A 9-year-old male, pseudohermaphrodite Labrador had intermittent urinary incontinence and haematuria. Laparotomy revealed two neoplastic intra-abdominal testes and a uterus which communicated with the urethra. Seminoma and Sertoli cell tumours were found in the testes.  相似文献

Objective— To determine the effects of clamp reuse for the Kirschner–Ehmer (KE); Securos; and the IMEX–SK clamp.
Study Design— Experimental bench test of mechanical properties.
Methods— Specially designed fixtures were used to mechanically test 18 clamps of each type with respect to 6 mechanical variables: fixator pin slippage, connecting bar slippage, fixator pin rotation, connecting bar rotation, and clockwise and counterclockwise clamp–bolt axis pivot. Each clamp was tested 6 times for each variable at 7.68 Nm of clamp–bolt tightening torque. Results were compared using repeated measures ANOVA.
Results— For the IMEX–SK clamp, a significant degradation in the force required to cause slippage was found for connecting bar slippage and fixator pin rotation; however, this clamp also had a significant increase in the force to initiate slippage for clockwise clamp–bolt axis pivot with reuse. The Securos clamp had significant degradation in connecting bar slippage, connecting bar rotation, and fixator pin rotation whereas the KE clamp had significant degradation in connecting bar slippage only.
Conclusions— All 3 external fixator clamp types degraded in 1 or more movement variables in their ability to resist motion with reuse. The IMEX and Securos clamps were more subject to degradation than the KE clamp and this may have clinical importance for fixator composite rigidity.
Clinical Relevance— Fracture-reduction stability is related to the ability to resist motion within a clamp. The unpredictable nature of degradation we found cautions against repeated use. Clinicians should consider reuse of external fixator clamps with the knowledge that repeated use degrades clamp mechanical performance.  相似文献

A prospective study was undertaken to compare the analgesic effect of intra-articular bupivacaine, morphine, or saline in the 24-hour period following cranial cruciate ligament repair in dogs. Thirty-six clinical patients with ruptured cranial cruciate ligaments were randomly assigned to one of three groups. After surgical stabilization, and before skin closure, an intra-articular injection was given; group one (n = 12) received 0.5% bupivacaine HCl at 0.5 mL/kg, group two (n = 12) received morphine at 0.1 mg/kg diluted with saline to a volume of 0.5 mL/kg, and group three (n = 12) received saline at 0.5 mL/kg. Heart rate, respiratory rate, mean arterial blood pressure, cumulative pain score, visual analog pain score, and pain threshold test on both stifles were recorded preoperatively and at 0 to 6 and 24 hours postoperatively. Surgeons and pain scoring investigators were unaware of the intra-articular medication given. Supplemental analgesia, if needed, was provided in the postoperative period according to subjective assessment of patient discomfort. Postoperative pain scores were lowest in the bupivacaine group and highest in the saline group. Pain threshold, measured by applying calibrated loads to the knee, was higher postoperatively in the bupivacaine group than in the saline group. Dogs in the morphine and bupivacaine groups required less supplemental analgesia than dogs in the saline group. The local provision of analgesia reduces the need for systemic drugs with potential side effects. Both intra-articular morphine and intra-articular bupivacaine provided better postoperative analgesia than intra-articular saline, with intra-articular bupivacaine showing the greatest effect.  相似文献

Nine stifle arthrodeses in eight dogs were reviewed retrospectively to evaluate use of the limb, each dog's comfort, complications, and factors that may have influenced the final outcome. Ability to use the limb after unilateral fusion was good (limb used at all times) in three dogs, fair (limb used at all gaits except a gallop) in three dogs, and poor (limb used only when running) in one dog. Factors that appeared to affect the outcome included angle at which the stifle was fused and lesions in the ipsilateral coxofemoral joint. One dog with bilateral arthrodesis had a good outcome with minor limitations. The only potentially devastating complications occurred in one dog in which infection and premature implant loosening jeopardized the fusion. None of the dogs exhibited signs of pain and all owners were satisfied with the results.  相似文献

