Two domestic shorthair cats, a 6-month-old castrated male and a 7-month-old intact female, were diagnosed with complex congenital heart disease. Transthoracic echocardiography in both cats revealed a dilated arterial trunk overriding the interventricular septum with a large ventricular septal defect. The pulmonary trunk and branch pulmonary arteries were not visible using standard echocardiographic views in either cat. The differential diagnosis for both cats included truncus arteriosus communis vs. pulmonary atresia with ventricular septal defect. Each cat underwent computed tomography angiography to determine the origin and extent of the pulmonary blood supply and to better define extra-cardiac anatomy. Computed tomography angiography led to a diagnosis of truncus arteriosus communis with unrestricted pulmonary blood flow in one cat, whereas the other cat was diagnosed with pulmonary atresia with ventricular septal defect and major aortopulmonary collaterals serving as the primary source of pulmonary blood flow. Computed tomography angiography allowed for the ante mortem differentiation of truncus arteriosus communis from pulmonary atresia with ventricular septal defect in these two cats, leading to an accurate diagnosis and providing valuable information to therapeutic decision-making for each case.  相似文献

Pulmonary atresia and ventricular septal defect (PA-VSD) was diagnosed in a 2-year-old castrated male Terrier mix. Transthoracic echocardiography identified a large ventricular septal defect, overriding aorta and severe right ventricular hypertrophy. A main pulmonary artery could not be identified, consistent with pulmonary atresia or persistent truncus arteriosus. Transesophageal echocardiography and angiography confirmed PA-VSD with aortopulmonary collateral circulation arising from the descending thoracic aorta. This case report describes the antemortem diagnosis of the rare congenital defect PA-VSD in an adult dog.  相似文献

The purpose of this study was to evaluate the feasibility, safety, and efficacy of transcatheter closure in dogs with a congenital perimembranous ventricular septal defect (VSD) by using a detachable coil. No dogs showed any symptoms, and results of chest X-rays and ECGs were normal. The diameters of VSD ranged from 2 to 4 mm on echocardiogram. The defect was 2-2.5 mm from the aortic valve. A detachable coil (size 5 mm with 5 loops) designed for patent ductus arteriosus was delivered via the transarterial route. The device was successfully employed in all dogs. A minimal residual shunt was observed in all dogs even though Qp/Qs decreased. Hemolysis and a rate-dependent right-bundle branch block were observed in 1 dog, but there was no clinical significance. No major complication was noted. Pathologic examination after 1 year revealed that the coils were covered with tissue without significant damage to the His-Purkinje conduction system. In conclusion, transcatheter closure of a small perimembranous VSD with a detachable coil can be achieved without major complications or significant pathologic damage at the lesion site.  相似文献

BACKGROUND: Transcatheter occlusion of patent ductus arteriosus (PDA) is a minimally invasive treatment option in dogs. Device selection based on appropriately acquired PDA dimensions and morphology can minimize procedural complications, including minimizing residual ductal flow and inappropriate embolization. HYPOTHESIS: Transesophageal echocardiographic (TEE) derived minimum ductal diameter measurements would most accurately reflect angiographic measurements. Color Doppler (CD) TEE and transthoracic echocardiographic (TTE) ductal measurements would overestimate two-dimensional (2D) measurements. ANIMALS: Fifteen client-owned dogs with PDA. METHODS: PDA dimensions obtained with angiography, 2D and CD TTE, 2D and CD TEE from the right and left views were prospectively evaluated. RESULTS: PDA dimensions were measured by using TEE most closely approximated angiographic measures. CD TTE and TEE frequently overestimated angiographic measures of minimum ductal diameter. TEE accurately identified a type III (tubular) PDA morphology in 1 dog. Fourteen of 15 dogs were determined to have hemodynamically significant reductions in left to right shunting documented by a first-pass nuclear angiocardiographic ratio of pulmonary to systemic blood flow < or = 1.2 and trivial (n = 3) to no (n = 11) CD TTE flow at approximately 1 month after PDA occlusion. CONCLUSIONS: TEE provided accurate anatomic information regarding PDA morphology and closely approximated angiographic ductal dimensions while aiding in both coil deployment and confirmation of intra-operative ductal closure.  相似文献

Objectives

The purpose of this study was to evaluate a canine patent ductus arteriosus (PDA) model developed for practicing device placement and to determine practices and perceptions regarding transcatheter closure of PDA from the veterinary cardiology community.

