Objective —To compare two external fixation clamp designs for their ability to resist movement of a fixation pin in relation to the connecting rod. Study Design —Two designs of external fixator clamps were attached to connecting rods mounted on a jig for mechanical testing. Fixator pins were placed perpendicular to the connecting rod. A mechanical testing machine was used to deflect each 3.2-mm pin at a distance that was 25 mm from the center of the clamp bolt. Both clamp designs were tightened to 4.4, 6.1, and 7.8 newton-meters (N m) torque, and loads were applied in a position ramp through 4 mm and resisting loads were measured. Two clamp orientations were used during load application, such that the deflection of the pin tended to tighten the clamp bolt or tended to loosen the clamp bolt. The tests were videotaped to determine mode of failure. Comparisons of the load/displacement curves for the two external fixator clamp designs were made using nonlinear equational curve fitting methods. The resultant plateau and rise coefficients were compared using analysis of variance. Results —Slippage of the pin in relation to the clamp occurred with the Kirschner-Ehmer clamp tightened to 4.4, 6.1, and 7.8 N-m, and slipping of the pin in relation to the clamp occurred with the experimental clamp design tightened to 4.4 and 6.1 N-m but not to 7.8 N-m. At 7.8 N-m, the 3.2-mm pin deformed plastically with the experimental clamp design. Increasing the torque of the clamp bolt resulted in superior plateau coefficients for both clamp designs. At each level of tightness and in each clamp orientation to applied pin load, the experimental clamp design provided greater plateau coefficients than did the Kirschner-Ehmer clamp design. At 7.8 N m of tightness, the Kirschner-Ehmer clamp and bolt bent, whereas only slight plastic deformation of the experimental clamp design occurred. Conclusions —The experimental external fixator clamp was more secure in resisting fixator pin movement at all levels of tightening compared with the Kirschner-Ehmer-type external fixator clamp. At 7.8 N m of tightening, the new clamp design did not allow slippage of the pin within the clamp. Clinical Significance—The experimental external fixator clamp should result in greater rigidity of fixator configurations, in addition to providing design features that allow addition of a clamp between two installed clamps, sleeved predrilling of pilot holes for all pins, measurement of pin depth, and placement of positive profile pins at all sites.  相似文献

Objective—To assess and compare the respiratory depressant and skeletal muscle relaxant effects of two low doses of a nondepolarizing neuromuscular blocker, pancuronium bromide. To determine if a “low dose” of pancuronium bromide can produce selective skeletal muscle relaxation in extraocular muscles sufficient to perform intraocular surgery while sparing or minimizing depression of muscles of ventilation. Study Design—Blinded, randomized crossover, placebo controlled study. Animals—Six healthy, adult mongrel dogs weighing 20.8 ±1.9 kg. Methods—Spontaneously breathing, isoflurane-anesthetized dogs received 0.02 mg/kg pancuronium bromide, intravenously (IV), (high dose [HD]), 0.01 mg/kg pancuronium bromide, IV, (low dose [LD]), or saline placebo IV in a blinded, randomized crossover study. Indices of patient ventilation including tidal volume (Vt), respiratory rate (RR), and minute ventilation (VE) were recorded throughout the study period. Serial arterial blood gas analyses were performed at timed intervals. Neuromuscular blockade of skeletal muscle was assessed at timed intervals with train-of-four stimulus/response ratios. Eye position scores, based on the degree of ocular rotation from a neutral gaze axis, were assigned by an ophthalmologist who was blinded to the treatment given. Results—Vt and VE in HD dogs decreased by 82% from baseline after administration of pancuronium bromide. Similarly, Vt and VE in LD dogs decreased 40% and 55%, respectively. Decreased ventilation in HD dogs corresponded with significant (P< .05) neuromuscular blockade, as indicated by train-of-four ratio less than 75% between 0 and 60 minutes. Eye position scores in HD and LD dogs were suitable for intraocular surgery between 0 and 60 minutes. Eye position scores in five of six control dogs were unsuitable for intraocular surgery at any time period. Conclusions—LD dogs experienced only transient, mild to moderate respiratory depression compared with HD dogs, which experienced prolonged, moderate to severe respiratory depression. Both LD and HD dogs acquired and maintained eye position scores suitable for intraocular surgery between 0 to 60 minutes. A “low dose” of pancuronium bromide, which would provide adequate extraocular muscle relaxation while minimizing ventilatory depression, was not identified. Clinical Relevance—All patients receiving any dose of neuromuscular blocking agents should be closely monitored and receive ventilatory assistance as needed.  相似文献