The effect of implantation of polyvinilidine spinal plates was studied when placed on spinous processes of 11 mixed breed dogs and when placed within the thoracolumbar musculature of 12 male white rats. Specimens were also obtained from soft tissues adjacent to polyvinilidine plates 2 to 3 months after clinical surgery to repair spinal subluxations in three dogs. A similar pattern of dense, organized fibrous tissue interspersed with focal congregations of lymphocytes, plasma cells, and multinucleated giant cells was observed in the dog experiment, rat experiment, and in all clinical cases. Birefringent debris with polarization patterns identical to small slivers of polyvinilidine was associated with the observed reactions. It would appear that either a chemical or physical characteristic of the plates elicited this foreign body reaction. Grooving the inner surface of the plates, which is performed to increase the friction grip when applied to spinous processes, may make the surface texture of the plate more reactive or it may predispose the surface of the plate to minute fragmentation ("wear products").  相似文献

The knot security of chromic gut, polyglycolic acid, polyglactin 910, polydioxanone, polypropylene, and monofilament nylon size 2-0 suture materials were tested biomechanically in vitro. Twenty reproducible knots were tied and incubated in canine serum at 37 degrees for 24 hours before testing. A "secure knot" was defined as a knot that, when tested to failure, broke rather than untied by slippage. The minimum number of throws necessary to make a secure, snug (1500 g tension) square knot was three for gut, polyglycolic acid, polyglactin 910, and polypropylene and four for polydioxanone and nylon. All throws including the first were counted. With all suture materials tested, surgeon's knots were as secure as square knots. Only gut, polyglycolic acid, and polydioxanone granny knots were as secure as square knots; no loosely tied (500 g tension) asymmetric square knots were as secure as snug square knots, and only polydioxanone and polypropylene loose square knots were as secure as snug square knots. Square knots used to start a continuous pattern required one additional throw with gut, polydioxanone, and nylon. Square knots used to end a continuous pattern required two to three additional throws with all materials tested.  相似文献

Osteomyelitis with sequestration of large fragments of cortical bone accompanied delayed union-nonunion in the tibia of three dogs. All fracture sites were grossly unstable at initial presentation, with the sequestration creating large bony deficits that required bone grafting. All three fractures were treated by the application of external fixation, seques-trectomy, debridement, cancellous bone grafting, and dependent wound drainage. One animal refractured the tibia causing an exacerbation of the osteomyelitis. Reapplication of an external fixation frame and direct current electrical stimulation resulted in bony union. There were no recurrences of infection on long-term follow-up in any of the three tibias.  相似文献

Four commonly used cranial cruciate ligament (CCL) repair techniques were compared biomechanically in canine cadaver stifles by assessment of immediate postoperative laxity (cranial drawer) and stiffness. Each joint was tested nondestructively before repair, with the CCL intact and with the CCL excised. The cranio-caudal laxity after each repair was expressed as a percentage of the laxity in the same stifle after CCL excision. The stiffness of a repair was expressed as a percentage of the same intact joint's stiffness at an equivalent displacement. Repaired stifles were tested three consecutive times to the same maximum load (180 N) and the comparative laxity and stiffness in each test were determined. Fibular head transposition resulted in significantly increased stiffness, less laxity, and less decay per test than the other repairs. Lateral imbrication resulted in significantly less laxity on all tests, and significantly greater stiffness than the fascial strip or patellar tendon on the second and third tests to maximum load. The fascial strip resulted in significantly less laxity on all tests than the patellar tendon, but it was not significantly greater in stiffness. These data suggest that, of the reconstructive methods tested, fibular head transposition has biomechanically superior properties in the immediate postoperative period.  相似文献

