Raccoon-variant rabies was confirmed in 7 pet rabbits and 1 pet guinea pig in New York State, and postexposure treatment was required in several adults and children. To prevent rabies virus infection, domestic rabbits and pet rodents should be protected from contact with wild animals, including double-cage housing when housed outside. Pet rabbits or rodents with any possible contact with a wild animal, particularly if the rabbit or rodent had wounds of unknown origin, should be quarantined for 6 months for observation, to prevent escape, and to avoid contact with humans, who will require treatment if the rabbit or rodent develops rabies. Bites and scratches to humans from rodents and lagomorphs should be evaluated for potential rabies exposure on an individual basis, with consideration of whether the animal was caged outside or permitted outdoors unsupervised.  相似文献

This article describes and contrasts the public health response to two human rabies cases: one organ recipient diagnosed within days of symptom onset and the transplant donor who was diagnosed 18 months post‐symptom onset. In response to an organ‐transplant‐related rabies case diagnosed in 2013, organ donor and recipient investigations were conducted by multiple public health agencies. Persons with potential exposure to infectious patient materials were assessed for rabies virus exposure. An exposure investigation was conducted to determine the source of the organ donor's infection. Over 100 persons from more than 20 agencies spent over 2700 h conducting contact investigations in healthcare, military and community settings. The 564 persons assessed include 417 healthcare workers [5.8% recommended for post‐exposure prophylaxis (PEP)], 96 community contacts (15.6% recommended for PEP), 30 autopsy personnel (50% recommended for PEP), and 21 other persons (4.8% recommended for PEP). Donor contacts represented 188 assessed with 20.2% recommended for PEP, compared with 5.6% of 306 recipient contacts recommended for PEP. Human rabies cases result in substantial use of public health and medical resources, especially when diagnosis is delayed. Although rare, clinicians should consider rabies in cases of encephalitis of unexplained aetiology, particularly for cases that may result in organ donation.  相似文献

Rabies     
The practicing veterinarian has a key role to play in rabies control in the maintenance of protection in the companion animal populations, in the education of the pet-owning community on rabies, and in the decision-making process that accompanies human exposure to potentially rabid animals. This role encompasses far more than the routine maintenance of health and well-being in pet animal populations. Although it is important to remain diligent in the immunization of domestic pets against rabies to provide a barrier between wildlife, the main reservoir of infection, and man, it is also important to be aware of the current trends in wildlife rabies within a region. Research is currently underway to investigate methods of immunizing wildlife populations, but the effective and widespread control of rabies in these species is still a long way off; in the meantime, there will be an increasing demand for practicing veterinarians to take the lead in the control of this most fearful zoonotic disease.  相似文献

Psittacosis, also known as parrot fever and ornithosis, is a bacterial infection of humans that can cause severe pneumonia and other serious health problems. It is caused by Chlamydophila psittaci, formerly known as Chlamydia psittaci. From 1988 through 2003, 935 human cases of psittacosis were reported to the CDC and most resulted from exposure to infected pet birds, usually cockatiels, parakeets, parrots, and macaws. In birds, C. psittaci infection is referred to as avian chlamydiosis. Infected birds shed the bacteria through feces and nasal discharges, and humans become infected from exposure to these materials. This compendium provides information about psittacosis and avian chlamydiosis to public health officials, physicians, veterinarians, the pet bird industry, and others concerned with controlling these diseases and protecting public health. The recommendations in this compendium provide standardized procedures for controlling avian chlamydiosis in birds, a vital step to protecting human health. Domestic cats are an important part of many Americans' lives, but effective control of the 60–100 million feral cats living throughout the country remains problematic. Although trap–neuter–vaccinate–return (TNVR) programmes are growing in popularity as alternatives to euthanizing feral cats, their ability to adequately address disease threats and population growth within managed cat colonies is dubious. Rabies transmission via feral cats is a particular concern as demonstrated by the significant proportion of rabies post‐exposure prophylaxis associated with exposures involving cats. Moreover, TNVR has not been shown to reliably reduce feral cat colony populations because of low implementation rates, inconsistent maintenance and immigration of unsterilized cats into colonies. For these reasons, TNVR programmes are not effective methods for reducing public health concerns or for controlling feral cat populations. Instead, responsible pet ownership, universal rabies vaccination of pets and removal of strays remain integral components to control rabies and other diseases.  相似文献

