Background: Otitis media is difficult to diagnose antemortem. Case reports have described computed tomography (CT) in the diagnosis, but not all cases were confirmed. Hypothesis: CT is a sensitive and specific imaging modality of the tympanic bullae and can be used as the gold standard for the diagnosis of otitis media. Animals: Sixteen Holstein calves 5–7 weeks of age were included. Methods: Prospective study. All calves were sedated with IV xylazine (0.05–0.15 mg/kg) for routine radiography (3 views) and CT of the tympanic bullae followed by necropsy. Results: Based upon necropsy findings, 10 of 16 calves were affected with otitis media, 4 unilaterally and 6 bilaterally. Imaging changes associated with otitis media included increased soft tissue opacity within the bulla, thickening of the bulla wall, enlarged bulla, and osteolysis of the bulla wall and trabeculations. The most frequent radiographic changes were lysis of trabeculations and increased soft tissue opacity, which were present in 56.3% of affected bullae. On CT, increased soft tissue opacity within the bulla was present in 93.8% of affected bullae. Sensitivity of radiography and CT was 68.8 and 93.8% and specificity was 50 and 100%, respectively. The κ value between radiography and CT with necropsy diagnosis was 0.19 for radiography, indicating poor agreement, and 0.94 for CT, indicating excellent agreement. Conclusion: CT is more specific, more sensitive, and easier to interpret than radiography and can be used as the gold standard in the diagnosis of otitis media in the calf.  相似文献

Rounded, sessile, hyperattenuating structures detected in computed tomography (CT) studies of canine tympanic bullae have been termed “otoliths.” These have been proposed to represent dystrophic mineralizations or heterotopic bone formations in the middle ear that are potentially related to chronic otitis media. Aims of the current study were to describe the prevalence, macroscopic, and histological features of structures consistent with “otoliths” in the canine tympanic cavity. Tympanic bullae from 50 routinely necropsied dogs and 139 retrospectively retrieved CT scans of canine clinical cases were examined. Small tympanic bone spicules with pointed or clubbed tips essentially arising from the free margin of the septum bullae were bilaterally present in the tympanic cavities of all 50 of the necropsied dogs. In 48% of the dogs, “otolith”‐like CT‐detectable bone spicules carrying drumstick‐like hyperostoses that were 1–6 mm in diameter were also present. In the retrospective survey of bulla CT scans of 139 cases, the prevalence of hyperostotic tympanic bone spicules (HTBS) was 20%. Findings from the current study indicated that the presence of small tympanic bone spicules in adult dogs is most likely due to physiological bone growth in the septum bullae and that HTBS represent osseous proliferations of small tympanic bone spicules. However, the factors inducing formation of hyperostotic spicules from small tympanic bone spicules remain unknown. The high prevalence of HTBS displaying a similar appearance in bulla CT scans in dogs suggests that these spicules should be included in a differential diagnosis list for “otoliths.”  相似文献

Fluid accumulation within the tympanic bulla (TB) is an important diagnostic indicator in clinical cases of canine otitis media although its identification can be a challenge using currently available imaging techniques. The aim of this study was to compare ultrasound with radiography and a single computed tomography (CT) slice for the identification of fluid within the TB of canine cadavers. A random number of TB in 66 cadavers were filled with ultrasound gel. Rostrocaudal open mouth (RCdoM) radiographs and CT images were interpreted by 2 blinded radiologists and ultrasound examinations were performed by 2 blinded sonographers. The heads were then frozen and sectioned to confirm the contents of each TB. Although CT remained the most accurate method, comparable results were obtained by one of the sonographers and even the inexperienced sonographer produced results superior to radiography. Ultrasound has several advantages over the other imaging techniques and this study suggests that it may have an application in the investigation of canine otitis media although further work in live animals would be required to confirm this.  相似文献

