A case of infectious arthritis of a cervical vertebral articulation and vertebral osteomyelitis in a foal caused by Salmonella is described. Ultrasound images of the cervical facet joints were suggestive of septic arthritis and ultrasound facilitated needle placement for arthrocentesis and joint lavage. Due to the uncommon location of the infection, diagnosis of septic arthritis was not immediate and, despite aggressive intra‐articular lavage and instillation of appropriate intra‐articular antibiotics after hospitalisation, we were unable to resolve the infection in this foal. Radiographs did not show evidence of osseous involvement until later in the course of the infection. Septic arthritis should be suspected in cases with similar clinical and laboratory findings and radiographic changes may not be present early in the course of infection. Ultrasound imaging provided the most useful diagnostic information regarding infection of the cervical joints, facilitating both diagnosis and treatment.  相似文献

Objective   To determine the clinical characteristics, short-term outcome and future athletic performance of foals with septic osteomyelitis.
Design   Retrospective clinical study of 108 Thoroughbred foals with radiographic evidence of bone infection that were presented at the Scone Veterinary Hospital between August 1995 and December 2001. Medical records were reviewed and information concerning signalment, the clinical, laboratory and radiographic findings, treatment and outcome was obtained. Racing records were obtained and evaluated for surviving foals that had reached racing age.
Results   Mean age of foals at initial evaluation was 39 days (range 1–180 days); 21 foals had multiple radiographic bone lesions (19.4%), and 76 had concurrent septic arthritis (70.4%). The most frequently affected bones were the femur, tibia and distal phalanx. In total, 87 foals were discharged from the hospital (80.6%), 79 survived long-term to reach racing age and 52 raced (65.8%). Overall, 48% (52/108) of the foals treated for osteomyelitis raced. Foals less than 30 days of age at the time of diagnosis, critically ill foals and those with multiple bones or joints affected were significantly less likely to be discharged from hospital. Multiple septic joints, but not multiple bone involvement, had an unfavourable prognosis for racing.
Conclusions   The prognosis for survival of foals with septic osteomyelitis or osteitis is favourable. Multiple bone or joint involvement is an important short-term prognostic indicator; however, the involvement of multiple joints, but not multiple infected bones, is associated with an unfavourable prognosis for racing.  相似文献

Computed tomography (CT) features of four immature to young adult dogs with osteomyelitis of the skull are described. Trauma or bite wounds were the cause of infection and Staphylococcus aureus was the most common pathogen. CT features were a combination of soft tissue thickening, bone lysis, and bone proliferation. Bone lysis was extensive in some dogs with a moth-eaten appearance and involved the calvarium, base of the skull, the frontal sinuses, and the temporomandibular joint. In other dogs it was more focal with thinning of the bone rather than complete lysis. Bone proliferation also varied in appearance from irregular palisading or spiculated to expansion and septation of the frontal bone. Sequestrum formation was seen in one dog. Widespread infection in one dog involved the tympanic bullae and the temporomandibular joint. Lysis of the calvarium resulted in bacterial meningitis in two dogs. One dog was euthanized and three were treated with surgical curettage of the affected bone and antibiotic therapy which resulted in resolution of the clinical signs in one dog whereas two dogs had recurrent disease. CT was very helpful for characterizing extent and localization of the infection. Despite the aggressive CT features, osteomyelitis should be considered especially in young animals with a history of trauma or bite wounds. The pathophysiology of skull bone infections is discussed.  相似文献

Osteomyelitis in adult horses, often associated with trauma or iatrogenic infection following surgery, usually presents as a focal area of infected bone. Diffuse osteomyelitis, affecting both the cortex and medulla, along the full length of a bone is encountered less frequently and treatment of such infections is not well reported in horses. The 2-year-old Warmblood mare in this case was diagnosed with diffuse osteomyelitis affecting the 4th metacarpal bone with concurrent unicortical fracture of the third metacarpal bone following traumatic injury. Computed tomography (CT) aided diagnosis in this case, providing critical additional detail to that obtained with radiography and ultrasound. This case highlights the value of CT in the diagnosis of diffuse osteomyelitis. This is the first reported case of diffuse osteomyelitis caused by Clostridium perfringens in horses. Successful treatment in this case consisted of surgical debridement of the associated abscess, followed by systemic and locoregional antimicrobial therapies.  相似文献

