Objective To estimate the risks of anaesthetic and sedation‐related mortality in companion animals in the UK. (The Confidential Enquiry into Perioperative Small Animal Fatalities, CEPSAF). Study design A prospective cohort study with nested case–control study. Animal population All small animals anaesthetized and sedated at participating centres between June 2002 and June 2004. Methods Patient outcomes at 48 hours (alive, dead and killed) were recorded. Anaesthetic and sedation‐related death was defined as death where surgical or pre‐existing medical causes did not solely cause death. Species‐specific risks of anaesthetic‐related death and 95% confidence intervals (95% CI) were calculated. Risks were also estimated in the sub‐sets of dogs, cats and rabbits that were either healthy or sick (ASA 1–2 and 3–5, respectively). Results One hundred and seventeen veterinary practices participated in the study and 98 036 dogs, 79 178 cats and 8209 rabbits were anaesthetized and sedated. Overall risks of anaesthetic and sedation‐related death in dogs were 0.17% (1 in 601, 95% CI 0.14–0.19%), in cats 0.24% (1 in 419, 95% CI 0.20–0.27%) and in rabbits 1.39% (1 in 72, 95% CI 1.14–1.64%) within 48 hours of the procedure. In healthy dogs, cats and rabbits, the risks were estimated to be 0.05% (1 in 1849, 95% CI 0.04–0.07%), 0.11%, (1 in 895, 95% CI 0.09–0.14%) and 0.73% (1 in 137, 95% CI 0.54–0.93%), respectively. In sick dogs, cats and rabbits, the risks were 1.33%, (1 in 75, 95% CI 1.07–1.60%), 1.40% (1 in 71, 95% CI 1.12–1.68%) and 7.37% (1 in 14, 95% CI 5.20–9.54%), respectively. Postoperative deaths accounted for 47% of deaths in dogs, 61% in cats and 64% in rabbits. Most other small animal species had higher mortality risks. Conclusions and clinical relevance Small animal anaesthesia appears to be increasingly safe. Greater patient care in the postoperative period could reduce fatalities. 相似文献

Pigmentary uveitis (PU), also known as Golden Retriever Pigmentary Uveitis (GRPU), is a common ocular condition of Golden Retrievers that has severe, vision‐threatening ocular complications and can require surgical intervention. In order to ensure consistency in the diagnosis of GRPU between examiners, a specified set of diagnostic criteria must be applied. This is critical to ensure owners, breeders, and veterinary ophthalmologists maintain confidence in the ocular certification process. Cytologic sampling of the ultrasonographically normal spleen and liver is not implemented routinely in the clinical staging of canine cutaneous mast cell tumors and normal ultrasound findings are often accepted as sufficient evidence for ruling out splenic or liver metastasis. Our objective was to define the specificity and sensitivity of ultrasound findings for diagnosis of mast cell infiltration when verified with cytologic evaluation, and to define the prognostic role of cytologic evaluation of liver and splenic aspirates. Dogs with a diagnosis of clinically aggressive grade II, or grade III mast cell tumor treated with a combination vinblastine/CCNU chemotherapy protocol, were selected retrospectively based on availability of cytologic evaluation of spleen plus or minus liver for staging. Out of 19 dogs, 10 dogs had a grade II tumor and nine a grade III tumor. Seven dogs had mast cell infiltration of the spleen, liver, or both. The sensitivity of ultrasound for detecting mast cell infiltration was 43% for the spleen and 0% for the liver. Dogs with positive cytologic evidence of mast cell infiltration to spleen, liver, or both had significantly shorter survival (100 vs. 291 days) than dogs without evidence of mast cell infiltration (P<0.0001). Routine splenic aspiration should be performed regardless of ultrasonographic appearance in dogs with a clinically aggressive mast cell tumor. 相似文献