Abstract— The objective of this double-blinded crossover study was to examine the effects of marine oil supplementation with high-dose eicosapentaenoic acid (EPA) on canine pruritic skin disease. Sixteen dogs that completed this study had clinical signs related to idiopathic pruritus, confirmed atopy and/or flea allergy. Each dog was randomly placed on one omega-3 fatty acid capsule (MVP: Meridian Veterinary Products, St. Augustine, FL, U.S.A.) which contained 1 ml of marine oil (180mg EPA and 120mg DHA) or one capsule containing 1 ml of corn oil (570 mg linoleic acid and 50 mg gamma linolenic acid) per 4.55 kg of body weight q 24 h for 6 weeks. After a 3-week washout period in which no supplement was given, each subject was crossed over to the other supplement for an additional 6 weeks. Dogs receiving marine oil showed a significant improvement in pruritus (P < 0.001), self-trauma (P <0.05) and coat character (P<0.01) over time. When compared to the corn oil control over time, marine oil supplementation significantly improved pruritus (P < 0.02), alopecia (P < 0.05) and coat character (P < 0.001). This study demonstrates the effectiveness of high doses of marine oil as an alternative anti-inflammatory for canine pruritic skin disease. Zusammenfassung— Ziel dieser Doppelblindkreuzstudie war die Untersuchung von Wirkungen der Substitution mit marinem Öl mit hohem Gehalt an Eicosapentensäure (EPA) auf juckende Hautkrankheiten beim hund. Sechzehn Hunde, die die Studie vollständig durchliefen, litten unter klinischen Symptomen, die mit idiopathischem Pruritus, nachgewiesener Atopie und/oder Flohallergie verbunden waren. Jeder Hund erhielt nach dem Zufallsprinzip eine omega-3-Fettsäurekapsel (MVP: Meridian Veterinary Product; St. Augustine, FL, U.S.A.), die 1 ml von marinem Öl enthielt (180 mg EPA und 120 mg DHA) oder eine Kapsel, die 1 ml Pflanzenöl enthielt (570 mg Linolensäure und 50 mg gamma-Linolensäure) bei einer Dosierung von 1 Kapsel pro 4,55 kg Körpergewicht alle 24 Stunden über 6 Wochen. Nach einer dreiwöchigen Ausschwemmphase, in der keine Substitution verabreicht wurde, wurde jedes Tier für weitere 6 Wochen auf die andere Supplementation gesetzt. Hunde, die marines Öl erhielten, zeigten eine signifikante Besserung des Juckreizes (P < 0,001), der Selbsttraumatisierung (P <0,05) und der Fellbeschaffenheit (P < 0,01) während des Unter-suchungszeitraumes. Im Verglich zur Pflanzenölkontrolle über den Zeitraum besserte die Substitution mit marinem Öl signifikant den Juckreiz (P < 0,02), die Alopezie (P < 0,05) und die Fellbeschaffenheit (P < 0,001). Diese Studie zeigt die Wirksamkeit von hohen Dosen von marinem Öl als alternativen Entzündungshemmer bei juckenden Hauterkrankungen des Hundes. Dieciséis perros fue completaron este estudio presentaban signos clinicos asociados con prurito idiopático, atopia confirmada y/o alergia a pulgas. Cada perro fue puesto al azar en una cápsula de ácido graso omega-3 (MVP: Meridian Veterinary Product; St. Augustine, FL, U.S.A.) fue contenia 1 ml de aceite marino (180mg EPA y 120 mg DMA) o una cápsula que contenia 1 ml de aceite de maiz (570 mg ácido linoleico y 50 mg ácido gamma linolénico) por 4,55 kg de peso corporal, 24 horas durante 6 semanas. Después de un periodo de 3 semanas durante el cual no se dió ningún suplemento, cada sujeto fue cambiado al otro suplemento por un periodo adicional de 6 semanas. Los perros que recibieron aceite marino mostraron una mejora significante en el prurito (P < 0,001), auto-trauma (P < 0,005) y carácter de la capa (P < 0,01) a lo largo del tiempo. Cuando se comparó al