Method

A silicone model was developed from images obtained from a dog with a PDA and device placement was performed with catheter equipment and a document camera to simulate fluoroscopy. A total of 36 individuals including 24 diplomates and 12 residents participated, and the feedback was obtained. The study included an initial questionnaire, practice with the model, observation of device placement using the model, and a follow-up questionnaire.

Results

A total of 92% of participants including 100% of residents indicated they did not have the opportunity to practice device placement before performing the procedure and obtained knowledge of the procedure from reading journal articles or observation. Participants indicated selecting the appropriate device size (30/36, 83%) and ensuring the device is appropriately positioned before release (18/36, 50%) as the most common areas of difficulty with device placement. Confidence level was higher after practicing with the model for residents when compared with diplomates and for participants that had performed 1–15 procedures when compared with those that had performed >15 procedures. These findings suggest those that have performed fewer procedures may benefit the most from practicing with a model.

Conclusions

This preliminary study demonstrates the feasibility of a PDA model for practicing device placement and suggests that there is a potential benefit from providing additional training resources.  相似文献

Background: Transarterial ductal occlusion with the Amplatzer vascular plug was first reported in dogs by Hogan et al in 2005. Hypothesis: Use of the Amplatzer vascular plug is a safe, efficacious method of patent ductus arteriosus (PDA) occlusion. Animals: Thirty‐one client‐owned dogs with PDA. Methods: Records of 31 dogs in which transarterial occlusion of PDA with an Amplatzer vascular plug was attempted were reviewed. Results: All dogs had a type II PDA, with 27 dogs having type IIA morphology and 4 dogs having type IIB morphology. Appropriate device deployment was achieved in 29 of 31 dogs. Postdeployment angiography in 21 dogs documented complete occlusion in 10 dogs, trivial residual flow in 5 dogs, mild residual flow in 2 dogs, moderate residual flow in 3 dogs, and severe residual flow in 1 dog. Transthoracic color Doppler echocardiography documented complete occlusion in 22 dogs, whereas 2 dogs had trivial residual flow, 2 dogs had mild residual flow, 2 dogs had mild to moderate residual flow, and 1 dog had severe residual flow. Of the 7 dogs with residual flow, 2 had complete occlusion 2–4 months postoperatively, 1 had moderate residual flow 1 month postoperatively, and 4 were lost to follow‐up. One dog required a larger device than was able to be deployed through the largest sheath placed in the femoral artery. Pulmonary embolization of the device occurred in 1 dog. Conclusion: We conclude that ductal occlusion with an Amplatzer vascular plug is a safe and efficacious therapy for PDA in dogs.  相似文献

Two dogs with severe dysplastic pulmonary valve stenosis and right-to-left shunting defects (patent foramen ovale, perimembranous ventricular septal defect) underwent palliative stenting of the right ventricular outflow tract and pulmonary valve annulus using balloon expandable stents. One dog received 2 over-lapping bare metal stents placed 7 months apart; the other received a single covered stent. Both procedures were considered technically successful with a reduction in the transpulmonary valve pressure gradient from 202 to 90 mmHg in 1 dog and from 168 to 95 mmHg in the other. Clinical signs of exercise intolerance and syncope were temporarily resolved in both dogs. However, progressive right ventricular concentric hypertrophy, recurrent stenosis, and erythrocytosis were observed over the subsequent 6 months leading to poor long-term outcomes. Stenting of the right ventricular outflow tract is feasible in dogs with severe dysplastic pulmonary valve stenosis, though further study and optimization of the procedure is required.  相似文献

