OBJECTIVE: To study pulmonary gas exchange and cardiovascular responses to sedation achieved with romifidine and butorphanol (RB) alone, or combined with acepromazine, and during subsequent tiletamine-zolazepam anaesthesia in horses. ANIMALS: Six (four males and two females) healthy Standardbred trotters aged 3-12 years; mass 423-520 kg. STUDY DESIGN: Randomized, cross-over, experimental study. MATERIALS AND METHODS: Horses were anaesthetized on two occasions (with a minimum interval of 1 week) with intravenous (IV) tiletamine-zolazepam (Z; 1.4 mg kg(-1)) after pre-anaesthetic medication with IV romifidine (R; 0.1 mg kg(-1)) and butorphanol (B; 25 microg kg(-1) IV). At the first trial, horses were randomly allocated to receive (protocol ARBZ) or not to receive (protocol RBZ) acepromazine (A; 35 microg kg(-1)) intramuscularly (IM) 35 minutes before induction of anaesthesia. Each horse was placed in left lateral recumbency and, after tracheal intubation, allowed to breathe room air spontaneously. Respiratory and haemodynamic variables and ventilation-perfusion (; multiple inert gas elimination technique) ratios were determined in the conscious horse, after sedation and during anaesthesia. One- and two-way repeated-measures anova were used to identify within- and between-technique differences, respectively. RESULTS: During sedation with RB, arterial oxygen tension (PaO(2)) decreased compared to baseline and increased mismatch was evident; there was no O(2) diffusion limitation or increase in intrapulmonary shunt fraction identified. With ARB, PaO(2) and remained unaffected. During anaesthesia, intrapulmonary shunt occurred to the same extent in both protocols, and mismatching increased. This was less in the ARBZ group. Arterial O(2) tension decreased in both protocols, but was lower at 25 and 35 minutes of anaesthesia in RBZ than in ARBZ. During sedation, heart rate (HR) and cardiac output (Qt) were lower while arterial-mixed venous oxygen content differences and haemoglobin concentrations were higher in RBZ compared with ARBZ. Total systemic vascular resistance, mean systemic, and mean pulmonary arterial pressures were higher during anaesthesia with RBZ compared to ARBZ. CONCLUSIONS AND CLINICAL RELEVANCE: Acepromazine added to RB generally improved haemodynamic variables and arterial oxygenation during sedation and anaesthesia. Arterial oxygenation was impaired as a result of increased shunt and mismatch during anaesthesia, although acepromazine treatment reduced disturbances and falls in PaO(2) to some extent. Haemodynamic variables were closer to baseline during sedation and anaesthesia when horses received acepromazine. Acepromazine may confer advantages in healthy normovolaemic horses.  相似文献

OBJECTIVE: Enoximone is a phosphodiesterase III inhibitor frequently used to improve cardiac output (CO) in man. As the use of enoximone has not been reported in horses, the effects of this inodilator were examined in isoflurane anaesthetized ponies. STUDY DESIGN: Prospective, randomised, experimental study. ANIMALS: Six healthy ponies, weighing 286 (212-367) +/- 52 kg, aged 5.0 +/- 1.6 years (4-6.5). METHODS: After sedation with romifidine [80 microg kg(-1) intravenously (IV)], general anaesthesia was induced with midazolam (0.06 mg kg(-1) IV) and ketamine (2.2 mg kg(-1) IV) and maintained with isoflurane in oxygen (Et Iso 1.7%). The ponies were ventilated to maintain eucapnia (PaCO(2) 4.66-6.00 kPa). Each pony was anaesthetized twice with an interval of 3 weeks; receiving enoximone 0.5 mg kg(-1) IV (E) or saline (S) 90 minutes post-induction. Heart rate (HR), arterial (AP) and right atrial pressure (RAP) were measured before treatment, every 5 minutes between T0 (treatment) and T30 and then every 10 minutes until T120. Cardiac output measurements (lithium dilution technique) and blood gas analysis (arterial and central venous samples) were performed before T0 and at T5, T10, T20, T40, T60, T80, T100 and T120. Stroke volume (SV), systemic vascular resistance (SVR), venous admixture (Qs/Qt) and oxygen delivery (DO(2)) were calculated. RESULTS: Enoximone induced significant increases in HR, CO, SV, Qs/Qt and DO(2) and a significant decrease in RAP. No significant differences were detected for AP, SVR and blood gases. No cardiac arrhythmias or other side effects were observed. CONCLUSIONS AND CLINICAL RELEVANCE: The present results suggest that in isoflurane anaesthetized ponies, enoximone has beneficial effects on CO and SV without producing significant changes in blood pressure. Despite an increase in Qs/Qt, DO(2) to the tissues was improved.  相似文献

