The aim of this project was to determine the effect of patient position on the L5-L6 mid-laminar distance (MLD). The lumbar area of 22 recently euthanatized dogs of various breeds was radiographed in three positions: lateral recumbency with the spine in neutral position, lateral recumbency with the spine flexed in a kyphotic position, and sternal recumbency with the spine flexed in a kyphotic position. Digital images of the radiographs were analyzed using a computer program that allowed measurement of the MLD between L5-L6 in the three positions. The L5 and L6 MLD was significantly larger in sternal recumbency with the spine flexed (142.3 units) than both in lateral recumbency with the spine flexed (138.7 units; P= 0.001) and lateral recumbency with the spine in the neutral position (135.8 units; P < or = 0.001). The MLD in lateral recumbency with the spine flexed was significantly larger than in lateral recumbency with the spine in neutral position (P = 0.005). Positioning a dog in sternal recumbency with the spine flexed produces a significantly larger MLD than in lateral recumbency with the spine flexed; this should simplify needle placement when performing a lumbar puncture.  相似文献

Six cases of mechanical lameness involving the coxofemoral joint are presented. All dogs had a history of chronic rear leg pain and lameness, and decreased range of motion of the hip. On physical examination, ventral subluxation of the coxofemoral joint during extension was present in three dogs. A radiographic diagnosis of subluxation was made in five dogs. In each dog, surgical exploration of the area caudal to the coxofemoral joint revealed a fibrous mass between the lesser or third trochanters and the ischium, diagnosed as a nonneoplastic fibrous reaction by microscopic examination. The clinical, radiographic, and histopathologic signs were similar to previously described cases of myositis ossificans. Five dogs returned to normal function after excision of the soft tissue mass, and one dog was normal when lost to follow-up 4 weeks postoperatively.  相似文献

Eighteen stored split thickness meshed skin grafts were applied to surgically created lesions on the metacarpal and metatarsal regions of six horses. Donor skin was harvested from the sternal region, meshed and stored at 4 degrees C in a cell culture medium containing 10% serum. Stored grafts were applied to the wounds at 1, 2, and 3 week intervals. Acceptance of the grafts stored for 1 week was generally poor (1 of 6 grafts), whereas that of the 2 and 3 week old grafts was generally excellent (10 of 12 grafts). Poor acceptance of the 1 week old grafts was attributed to streptococcal infection of the recipient wounds. Using the storage medium and grafting technique described, excellent acceptance can be expected after graft storage of up to 3 weeks.  相似文献

Dorsal frontal fractures of the first phalanx in nine horses are described. All fractures were in hindlimbs, and two horses were affected bilaterally. Of the eleven fractures, seven were incomplete and nondisplaced, one was complete and nondisplaced, and three were complete and displaced. Two horses with complete displaced fractures had their fractures repaired with lag screw fixation combined with open reduction, and the other seven horses were treated nonsurgically.
All horses were sound on the injured limb within 3 to 6 months of the injury. Lag screw fixation was recommended for horses with complete, displaced dorsal frontal fractures in order to reduce the articular defect and help prevent secondary joint changes. Nonsurgical therapy in the form of stall rest with a heavy support wrap resulted in bony healing of the fracture in 4 to 6 months.
Follow-up was obtained on all nine horses: two were retired for breeding; five performed equal to or better than their previous level of racing performance; one was performing successfully as a combined training horse; and one returned to a lower level of performance due to an unrelated forelimb lameness.  相似文献

Objective —To describe incomplete ossification of the humeral condyle and fragmentation of the medial coronoid process in a Rottweiler.
Study Design —Clinical report.
Animal Population —A 4-year-old sexually intact male Rottweiler.
Methods —Physical examination, radiography, and computed tomography of both elbow joints were performed initially. Drill holes were made across the humeral condyle to promote ossification. Radiography and computed tomography were repeated 14 weeks later. Radiography was repeated 15 months later. A mild, intermittent lameness remained.
Results —Preoperatively a radiolucent line was present across the right humeral condyle. This radiolucent line remained unchanged 14 weeks after drill holes were made across the condyle.
Conclusions —Incomplete ossification of the humeral condyle is present in Rottweilers.
Clinical Relevance —Incomplete ossification of the humeral condyle is present in Rottweilers and may coexist with fragmentation of the medial coronoid process in that breed. The radiographic diagnosis may be difficult because precise positioning is required to see the area of incomplete ossification. Computed tomography may be required to confirm the presence of incomplete ossification of the humeral condyle. Drilling holes across the humeral condyle does not appear to lead to union of the area of incomplete ossification.  相似文献

