Lactate Kinetics in veterinary Critical Care: A Review   总被引:3,自引:0,他引:3  
Elevation in blood lactate concentration, with or without accompanying metabolic acidosis, is a hallmark finding in patients with circulatory compromise, and is also consistently noted in other conditions affecting critically ill or injured individuals. Little is reported in a veterinary literature regarding lactate measurement in the emergency and critical care setting, despite impressive reports of the clinical usefulness of lactate measurement in people. The purpose of this article is to review lactate kinetics and the clinical utility of lactate measurement. Limitations to lactate evaluation will also be discussed.  相似文献

Five identifiable patterns of deforestation are recognized - internal, indentation, cropping, fragmentation, and removal - and each has a distinct effect on habitat quality of forest patches in the eastern United States. By overlaying land use maps from 1973 and 1981 for three counties in the State of Maryland (Prince Georges, Anne Arundel, and Wicomico), changes in the interior core area and edge length of individual patches were measured. Forest interior declined by 23.8 km2 in Anne Arundel, 16.3 km2 in Prince Georges, and 8.4 km2 in Wicomico. Within Anne Arundel and Prince Georges Counties, deforestation increased edge length by 52.1 km and 31.2 km, respectively, whereas, within Wicomico, it decreased edge length by 8.7 km. Differences among counties resulted from current land use patterns, percentage of forest cover, and the dominant deforestation pattern.  相似文献

Over the past several decades, recognition of acute respiratory failure as the cause of death in patients suffering from various clinical conditions has prompted aggressiv investigation into the area of respiratory physiology and supportive respiratory care. With the evolution of emergency medicine and critical care services in both human and veterinary medicine, many patients previously considered unsalvageable due to the severity of their underlying disease are now being resuscitated and successfully supported, creating a new population of critically ill patients. Where only a decade ago these patients would have succumbed to their underlying disease, they now survive long enough to manifest the complications of shock and tissue injury in the form of acute respiratory failure. Investigation into the pathophysiology and treatment of this acute respiratory distress syndrom (ARDS) has facilitated increased clinical application of respiratory theerapy and machanical ventilation.1 The purpose of this paper is to provide a basic review of respiratory mechanics and the pathophysiology of hypoxemia as they relate to airway pressure therapy in veterinary patients and to review the use of airway pressure therapy in veterinary patients This paper is divided into two parts; part I reviews respiratory mechanics and hypoxemia as they apply to respiratory therapy, while part II deals specifically with airway pressure therapy andits use in clinical cases.  相似文献

As the specialties of emergency medicine and critical care have grown and evolved in both human and veterinary medicine, so has the need for more advanced care of patients with primary lung disease. Treatment of acute respiratory failure has been the focus of several articles in the human medical literature of the past few years.1,8 This paper deals with airway pressure therapy and its application in cases of acute respiratory failure in veterinary medicine. Abstract. Recent developments in in situγ ray spectrometry offer a new approach to measuring the activity of radionuclides such as 137Cs and 40K in soils, and thus estimating erosion or deposition rates and field moist bulk density (ρm). Such estimates would be rapid and involve minimal site disturbance, especially important where archaeological remains are present. This paper presents the results of a pilot investigation of an eroded field in Scotland in which a portable hyper pure germanium (HPGe) detector was used to measure γ ray spectra in situ. The gamma (γ) photon flux observed at the soil surface is a function of the 137Cs inventory, its depth distribution characteristics and ρm. A coefficient, QCs, derived from the forward scattering of 137Cs γ ray photons within the soil profile relative to the 137Cs full energy peak (662 keV), was used to correct the in situ calibration for changes in the 137Cs vertical distribution in the ploughed field, a function of tillage, soil accumulation and ρm. Based on only 8 measurements, the agreement between in situγ ray spectrometry and soil sample measurements of 137Cs inventories improved from a non significant r2=0.05 to a significant r2=0.62 (P<0.05). Erosion and deposition rates calculated from the corrected in situ137Cs measurements had a similarly good agreement with those calculated from soil cores. Mean soil bulk density was also calculated using a separate coefficient, QK, derived from the forward scattering γ photons from 40K within the soil relative to the 40K full energy peak (1460 keV). Again there was good agreement with soil core measurements (r2=0.64; P<0.05). The precision of the in situ137Cs measurement was limited by the precision with which QCs can be estimated, a function of the low 137Cs deposition levels associated with the weapons testing fallout and relatively low detector efficiency (35%). In contrast, the precision of the in situ ρm determination was only limited by the spatial variability associated with soil sampling.  相似文献