OBJECTIVE: To compare rectal (C(R) degrees ), coronary band (P(CB) degrees ) and ear base (P(E) degrees ) temperatures with pulmonary arterial temperature (C(PA) degrees ), and to investigate the relationship between core-peripheral temperature difference ([C - P]t degrees ) and cardiac output (Qt) in an acute ovine trauma model (PIL 60/9064). STUDY DESIGN: Observational study. ANIMALS: Thirteen mixed-breed male sheep, body mass (mean +/- SD) 45.50 +/- 4.21 kg, aged approximately 1 year. MATERIALS AND METHODS: Anaesthesia was induced with intravenous etomidate 0.5 mg kg(-1) and midazolam 0.5 mg kg(-1) and following endotracheal intubation maintained with halothane in a 2:1 N(2)O:O(2) mixture. The lungs were mechanically ventilated to maintain normocapnia. Core temperature was measured using a pulmonary arterial thermistor. Rectal temperature and peripheral temperatures at the thoracic limb coronary band and ear base were recorded using thermistor probes with a multi-channel recorder. Cardiac output was measured by continuous thermodilution. Recordings were taken hourly over a 24-hour period. After baseline data collection, hypovolaemia was induced by haemorrhage from bilateral femoral fractures. The development of hypovolaemic shock was confirmed by falling Qt, central venous pressure, mean arterial pressure and urine output. Resuscitation was attempted using intravenous fluids, inotropes and vasoconstrictors. Nine sheep received lumbosacral extradural bupivacaine. RESULTS: The rectal temperature correlated closely with pulmonary arterial temperature in all sheep (r(c) = 0.924). There was no consistent statistically significant relationship between Qt and [C - P]t degrees . CONCLUSIONS AND CLINICAL RELEVANCE: Rectal temperature accurately reflected core temperature in anaesthetized sheep but [C - P]t degrees was unrelated to Qt, under the conditions described in this study.  相似文献

OBJECTIVE: To compare the effects of spontaneous breathing and mechanical ventilation on haemodynamic variables, including muscle and skin perfusion measured with laser Doppler flowmetery, in horses anaesthetized with isoflurane. STUDY DESIGN: Prospective controlled study. ANIMALS: Ten warm-blood trotter horses (five males, five females). Mean mass was 492 kg (range 420-584 kg) and mean age was 5 years (range 4-8 years). MATERIALS AND METHODS: After pre-anaesthetic medication with detomidine (10 microg kg(-1)) anaesthesia was induced with intravenous (IV) guaifenesin and thiopental (4-5 mg kg(-1) IV) and maintained using isoflurane in oxygen. The horses were positioned in dorsal recumbency. In five animals breathing was initially spontaneous (SB) while the lungs of the other five were ventilated mechanically using intermittent positive pressure ventilation (IPPV). Total anaesthesia time was 4 hours with the ventilatory mode changed after 2 hours. During anaesthesia, heart rate (HR) cardiac output (Qt) stroke volume (SV) systemic arterial blood pressures (sAP), and pulmonary arterial pressure (pAP) were recorded. Peripheral perfusion was measured in the semimembranosus and gluteal muscles and on the tail skin using laser Doppler flowmetry. Arterial (a) and mixed venous (v) blood gases, pH, haemoglobin concentration [Hb], haematocrit (Hct), plasma lactate concentration and muscle temperature were measured. Oxygen content, venous admixture (s/Qt) oxygen delivery (DO(2)) and oxygen consumption (VO(2)) were calculated. RESULTS: During mechanical ventilation, HR, sAP, pAP, Qt, SV, Qs/Qt and PaCO(2) were lower and PaO(2) was higher compared with spontaneous breathing. There were no differences between the modes of ventilation in the level of perfusion, DO(2), VO(2), [Hb], (Hct), or plasma lactate concentration. After the change from IPPV to SB, left semimembranosus muscle and skin perfusion improved, while muscle perfusion tended to decrease when SB was changed to IPPV. Low-frequency flow motion was seen twice as frequently during IPPV compared with SB. CONCLUSIONS: Mechanical ventilation impaired cardiovascular function compared with SB in horses during isoflurane anaesthesia. Muscle and skin perfusion changes occurred with ventilation, although further studies are needed to elucidate the underlying mechanisms.  相似文献

