Hip palpation has been used to provide semiquantitative information regarding passive joint laxity and susceptibility to hip dysplasia. The purpose of this study was to: (1) evaluate the intra- and inter-examiner repeatability of the hip reduction angle measured at 4 months of age by three examiners using manual goniometry and an electromagnetic tracking system; (2) compare the hip reduction angle measured with manual goniometry to the hip reduction angle measured with the electromagnetic tracking system; and (3) evaluate the hip reduction angle, distraction index and Ortolani manoeuvre at 4 months of age as predictors of the development of hip osteoarthritis at 12 months of age in 11 Labrador Retriever dogs. Intra- and inter-examiner repeatability was demonstrated for both the manual and electromagnetic goniometric measurement of the hip reduction angle (coefficient of variation < 4.3% and < 6.1%; and P = 0.163 and P = 0.836 respectively). The hip reduction angle measured by manual goniometry was moderately correlated to the hip reduction angle measured by the electromagnetic tracking system (r = 0.603, P < 0.0000). The hip reduction angle measured by manual and electromagnetic goniometry was a poor predictor of osteoarthritis at 12 months of age (r = 0.231, P < 0.062, and r = 0.321, P < 0.01). The distraction index was moderately correlated with the development of osteoarthritis by 12 months of age (r = 0.493, P < 0.0000). The Ortolani sign was sensitive (100%) but not specific (41%) for the development of osteoarthritis at 12 months of age. The hip reduction angle did not further quantify the Ortolani manoeuvre as a predictor of osteoarthritis in Labrador Retrievers.  相似文献

Changes in hip joint congruity was evaluated in dogs with hip dysplasia before and after triple pelvic osteotomy by computed tomography examination in the standing position. Lateral center edge angle significantly increased, and center distance (CD) significantly decreased after surgery compared to the values before surgery, respectively. There was an inverse proportion between the postoperative period and the change in the ratio of CD. These results suggested that joint laxity was improved with time after surgery, providing evidence of the clinical usefulness of this surgery.  相似文献

Total hip replacement in the dog has proved to be a highly successful method of restoring normal, pain-free motion to the coxofemoral joint. In a prospective study done at The Ohio State University College of Veterinary Medicine and the Berwyn Veterinary Associates Hospital, the indications and contraindications, the most effective surgical technique, the complication rate, and the long-term functional evaluation have been established for canine total hip replacements. This study has established that a 95 per cent success rate for function of the hip joint can be achieved if the established guidelines are followed.  相似文献

Cemented THA is an established procedure for treating arthropathies of the hip in large, healthy mature dogs. CHD with secondary osteoarthritis is the most common indication. Although comparative studies between THA and excision arthroplasty have not been reported, our experience with both procedures and studies in the literature support THA as the best available treatment for crippling degenerative joint disease secondary to CHD in large, mature dogs. Negative aspects of the procedure include high cost, the potential of significant complications, and the sophisticated surgical technique required. Improvements in patient selection, design of implants, surgical technique, and postoperative care have decreased the incidence of complications and improved the success rate to over 90%. However, concerns in human applications about cement disease and the desire to increase the functional life of the prosthesis have renewed interest in cementless systems. Advantages of cementless THA are fixation via bony ingrowth and avoidance of problems associated with PMMA. Important technical aspects of cementless THA relate to optimal fit and fill so that the prosthesis is stable, bone ingrowth is promoted, and weight-bearing forces are transferred to the proximal femur physiologically. Problems recognized with cementless systems include fissure fracture, bone resorption, and excessive motion between the implant and bone. Currently, investigations are being conducted to develop prosthetic materials that more closely match the stiffness of bone, stem designs that provide optimal fit and maintain normal strain patterns in the bone, and coating materials that promote permanent fixation by bone ingrowth. Early clinical results in humans and research results in dogs have been encouraging but have not withstood the test of time. Results of cementless techniques must be compared to the standards set by cemented THA over the last 20 years in humans and the last 10 years in dogs.  相似文献