Heart rate is a major factor influencing diagnostic image quality in computed tomographic coronary artery angiography (MDCT‐CA), with an ideal heart rate of 60–65 beats/min in humans. The purpose of this prospective study was to compare effects of two different clinically applicable anesthetic protocols on cardiovascular parameters and 64‐MDCT‐CA quality in 10 healthy dogs. Scan protocols and bolus volumes were standardized. Image evaluations were performed in random order by a board‐certified veterinary radiologist who was unaware of anesthetic protocols used. Heart rate during image acquisition did not differ between protocols (P = 1), with 80.6 ± 7.5 bpm for protocol A and 79.2 ± 14.2 bpm for protocol B. Mean blood pressure was significantly higher (P > 0.05) using protocol B (protocol A 62.9 ± 9.1 vs. protocol B 72.4 ± 15.9 mmHg). The R‐R intervals allowing for best depiction of individual coronary artery segments were found in the end diastolic period and varied between the 70% and 95% interval. Diagnostic quality was rated excellent, good, and moderate in the majority of the segments evaluated, with higher scores given for more proximal segments and lower for more distal segments, respectively. Blur was the most commonly observed artifact and mainly affected the distal segments. No significant differences were identified between the two protocols for optimal reconstruction interval, diagnostic quality and measured length individual segments, or proximal diameter of the coronary arteries (P = 1). Findings indicated that, when used with a standardized bolus volume, both of these anesthetic protocols yielded diagnostic quality coronary 64‐MDCT‐CA exams in healthy dogs.  相似文献

Transcatheter coil embolization of patent ductus arteriosus (PDA) was performed in 206 dogs between 1994 and 2003 at Texas A&M University, of which 7 (3%) had embolization of coils to the pulmonary vasculature. Thoracic radiographs indicated that coils were located in the right pulmonary artery in 6 of the 7 dogs. Pulmonary perfusion scans were available for review in 5 dogs, and moderate perfusion defects were observed in the right caudal lung lobe in 4 dogs within 24 hours of embolization. Perfusion deficits observed initially in 2 of the dogs resolved on perfusion scans performed at 6 months and 3.1 years. One dog did not have evidence of focal perfusion defects on a perfusion scan performed 4.5 months after embolization. All pulmonary embolizations occurred during the procedure. Attempts at retrieval of coils were unsuccessful in the 2 dogs in which it was attempted. No short- or long-term clinical complications were observed in any of the dogs with pulmonary embolization. We conclude that pulmonary embolization of vascular occlusion coils is an uncommon event and is not typically associated with adverse clinical effects in dogs with PDA.  相似文献

Transvenous embolization of small patent ductus arteriosus (PDA; < or = 4 mm) with a single detachable coil was attempted in 24 dogs (median age 5.7 months, range, 2.6-65.5 months; median body weight 5.5 kg, range, 1.5-30.0 kg). Angiographic imaging of the duct and pressure measurements were made before and after embolization. The minimal ductal diameter was 2.7 +/- 0.7 mm. In all dogs, a single coil was employed regardless of residual shunting. Ten dogs (PDA minimal diameter range, 1.5-2.2 mm) received a 5-mm coil, and 14 dogs (PDA minimal diameter range, 2.9-3.6 mm) received a 8-mm coil. After coil embolization the angiographic shunt grade decreased significantly (n = 20, P < .001). Residual shunts were assessed by angiography 15 minutes after and by Doppler echocardiography 1-3 days and 3 months after the intervention. In the dogs treated with the 5-mm coils the residual shunt rate was low (0%, 10%, and 0% for angiography and Doppler echocardiography at 1-3 days and 3 months, respectively), in contrast to the dogs treated with the 8-mm coils (91%, 79%, and 67% for angiography and Doppler echocardiography at 1-3 days and 3 months, respectively). After 3 months, no residual murmur was found in dogs treated with the 5-mm coils (0/7), in contrast to murmurs in 5 of 12 (42%) dogs treated with the 8-mm coils. Despite incomplete closure in these dogs, volume loading of the left heart decreased in all dogs. Pulmonic or aortic coil embolism did not occur. Analysis of initial results shows that single detachable coil embolization is possible in all dogs with a small PDA (< or = 4 mm), but only very small PDA (< or = 2.5) could be treated effectively, and for the moderate PDA (2.6-4.0 mm) longer coils or multiple coils may be necessary to achieve complete occlusion.  相似文献

A 12-week-old male Golden Retriever was presented with signs of right-sided congestive heart failure and a grade V/VI left craniosternal systolic murmur. Echocardiography identified a double-chambered right ventricle and dilated coronary sinus (CS) running into an inter-atrial chamber. This was confirmed to be an imperforate cor triatriatum dexter (CTD) by selective angiographic studies. To the authors’ knowledge this is the first case reported of imperforate CTD successfully treated by membranostomy and balloon dilation. Cardiac MRI confirmed the echocardiographic and angiographic findings and provided a more precise understanding of the venous abnormalities.  相似文献

Objectives

To create reference intervals for right ventricular outflow tract fractional shortening (RVOT-FS) in healthy dogs and examine diagnostic performance of this index in dogs with pulmonary hypertension (PH). In addition, we examine the impact of myxomatous mitral valve disease (MMVD) without PH on RVOT-FS.