OBJECTIVE: To evaluate the suitability of a 'mini parallel Lack' (MPL) breathing system for use in spontaneously breathing cats and to compare the fresh gas flow requirement with that of a modified Ayre's T-piece (MATP). ANIMALS: Twenty client-owned cats, ASA I and II, presented for elective procedures requiring anaesthesia. MATERIALS AND METHODS: Pre-anaesthetic medication and induction of anaesthesia were carried out using several techniques commonly used in our teaching hospital. Anaesthesia was maintained with halothane or isoflurane vaporized in either oxygen or with a mixture of oxygen and nitrous oxide. Both breathing systems were evaluated in each cat, with the order of use randomized. Initial fresh gas flows were 300 mL kg(-1) minute(-1) for the MPL and 500 mL kg(- 1) minute(-1) for the MATP. After a 20-minute stabilization period, fresh gas flow was reduced by 200 mL minute(-1) every 5 minutes until re-breathing--defined as an increase in the inspired partial pressure of carbon dioxide to 0.3 kPa (2 mm Hg)--was detected. The fresh gas flow was then increased in 100 mL minute(-1) increments until re-breathing was no longer detectable, and this value was recorded as the minimum fresh gas flow requirement for the breathing system in use. The procedure was then repeated for the second breathing system. Minimum fresh gas flow requirements were compared using a paired Students t-test. Cardiopulmonary variables were compared using anova. Valve opening pressure was measured in the MPL using a manometer. RESULTS: The mean (+/-SD) fresh gas flow that prevented re-breathing with the MPL (510 +/- 170 mL minute(-1); equivalent to 142 +/- 47 mL kg(-1) minute(-1)) was significantly lower than that required for the MATP (1430 +/- 560 mL minute(-1); equivalent to 397 +/-155 mL kg(-1) minute(-1)). There were no significant differences in cardiopulmonary variables attributable to the use of the two breathing systems. The MPL valve opening pressure was 1.1 cm H2O. CONCLUSIONS: The MPL breathing system used lower gas flows than the MATP without affecting cardiovascular or respiratory function. Clinical relevance In spontaneously breathing cats, the MPL offers the advantages of a reduction in cost and atmospheric pollution because less volatile agent is vaporized.  相似文献

OBJECTIVE: To evaluate the use of a lithium dilution cardiac output (LiDCO) technique for measurement of CO and determine the agreement between LiDCO and thermodilution CO (TDCO) values in anesthetized cats. ANIMALS: 6 mature cats. PROCEDURE: Cardiac output in isoflurane-anesthetized cats was measured via each technique. To induce different rates of CO in each cat, anesthesia was maintained at > 1.5X end-tidal minimum alveolar concentration (MAC) of isoflurane and at 1.3X end-tidal isoflurane MAC with or without administration of dobutamine (1 to 3 microg/kg/min, i.v.). At least 2 comparisons between LiDCO and TDCO values were made at each CO rate. The TDCO indicator was 1.5 mL of 5% dextrose at room temperature; with the LiDCO technique, each cat received 0.005 mmol of lithium/kg (concentration, 0.015 mmol/mL). Serum lithium concentrations were measured prior to the first and following the last CO determination. RESULTS: 35 of 47 recorded comparisons were analyzed; via linear regression analysis (LiDCO vs TDCO values), the coefficient of determination was 0.91. The mean bias (TDCO-LiDCO) was -4 mL/kg/min (limits of agreement, -35.8 to + 27.2 mL/kg/min). The concordance coefficient was 0.94. After the last CO determination, serum lithium concentration was < 0.1 mmol/L in each cat. CONCLUSIONS AND CLINICAL RELEVANCE: Results indicated a strong relationship and good agreement between LiDCO and TDCO values; the LiDCO method appears to be a practical, relatively noninvasive method for measurement of CO in anesthetized cats.  相似文献

OBJECTIVE: To evaluate the effects of meloxicam on renal function in dogs anaesthetized and rendered hypotensive with acepromazine-thiopental-isoflurane. ANIMALS: Eight healthy beagles, four males and four females, 25.6 +/- 19.3 months old and weighing 12.8 +/- 2.0 kg. MATERIALS AND METHODS: Either meloxicam suspension at a dose of 0.133 mL kg(-1) (0.2 mg kg(-1)) or 0.133 mL kg(-1) saline solution (control), were given by mouth (PO) in a randomized, cross-over fashion. The treatment or control was given 3 hours before anaesthesia. Dogs were sedated with intramuscular acepromazine 0.1 mg kg(-1). Anaesthesia was induced with intravenous thiopental, followed by tracheal intubation and maintenance with isoflurane in oxygen and air, delivered using a semi-closed breathing system. Renal function was quantified using serum biochemistry, urinalysis and glomerular filtration rate measured by scintigraphy. Analysis of variance or Friedman anova were used for statistical analysis. RESULTS: Values (mean +/- SD) for mean arterial blood pressure did not differ significantly between treatments but was low (54 +/- 7 mmHg) during anaesthesia. Glomerular filtration rate did not differ significantly between treatments or over time, and results of urine and serum analysis were within reference ranges after meloxicam treatment. CONCLUSIONS AND CLINICAL RELEVANCE: Meloxicam caused no adverse effects on renal function when given to healthy dogs anaesthetized and rendered hypotensive with acepromazine, thiopental and isoflurane.  相似文献

