The need for alternative methods of teaching veterinary medicine and surgery has increased in recent years because of increasing costs and changing public opinion. For these reasons a hemostasis model was developed that mimics the arteries and veins of the peripheral vascular system, and can be used to teach the basic skills involved in blood vessel ligation and division. This study evaluated the effectiveness of the fluid hemostasis model compared with using live animals for teaching these skills. Forty sophomore veterinary students participated in the study. Two groups of 20 students each received identical instruction in the basic techniques required for vessel ligation and division. The students then completed various exercises using inanimate models to objectively evaluate their psychomotor skills. Both groups then practiced the techniques for equal time periods; one group used the hemostasis model and the other performed a splenectomy on live dogs. After the practice session, the students were videotaped (for later evaluation), as they performed vessel ligations and divisions. The students then repeated the exercises using the inanimate models for evaluation of skills improvement. Questionnaire responses before and after the project were obtained to determine the students' views on the need for inanimate models for teaching purposes. Results of this study indicate that the hemostasis model was as effective as live animals for teaching the basic skills involved in blood vessel ligation. The students' opinions regarding the use of properly designed inanimate models for teaching these skills were dramatically changed.  相似文献

Fibrocartilage degeneration is the earliest pathologic finding in navicular disease but remains difficult to detect, even with magnetic resonance (MR) imaging. We hypothesized that injection of the navicular bursa with saline would improve accuracy of MR imaging evaluation of palmar fibrocartilage. Thoracic limbs were collected from 11 horses within 6 h of death. Imaging was performed with a 1.5 T magnet using sagittal 2D proton density and transverse 3D FLASH sequences with fat saturation. For the purpose of determining sensitivity and specificity of the MR images, fibrocartilage was classified as normal or abnormal, based on combination of the findings of gross and microscopic pathology. Thickness of fibrocartilage was measured on histologic sections and corresponding transverse FLASH MR images before and after injection of saline. A paired Student's t- test was used for comparison of measurements. Partial thickness fibrocartilage loss was present in 6 of 22 limbs. Sensitivity of precontrast MR images for detection of lesions was 100% while specificity was 6%. Saline MR arthrography resulted in both sensitivity and specificity of 100% based on consensus review. Mean histologic fibrocartilage thickness was 0.75±0.12 mm. Mean fibrocartilage thickness on precontrast transverse FLASH images was 0.93±0.065 and 0.73±0.09 mm on postsaline images. The histologic cartilage thickness was signficantly different from that in precontrast images ( P <0.001) but not in images acquired after saline injection ( P =0.716). Based on our results, and using pulse sequences as described herein, navicular fibrocartilage can only be evaluated reliably for the presence of partial thickness lesions after intrabursal injection of saline.  相似文献

Objective— To (1) examine the outcome in horses with osteoarthritis or intra‐articular soft tissue injuries of the stifle after arthroscopic exploration and debridement and (2) to determine any imaging or surgical findings that may influence prognosis. Design— Case series. Animals— Horses (n=44) with lameness referable to the stifle, diagnosed with osteoarthritis, meniscal tears, or other intra‐articular soft tissue injuries based on arthroscopic examination. Methods— Medical records of horses with stifle lameness that had arthroscopic exploration were reviewed. Horses with osteochondrosis lesions, intra‐articular fractures, or osseous cyst‐like lesions were excluded. Pertinent case information was analyzed and short‐ and long‐term outcome was assessed. Results— There was no association between radiographic score and surgery score. Diagnostic ultrasound had a sensitivity of 79% and a specificity of 56% for identifying meniscal injuries. Follow‐up information was available for 35 horses; 23 horses (60%) improved after surgery, 16 (46%) became sound, and 13 (37%) returned to their previous level of function. A negative association was observed between age and degree of preoperative lameness and outcome. More severe changes observed on preoperative radiographs were also negatively associated with prognosis. No horses with grade 3 meniscal tears improved postoperatively and increasing meniscal pathology was negatively associated with return to previous function. A weak association between surgery grade and outcome was also observed. Degree of chondral damage, location of primary pathology, and microfracture techniques had no effect on outcome. Conclusions— Advanced horse age, severe lameness and preoperative radiographic changes, and presence of large meniscal tears are associated with a negative postoperative outcome for horses with stifle lameness. Appearance of the articular surface at surgery appears to be an inconsistent prognostic indicator. Clinical Relevance— Some horses with extensive cartilage damage may return to athletic function after arthroscopic debridement and lavage. A more pessimistic prognosis may be given to older horses, those with more severe preoperative lameness, and those with severe radiographic changes or large meniscal tears.  相似文献

Sablefish (Anoplopoma fimbria) supports substantial fisheries in both the eastern and western Pacific Oceans. Juvenile recruitment along the west coast of the continental United States has been highly variable over the past three decades. Using a generalized additive model, we demonstrate that physical oceanographic variables within the California Current System have significant effects on sablefish recruitment. Significant relationships were found between juvenile recruitment and northward Ekman transport, eastward Ekman transport, and sea level during key times and at key locations within the habitat of this species. The model explains nearly 70% of the variability in sablefish recruitment between the years 1974 and 2000. The predictive power of the model was demonstrated by refitting without the last 5 yr of data and subsequent prediction of those years. Bootstrap assessments of bias associated with parameter estimates and jackknife‐after‐bootstrap assessments of the influence of individual data on parameter estimates are presented and discussed. Using this model, it is possible to draw preliminary conclusions concerning year‐class strength of cohorts not yet available to the survey gear as well as historic year‐class strengths. We discuss changes in zooplankton abundance and shifts in species of copepods associated with fluctuations in the physical variables that appear to have a major influence on sablefish recruitment.  相似文献

Perineal urethrostomy was performed in five male cats with minimal, sharp intrapelvic transection of the ischiocavernous and ischiourethralis muscles and ventral penile ligaments, and in five male cats with extensive blunt intrapelvic dissection and sharp muscle transection. Urethral pressure profiles and cystometrograms with simultaneous fine wire electrode sphincter electromyography were performed in sedated cats before surgery and on days 14 and 28. Premicturition pressure, urethral opening pressure, maximum detrusor pressure, total volume infused, and residual volume were measured during cystometry, and maximal urethral closure pressure was measured during profilometry. A grading of (+), (++), or ( ) was used to represent the intensity of electromyographic activity. There was no difference between minimal and extensive dissection for any cystometrogram or profile variable at days 14 or 28. Urethral opening pressure and maximal detrusor pressure during cystometrography were higher in preoperative studies than after minimal or extensive dissection. Sphincter electromyography after surgery was not significantly different from preoperative results in either surgical group. The results of this study indicate that neither sharp nor blunt intrapelvic dissection significantly alters the postoperative urodynamic status in male cats.  相似文献