Animals, materials and methods

The study population included 52 healthy adult dogs, 51 dogs with MMVD but without PH, and 51 dogs with PH. This is a prospective study. Complete echocardiographic evaluations were performed on all dogs. Right ventricular outflow tract fractional shortening was obtained by two-dimensional guided M-mode recordings from the right parasternal short axis view. Right ventricular outflow tract fractional shortening was evaluated in healthy dogs of various breeds, and reference intervals were generated. We examined the effect of PH on RVOT-FS with receiver operating characteristic analysis and evaluated the effect of MMVD on RVOT-FS in dogs without PH. Intraobserver and interobserver reproducibility was calculated.

Results

Healthy dogs had RVOT-FS > 44%. Right ventricular outflow tract fractional shortening values of healthy dogs and MMVD dogs without PH did not differ (p=0.84). In dogs with PH, RVOT-FS decreased with increasing tricuspid regurgitation velocity (p<0.0001). Pimobendan use in dogs with PH increased RVOT-FS as PH worsened. Right ventricular outflow tract fractional shortening was acquired with clinically acceptable intraobserver and interobserver reproducibility.

Conclusions

Right ventricular outflow tract fractional shortening is a novel, easy applicable, and repeatable index for evaluating RV systolic function. Studies comparing this index with common echocardiographic indices used to assess RV function in dogs are needed.  相似文献

ObjectiveTo analytically validate a commercially available high-sensitivity immunoassay for measurement of cardiac troponin I (cTnI) in humans for use in dogs and to evaluate serum cTnI concentrations in healthy dogs and 3 well-defined groups of dogs with common cardiac diseases.AnimalsCanine serum samples were used for validation. 85 client-owned dogs including 24 healthy controls, 20 with myxomatous mitral valve disease, 19 with congenital heart disease, and 22 with arrhythmias.MethodsFour serum samples were used to analytically validate the ADVIA Centaur TnI-Ultra assay by assessing intra-assay variability, inter-assay variability, spiking recovery, and dilutional parallelism. Dogs were grouped based on examination, echocardiography, and additional testing as clinically indicated, and serum cTnI concentrations were compared.ResultsAnalysis of the serum samples used for validation revealed an intra-assay coefficient of variation between 3.6% and 5.7%, and an inter-assay coefficient of variation between 2.4% and 5.9%. Observed to expected ratios for spiking recovery were 97.9 ± 8.6% (mean, SD). Observed to expected ratios for dilutional parallelism were 73.0 ± 11.5% (mean, SD). Dogs with cardiac disease had significantly higher serum cTnI concentrations (P < 0.005) than healthy dogs.ConclusionsThe ADVIA Centaur TnI-Ultra's low limit of detection allows measurement of serum cTnI in the majority of dogs even with no or mild cardiac disease. Dilution of samples for measurement of values above the upper limit of detection is not reliable and therefore not recommended. Serum cTnI concentrations are significantly higher in dogs with cardiac disease compared to healthy dogs.  相似文献

BackgroundSubaortic stenosis (SAS) is a commonly diagnosed canine congenital cardiac defect, with severe forms of carrying a poor long-term prognosis. To date, an effective treatment strategy has not been developed in veterinary medicine. This study sought to determine if sotalol, a class III antiarrhythmic, may have salient echocardiographic and antiarrhythmic benefits for medical management for dogs affected with severe SAS.MethodsTen dogs diagnosed with severe SAS were enrolled in this prospective, double-blinded, crossover study. Dogs underwent physical exam, non-invasive blood pressure measurement, electrocardiography, echocardiography, and 24-h Holter monitoring. Diagnostics were repeated 12–16 days following randomization to oral atenolol (0.5–1 mg/kg) or sotalol (1–2 mg/kg) twice daily. After a medication taper and four-day washout, dogs were crossed-over to the alternate study medication, and the diagnostics were repeated in 12–16 days. Linear and multinomial mixed models were developed to evaluate the effects of treatments on echocardiographic and electrocardiographic variables.ResultsIndices of left ventricular systolic function were reduced based on the volumetric assessment when dogs received sotalol compared to atenolol. No difference was noted between groups in left ventricular systolic function based on the linear assessment. No difference was observed in the reduction in left ventricular outflow tract velocity. No significant differences were observed between treatment groups for any variable on 24-h Holter monitor.ConclusionsSotalol may be a viable therapy to consider for dogs with severe SAS based on this pilot study. A larger, prospective study is necessary to investigate further.  相似文献