A survey for the prevalence of antibodies to Hepatozoon canis and for intraneutrophilic H. canis gametocytes in the peripheral blood neutrophils of dogs in Israel showed that 33.1% were seropositive, while only 1% of the dogs sampled had detectable parasites in their blood smears. The pattern of the specific 5-HT2A (5-hydroxytryptamine 2A receptor) antagonist 123I-5-I-R91150 was measured in 10 healthy dogs without neurologic and behavior abnormalities. Eight cortical regions (left and right fronto-, temporo-, parieto-, and occipitocortical area), one global subcortical region (including the thalamic system) were compared with a reference region lacking receptors; that is, the cerebellum. The 123I labeled radioligand was injected intravenously 100-200 minutes before acquisition. Both transmission and emission data were obtained with a triple head gamma camera equipped with high-resolution fanbeam collimators. The emission data were corrected for scatter and attenuation. To delineate different cerebral regions more accurately, the regions of interest (ROI) defined in a former study on brain perfusion measured with 99mTc-ethyl cysteinate dimer (ECD) in the same dogs were used. The co-registration of the 99mTc-ECD and the 123I-5-I-R91150, obtained from each dog, was realized with the help of corresponding transmission maps. By normalizing each regional cerebral activity to the activity observed in the cerebellum, the regional radioactivity (binding index) could be relatively quantified. Highest brain uptake was noted in the frontocortical brain areas (right: 1.85, left: 1.89), followed by the temporocortical region (right: 1.58, left: 1.56). Least uptake was noted in the more caudal and middle brain regions [occipito- (right: 1.46, left: 1.41), parietocortical (right: 1.30, left: 1.26), and striatal region (1.19)]. No gender nor age influence was noted in this series. The 123I labeled serotonin-2A receptor ligand seems to have similar cortical binding in the normal canine brain, as shown in humans and other animal species. A frontocortical to occipitocortical (rostrocaudal) binding index gradient was identified within the dog, which has not been seen in imaging studies from humans and other animal species. The significance of these results will need further investigation. This normative data can be used to compare regional brain uptake of the 123I-radioligand to dogs with behavioral disorders related to the serotonergic system, in future studies. 相似文献

ObjectiveTo describe a contrast medium distribution pattern after peribulbar injection in cadavers.Study designA cadaveric study.AnimalsA group of eight male dogs and four canine cadaver heads.MethodsIn phase 1, ultrasound-guided peribulbar injections (1.0 mL dye) were performed in four orbits using a subzygomatic approach. After dissection, the procedure was considered successful if 1) no dye was found in the retrobulbar space, 2) the peribulbar compartment was stained and 3) the dye reached the orbital fissure. In phase 2, using 16 orbits, the same technique was used to inject 0.1 mL kg–1 contrast medium into (group A, n = 8) or outside (group B, n = 8) the peribulbar compartment. After freezing at –20°C, orbits were imaged using computed tomography, and the contrast medium distribution was analysed.ResultsIn phase 1, the procedure was successful in all four eyes. In phase 2, two orbits (one in each group) were removed from the study: one was not injected and the other was injected twice. The contrast medium was found inside the peribulbar compartment in six of seven (85.7%) group A orbits and two of seven (28.6%) group B orbits. In all of these cases, distribution to the orbital fissure, rostral alar foramen, mandibular foramen and inside the skull was observed. The oval foramen was reached in three of seven (42.9%) group A orbits and one of seven (14.3%) group B orbits. In one/seven (14.3%) group A orbits and five/seven (71.4%) group B orbits, the contrast medium was found inside the masseter and temporal muscles, and no distribution towards the aforementioned structures was observed.Conclusions and clinical relevanceUltrasound-guided peribulbar injection of 0.1 mL kg–1 contrast medium in canine cadavers can lead to an extensive distribution of the injectate that reaches the orbital fissure, the rostral alar, oval, and mandibular foramina and can be found intra-cranially. 相似文献

OBJECTIVE: To report a technique for minimally invasive occlusion of patent ductus arteriosus (PDA) and outcome in 5 dogs. STUDY DESIGN: Clinical cases. Animals: Five, 4-6-month-old, dogs with PDA. MATERIALS AND METHODS: Titanium ligating clips were used for PDA closure in all dogs. Three dogs had video-enhanced mini-thoracotomy PDA occlusion. Two other dogs had thoracoscopic PDA occlusion using a custom-designed thoracoscopy clip applicator. RESULTS: Thoracoscopic PDA occlusion was successful in both dogs in which it was attempted. Complete PDA closure was achieved in 4 dogs. Three months after surgery, the largest dog had residual ductal flow that hemodynamically was insignificant. CONCLUSIONS: Although technically demanding, minimally invasive PDA occlusion is a safe and reliable technique in dogs. Preoperative measurement of the diameter of the PDA is crucial to determine if complete closure with metal clips can be achieved. CLINICAL RELEVANCE: Minimally invasive PDA occlusion should be considered as an alternative to occlusion via conventional thoracotomy. 相似文献
