Objective— To report diagnosis and treatment of bilateral iliopsoas muscle contracture in a dog with spinous process impingement. Study design— Case report. Animals— German Shepherd dog. Methods— A dog with chronic progressive lameness, flexion contracture of the coxofemoral joints, severe pain, and decreased femoral reflexes had severe spondylosis bridging the vertebral bodies from L1 to L4 and enlarged dorsal spinous processes from T8 to L6 with impingement and bony proliferation. Ultrasonographic and magnetic resonance imaging (MRI) findings were consistent with fibrosis, mineralization, and atrophy of the iliopsoas muscles bilaterally which was treated by staged tenectomy of the insertions of the iliopsoas muscles. Results— Because of severe perivascular fibrosis, the femoral vessels required ligation. Bilateral iliopsoas muscle tenectomy improved gait and provided pain relief. Histologic findings were consistent with fibrotic myopathy. Conclusions— Slow progression of severe clinical signs observed bilaterally in this dog differs from previous reports of iliopsoas myopathy. Findings were similar to the fibrotic myopathy of the gracilis or semitendinosus muscles described in dogs. Clinical Relevance— Iliopsoas muscle abnormalities should be considered in dogs with limited hip extension and pain. MRI is useful for diagnosing muscle fibrosis. Iliopsoas tenectomy may improve clinical function in dogs with fibrotic myopathy.  相似文献

The normal radiographic anatomy of the equine thoracolumbar (TL) spine is described from birth to maturity. Closure of the vertebral epiphyses was found to take place from 30 to 38 months of age. In a series of 110 normal horses, there were few incidental radiologic abnormalities seen apart from changes associated with crowding and overriding of the dorsal spinous processes. Sixty-six percent of horses were free from any detectable radiologic lesion, and an additional 12 percent showed only impingement of the dorsal tips of one or more summits. The remainder (22 percent) exhibited some evidence of overriding of the dorsal spinous processes, but in only 3 percent were the changes pronounced. This reduction or obliteration of the normal interspinous spaces occurred chiefly in the midback from T13 to T18 and was associated with pressure points between adjacent spines with local periosteal reaction and focal areas of radiolucency. The tips of the processes sometimes overlapped one another and became misshapen, presumably as a result of continued pressure from the adjacent impinging spines.  相似文献

Back pain is common in horses, but there has been no large‐scale in‐depth study describing radiographic changes of the spinous processes, the relationship between radiographic and scintigraphic findings, and the effect of size, age, breed, or discipline. The objectives were to investigate the frequency of occurrence in horses with perceived back pain of: (1) radiographic alteration of the spinous process structure; (2) increased radiopharmaceutical uptake (IRU) in the spinous processes; and (3) to compare radiographic and scintigraphic findings; and to determine if there was breed, gender, age, bodyweight, height, or work discipline predisposition for close, impinging, or overriding spinous processes. Radiographic and scintigraphic images of the thoracolumbar spine of 604 horses were graded. A radiographic grade for each spinous process (T8–L6) was assigned (0–7). The maximum radiographic grade for each horse was defined as the highest grade assigned to any spinous process; the total radiographic grade was the sum of all grades for each horse. A scintigraphic grade for each spinous process was determined (0–3). The maximum scintigraphic grade for each horse was defined as the highest grade assigned to any spinous process; the total scintigraphic grade was the sum of all grades for each horse. Associations between radiology and scintigraphy and age, gender, breed, height, weight, and discipline were analyzed statistically. The severity of radiographic lesions of the spinous processes was associated with the severity of scintigraphic abnormalities. The caudal thoracic spine (T14–T17) was most frequently affected. There was a significant breed and age effect, with Thoroughbreds and older horses having higher total and maximum radiographic grades. The severity of the lesions of the spinous processes was significantly associated with the presence of osteoarthritis of the articular process joints. It was concluded that there is a wide range of radiographic abnormalities of the spinous processes seen in horses with or without back pain. There is an association between radiographic and scintigraphic grades of the spinous processes.  相似文献