Objective—A model system was developed to objectively assess the quality of articular cartilage after surgical reconstruction of focal defects in the medial femoral condyle using osteochondral dowel grafts.
Study Design—The surgical technique was developed and customized to reproducibly minimize surgical trauma and graft instability in order to improve the survival of the transplanted cartilage and the long-term integrity of the joint surfaces.
Animals or Sample Population—24 adult female Suffolk-Romanoff crossbred sheep.
Methods—Biomechanical creep testing, paravital staining for chondrocyte viability, histological analysis, and gross morphological analysis were performed at 3, 6, and 12 months postoperatively to compare fresh autografted osteochondral dowels with allografts that had been subject to a freezing protocol known to kill chondrocytes. The latter was used to investigate the time course of cartilage degeneration after injury. These two groups were also compared with normal unoperated control tissue.
Results—Biomechanical behavior, chondrocyte survival, and cartilage histology differed significantly between fresh grafts and those that had been frozen.
Conclusions —Indentation testing and paravital staining were able to identify degenerative changes earlier than other methods of assessment. The technique developed here reproducibly and reliably transplanted osteochondral dowel grafts while minimizing the confounding effects of surgical trauma and graft instability.
Clinical Relevance —The technique provides both a promising surgical technique for the repair of focal defects of the medial femoral condyle and a sensitive model for the future study of cryopreservation strategies for articular cartilage.  相似文献

Ventral Abdominal Approach for Laparoscopic Cryptorchidectomy in Horses   总被引:1,自引:0,他引:1  
Objective —To report a ventral abdominal approach and a ligating loop technique for laparoscopic cryptorchidectomy in horses.
Study Design —Prospective.
Sample Population—Six horses, aged 1 to 5 years, with retained testes.
Methods —One laparoscopic portal and three to four instrument portals were used for ventral abdominal laparoscopic cryptorchidectomy. Laparoscopic instruments were used to maneuver and secure the testis through a ligating loop (modified Roeder knot) that was secured from outside the abdominal cavity. Only minimal enlargement of one instrument portal was used to remove the testicle.
Results —Three horses were bilateral cryptorchids, and three were unilateral (left side, two; right side, one) cryptorchids. Operative time, defined as the time from laparoscope insertion to removal, ranged from 20 to 25 minutes for unilateral cryptorchids and from 40 to 50 minutes for bilateral cryptorchids.
Conclusions —The reported technique allowed decreased tension on the tissues during ligation and removal of the testis from the peritoneal cavity. Improved observation of the abdominal cavity, ligation security, shortened patient confinement time, and minimally invasive technique are all considered to be benefits of laparoscopic cryptorchidectomy.
Clinical Relevance —Direct observation of retained testes and intraabdominal castration are distinct advantages of the use of laparoscopy in horses that have had previous unsuccessful surgical attempts, horses with unknown histories that have retained testicular tissue, or bilateral abdominal cryptorchids.  相似文献

Objective— To compare the efficacy of recombinant human bone morphogenetic protein‐2 (rhBMP‐2)/calcium phosphate (CP) to autogenous cancellous bone graft (CBG) and to no treatment on bone healing, in surgically induced osteotomies and ostectomies of the accessory metatarsal bones in an equine model. Study Design— Experimental. Animals— Adult horses (n=9). Methods— Segmental ostectomies of the second metatarsal bone (MT2) and osteotomies of the fourth metatarsal bone (MT4) were performed bilaterally in 9 horses. There were a total of 35 defects (1 MT4 was previously fractured) created and supplemented randomly either with no treatment (untreated control), rhBMP‐2/CP cement, or matrix (CPC or CPM), or CBG. Radiography was performed every 2 weeks until study endpoint at 12 weeks. After euthanasia, bone healing was evaluated using radiography, mechanical testing, and histology. Data were analyzed with ANOVA followed by the Duncan's Multiple Range Test or nonparametric analyses. Results— At 12 weeks, radiographic scores for union were significantly greater for the rhBMP‐2 (P<.0001) and CBG (P=.004) groups compared with the untreated control group, for both MT2 ostectomies and MT4 osteotomies. The rhBMP‐2 treated MT2 had greater maximum torque to failure in torsion than CBG and control limbs at 12 weeks (P=.011). Histologic analysis demonstrated increased bone formation and more mature bone at the ostectomy site for MT2 in the rhBMP‐2 and CBG groups compared with the untreated control group. Conclusion— Injection of rhBMP‐2/CP into surgically induced ostectomies and osteotomies of the accessory metatarsal bones might accelerate early bone healing in the horse. Clinical Relevance— RhBMP‐2/CP may be as effective if not superior to CBG as an adjuvant treatment to accelerate healing of bone defects.  相似文献

ABSTRACT This paper examines the relationship between U.S. metropolitan county employment growth and poverty. Differential job growth–poverty linkages are found across metropolitan size and type of county. Own‐county employment growth significantly reduces central‐county poverty in large metropolitan areas relative to suburban county poverty. Compared with larger metropolitan areas, broader metropolitan‐wide job growth has more poverty reducing benefits in medium and smaller metropolitan areas, suggesting fewer metropolitan‐wide job‐accessibility constraints. The results suggest that targeted place‐based efforts to spur job growth may help reduce poverty.  相似文献