Objective —To examine the efficacy of psyllium mucilloid in evacuating sand from the equine large intestine.
Animals —12 clinically healthy pony geldings.
Procedure—Twelve ponies were assigned to 2 groups of six each. One group was treated with psyllium and the second was a control group. All ponies had an exploratory celiotomy and 10 g/kg body weight of sand was placed into the cecum. Ponies were fed a grain mixture alone at 1 g/kg (controls), a grain mixture plus psyllium pellets, each at 1 g/kg body weight (3 ponies), or fed a grain mixture and given psyllium powder by nasogastric tube at 1 g/kg body weight divided into two daily doses in 3 L of water (3 ponies). Radiographs were taken on days 1 (3 per group), 5 (all ponies), and 11 (3 per group) to monitor sand transit through the large intestine. Ponies were euthanatized 11 days after surgery. Sand was collected from the contents of the cecum, ventral colon, dorsal colon, and small colon. Dry weight of the recovered sand was compared between the two treatment groups as a percentage of the dry weight of sand placed in the cecum.
Results —No significant differences were detected in the mean percentage of sand recovered between the two treatment groups ( P < .05), with 39.2% recovered in ponies treated with psyllium and 27.4% recovered in control ponies.
Clinical Relevance —Psyllium mucilloid had no apparent effect on sand evacuation from the equine large intestine. When intake of sand is prevented, the equine large intestine can reduce and possibly eliminate its sand burden.  相似文献

ObjectiveTo compare isoflurane alone or in combination with systemic ketamine and lidocaine for general anaesthesia in horses.Study designProspective, randomized, blinded clinical trial.AnimalsForty horses (ASA I-III) undergoing elective surgery.MethodsHorses were assigned to receive isoflurane anaesthesia alone (ISO) or with ketamine and lidocaine (LKI). After receiving romifidine, diazepam, and ketamine, the isoflurane end-tidal concentration was set at 1.3% and subsequently adjusted by the anaesthetist (unaware of treatments) to maintain a light plane of surgical anaesthesia. Animals in the LKI group received lidocaine (1.5 mg kg−1 over 10 minutes, followed by 40 μg kg−1 minute−1) and ketamine (60 μg kg−1 minute−1), both reduced to 65% of the initial dose after 50 minutes, and stopped 15 minutes before the end of anaesthesia. Standard clinical cardiovascular and respiratory parameters were monitored. Recovery quality was scored from one (very good) to five (very poor). Differences between ISO and LKI groups were analysed with a two-sample t-test for parametric data or a Fischer's exact test for proportions (p < 0.05 for significance). Results are mean ± SD.ResultsHeart rate was lower (p = 0.001) for LKI (29 ± 4) than for ISO (34 ± 6). End-tidal concentrations of isoflurane (ISO: 1.57% ± 0.22; LKI: 0.97% ± 0.33), the number of horses requiring thiopental (ISO: 10; LKI: 2) or dobutamine (ISO:8; LKI:3), and dobutamine infusion rates (ISO:0.26 ± 0.09; LKI:0.18 ± 0.06 μg kg−1 minute−1) were significantly lower in LKI compared to the ISO group (p < 0.001). No other significant differences were found, including recovery scores.Conclusions and clinical relevanceThese results support the use of lidocaine and ketamine to improve anaesthetic and cardiovascular stability during isoflurane anaesthesia lasting up to 2 hours in mechanically ventilated horses, with comparable quality of recovery.  相似文献