A 9-year-old spayed female Bengal Red cat was evaluated for high-rise syndrome. The cat had paraplegia of the hind limbs, intact reflexes and pain perception, and hyperesthesia in the caudal thoracic area. Mentation, cranial nerve function, forelimb proprioceptive responses, and spinal reflexes were normal. There were no abnormalities on radiographs or computed tomography scan, but magnetic resonance imaging revealed a hyperintense intraparenchymal spinal cord lesion on T2-weighted and T2 fat saturation images.  相似文献

Objective— To describe outcome after an alternative unilateral approach to the thoracolumbar spine for dorsal laminectomy.
Study Design— Retrospective clinical study.
Animals— Dogs (n=14) with thoracolumbar spinal cord compression.
Methods— Thoracolumbar spinal cord compression was lateral (6 dogs), dorsal (4), and dorsolateral (4) caused by subarachnoid (7) and synovial cysts (2) and intradural-extramedullary neoplasia (5). All dogs were treated by dorsal laminectomy with osteotomy of the spinous process using a unilateral paramedian approach. The contralateral paraspinal muscles were not stripped from the spinous process and the osteoligamentous complexes were preserved. Retraction of the spinous process and muscles to the contralateral side resulted in complete visualization of the dorsal vertebral arch thereby allowing dorsal laminectomy to be performed.
Results— No technique complications occurred. Approximately 75% exposure of the spinal cord (dorsal and lateral compartments) was achieved providing adequate visualization and treatment of the lesions. Transient deterioration of neurologic state occurred in 5 dogs because of extensive spinal cord manipulation. At long-term follow-up, 6 dogs were normal, 6 had clinical improvement, and 2 were unchanged.
Conclusion— Dorsal laminectomy after osteotomy and retraction of the spinous process may be considered in canine patients with dorsal, dorsolateral, or lateral compression to facilitate adequate decompression of the spinal cord.
Clinical Significance— This surgical technique offers an alternative approach to the thoracolumbar spine and spinal cord by a modified dorsal laminectomy that preserves the paraspinal muscle integrity on the contralateral side.  相似文献

REASONS FOR PERFORMING STUDY: Radiography and scintigraphy are used to aid diagnosis of the cause of back pain, but a large variation in appearance and radiopharmaceutical uptake in fully functioning horses make diagnosis difficult. OBJECTIVES: To describe the range of and compare scintigraphic and radiographic findings in the spinous processes of horses without clinical signs of back problems. METHODS: Thirty-three apparently normal riding horses underwent scintigraphic and radiographic examinations of the spinous processes in the thoracolumbar spine. Scintigraphic images were evaluated in a continuous blue, green and red colour scale, and the level of radiopharmaceutical uptake in the spinous processes from T10-L2 was graded into none, mild, moderate or severe increased radiopharmaceutical uptake. Structural changes along the borders of the spinous processes and the width of the interspinous spaces from T10-L2 were recorded. RESULTS: Only 7 horses had no scintigraphic or radiographic findings. Nine horses had no increased radiopharmaceutical uptake, 17 had no sclerosis, 21 had no radiolucencies and 11 had normal spacing of the spinous processes (>4 mm wide). The majority of findings in 26 horses were located from T13-18 and were mild. CONCLUSIONS: The findings of a wide spectrum of scintigraphic and radiographic changes leads to the conclusion that changes within this range found in affected horses cannot be interpreted as clinically significant. POTENTIAL RELEVANCE: To determine whether scintigraphy and/or radiography can be used to separate horses with back pain from horses without clinical signs, the results from this study should be compared to the scintigraphic and radiographic findings in horses with clinical signs.  相似文献

A modified segmental spinal stabilization technique was used in four dogs weighing 10 to 56 kg with thoracolumbar and lumbosacral spinal fractures/luxations. A Steinmann pin was bent to encircle the dorsal spinous processes and was wired together with longitudinal pins to the cranial articular facets and dorsal spinous processes in the thoracolumbar application. The central pin was omitted, and the longitudinal pins were bent at a right angle and passed through the ilial wings in the lumbosacral application. Fracture fixation and stabilization were excellent in all dogs. One broken pin was detected in one dog after 18 months. No other complications due to the spinal instrumentation were observed. Neurologic recovery was excellent in two dogs, good in one dog, and fair in one dog.  相似文献

