blood pressure increased (p < 0.05) by 90% and 25%, respectively, with dobutamine, this stimulant effect was not evident in the corresponding fetal variables. Maternal haemoglobin concentration increased 30% (p < 0.05) with each infusion. Conclusions Dobutamine at 4 µg kg?1minute?1 increases fetal oxygenation that is not improved by a dose of 10 µg kg?1minute?1. This increase is largely due to an increase in maternal haemoglobin concentration that, in turn, increases oxygen delivery to the placenta. Clinical relevance The use of dobutamine to treat hypotension in pregnant sheep during isoflurane anaesthesia improves fetal oxygenation. This may be true in other species.  相似文献

Objective To determine the effect of applying plastic clips to the breech and tail of lambs on the perineal and tail bare areas, breech wrinkle, dag (faecal) accumulation, urine stain, body weight and survival of lambs, compared with unclipped, unmulesed lambs and mulesed lambs. Method The study was conducted on five commercial wool‐growing farms in southern Australia. On each study site lambs (2–12‐weeks‐old) were divided into three groups: (1) unclipped, unmulesed control (tail dock only); (2) treated with clips; and (3) treated with the mules operation. Evaluations of effects, including visual scoring, bare area measurements and body weight, were performed before treatment and on or approximately days 30, 60, 90 and 180 after treatment. On each occasion, lambs were recorded as either present or absent to allow estimates of survival. Results The clip treatment increased the size of the perineal and tail bare areas compared with the unclipped, unmulesed control lambs (P < 0.05), although the increases were less than in mulesed lambs (P < 0.05). The clips reduced breech wrinkle, dag and urine stain to levels partway between those recorded in the unclipped, unmulesed controls and the mulesed lambs (P < 0.05). Clipped lambs weighed more than mulesed lambs after treatment (days 30–90 P < 0.001; day 180 P < 0.01) and had higher cumulative percentage survival to 90 days after treatment (P = 0.03). Conclusions The clips successfully modified the breech region of lambs, although the changes were less than with mulesing. The clips did not reduce the body weight of lambs, compared with the unclipped, unmulesed control lambs, and improved 90‐day cumulative percentage survival compared with mulesed lambs.  相似文献

Buprenorphine is an effective analgesic when administered epidurally to humans. The purpose of this study was to compare epidural buprenorphine (B; n = 10) with epidural morphine (M; n = 10) for post‐operative analgesia in dogs undergoing cranial cruciate ligament repair. All dogs were premedicated with acepromazine (0.1 mg kg?1 IM), induced with propofol (4–6 mg kg?1 IV) and maintained with halothane in oxygen. Dogs were randomly assigned to receive B (0.004 mg kg?1) or M (0.1 mg kg?1) in the lumbosacral epidural space in a total volume of 0.2 mL kg?1. End‐tidal halothane and CO2 and temperature were recorded every 15 minutes until extubation (t = 0). A numerical rating pain score (SPS) was recorded at t = 0, 1, 2, 4, 6, 10 and 24 hours by a blinded observer. Dogs received rescue morphine (1.0 mg kg?1 IM) if indicated by SPS and the time of rescue analgesic administration was recorded. Observable side‐effects such as urinary retention, sedation or pruritus were recorded. Data were analyzed with repeated measures anova . Mean ± SD body weight (kg) and age (yrs) for B dogs was 34.2 ± 10.8 and 5.5 ± 2.8; for M dogs these values were 36.6 ± 13.5 and 5.9 ± 3.3. Mean ± SD SPS for B dogs at t = 0, 1, 2, 4, 6, 10 and 24 hours were 1.2 ± 0.75, 3.2 ± 2.0, 4.5 ± 4.3, 4.6 ± 3.4, 4.7 ± 3.0, 5.0 ± 4.9 and 5.1 ± 3.5. For M dogs these values were 1.7 ± 0.5, 2.6 ± 2.0, 3.7 ± 0.75, 4.2 ± 2.2, 4.1 ± 3.0, 3.1 ± 2.1 and 3.9 ± 1.9. There were no significant differences between B and M with respect to SPS, times or frequency of rescue morphine administration, end‐tidal halothane and CO2, or esophageal temperature. Fifty per cent of dogs in both groups required rescue morphine. Buprenorphine is as effective as morphine for epidural analgesia in healthy dogs undergoing hindlimb orthopedic surgery.  相似文献