The feasibility of virtual otoscopy (VO) imaging was evaluated in five dogs with experimentally induced otitis media, two control dogs, and two canine patients with otitis media. VO images of the tympanic cavity and ossicles were generated with commercially available software using raw computed tomography (CT) data. Eight out of 10 ears inoculated with pathogen exhibited obvious clinical signs associated with otitis externa. CT images revealed soft tissue density material occupying the tympanic bulla compatible with otitis media in three dogs with experimentally induced otitis media and two patients. No remarkable features were observed on the radiographs. Four different VO views (ear canal, tympanic bulla, eustachian tube, and ossicular chain) were created. Promontory, cochlea window, tympanic, and septum bulla as well as ossicles were easily and clearly distinguished except for the incus and stapes of the clinical patients. VO images were not more suitable than images created with conventional CT for accurately diagnosing otitis media in this study. However, it appears that VO could be more feasible for assessing the complex structure of the inner ear in dogs with fluid-filled tympanic cavities since fluid accumulation within the tympanic bulla did not affect the evaluation of bony tissue in the middle ear on VO images.  相似文献

Evaluation of the tympanic bulla (TB) in cases of acute feline otitis media can be a diagnostic challenge, although a feature often associated with this condition is the accumulation of fluid or material within the middle ear cavity. A technique is reported allowing optimum imaging of the feline TB using ultrasound (US) and recording of the appearance of gas and fluid-filled TB. A random number of bullae in 42 feline cadavers were filled with lubricant and rostroventral-caudodorsal oblique radiographs, single slice computed tomography (CT) images and US images were created and interpreted by blinded operators. The content (fluid or gas) of each TB was determined using each technique and the cadavers were then frozen and sectioned for confirmation. CT remained the most accurate diagnostic method, but US produced better results than radiology. Given the advantages of US over other imaging techniques, these results suggest that further work is warranted to determine applications of this modality in the evaluation of clinical cases of feline otitis media.  相似文献

Radiographic evaluation of the tympanic bulla is limited. Improper obliquity or angulation of the skull for radiographs or malpositioning of the tongue can result in an inadequate study [4]. Additionally, because of the complex anatomy of the canine and feline skull, with superimposition of multiple osseous structures, radiography can result in false-negative examinations or understimation of the disease present [2]. When compared with surgical findings of 19 clinical cases of presumptive middle ear disease, false-negative radiographic findings were found in 25% of the surgically confirmed cases of otitis media [4]. Therefore, radiographs are not considered to be a highly sensitive mode of diagnosing otitis media. Radiographs may be helpful in determining lysis associated with neoplasia; however, the extent of involvement is still underestimated. In a study comparing CT and radiographic evaluation of otitis media, CT was determined to be more sensitive but less specific than radiography [11]. CT is the preferred modality for the evaluation of the tympanic bulla at our institution. This opinion may be biased by the fact that most cases are presented for recurrent or chronic otitis. Although radiography is more accessible, the availability of CT in local referral settings is increasing as opposed to being limited to university settings. After the cost (i.e., anesthesia, radiographs) and amount of time necessary to make optimum radiographic studies are taken into account, a CT study may actually be faster and more informative. Given our preference for the CT examination, the cost of the examination has been adjusted to make it more appealing to the owner and clinician. For example, a bulla CT study is only 1.3 times the cost of a radiographic bulla study. In conclusion, imaging of the ear canal can provide important information about ear disease, including unilateral or bilateral ear disease, the degree of middle or inner ear involvement, peripheral versus central vestibular disease, an infectious or inflammatory versus neoplastic process, the chronicity of the disease process, involvement of adjacent structures, and postsurgical complications.  相似文献

The tympanic bullae of 20 previously healthy cadaver animals were randomly filled with sterile saline or air. The heads were positioned using tape ties and a rostrocaudal open mouth radiograph taken. The resulting radiographs were assessed by a blinded ECVDI diplomate, with each tympanic bulla being recorded as being either air or fluid filled. Ultrasound examination of the bullae was performed, by a blinded assessor, using a B-mode 5 mHz sector probe (Sonotron-VingMed Sound A/S, PO Box 141, N3191, Horten, Norway) operating at 3 mHz. Each bulla was recorded as being either air or fluid filled. Results of radiography and ultrasonography were each compared to the actual status of the bulla in a chi square analysis. Sensitivity of radiography was 80%, with specificity of 65%. Ultrasonographic sensitivity was 100% and specificity 100%. Ultrasonography may in future provide a cheap, noninvasive, rapid and widely available method for diagnosing otitis media in clinical cases.  相似文献