At the slaughterhouse, 1465 culled sheep heads were studied in order to identify disorders of the mandibular cheek teeth. Of these, 227 (15.5%) had evidence of osteomyelitis. The lesional profile showed that the mandible was affected in a similar proportion on both sides (46.7% on the right side vs 50.7% on the left side), mainly in the middle region (55.3%) and with most of the lesions closed without fistulisation (89.4%). In addition, swelling was palpable, with an increase in thickness in the area of the affected body (2.65 ± 0.065 cm vs 1.74 ± 0.030 cm). In 78.6% of the animals, the regional lymph nodes were enlarged. Concerning the mandibular cheek teeth, more than half of the animals lacked at least one tooth (57.0%), with the first premolar being the most frequently missing tooth (34.8%) and the third molar the least (8.2%). The impaction of food around the teeth was very common with the posterior molars being more frequently affected. In the knowledge of the authors, this is the first study that analyses the prevalence of mandibular molar and premolar disorders in sheep, and these are revealed as an important condition affecting culled sheep.  相似文献

Osteolytic appendicular skeletal lesions in eight-stranded, cold-stunned Kemp's ridley sea turtles (Lepidochelys kempii) were evaluated using radiography and skeletal scintigraphy. Radiographic studies were performed monthly in most animals. Follow-up scintigraphy was performed 45-120 days after the initial exams in six turtles. Radiographically, lesions slowly progressed from an early osteolytic process contained to either the proximal or distal end of long bones, to a later stage characterized by thickening of the affected bone, sclerosis, and remodeling of the lesion borders. In seven turtles, the initial scintigrams were characterized by at least one focus of abnormal radiopharmaceutical uptake that correlated with a lytic site noted in radiographs. In five turtles, scintigraphic lesions were characterized by asymmetric radiopharmaceutical uptake rather than by increased intensity of uptake. Scintigraphic studies obtained more than 4 months after the appearance of clinical and radiographic signs had minimal, if any, abnormal radiopharmaceutical uptake, despite the persistence of abnormal radiographic findings. Skeletal scintigraphy is an effective method for more precisely determining if and when these animals can be returned to the wild. Animals were released if normal radiopharmaceutical uptake was seen during initial examination, or if decreased uptake was noted between serial examinations. In four of the turtles, resolution of abnormal scintigraphic findings permitted an objective decision to discontinue antibiotic and antifungal therapy. Seven of the eight turtles were released after correlation of the clinical signs with the imaging findings. Radiographs, however, are still needed to facilitate the correct identification of lesions with scintigraphy.  相似文献

A 7‐year‐old Quarter Horse gelding was referred for magnetic resonance (MR) imaging due to chronic left hindlimb lameness localised to the foot. On presentation, a previously undiagnosed draining tract was identified at the plantar aspect of the pastern. Radiographs revealed severe osteolysis of the navicular bone. Positive contrast MR fistulography was performed using a gadolinium based contrast agent following conventional MR imaging of the left hind foot. Fistulography allowed characterisation of a fistulous tract, which was closely associated with the deep digital flexor tendon, navicular bursa and osteomyelitis of the navicular bone.  相似文献

Three foals aged between 21 and 44 days were presented for evaluation of pyrexia and lameness. Each had subcutaneous abscessation associated with osteomyelitis of a rib at the costochondral junction and pathological fracture of the affected rib; one foal had more than one rib affected. Other localised sites of infection included other sites of osteomyelitis, omphalophlebitis, uveitis, enterocolitis, and calcaneal bursitis. The subcutaneous abscessation and rib osteomyelitis was treated surgically in two foals. Salmonella typhimurium was isolated from the subcutaneous abscess in one foal and from the faeces of another. Two of the three foals were euthanased. Ultrasonography was a more useful modality than radiography for establishing the diagnosis of costal osteomyelitis.  相似文献