OBJECTIVE: To compare radiographic and arthroscopic abnormalities in juvenile dogs with clinically apparent hip dysplasia. DESIGN: Case series. ANIMALS: 52 dogs (70 hip joints) with clinical signs of hip dysplasia scheduled to undergo triple pelvic osteotomy. PROCEDURE: A ventrodorsal radiographic projection of the pelvis was evaluated by a radiologist unaware of clinical and arthroscopic findings, and radiographic osteoarthritic abnormalities were judged and scored as absent (0), mild (1), moderate (2), or severe (3). Arthroscopy was performed by a surgeon unaware of clinical and radiographic findings, and arthroscopic abnormalities were graded from 0 (normal) to 5 (exposed, eburnated subchondral bone). RESULTS: In 30 of the 70 (43%) hip joints, no radiographic osteoarthritic abnormalities were seen. Severe, full-thickness articular cartilage lesions (grade 4) of the femoral head or acetabulum were seen arthroscopically in 14 (20%) joints. Lesions > or = grade 2 were seen in 60 (86%) joints. Partial tearing of the ligament of the femoral head was present in 57 (81%) joints, and complete rupture was seen in 5 (7%). Radiographic abnormalities were seen in 13 of the 14 (93%; 95% confidence interval, 66% to 99.8%) joints with grade 4 arthroscopic abnormalities but in only 23 of the 46 (50%; 95% confidence interval, 35% to 65%) joints with grade 2 or 3 arthroscopic abnormalities. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that radiography is not a sensitive method for identifying moderate cartilage lesions in juvenile dogs with hip dysplasia. If moderate cartilage lesions are an important prognostic indicator for the success of triple pelvic osteotomy, then methods other than radiography should be used to detect these lesions.  相似文献

OBJECTIVE: To determine in dogs what effect using hip conformation scores assigned by the Orthopedic Foundation for Animals (OFA) as a criterion for breeding selections would have on hip conformation scores of the progeny. DESIGN: Longitudinal study. ANIMALS: English Setters, Portuguese Water Dogs, Chinese Shar-peis, and Bernese Mountain Dogs for which OFA hip conformation scores were known. PROCEDURE: Pedigree data were obtained from the national breed clubs and the American Kennel Club and merged with data from the OFA hip conformation score database. An ANOVA was used to evaluate the effects of sex, age at the time of radiographic evaluation, and year of birth on the variation in hip conformation scores among the progeny. Heritability was estimated by use of within-year midparent offspring regression analyses. RESULTS: Significant differences in progeny hip conformation scores between sexes were not detected, but age at the time of radiographic evaluation and year of birth had a significant effect on hip joint conformation of the progeny. Estimated heritability (mean +/- SE) was 0.26 +/- 0.03, and dam and sire hip conformation scores had a significant effect on progeny hip conformation scores. Annual decreases in percentage of dysplastic progeny and increases in percentages of progeny and breeding dogs with phenotypically normal hip joint conformation were detected. CONCLUSIONS AND CLINICAL RELEVANCE: Results indicated that hip conformation scores have moderate heritability in dogs and selection of breeding stock with better hip conformation scores will increase the percentage of progeny with phenotypically normal hip joint conformation.  相似文献

Denervation of the canine hip joint capsule is described as a surgical therapy method in the treatment of canine hip joint dysplasia and arthrosis. The goal of this operation is a removal of the pain immediately and a reactivation of the dynamic active component of the hip joint in moving the body forward. Simple removal of the periosteum of the craniolateral acetabulum edge destroys the rami articulares of the cranial glutaeal nerve and the rami articulares dorsalis of the sciatic nerve. Within 10 years now we performed the denervation of the canine hip joint capsule. Evaluation of the post operative clinical course in 269 cases revealed an impressive improvement of lameness due to pain relief in almost 92% of the cases.  相似文献