A 5-month-old female chinchilla (Chinchilla lanigera) was evaluated for failure to thrive, difficulty drinking, and respiratory signs. Physical examination demonstrated a thin body condition score, purulent nasal and ocular discharge, and an abnormal facial appearance. Endoscope-guided oral examination and computed tomography (CT) scan demonstrated a cleft soft palate, absent nasal turbinates, and bilateral otitis media. The animal was euthanized and necropsy confirmed the diagnosis of palatoschisis (cleft palate) of the soft palate. Other relevant postmortem findings included otitis media, interna, and externa, bony bullae proliferative changes, absent nasal turbinates and rhinosinusitis. Congenital abnormalities are rarely reported in chinchillas. While cleft palates have been reported in many other domestic animals, there are no published reports of this condition in chinchillas to the best of the authors’ knowledge.  相似文献

Background

Otitis media is a common disease in calves that can be subclinical, making antemortem on‐farm diagnosis challenging.

Objectives

To evaluate the sensitivity and specificity of ultrasonography of tympanic bullae for the diagnosis of clinical and subclinical otitis media and to evaluate the reproducibility of the technique.

Animals

Forty calves 19–50 days of age were selected from a veal calf farm.

Methods

Prospective study. Ultrasonography was first performed on the farm by ultrasonographer A (US A). Ultrasonography was repeated by ultrasonographer A (US A'') and another ultrasonographer (US B) at the Centre Hospitalier Universitaire Vétérinaire. Images were later reread by both examiners and a diagnosis was recorded. The calves were euthanized and submitted for necropsy, and histopathologic diagnosis was used as the gold standard.

Results

Forty‐five bullae were affected by otitis media and 35 bullae were normal. Sensitivity and specificity of the ultrasound technique ranged from 32 to 63% and 84 to 100%, respectively, depending on the examiner and classification of suspicious ultrasonography results. Kappa analysis to evaluate interobserver agreement between A'' and B yielded a к value of 0.53. Agreement within the same examiner (A versus A'') yielded a к value of 0.48, and real‐time ultrasound versus rereading of recorded images for A'' and B yielded к values of 0.58 and 0.75, respectively.

Conclusions

Sensitivity and specificity of the ultrasound imaging technique are, respectively, low and high for diagnosis of clinical and subclinical otitis media in calves, with moderate reproducibility.  相似文献

Epidemiological data, clinical signs, complementary examination findings, antimicrobial treatments, and outcome were reviewed in 15 calves diagnosed with otitis media at the Centre hospitalier universitaire vétérinaire de l'Université de Montréal between 1987 and 2002. Age at presentation ranged from 2 to 18 weeks. A purulent ear discharge and epiphora were seen in 8/12 and 6/15 cases, respectively. Neurological signs observed were head tilt (13), eyelid ptosis (7), paresis/paralysis of the pinna (8), ataxia (2), strabismus (2), and convulsions (1). Concurrent pneumonia was frequently diagnosed (n = 11). A Mycoplasma sp. was the principal pathogen isolated from ear discharge; 6 out of 6 samples submitted were positive for mycoplasma. Tympanic bullae radiographs were considered abnormal in 12 out of 13 cases. Cerebrospinal fluid analysis was considered abnormal in 2 out of 5 cases. The antibiotic most commonly used was enrofloxacin (n = 7). Average treatment duration was 19.6 days. Four out of 8 treated animals for which follow-up information was available completely recovered. These results suggest that M. bovis is a major pathogen of otitis media in dairy calves and effective antimicrobial therapy should be of long duration.  相似文献

An ultrasonographic approach for the tympanic bulla in calves is described. Ultrasound evaluation was performed through a lateral approach on 7 fetuses, 2 newborn calves, and 4 living calves of various ages. The probe was positioned caudal to the vertical mandibular ramus, ventral to the base of the ear. Three different positions of the probe were used. The tympanic bulla and adjacent structures were successfully identified and their ultrasonographic appearance described. Fluid within the tympanic bulla was visualized in 12 bullae in the cadavers and in 6 bullae in live calves. Ultrasound imaging has been shown to be useful but its diagnostic value remains to be studied in clinical cases of otitis media in calves.  相似文献

