Improvements in human patient monitoring despite their development in animals, do not always find their way into veterinary clinical use due to financial constraints. Gastrointestinal intraluminal CO2 partial pressure (Gip1 CO2 ) monitoring, however, is not only proving very beneficial in human trauma and critical patient care but is also very likely to become relatively inexpensive. By providing information on the perfusion adequacy of a high risk, critically important tissue, the GI mucosa, GI P1 CO2 monitoring offers an easily accesible indicator of the efficacy and adequacy of resuscitative interventions. The potential for decreasing morbidity and mortality is enormous. Therefore, the practicing veterinarian should become familiar with GI P1 CO2 monitoring theory and technology so he or she can be better prepared to incorporate it into practice when in becomes available. 相似文献

A juvenile female black rhinoceros (Diceros bicornis) was successfully treated after overdose of drugs used for chemical restraint. Subsequent general anaesthesia for surgical reduction of a recurrent rectal prolapse was uneventful. Over a 25-minute period before transportation to the veterinary hospital, the animal received a total dose of 1.225 mg etorphine, 30 mg acepromazine and 30 mg detomidine. Based on an estimated mass of 200 kg, these corresponded to doses of 6.1 microg kg(-1) etorphine, 150 microg kg(-1) acepromazine, and 150 microg kg(-1) detomidine which constitutes considerable overdose for each drug given separately, notwithstanding the synergy that probably resulted when the three drugs were present concurrently. The estimated body mass may have substantially overestimated the actual body mass and exacerbated overdosage. The animal was recumbent and apnoeic on arrival at the hospital. Heart sounds were auscultated and a weak peripheral pulse was palpated; no pulse deficits were detected, although the heart rate was low. The trachea was intubated, inspired breath was enriched with oxygen and the lungs ventilated manually. Diprenorphine (1.5 mg) was given intravenously and spontaneous breathing resumed 11 minutes later. After induction of general anaesthesia using isoflurane, emergency surgery for correction of rectal prolapse was performed, from which the animal recovered uneventfully. The case highlights some of the practical problems that may be encountered in dealing with dangerous and unfamiliar species. 相似文献

A 3‐month‐old female Warmblood foal, weighing 150 kg, was presented to the Equine Clinic of the University of Hannover with a fracture of the ulna (type 5 fracture of the olecranon). Anaesthesia was induced with midazolam and ketamine after sedation with xylazine and maintained with isoflurane. Anaesthesia was uneventful until cardiac arrest occurred in the early recovery period. Cardiopulmonary resuscitation (CPR) was performed with chest compressions and electrical defibrillation. During resuscitation palpebral reflexes and spontaneous breathing returned but ventricular fibrillation could not be converted to sinus rhythm by transthoracic defibrillation of the heart by electrical shock. 相似文献