A cross-sectional questionnaire-based survey was carried out in Harare to assess pet ownership and public awareness with regard to pet zoonoses. The questionnaire was designed to obtain information on pet ownership, health and welfare of pets, pet owners’ knowledge and awareness of pet zoonoses with particular emphasis on hookworms and roundworms. The results demonstrated that the proportion of pet owners who knew helminths as zoonoses in dogs (21.3%) and cats (1.1%) was low compared to rabies (95.7%) with ancylostomosis (4.3%) and toxocariosis (2.1%) being the specific parasitic zoonoses known to occur in dogs and toxoplasmosis (2.1%) in cats. More than 50% of the pet owners indicated that veterinarians never discussed the potential hazards of zoonoses or discussed it only when asked and 33% indicated that veterinarians initiated discussion of the subject whenever zoonoses were diagnosed in pets. Over 90% of the pet owners indicated that veterinarians should discuss zoonoses with them. Further investigations are necessary to determine the current prevalence of intestinal nematode infections in dogs and cats in the various regions of the country.  相似文献

Rabid free‐ranging cats have been a public health concern in Pennsylvania since raccoon variant rabies first was recognized in the state in the early 1980s. Over the last decade, between 1.5 and 2.5% of cats submitted to Pennsylvania's state laboratories for rabies testing have been positive. In this report, we describe the extent of rabies in free‐ranging cats in Pennsylvania. We also present two examples of human exposure to rabid free‐ranging cats that occurred in Pennsylvania during 2010–2011 and the public health actions taken to address rabies exposure in the humans and animals. We then describe the concerns surrounding the unvaccinated and free‐ranging cat population in Pennsylvania and possible options in managing this public and animal health problem.  相似文献

MEEREB is an informal network of rabies experts from the Middle East, Eastern Europe and Central Asia, seeking to eliminate rabies from the region. They met for the second time to review the current rabies situation, both globally and in their respective countries, highlighting current rabies control problems and potential solutions. Success stories in Latin America, in Western Europe, in some Asian countries, as well as in Croatia and Serbia prove that elimination of human rabies is achievable in the MEEREB region. It requires political willingness and cooperation of all stakeholders, including Ministries of Health and of Agriculture; adequate management of animal bites through post‐exposure prophylaxis; pre‐exposure prophylaxis for populations at high risk of rabies exposure, animal vaccination and humane control of stray dog populations. MEEREB members called for a regional initiative for rabies elimination in Eastern Europe and the Middle East. They are confident that the elimination of human rabies of canine origin can be achieved in the region through adopting a One Health approach, and that campaigns for rabies elimination will have significant benefit for public health, including strengthening the structure for control of other zoonoses.  相似文献

The emphasis on rabies control and prevention in the United States seems to be a function of our perception of proximity of the threat. Wildlife rabies epizootics within a state may be of little concern to the uninformed urban dweller. Additionally, many parts of the western United States are free of terrestrial rabies; were it not for the presence of bat rabies, people in those areas would likely interpret rabies control as a minor public health concern. It is essential that federal, state, and local public health programs emphasize the importance of rabies control through activities that include rabies education, sponsorship of legislated requirements for domestic animal vaccination, support for local animal control programs, and the promotion of recommendations that encourage the appropriate use of PEP. We are almost guaranteed that rabies is going to remain a major public health issue well into the next century because of expanding wildlife rabies epizootics, identification of new rabies viral variants with increased public health concern, emotional and legal concerns associated with rabies exposure, and increasing national cost associated with rabies control and prevention. Nevertheless, the development of new laboratory technology that allows an understanding of the epidemiologic nature of the rabies virus based on an evolving genetic history and the interrelationship with wildlife reservoirs should allow access to valuable tools for rabies control. When combined with programs using new developments in oral rabies vaccine that can immunize whole populations of wildlife reservoirs, that technology offers encouragement in our effort to control one of the diseases of antiquity.  相似文献