Mineral opacities within the tympanic bullae, termed otoliths, were detected in three dogs by means of radiography and computed tomography. Radiographic signs of otitis externa were present in two dogs. One dog had clinical signs of vestibular disease, whereas the other two dogs had no clinical evidence of ear disease. Medical regimens for the successful treatment of infectious otitis may vary widely depending on the degree of pathologic changes of the external ear canals, the status of the tympanic membranes, and the specific microorganisms involved. Chronic otitis media is especially problematic, because impaction of the bullae can be difficult to resolve and these cases often involve antibiotic-resistant bacterial organisms. In fact, the continuing emergence of resistant Pseudomonas spp and Staphylococcus spp will ensure that the medical therapy of otitis remains a dynamic art for the foreseeable future. This article considers the specific antimicrobial agents available for otic use, their potential toxicities, and guidelines for their use.  相似文献

OBJECTIVE: To evaluate use of computed tomography (CT) of the lungs, compared with conventional radiography, for detection of blebs and bullae associated with spontaneous pneumothorax in dogs. DESIGN: Retrospective case series. ANIMALS: 12 dogs with spontaneous pneumothorax. PROCEDURE: Medical records were reviewed, and information was collected that included signalment, body weight, initial owner complaint, laboratory findings, radiographic findings, CT findings, medical and surgical treatment, histologic findings, complications, duration of hospitalization, and final outcome. RESULTS: Radiographs were excellent for identifying pneumothorax (sensitivity, 100%) but poor for identifying the underlying cause (bullae or blebs); these were identified in radiographs of only 2 of 12 dogs. Computed tomography allowed identification of bullae or blebs in 9 of 12 dogs. Ten of the 12 dogs were treated via surgery, and 17 affected lung lobes were identified. Four of the 17 affected lobes were identified via radiography. Thirteen of the 17 affected lobes were identified via CT; however, 1 lobe was incorrectly identified as the right caudal lobe instead of the right cranial lobe. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggested that CT is better than radiography for identifying the underlying causes of spontaneous pneumothorax.  相似文献

Epidemiological data, clinical findings, laboratory data, medical imaging, and outcomes were reviewed in 29 dairy calves with otitis media/interna. Age at admission ranged from 1 to 24 wk. The majority of calves were referred during winter. Clinical signs included drooping ear, ptosis, head tilt, abnormal nystagmus, strabismus, dysphagia, regurgitation, stiff neck, opisthotonos, facial hyperesthesia, and purulent aural discharge. Intranasal endoscopic examination of 5 animals revealed nasopharyngeal collapse in 4. Cerebrospinal fluid (CSF) was abnormal in all of 7 cases. Mycoplasma bovis was cultured from all but 1 case with external ear or tympanic bullae samples (n = 12), and Mycoplasma arginini was cultured from the remaining ear sample. Radiographs of the tympanic bullae were performed in 24 calves, tomodensitometry (CT) in 3 calves and ultrasound in 4 calves. According to medical imaging techniques or necropsy, 69% of the cases were classified as chronic. Mean duration of treatment was 23.3 d. The rate of clinical recovery was 75%.  相似文献

The medical records of five cats that were diagnosed with otitis media and soft palate abnormalities, three of which had concurrent otitis interna, were reviewed retrospectively. The animals presented with unilateral or bilateral otitis media or otitis interna associated with soft palate hypoplasia (four cases) or unilateral soft palate cleft (one case). Otitis media was confirmed by radiography, CT or MRI. The soft palate abnormalities present were discovered on oropharyngeal examination at induction of anaesthesia. These five cases provide additional support of a link between otitis media and soft palate abnormalities in cats, as reported in humans and dogs.  相似文献

