Objectives

To describe the ventral spinal nerve rami contribution to the formation of the brachial plexus (BP), and to compare ease of performing and nerve staining between three blind techniques for BP blockade in dogs.

Study design

Prospective, randomized, blind study.

Animals

A total of 18 dog cadavers weighing 28.2 ± 9.7 kg (mean ± standard deviation).

Methods

Dogs were randomly assigned to two of three BP treatments: traditional approach (TA), perpendicular approach (PA), and axillary approach (AA). Dye (0.2 mL kg?1) was injected in the left BP using a spinal needle; another BP treatment was used in the right BP. Landmarks (L) included: L1, midpoint between point of the shoulder and sixth cervical (C6) transverse process; L2, scapulohumeral joint; and L3, first rib. For TA, the needle was introduced craniocaudally through L1, medial to the limb and cranial to L3. For PA, the needle was directed perpendicular and caudal to L2, aligned with L1, until cranial to L3. For AA, the needle was directed ventrodorsally, parallel and cranial to L3 until at L1. All BPs were scored for dyeing quality [0 (poor) to 5 (excellent)]. The left BP was dissected for nerve origins. Durbin test was used to compare scores (p < 0.05).

Results

In all dogs, the musculocutaneous nerve originated from C7 and C8; the radial nerve from C8, the first thoracic vertebra (T1) (16/18 dogs) and C7 (2/18); and the median and ulnar nerves from C8, T1 (17/18) and C7 (1/18). Respective raw scores and adjusted scores for the incomplete block design were not significantly different (p = 0.72; ranks TA 16.5, PA 19.0, AA 18.5).

Conclusions and clinical relevance

The musculocutaneous, median, ulnar and radial nerves originate from C7, C8 and T1. Regardless of the technique, knowledge of anatomy and precise landmarks are relevant for correct dye dispersion.  相似文献

Objective

To examine changes in the distribution of ventilation and regional lung compliances in anaesthetized horses during the alveolar recruitment manoeuvre (ARM).

Study design

Experimental study in which a series of treatments were administered in a fixed order on one occasion.

Animals

Five adult Warmblood horses.

Methods

Animals were anaesthetized (xylazine, midazolam–ketamine, isoflurane), placed in dorsal recumbency and ventilated with 100% oxygen using peak inspiratory pressure (PIP) and positive end-expiratory pressure (PEEP) of 20 cmH2O and 0 cmH2O, respectively. Thoracic electrical impedance tomography (EIT), spirometry and routine anaesthesia monitoring were performed. At 90 minutes after induction of anaesthesia, PIP and PEEP were increased in steps of 5 cmH2O to 50 cmH2O and 30 cmH2O, respectively, and then decreased to baseline values. Each step lasted 10 minutes. Data were recorded and functional EIT images were created using three breaths at the end of each step. Arterial blood samples were analysed. Values for left-to-right and sternal-to-dorsal centre of ventilation (COV), lung compliances and Bohr dead space were calculated.

Results

Distribution of ventilation drifted leftward and dorsally during recruitment. Mean ± standard deviation (SD) values at baseline and highest airway pressures, respectively, were 49.9 ± 0.7% and 48.0 ± 0.6% for left-to-right COV (p = 0.009), and 46.3 ± 2.0% and 54.6 ± 2.0% for sternal-to-dorsal COV (p = 0.0001). Compliance of dependent lung regions and PaO2 increased, whereas compliance of non-dependent lung regions decreased during ARM and then returned to baseline (p < 0.001). Bohr dead space decreased after ARM (p = 0.007). Interestingly, PaO2 correlated to the compliance of the dependent lung (r2 = 0.71, p < 0.001).

Conclusions and clinical relevance

The proportion of tidal volume distributed to dependent and left lung regions increased during ARM, presumably as a result of opening atelectasis. Monitoring compliance of the dependent lung with EIT may substitute PaO2 measurements during ARM to identify an optimal PEEP.  相似文献

This report documents the feasibility and clinical information provided by a new method for spirometric monitoring adapted for equine anaesthesia. Monitoring of ventilatory function was done with continuous spirometry during general anaesthesia of client‐owned horses presented for various diagnostic and surgical procedures. An anaesthetic monitor with a spirometry unit for human anaesthesia was used. To allow the measurement of large tidal volumes, a remodelled larger version of the pitot tube‐ based flow sensor was used. This technology provided reliable spirometric data even during prolonged anaesthesia when water condensation accumulated in the anaesthetic circuit and the sensor. In addition to flow and volume measurement and respiratory gas analysis, the continuous display of flow‐volume and pressure‐volume loops offered visually recognisable information about compliance, airway resistance and integrity of the circuit. Continuous spirometry with this monitoring system was helpful in evaluating the efficacy of spontaneous ventilation, in adjusting intermittent positive pressure ventilation and detecting technical faults in the anaesthetic apparatus and connection with the patient. This adapted spirometry method represents a practical and reliable measuring system for use during equine anaesthesia. The variety of information provides an opportunity to optimise anaesthetic management in this species.  相似文献

BACKGROUND: Spray volume can influence the amount of free water on the leaf surface and subsequently the ability of entomopathogenic nematodes (EPNs) to move. In this study, an investigation was made of the effect of spray volume (548, 730 and 1095 L ha−1) on the deposition, viability and infectivity of EPNs against Galleria mellonella on savoy cabbage, cauliflower and leek. RESULTS: Increasing spray volume decreased nematode deposition on 7.1 cm2 leek leaf discs at a 15° angle with the spray nozzle. Although the number of living nematodes observed on leek after 240 min of exposure was not significantly different between the low-volume application (548 L ha−1) and the high-volume application (1095 L ha−1), a greater infectivity was obtained in the latter application. The higher number of droplets deposited on the leek discs in the high-volume application may have stimulated nematode movement. No significant effect of spray volume was observed on the relative deposition of Steinernema carpocapsae on the bottom side of cauliflower and savoy cabbage leaf discs. In spite of the low S. carpocapsae deposition on the bottom side of the savoy cabbage discs, high infectivity was obtained against G. mellonella. Using the lowest spray volume on savoy cabbage, infectivity decreased with increasing exposure time, while infectivity was not affected by exposure time when a spray volume of 730 L ha−1 or more was used. CONCLUSION: Spray volume is an important application parameter, as it affects nematode infectivity.