The prevalence of human allergy to pet danders has increased rapidly over the past six decades, as a consequence of lifestyle changes that have enhanced ambient exposure to pet allergens. This is a problem of global public health importance, as the morbidity associated with allergic diseases disproportionately affects socio‐economically disadvantaged populations, particularly children. Although the public often seeks advice from veterinary health‐care professionals regarding healthy pet ownership practices, including strategies for reducing residential pet dander exposure, many misconceptions persist in the public domain regarding pet dander allergy, such as the belief that certain dog and cat breeds are ‘hypoallergenic’ due to their hair/coat type. This review considers the epidemiology of human exposure to the major cat and dog dander allergens, Fel d 1 and Can f 1 respectively, and the sensitization mechanisms to them, including the hygiene hypothesis and the putative role of bacterial endotoxin. The literature regarding primary and secondary exposures in different ambient environments is explored including threshold effects that influence allergen sensitization and elicitation of symptoms, and environmental intervention strategies that seek to reduce allergen exposure. Pet‐specific factors, including the aetiopathogenesis of the several cat and dog allergens that have been characterized to the molecular level, individual animal characteristics that influence Fel d 1 and Can f 1 shedding, and pet‐directed interventions intended to reduce allergen dispersal, are discussed.  相似文献

Campylobacter is regarded as a leading cause of human bacterial gastroenteritis in the United States. We report on a case of laboratory‐confirmed Campylobacter jejuni infection in the Commonwealth of Pennsylvania among members of a household living with a laboratory‐confirmed but non‐speciated Campylobacter‐infected puppy. We describe an outbreak of likely dog‐associated campylobacteriosis, the risk factors, potential routes of exposure and the clinical features in the exposed family members, which began shortly after exposure to the recently purchased dog. We also provide public health recommendations to prevent Campylobacter infections in veterinary care providers, pet owners and those planning to adopt pets in the future. Finally, this report underscores the importance of the One Health approach when public health responders, human and animal healthcare providers and clinical diagnostic laboratories are tasked with developing effective strategies when investigating, detecting and responding to zoonoses (diseases shared between animals and humans).  相似文献

Historically, Australia was considered free of rabies and rabieslike viruses. Thus, the identification of Australian bat lyssavirus (ABLV) in 1996 in a debilitated bat found by a member of the public precipitated both public health consternation and a revision of lyssavirus taxonomy. Subsequent observational studies sought to elaborate the occurrence and frequency of ABLV infection in Australian bats. This paper describes the taxonomic diversity of bat species showing evidence of ABLV infection to better inform public health considerations. Blood and/or brain samples were collected from two cohorts of bats (wild‐caught and diagnostic submissions) from four Australian states or territories between April 1996 and October 2002. Fresh brain impression smears were tested for ABLV antigen using fluorescein‐labelled anti‐rabies monoclonal globulin (CENTOCOR) in a direct fluorescent antibody test; sera were tested for the presence of neutralising antibodies using a rapid fluorescent focus inhibition test. A total of 3,217 samples from 2,633 bats were collected and screened: brain samples from 1,461 wild‐caught bats and 1,086 submitted bats from at least 16 genera and seven families, and blood samples from 656 wild‐caught bats and 14 submitted bats from 14 genera and seven families. Evidence of ABLV infection was found in five of the six families of bats occurring in Australia, and in three of the four Australian states/territories surveyed, supporting the historic presence of the virus in Australia. While the infection prevalence in the wild‐caught cohort is evidently low, the significantly higher infection prevalence in rescued bats in urban settings represents a clear and present public health significance because of the higher risk of human exposure.  相似文献