OBJECTIVE: To record the electroencephalographic changes during castration in ponies anaesthetized with halothane and given intravenous (IV) lidocaine by infusion. The hypothesis tested was that in ponies, IV lidocaine is antinociceptive and would therefore obtund EEG changes during castration. ANIMALS: Ten Welsh mountain ponies referred to the Department of Clinical Veterinary Medicine, Cambridge for castration under general anaesthesia. MATERIALS AND METHODS: Following pre-anaesthetic medication with intramuscular acepromazine (0.02 mg kg(-1)) anaesthesia was induced with IV guaiphenesin (60 mg kg(-1)) and thiopental (9 mg kg(-1)) and maintained with halothane at an end-tidal concentration (FE'HAL) of 1.2%. A constant rate infusion of IV lidocaine (100 microg kg(-1) minute(-1)) was administered throughout anaesthesia. The electroencephalogram (EEG) was recorded continuously using subcutaneous needle electrodes. All animals were castrated using a closed technique. The raw EEG signal was analysed after completion of each investigation, and the mean values of EEG variables (median frequency, spectral edge frequency, total amplitude) recorded during a baseline period (before surgery began) and the removal of each testicle were compared using anova for repeated measures. RESULTS: Spectral edge frequency (SEF) 95% decreased during removal of the second testicle compared with baseline recordings. No other significant EEG changes during castration were measured. CONCLUSIONS: Lidocaine obtunded the EEG changes identified during castration in a previous control study, providing indirect evidence that lidocaine administered peri-operatively was antinociceptive and contributed to anaesthesia during castration. CLINICAL RELEVANCE: The antinociceptive effect of lidocaine combined with its minimal cardiovascular effects indicate a potential use for systemic lidocaine in clinical anaesthetic techniques.  相似文献

OBJECTIVE: To determine the cardiopulmonary effects of desflurane (DES) in horses. ANIMALS: Six healthy adult horses, three males and three females, aged 9 +/- 4 (mean +/- SD) years and weighing 370 +/- 36 kg. MATERIALS AND METHODS: Anaesthesia was induced with an O2 (10 L minute(-1)) and DES mixture (vaporizer setting 18%). After oro-tracheal intubation, horses were positioned in right lateral recumbency. Anaesthesia was maintained with DES in O2 (20 mL kg(-1) minute(-1)) delivered through a large animal circle breathing system. The minimum alveolar concentration of DES (MAC(DES)) that prevented purposeful movement in response to 60 seconds of electrical stimulation of the oral mucous membranes was determined for each horse. The delivered concentration of DES was then increased to achieve end-tidal concentrations corresponding to 1.5 x MAC(DES), 1.75 x MAC(DES), and 2.0 x MAC(DES). Heart rate (HR), mean arterial blood pressure (MAP), respiratory rate (fr), tidal volume (VT), minute volume (VM) and core temperature were determined, and blood samples for arterial blood gas analysis taken at each DES concentration. All data were analysed by two-way anova for repeated measures and Fisher's test for multiple comparisons. A probability level of p < 0.05 was applied. RESULTS: Desflurane concentrations of 2.0 x MAC(DES) increased HR whereas lower concentrations did not. Mean arterial pressure was not affected by 1.0 x MAC(DES) 1.5 x MAC(DES) or 1.75 x MAC(DES), whereas it decreased at 2.0 x MAC(DES). All concentrations of DES examined significantly depressed fr, VT and VM. CONCLUSIONS AND CLINICAL RELEVANCE: Desflurane concentrations between 1.0 and 1.75 x MAC(DES) reduces fr and VM but does not affect HR or MAP in horses.  相似文献

OBJECTIVE: To evaluate the effects of three anaesthetic combinations in adult European badgers (Meles meles). STUDY DESIGN: Prospective, randomized, blinded, experimental trial. ANIMALS: Sixteen captive adult badgers. METHODS: The badgers were each anaesthetized by intramuscular injection using the three techniques assigned in random order: romifidine 0.18 mg kg(-1), ketamine 10 mg kg(-1) and butorphanol 0.1 mg kg(-1) (RKB); medetomidine 0.1 mg kg(-1), ketamine 9 mg kg(-1) and butorphanol 0.1 mg kg(-1) (MKB); and medetomidine 0.1 mg kg(-1) and ketamine 10 mg kg(-1) (MK). Initial drug doses were calculated based on a body mass of 10 kg. Additional anaesthetic requirements, time to drug effect, duration of action and recovery from anaesthesia were recorded. Heart rate and rhythm, respiratory rate and rhythm, rectal and subcutaneous microchip temperature and oxygen saturation were recorded every 5 minutes. Depth of anaesthesia was assessed using: muscle tone; palpebral and pedal reflexes; and tongue relaxation at these time points. Blood samples and a tracheal aspirate were obtained under anaesthesia. Atipamezole was administered if the badger had not recovered within 60 minutes Parametric data were analysed using anova for repeated measures, and nonparametric data using Friedman's, and Cochran's Q tests: p < 0.05 was considered significant. RESULTS: All combinations produced good or excellent muscle relaxation throughout the anaesthetic period. RKB had the shortest duration of anaesthesia (16.8 minutes compared with MKB 25.9 minutes and MK 25.5 minutes) and antagonism was not required. RKB depressed respiratory rate less than MK and MKB. There was no significant difference between techniques for heart rate and rhythm. CONCLUSIONS AND CLINICAL RELEVANCE: All combinations provided anaesthetic conditions suitable for sampling and identification procedures in adult badgers. The RKB protocol provided a significantly shorter period of anaesthesia when compared with the combinations containing medetomidine.  相似文献