control del aceite de maiz a lo largo del tiempo, el suplemento de aceite marino mejoró significantemente el prurito (P < 0,02), alopecia (P < 0,005) y carácter de la capa (P < 0,001). Este estudio demuestra la efectividad de dosis altas de aceite marino como una alternativa antiinflamatoria para la enfermedad canina pruritca de la piel. Résumé— Un chien, jeune adulte, a été présenté pour une masse sous-cutanée évoluant depuis un mois sur la partie droite du thorax et une masse ulcérée sous-cutanée sur la cuisse droite. L'examen histo-pathologique a révélé une inflammation pyogranulomateuse du derme et du tissu sous-cutané avec de larges hyphes fongiques. De nouvelles lésions sont apparues. Un traitement à base d'itraconazole pendant 2 semaines a été inefficace. L'euthanasie a été pratiquée 10 semaines après la première consultation. La culture fongique faite à partir de prélèvements chirurgicaux a permis d'isoler un Conidiobolus sp. Des infections àConidiobolus sp. ont été rapportées chez l'homme, le cheval, le mouton et le lama. Histologiquement, les hyphes sont difficiles à différencier de ceux d'autres zygomycètes ou de Pythium insidiosum. La culture est nécessaire pour une identification définitive. Zusammenfassung— Ein junger ausgewachsener Hund wurde vorgestellt, der seit einem Monat an der rechten Thoraxwand ein subkutanes Gebilde und proximal an der rechten Hintergliedmaße eine ulzer-ierende, subkutane Masse aufwies. Bei der histopathologischen Untersuchung wurde eine pyogranulomatöse Entzündung der Dermis und Subkutis mit breiten Pilzhyphen entdeckt. Trotz des “Auf und Ab” der Hautkrankheit erschionon laufend neue Hautveränderungen. Eine zweiwöchige Behandlung mit Itrakonazol veränderte den Verlauf der Krankheit nicht. 10 Wochen nach der Erstvorstellung des Patienten wurde en euthenesiert. In der Kultur von chrirugischen Proben wurde Conidiobolus sp. isoliert. Conidiobolus sp.-Infektionen sind bei Menschen, Pferden, Schafen und Lamas beschrieben. Die Hyphen können histologisch schlecht von denen anderen Zygomyzeten oder von Pythium insidiosum unterschieden werden. Für die endgültige Diagnose ist eine Identifizierung mittels Pilzkultur notwendig. Die Erscheinungsbilder von kutanen Conidiobolus sp.-Infektionen werdon diskutiert und mit denen kutaner Phytiosis verglichen. [Canine subcutaneous zygomycosis caused by Conidiobolus sp.: a case report and reviews of Conidiobolus infections in other species (Subkutane Zygomykose des Hundes durch Conidiobolus sp.: Fallbericht und übersicht über Conidioblus-Infektionen bie anderen Tierarten). Abstract— A young adult dog was presented with a 1-month history of a subcutaneous mass on the right thoracic wall and an ulcerative, subcutaneous mass of the proximal right hindlimb. Pyogranulomatous inflammation of the dermis and subcutis with broad fungal hyphae were found on histopathologic examination. Despite some waxing and waning of the skin disease, new lesions continued to appear. A 2-week course of itraconazole did not alter the course of the disease. Euthanasia was performed 10 weeks after initial presentation. Conidiobolus sp. was isolated in culture from surgical specimens. Conidiobolus sp. infection has been reported in humans, horses, sheep and llamas. The hyphae cannot be easily distinguished histologically from those of some other zygomycetes or Pythium insidiosum and fungal culture identification of the organism is required for definitive diagnosis. A discussion of the features of cutaneous Conidiobolus sp. infection is given and comparative features of cutaneous pythiosis are presented.  相似文献