Rabies is a highly fatal viral infection of the central nervous system affecting all warm-blooded animals including humans. To implement the preventive and control measures, it is important to decide the status of anti-rabies antibodies in dogs. Out of 120 serum samples, 47 (39.2 %) serum samples, showed an antibody titre equal to or above the cut off value of 0.5 IU/ml. The maximum number of dogs showed anti-rabies antibody titres equal to or above the cut-off value of 0.5 IU/ml after <1 month pre-exposure to the rabies vaccine. In 15 serum samples of pet dogs, we observed 13 (86.66 %) dogs with protective anti-rabies antibody titre. Statistical analysis suggests that the age of the animal had no significant effect on anti-rabies antibody titre in vaccinated pet dogs. The overall low seroprevalence of anti-rabies antibody in stray dogs indicates their susceptibility to rabies infection and thus posing a risk of rabies to other animals and humans.  相似文献

In the context of all causes of human morbidity and mortality, or even within the context of all infectious diseases affecting the public health, pet-associated zoonotic infections are of moderate importance. The data documented in Table 1, however, indicate that they do exact significant human health and economic costs. If complete data were available for all of the infections shown in Table 1, the costs would be considerably higher. Moreover, most of these diseases are preventable through educating the public, particularly pet owners, of the zoonotic potential of these diseases, so that they may take precautions to minimize the risks leading to infection. These measures include appropriate health care of pets to eliminate infectious agents, reducing the number of uncontrolled, ownerless pets as well as unwanted or poorly supervised pets, preventing pets from soiling public places with their feces, excluding animals from areas where children play, enforcing leash laws, and promoting responsible pet ownership. Veterinarians, physicians, and public health agencies can aid in these efforts; ultimately, however, the responsibility lies with the pet owner.  相似文献

While most human Salmonella infections result from exposure to contaminated foods, an estimated 11% of all Salmonella infections are attributed to animal exposures, including both direct animal handling and indirect exposures such as cleaning cages and handling contaminated pet food. This report describes the epidemiologic, environmental and laboratory investigations conducted in the United States as part of the response to an international outbreak of tetracycline‐resistant Salmonella enterica serotype I 4,[5],12:i:‐ infections with over 500 illnesses occurring from 2008 to 2010. This investigation found that illness due to the outbreak strain was significantly associated with exposure to pet reptiles and frozen feeder rodents used as food for pet reptiles. Salmonella isolates indistinguishable from the outbreak strain were isolated from a frozen feeder mice‐fed reptile owned by a case patient, as well as from frozen feeder mice and environmental samples collected from a rodent producing facility (Company A). An international voluntary recall of all Company A produced frozen feeder animals sold between May 2009 and July 2010 occurred. Only 13% of cases in our investigation were aware of the association between Salmonella infection and mice or rats. Consumers, the pet industry, healthcare providers and veterinarians need to be aware of the potential health risk posed by feeder rodents, whether live or frozen. Frozen feeder rodent producers, suppliers and distributors should follow the animal food labelling requirements as described in 21 CFR §501.5, and all packages of frozen feeder rodents should include safe handling instructions. Persons should wash their hands thoroughly with soap and water after handling live or frozen feeder rodents, as well as reptiles or anything in the area where the animals live. Continued opportunities exist for public health officials, the pet industry, veterinarians and consumers to work together to prevent salmonellosis associated with pet food, pets and other animals.  相似文献

Animal rabies cases have increased steadily in South Carolina (SC) for the past decade. An understanding of the population's awareness and understanding of the disease is needed to tailor public health interventions. A marketing list-serv of SC residents' email addresses was used to recruit anonymous respondents for a Knowledge Attitudes and Practices (KAP) electronic survey. A total 516 South Carolinians completed the 31-question survey. Quantile regression and a Pearson's correlation evaluated potential associations between respondent's rabies knowledge and their attitudes and practices. Knowledge was assessed on topics of rabies biology, state animal case counts and rabies pet-related laws. Level of concern and level of knowledge were positively correlated. Additionally, statewide hotspot analysis revealed geographic areas warranting targeted public health interventions; counties with low public concern juxtapositioned with high animal rabies case counts. This study demonstrates the utility of statewide KAPs to gauge populations rabies perception and related preventative actions to tailor appropriate educational programs to limit human-animal rabies exposures.  相似文献