Studies evaluating the effects of dobutamine in horses do not consistently report increases in cardiac output despite increases in arterial blood pressure. The concurrent administration of the α2 agonist clonidine, in people, inhibited the chronotropic effects of dobutamine and increased left ventricular stroke work ( Zimpfer et al. 1982 ). Our study was performed to determine if pre‐medication with an α2 agonist affects the response to dobutamine in anaesthetized horses. Eleven horses were anaesthetized on four separate occasions for one of four randomly assigned treatments; (I) no xylazine, no dobutamine (II) xylazine, no dobutamine (III) no xylazine, dobutamine, and (IV) xylazine, dobutamine. Horses received 0.02 mg kg?1 of butorphanol IV 10 minutes prior to anesthetic induction. Two minutes prior to induction, groups II and IV received 0.5 mg kg?1 of IV xylazine. Anaesthesia was induced with 6–7 mg kg?1 of thiopental and maintained with halothane. End‐tidal halothane concentrations were maintained between 1.1 and 1.2% in groups I and III, and 0.9–1.0% for groups II and IV. Heart rate, cardiac output, right atrial pressure, and systolic (SAP), diastolic (DAP) and mean (MAP) arterial pressure were recorded 30 minutes after beginning halothane anaesthesia (T10). Cardiac output was estimated using Lithium dilution ( Linton et al. 2000 ). Baseline measurements were repeated twice, at 5‐minute intervals (T5 and T0). At time 0 (T0), an IV infusion of either saline (100 mL hour?1) or dobutamine (0.001 mg kg?1 minute?1) was started and data recorded at 5‐minute intervals for 30 minutes (T5 – T30). Stroke volume and systemic vascular resistance (SVR) were calculated. Data were analysed using repeated measures anova (p < 0.01 significant) and Newman–Keuls for multiple comparisons. Cardiac output and stroke volume increased over time in groups III and IV. Cardiac index was higher in groups III and IV than in groups I and II from T10 until completion of the study. Estimates of cardiac index at T30 for groups I–IV were 45 ± 9, 46 ± 11, 71 ± 11, and 78 ± 19 mL kg?1 minute?1, respectively (mean ± SD). Stroke index was higher in groups III and IV than in groups I and II from T15 to T30. Values for stroke index at T30 for groups I–IV were 0.98 ± 0.19, 1.11 ± 0.18, 1.46 ± 0.21, 1.74 ± 0.33 mL kg?1. Heart rate decreased from T10–T30 in groups I and II. Heart rate was greater in groups I and III than in groups II and IV at T5 and T0. Values for heart rate at T0 for groups I–IV were 48 ± 5, 42 ± 5, 50 ± 4, 43 ± 4 beats minute?1. Systolic arterial pressure, DAP and MAP were higher in groups III and IV than in groups I and II from T5 to T30. There were no differences in SVR between groups. Dobutamine at 0.001 mg kg?1 minute?1 increased cardiac output, blood pressure, and stroke volume. Premedication with xylazine at 0.5 mg kg?1 did not appear to affect the response to dobutamine.  相似文献

ObjectiveTo assess oxygenation, ventilation‐perfusion (V/Q) matching and plasma endothelin (ET‐1) concentrations in healthy horses recovering from isoflurane anaesthesia administered with or without pulse‐delivered inhaled nitric oxide (iNO).Study DesignProspective experimental trial.AnimalsHealthy adult Standardbred horses.MethodsHorses were anaesthetized with isoflurane in oxygen and placed in lateral recumbency. Six control (C group) horses were anaesthetized without iNO delivery and six horses received pulse‐delivered iNO (NO group). After 2.5 hours of anaesthesia isoflurane and iNO were abruptly discontinued, inhaled oxygen was reduced from 100% to approximately 30%, and the horses were moved to the recovery stall. At intervals during a 30‐minute period following the discontinuation of anaesthesia, arterial and mixed venous blood gas values, shunt fraction (Qs/Qt), plasma ET‐1 concentration, pulse rate and respiratory rate were measured or calculated. Repeated measures anova and a Bonferroni post hoc test was used to analyze data with significance set at p <0.05.ResultsAt all time points in the recovery period, NO horses maintained better arterial oxygenation (oxygen partial pressure: NO 13.2 ± 2.7–11.1 ± 2.7 versus C 6.7 ± 1.1–7.1 ± 1.1 kPa) and better V/Q matching (Qs/Qt NO 0.23 ± 0.05–0.14 ± 0.06 versus C 0.48 ± 0.03–0.32 ± 0.08%) than C horses. Mixed venous oxygenation was higher in NO for 25 minutes following the discontinuation of anaesthesia (NO 6.3 ± 0.2–4.5 ± 0.07 versus C 4.7 ± 0.6–3.7 ± 0.3 kPa). In both groups of horses arterial oxygenation remained fairly stable; venous oxygenation declined over this time period in the NO group but still remained higher than venous oxygen in the C group. ET‐1 concentrations were higher at most time points in C than NO. Changes in other parameters were either minor or absent.Conclusions and Clinical RelevanceDelivery of iNO to healthy horses during anaesthesia results in better arterial and venous oxygenation and V/Q matching (as determined by lower Qs/Qt) and lower ET‐1 concentrations throughout a 30‐minute anaesthetic recovery period.  相似文献