This case report describes the evaluation, surgical approach and outcome of an 8-month-old foal with severe angular limb deformities of both metatarsophalangeal joints (MTPJ). Radiography and computed tomography (CT) were used to diagnose a windswept deformity of the hindlimbs with associated bilateral physitis. The orthopaedic surgery was planned and rehearsed using three-dimensional (3D) bone models. A bilateral closing wedge ostectomy was performed on the distal MtIII, and 3.5 mm LCP® medial distal tibia plates were used bilaterally for osteosynthesis. Even though convalescence was prolonged and complicated by partial unilateral implant failure and osteoarthritis (OA), reduction of the deformities was successful and resulted in a sound foal.  相似文献

Complete assessment of vertebral trauma in dogs currently requires CT and MRI for evaluation of the osseous and soft tissue structures that contribute to vertebral stability. Some studies in people have suggested that MRI may be sensitive and specific at detecting vertebral fractures making this potentially a single modality that could be used in spinal trauma evaluation. This study aimed to assess the ability for observers to evaluate vertebral fractures using MRI when compared to CT, which was used as the reference standard. Twenty‐nine dogs with previously diagnosed acute vertebral fractures and four dogs with no vertebral fracture that had undergone sequential CT and MRI were included into the study. One hundred twenty‐eight vertebrae were evaluated for the presence of fractures. Imaging studies were read by two observers blinded to the history. While both observers had similarly high sensitivity and specificity for simple detection of any fractured vertebrae, interobserver agreement was only moderate (κ = 0.584). When evaluations were specifically limited to detection of structurally unstable fractured vertebrae both observers showed improved specificity and interobserver agreement became substantial (κ = 0.650). Complete agreement for exact fracture location between MRI and CT results was only achieved in 14.3‐32.6% of fractured vertebra with up to 79% of fractures being missed in some vertebral structures. This suggests that although MRI may be able to detect the presence of fractured vertebrae, it is not able to replace CT for the complete evaluation of the traumatized spine and documentation of fracture morphology.  相似文献

BACKGROUND: B-type natriuretic peptide (BNP) is increased in dogs with congestive heart failure (CHF). HYPOTHESIS: The purpose of this study was to evaluate the clinical utility of a novel canine-specific enzyme-linked immunosorbent assay of BNP for the diagnosis of CHF in dogs presenting with either cough or dyspnea. ANIMALS: Three hundred and thirty dogs from 2 large university teaching hospitals. METHODS: We prospectively measured plasma BNP concentrations in 3 groups of dogs: (1) normal adult dogs (n = 75), (2) dogs with asymptomatic heart disease (n = 76), and (3) dogs with cough or dyspnea (n = 179). The final diagnosis of dogs with cough or dyspnea and the severity of CHF (International Small Animal Cardiac Health Council Heart Failure Classification [ISACHC]) were determined by medical record review by a study cardiologist who was blinded to the results of the BNP assay. RESULTS: Dogs with CHF had a higher median BNP concentration (24.6 pg/mL) than dogs with noncardiac causes of cough or dyspnea (2.6 pg/mL) (P < .0001). The area under the curve was 0.91 for the receiver operating curve analysis of the diagnostic accuracy of the BNP measurement to differentiate CHF from other causes of cough or dyspnea. The median BNP concentrations in dogs were 3.0 pg/mL with ISACHC I, 17.8 pg/mL with ISACHC II, and 30.5 pg/mL with ISACHC III. (P < .0001) CONCLUSION AND CLINICAL IMPORTANCE: Measurement of BNP is useful in establishing or in excluding the diagnosis of CHF in dogs with cough or dyspnea. B-type natriuretic peptide concentrations rose significantly as a function of severity of CHF.  相似文献