Dogs were examined ophthalmoscopically to determine the prevalence and type of ocular lesions associated with naturally acquired Rocky Mountain spotted fever (RMSF). In a consecutive series of 11 dogs with serologically confirmed RMSF, 9 had ocular involvement reflecting various degrees of vascular damage, including subconjunctival hemorrhage, hyphema, anterior uveitis, iris stromal hemorrhage, retinal petechiae, and focal areas of retinal edema and perivascular inflammatory cell infiltrate. The ophthalmic lesions generally were mild, and most resolved without complication after systemic administration of appropriate antibiotics. The high prevalence of ocular lesions in these dogs suggested that critical ophthalmologic evaluation is a useful diagnostic tool for the early detection and management of RMSF in dogs.  相似文献

Reproductive clinicians striving to practice good quality medicine readily identify topics of common concern: causes that are not identified or therapeutics that are not available or applicable. Three dogs with multilobular osteochondrosarcoma of the skull were evaluated using magnetic resonance (MR) imaging. Spin echo T1, T2, proton weighted and post contrast T1W images were obtained with a 1.5 Tesla magnet. The MR imaging findings were similar in all three dogs with mixed signal intensities in the T1W, T2W and proton weighted images and fairly large areas of contrast enhancement in the post contrast T1W images. The extent of brain and soft tissue involvement were well delineated and provided useful information concerning surgical planning. MR imaging provided a useful method of evaluating dogs with skull tumors.  相似文献

The use of saline solutions was tested to determine their efficacy as replacements for ovarian fluid as sperm activators and to eliminate variability encountered with the use of Ovarian fluid. We tested fertilization rate of semen from eight males on Atlantic salmon Salmo salar eggs after five sperm-activating solutions and a non-activating saline were substituted for ovarian fluid. We used solutions shown acceptable for use with other salmonid species. The six solutions tested were a non sperm-activating phosphate-buffered saline (PBS, 7.2 g/L NaCl, 1.48 g/L Na2HPO4, 0.43 g/L K H2PO4), a Tris buffer (6.99 g/L NaCl, 3.63 g/L Tris and 2.42 g/L glycine), a Borax buffer (12.2 g boric acid/L in solution 1, 76 g disodium tetraborate/L combined 100:118, then 1 L combined with 3.7 L water and 18 g NaCl), and three solutions of 9.25 g/L (125 mM) NaCl buffered to pH 6.0, 7.5, and 8.9. The latter five solutions activated sperm immediately on contact, while PBS required additional water to activate sperm. The PBS solution was the least effective (mean percent eyed eggs 37.6%) for egg fertilization. The mean percent eyed eggs for the other five saline solutions (range 78% to 86%) were not significantly different. Sperm from one male provided significantly lower egg fertilization (39.6%) when compared with the other seven males (67.2–87.4% egg fertilization). Percent egg fertilization was not related to number of live sperm cells per egg. Our results show that osmotically-balanced sperm-activation solutions, even those with a pH range from 6.0 to 8.9 provide adequate media for fertilization of Atlantic salmon eggs. Fertilization in a deactivation saline and water reactivation of sperm resulted in low egg fertilization.  相似文献

Thromboembolic meningoencephalitis (TEME) caused by Huemophilus somnus infection was diagnosed for the first time in New Zealand in an eight month old bull calf. Clinical signs included pyrexia, excessive salivation, recumbency, opisthotonus and rigidity of the front legs. Multiple red, malacic foci (1-10 mm in diameter) were visible grossly on the surface of the cerebrum and cerebellum, and were scattered throughout the brain parenchyma. Histologically these lesions consisted of focal haemorrhage, necrosis and neutrophilic infiltration. Fibrin thrombi and colonies of gram negative bacteria were present in many small blood vessels within necrotic foci. An organism with the morphological and biochemical characteristics of Haemophilus somnus was cultured from the brain. Although TEME is an important disease of feedlot cattle in North America and Europe it is relatively uncommon in cattle grazing pasture and is therefore unlikely to become a major problem in New Zealand.  相似文献