OBJECTIVE: To investigate alterations in peri-operative body temperatures and oesophageal-skin temperatures in isoflurane-anaesthetized rabbits following either ketamine-midazolam or ketamine-medetomidine induction of anaesthesia. ANIMAL POPULATION: Fifty client-owned rabbits, (25 male, 25 female) of different breeds anaesthetized for elective neutering (age range: 3-42 months; mass range: 1.15-4.3 kg). STUDY DESIGN: Randomized, blinded clinical study. METHODS: Pre-anaesthetic rectal temperature was measured. A 24 SWG catheter was placed in a marginal ear vein after local anaesthesia. Ketamine (15 mg kg(-1)) with medetomidine (0.25 mg kg(-1)) (group KMT) or with midazolam (3 mg kg(-1)) (group KMZ) was injected intramuscularly (IM). Following endotracheal intubation anaesthesia was maintained with isoflurane in oxygen. Carprofen (3 mg kg(-1)) and glucose saline (5 mL kg(-1) hour(-1)) were administered through the intravenous catheter. Room temperature and humidity, skin temperature (from tip of pinna) and oesophageal temperature were measured during anaesthesia. Ovariohysterectomy or castration was performed. Rectal temperature was taken when isoflurane was discontinued (time zero) and 30, 60 and 120 minutes thereafter. Atipamezole (0.5 mg kg(-1)) was administered IM to rabbits in group KMT at zero plus 30 minutes. Mass, averaged room temperature and duration of anaesthesia data were compared using a two-tailed t-test. Age, averaged room humidity, rectal temperature decrease, oesophageal temperature decrease and oesophageal-skin difference data were compared using a Kruskal-Wallis test. p < 0.05 was considered significant. RESULTS: The averaged oesophageal-skin temperature difference was significantly greater in group KMT [median 9.85 degrees C (range 6.42-13.85 degrees C)] than in group KMZ [4.38 degrees C (2.83-10.43 degrees C)]. Rectal temperature decreased over the anaesthetic period was not significantly different between the two groups; however, oesophageal temperature decrease was significantly less in group KMT [1.1 degrees C (-0.1-+2.7 degrees C)] than in group KMZ [1.4 degrees C (0.6-3.1 degrees C)]. CONCLUSIONS: Oesophageal-skin temperature difference is larger in rabbits anaesthetized with ketamine-medetomidine combination than ketamine-midazolam. CLINICAL RELEVANCE: The oesophageal temperature in rabbits anaesthetized with ketamine-medetomidine and isoflurane decreases significantly less than in animals anaesthetized with ketamine-midazolam and isoflurane, during anaesthesia.  相似文献

An 18-month-old Lurcher was anaesthetized for surgical ligation of a patent ductus arteriosus using a target-controlled infusion (TCI) of propofol and a variable rate infusion of remifentanil. Before anaesthesia, radiographic and echocardiographic examination indicated that the dog had left-sided congestive heart failure and impaired left ventricular systolic function. Ramipril and furosemide were administered pre-operatively. Following pre-anaesthetic medication with morphine, 0.5 mg kg(-1), by intramuscular injection, and pre-oxygenation, remifentanil was infused for 5 minutes at 0.2 microg kg(-1) minute(-1), followed by induction of anaesthesia using intravenous propofol administered by TCI, set at a target concentration of 3.5 microg mL(-1) of propofol in blood. Tracheal intubation was performed and 100% oxygen delivered through a non-rebreathing (Bain) system and then a circle system in the operating theatre. Anaesthesia was maintained with propofol and remifentanil, adjusted according to clinical requirements. Peri-operative analgesia consisted of intercostal bupivacaine nerve block, with meloxicam, morphine and remifentanil.  相似文献