In November 1989, the epizootic of rabies affecting raccoons in the mid-Atlantic states reached New Jersey. An economic evaluation was conducted in 2 counties first affected by the epizootic to estimate the costs of the epizootic and to assess the costs and benefits of orally administering a newly developed recombinant rabies vaccine to prevent further spread of the disease. Data on expenditures associated with prevention of rabies in human beings and domestic animals and laboratory testing of suspect animals were collected and analyzed for 1988 (before the epizootic) and 1990 (first full year of the epizootic). Benefit-cost ratios were calculated and used to evaluate the economic advisability of the vaccine at various vaccination program alternatives. Two indices of capital investment analysis, payback period and net present value, were used to evaluate the economic benefits of the rabies vaccine. Expenditures were estimated to be $1,952,014 in 1990 (primarily for pet animal vaccinations), compared with $768,488 in 1988. Benefit-cost ratios ranged from 2.21 for the most expensive vaccination program alternative to 6.80 for the least expensive alternative. The payback period varied from 0.69 to 2.11 years, and the net present value ranged from $2,105,453 to $4,877,452. The high costs of this epizootic necessitated the reallocation of scarce public health resources to various rabies prevention activities, particularly the vaccination of dogs. This study also demonstrated the usefulness of benefit-cost analysis in developing public health strategies. Although the mass application of this recombinant vaccine was found to be economically beneficial, other qualitative considerations must be used to supplement these findings.  相似文献

Veterinary surgeons in Australia must be aware of the emerging viral diseases and their potential effects on public health generally and, more specifically, on the veterinary profession. Australian bat lyssavirus was identified in 1996 and causes rabies-like disease in bats and humans. Two humans from Queensland have died of Australian bat lyssavirus encephalitis. Surveillance has shown that all Australian bats must be considered carriers of this new virus, therefore protective apparel should be used when handling bats. The pre-exposure regimen of inactivated rabies vaccine (Pasteur Mérieux) provides protection against infection. As part of the preventive regimen, at risk groups, such as veterinary surgeons, should seriously consider pre-exposure rabies vaccination. The post-exposure protocol involves administration of human rabies immunoglobulins and five intramuscular injections of the inactivated rabies vaccine.  相似文献

To evaluate sampling biases, pathologic findings in accidentally killed skunks (ie, killed by motor vehicles) were compared with those in random-source skunks (live-trapped and euthanatized, or trap-killed during research) and skunks submitted to a public health laboratory as rabies-suspect. Presence or absence of microscopic lesions in the brain, kidneys, liver, and lungs were used to test the null hypothesis that prevalence of disease did not differ by source of collection. Brain lesions differed with the source; rabid and nonrabid skunks submitted to a public health laboratory had higher prevalences of lesions than did other skunks. Kidney, liver, and lung lesions did not differ among skunks by source of collection. Liver and lung lesions were attributed mainly to parasitism, were not severe, and did not cause debilitated condition. Lesions were seen more often in the kidneys than in other tissues. Usually, lesions were mild to severe, focal, chronic, nonsuppurative, interstitial nephritis (possibly a consequence of leptospirosis). Six of 177 skunks necropsied appeared cachectic. Aleutian disease was diagnosed in one skunk and histoplasmosis was diagnosed in another, but rabies and canine distemper virus infection were the only diseases found with the potential to cause the high population mortality. Public health surveillance cases were biased toward diseased animals (rabies and canine distemper virus infection), but random-source or accidentally killed animals provided unbiased data. Although other factors must be considered, accidentally killed skunks provided cost-effective and useful data for the evaluation of enzootic rabies.  相似文献