The purpose of this study was to compare computed tomography (CT) and radiography for diagnosing the presence and severity of middle ear disease in dogs with a history of chronic otitis externa. Thirty-one dogs undergoing a total ear canal ablation and bulla osteotomy were studied. Three normal dogs served as controls. All dogs were examined using radiography and CT. Three radiologists independently evaluated imaging studies in random order. A visual analog scale method was used for scoring certainty and severity of middle ear disease. Surgical findings were recorded intra-operatively. Bulla lining samples were submitted for histopathologic evaluation and scored by a single pathologist who also used a visual analog scale system. Findings from both imaging modalities agreed more closely with surgical findings than with histopathologic findings. With either surgical or histopathologic findings as the gold standard, CT was more sensitive than and as specific as radiographs for predicting presence and severity of middle ear disease. Observer performance with CT was more consistent than the performance with radiographs in the detection of changes that occur with middle ear disease. Both radiography and CT were more accurate for predicting the severity of the disease than its presence. Findings indicate that CT is more accurate and reliable than radiography in diagnosing middle ear disease for dogs having concurrent otitis externa, but only when severity of disease is moderate or high. With low severity of disease, diagnostic certainty for both modalities becomes more variable.  相似文献

Clinical signs associated with otic disease in rabbits (Oryctolagus cuniculus) are very similar to those demonstrated in dogs and cats. The surgical treatment options to resolve diseases involving the rabbit ear are also comparable with those used for canine and feline species. However, there are key anatomic and physiologic differences that must be accounted for to perform proper surgical treatment on a rabbit that has been diagnosed with ear disease. This article describes the diagnostic and treatment options for otitis externa, otitis media, and otitis interna, with an emphasis on surgical procedures and the important distinctive otic features of the rabbit patient. The ventral bulla osteotomy procedure is recommended for management of otitis media in rabbits without otitis externa. In rabbits with pathology in the external ear canal and the middle ear, total ear canal ablation and lateral bulla osteotomy should be performed. This combined procedure will allow the surgeon to address pathology in both sites, with the ultimate goal of resolution of the underlying disease etiology. Lateral ear canal resection is only appropriate in rabbits with otitis externa without otitis media; however, this surgical procedure may not resolve the otitis externa. If this is a concern, a total ear canal ablation should be done because it offers a greater chance of success.  相似文献

Inflammatory polyps of the feline middle ear and nasopharynx are non-neoplastic masses that are presumed to originate from the epithelial lining of the tympanic bulla or Eustachian tube. The exact origin and cause are unknown, however, it is thought that inflammatory polyps arise as a result of a prolonged inflammatory process. It is unclear whether this inflammation initiates or potentiates the development and growth of inflammatory polyps. Cats with inflammatory polyps typically present with either signs of otitis externa and otitis media or with signs consistent with upper airway obstruction. Traditional diagnostics involve imaging of the tympanic bulla either with skull radiographs or computed topography (CT). Treatment consists of traction and avulsion of the polyp with or without ventral bulla osteotomy (VBO) to remove the epithelial lining of the tympanic bulla. The three cases described here are unusual manifestations or presentations of feline inflammatory polyps that address the following issues: (1) concurrent otic and nasopharyngeal polyps, (2) potential association with chronic viral infection, (3) polyp development in the contralateral middle ear, (4) CT appearance of the skull following VBO, and (5) development of secondary pulmonary hypertension.  相似文献

A 7-month-old female llama was examined because of chronic otitis media and externa of 7 months' duration. Radiographically, the tympanic bullae appeared thicker than normal, and the ventral borders were poorly defined; the left external acoustic meatus (ear canal) appeared to be narrower than the right. The llama was treated with penicillin, and the ear canals were lavaged daily. Contrast radiography was performed on day 15 to determine the shape and size of the ear canals and evaluate the integrity of the tympanic membranes. Contrast medium was visible radiographically in the left tympanic bulla, indicating that the left tympanic membrane was ruptured, but the right tympanic membrane appeared to be intact. The left ear canal was narrower than the right, and the bony ear canals had a well-defined sigmoid shape. The right ear improved with medial treatment alone, but the left ear did not. Therefore, lateral ear canal resection was performed. After surgery, however, exudate was still evident in the left ear canal, and the llama became more lethargic and more reluctant to eat. Lateral bulla osteotomy was attempted, but no purulent material was obtained, and curettage of the bulla resulted in hemorrhage. Because of this and because of the llama's poor physical condition, a decision was made to euthanatize the llama. The sigmoid shape of the bony ear canal and the multicompartmental nature of the tympanic bulla make surgical treatment of otitis media and externa in llamas difficult. Further study of surgical treatments for otitis media in llamas is needed.  相似文献