OBJECTIVE: To evaluate the effectiveness of four ketamine-based anaesthetics in badgers using a quantitative anaesthesia assessment technique. STUDY DESIGN: Prospective randomized 'blinded' experimental trial. METHODS: The quality of induction, of anaesthesia (at 5-minute intervals) and of recovery were assessed in 93 badgers, given either one of three ketamine (K)-medetomidine (M)-butorphanol (B) combinations: group A - M K B at 20/40/80 microg kg(-1); group B - M K B at 20/40/60 microg kg(-1); and group C - M K B at 20/60/40 microg kg(-1), or ketamine (K) alone at 2 mg kg(-1) (group D). The assessor was ignorant of the combination administered. Physiological variables (heart and respiratory rates and rectal temperature) were measured at 5-minute intervals during anaesthesia. Gingival mucus membrane colour was also recorded. RESULTS: Induction to anaesthesia was most rapid with ketamine (2 mg kg(-1)) although induction quality did not differ between techniques. Ketamine used alone gave the poorest score for anaesthesia quality. Heart rate (HR) and scores for gingival mucus membrane colour were higher in animals anaesthetized with ketamine alone. Rectal temperature did not differ significantly between the techniques at any time during anaesthesia. Ketamine used alone produced the poorest quality of recovery. CONCLUSION AND CLINICAL RELEVANCE: The M-K-B combinations investigated overcame several side effects associated with ketamine anaesthesia, but at the expense of more variable induction times, lower HRs, and poorer mucus membrane coloration.  相似文献

OBJECTIVE: To compare minimum fresh gas flow (V(min)) requirements and respiratory resistance in the Uniflow and Bain anaesthetic breathing systems used in the Mapleson D mode. Animals Seven pigs, aged 8-12 weeks, anaesthetized for ophthalmic surgery. MATERIALS AND METHODS: Anaesthesia was maintained with halothane delivered in oxygen using a (Mapleson D) Bain breathing system. The V(min) that prevented re-breathing was found, and peak inspiratory (PIP) and peak expiratory (PEP) pressures measured. The fresh gas flow (V(f)) was then increased to V(min) + 50%, then V(min) + 100%, and respiratory pressures re-measured. A heat and moisture exchanger (HME) was inserted at the endotracheal tube and the procedure repeated. The breathing system was then exchanged for a Uniflow and the protocol repeated. After final disconnection from the breathing system, the animals' peak inspiratory and expiratory flows, tidal, and minute volumes (Vm) were measured over five respiratory cycles. RESULTS: The V(min) (L minute(-1); mL kg(-1) minute(-1)) required to prevent rebreathing in the Uniflow system [8.1(mean) +/-1.7 (SD); 332 +/- 94] was significantly greater than the Bain system (6.5 +/- 1.1; 256 +/- 64). At V(min), PEP with the Uniflow (3.5 +/- 0.1 cm H(2)O) was significantly higher than the Bain system (2 +/- 0.7 cm H(2)O), but PIP values did not differ (Uniflow -0.6 +/- 2.1 cm H(2)O; Bain system -0.2 +/- 0.6 cm H(2)O). With both systems, PEP increased significantly (p < 0.001) with each increase in V(f): Uniflow system 4.2 +/- 0.4 (V(min) + 50%) and 5.5 +/- 0.5 cm H(2)O (V(min) + 100%); Bain system 2.8 +/- 0.7 (V(min) + 50%) and 3.5 +/- 0.7 cm H(2)O (V(min) + 100%). Insertion of the HME did not alter pressures. The mean tidal volume was 6.4 +/- 1.6 mL kg(-1); mean Vm was 184.9 +/- 69.8 mL kg(-1) and mean respiratory rate was 28 +/- 5 breaths minute(-1). In one pig breathing with the Uniflow system PEP rose sharply; respiratory and heart rates increased, and ventricular dysrhythmias occurred. When the system was changed and V(f) reduced, physiological variables became normal. CONCLUSION: The study discredited the hypothesis that the two breathing systems behave similarly. Values for V(min) and PEP were higher with the Uniflow system. Increasing V(f) increased PEP with both systems. Insertion of an HME did not affect respiratory pressures. CLINICAL RELEVANCE: The Uniflow used in Mapleson D mode is not suitable for anaesthesia in young spontaneously breathing pigs.  相似文献

OBJECTIVE: To determine the pharmacokinetic parameters of alfaxalone in dogs after the intravenous (IV) administration of clinical and supra-clinical doses of a 2-hydroxypropyl-beta-cyclodextrin (HPCD) alfaxalone formulation (Alfaxan-CD RTU). EXPERIMENTAL DESIGN: Prospective two-period crossover design. Animals Eight (four male and four female) young adult healthy Beagle dogs. Methods The steroid anaesthetic alfaxalone was administered IV at two doses in a crossover design (2 and 10 mg kg(-1)) with a washout period of 21 days. Blood samples were collected before and up to 8 hours after dosing. Plasma concentrations of alfaxalone were assayed using a liquid chromatograph/mass selective detector technique and analyzed to estimate the main pharmacokinetic parameters by noncompartmental analysis. Results were expressed as mean +/- SD. RESULTS: The mean duration of anaesthesia from endotracheal intubation to extubation was 6.4 +/- 2.9 and 26.2 +/- 7.5 minutes, for the 2 and 10 mg kg(-1) doses, respectively. The plasma clearance of alfaxalone for the 2 and 10 mg kg(-1) doses differed statistically at 59.4 +/- 12.9 and 52.9 +/- 12.8 mL kg(-1) minute(-1), respectively (p = 0.008) but this difference was deemed clinically unimportant; the harmonic mean plasma terminal half-lives (t(1/2)) were 24.0 +/- 1.9 and 37.4 +/- 1.6 minutes respectively. The volume of distribution was between 2 and 3 L kg(-1) and did not differ between the two doses. No sex effect was observed. CONCLUSIONS AND CLINICAL RELEVANCE: Alfaxalone, as an HPCD formulation (Alfaxan-CD RTU) administered in the dog provides rapid and smooth induction of anaesthesia, satisfactory conditions for endotracheal intubation and a short duration of anaesthesia. There was no clinically significant modification of the pharmacokinetic parameters between sexes and between the clinical (2 mg kg(-1)) and supra-clinical (10 mg kg(-1)) doses.  相似文献

The influence of pharmacologic enhancement of cardiac output on the alveolar-to-arterial oxygen tension (difference (P[A-a]O2), physiologic right-to-left shunt fraction (Qs/Qt), and physiologic dead space-to-tidal volume ratio (VD/VT) ws studied in halothane-anesthetized horses in left lateral, right lateral, and dorsal recumbencies. Adult horses were anesthetized, using xylazine (2.2 mg/kg, IM), guaifenesin (50 mg/kg, IV), thiamylal (4.4 mg/kg, IV), and halothane (1.5% to 2% inspired) in 100% O2. Mechanical ventilation was controlled to maintain arterial eucapnia (PaCO2) 35 to 45 mm of Hg) for a period lasting at least 1 hour. Dobutamine was administered at dosages of 1, 3, and 5 micrograms/kg/min, IV, on a randomized basis. The P(A-a)O2, Qs/Qt, and VD/VT were calculated during equilibration and after each dobutamine infusion was given. The P(A-a)O2 and Qs/Qt were significantly (P less than 0.05) greater and VD/VT tended to be greater in horses in dorsal recumbency, compared with those values in horses in left lateral or right lateral recumbency. Cardiac output was similar in all horses, regardless of body position (recumbency). The qualitative relationship between horses in the 3 recumbent positions were not altered by dobutamine. Cardiac output was significantly (P less than 0.05) increased by 3 or 5 micrograms of dobutamine/kg/min in all horses, whereas P(A-a)O2, Qs/Qt, and VD/VT were not significantly altered by dobutamine. The results of the present study failed to substantiate our clinical observations of decreased P(A-a)O2 and Qs/Qt in anesthetized compromised horses given dobutamine.  相似文献

Objective To compare the magnitude and duration of the peri‐operative haematological, endocrine and metabolic effects of surgery performed under sevoflurane anaesthesia. Study Design Prospective randomized study. Animals Ten, 55‐day‐old lambs of both sexes, mean weight 20.8 ± 0.3 kg (range 18.5–23.6 kg). Methods Animals were randomly allocated to two equal groups. All were anaesthetized with sevoflurane for 3 hours. Surgery (end‐to‐end anastomosis of the right carotid artery and right jugular vein) was performed in animals of Group 1 only. The electrocardiogram, pulse oximetry, cardiac output and noninvasive arterial blood pressure (NIBP) were monitored. Venous blood samples (5 mL) were taken 30 minutes before induction of anaesthesia (T = 0) and 1 (T1), 24 (T2), 48 hours (T3) and 7 days (T4) after anaesthesia in order to measure plasma cortisol, ACTH, insulin, cyclic adenosine monophosphate (cAMP), glucose, protein concentrations and haematological variables. Results Sevoflurane decreased NIBP (minimum mean value: 64 ± 3 mm Hg) in both groups. Plasma cortisol and ACTH concentration increased in Group 1 (maximum mean values: cortisol: 136.2 nmol L?1, ACTH: 54.5 pmol L?1) and Group 2 (maximum mean values: cortisol: 128.7 nmol L?1, ACTH: 44.0 pmol L?1). Cyclic AMP increased only in Group 1 (9.3 nmol) 1 hour after anaesthesia. Neutrophilia, lymphopaenia and a decreased PCV were observed in both groups 1 hour after anaesthesia. Plasma protein and glucose concentrations did not change. Conclusions Increased ACTH and cortisol concentrations recorded 1 hour after anaesthesia suggest that sevoflurane induces a stress response in lambs. Clinical relevance The study did not identify the mechanism by which sevoflurane induces a stress response although hypotension is implicated.  相似文献