Severe necrotizing myelitis secondary to localization and reactivation of Toxoplasma gondii within the spinal cord of a domestic shorthair cat was diagnosed by use of light and electron microscopy and immunohistochemistry. The cat also was infected with feline immunodeficiency virus. This case may have useful comparative features to T gondii infections in human patients with acquired immunodeficiency syndrome.  相似文献

Reason for performing study: The aetiological factors behind impinged or overriding of dorsal spinous processes (‘kissing spine syndrome’, KSS) are not clearly understood. Back conformation, breed, age, training and gender may play important roles in this condition. Radiographic changes vary and abnormalities are seen in many clinically normal horses, but the conclusion of previous studies in mature horses is that interspinous spaces <4 mm are considered too narrow and potentially indicative of KSS. Objectives: To evaluate whether narrowing of the interspinous space was present in a population of normal Warmblood foals. Materials and methods: The mean interspinous space width in the area of T10–L1 was measured on radiographs from 25 Warmblood foals aged 9–88 days. Results: Mean ± s.d. interspinous space width was found to be between 5.9 ± 1.2 and 8.9 ± 2.6 mm with the narrowest space in the area T16–T17 and the widest space in T10–T12. No interspinous spaces were <4 mm wide. Gender and location of the interspinous space significantly affected the width of the distance between the spinous processes. Conclusions and potential relevance: In this study none of the interspinous spaces were <4 mm and therefore none of the foals showed signs of impinged or overriding of dorsal spinous processes known as KSS based on the current definitions. Consequently, in this population, there did not appear to be a congenital narrowing of the interspinous space. However, long‐term follow‐up studies, including detailed information on imposed factors such as training, are needed in order to further elucidate a possible congenital component in the aetiology of KSS.  相似文献

A 5‐day‐old Friesian colt was presented with a history of severe bilateral pelvic limb weakness since birth. Clinical examination revealed a painful dorsal deviation of the spinous processes of the lumbar vertebrae, pelvic limb paraparesis and grade 4/5 ataxia. Radiographic examination revealed kyphosis due to hypoplasia and malformation of the 5 lumbar vertebrae present. At myelography performed immediately after euthanasia, dorsal deviation of the myelographic contrast column was demonstrated at the level of T18–L4 with suspected spinal cord compression noticeable at L1. There was no lateral deviation of the spinal cord or scoliosis of the vertebrae visible.  相似文献

Lumbar and thoracic vertebrae of the extinct horse, Equus occidentalis, were examined for gross and radiographic evidence of overriding spinous processes. Of 2,661 vertebrae examined, 580 had intact spinous processes. Thirty-six intact spinous processes, which appeared grossly similar to overriding spinous processes in the modern domestic horse, E caballus caballus, were radiographed. Of these 36 vertebrae, 2 had radiographic signs compatible with a radiographic diagnosis of overriding spinous processes, ie, radiographically observed lysis and/or sclerosis. Seemingly, weight bearing or other stresses imposed by human beings may not have induced the signs of overriding spinous processes.  相似文献

Plastic plates are a rapid and easy method to realign and stabilize the spinal column following a traumatic injury and may give greater bone-to-plate contact than metal plates. They distribute the pressure on the spinous processes over a larger surface area although plate slippage and spinous process fractures can occur as with any plate. Laminectomies should not be a routine procedure with plating but each patient should be evaluated individually and autogenous bone grafts may be used to advantage to promote fusion of vertebral bodies. In the two cases presented, the plastic plates achieved their purpose. However, it is essential that fusion of the vertebrae occurs before the plates slip or the spinous processes fracture.  相似文献

Anecdotal comments in the literature suggest that spinous fracture many weeks after application of polyvinilidine plates to the dorsal spinous processes of dogs may be due to associated adverse vascular changes. This study was undertaken to evaluate that hypothesis. Large size Lubra plates were cut to the appropriate length and applied to the dorsal spines of vertebrae L3 and L4. Vertebrae L2 and L5 served as operated controls. Vertebrae L1 and L6 served as unoperated normals. Two dogs were euthanized 1 week postoperatively, and three dogs each at 4, 8, and 12 weeks. The effect of the plate on the dorsal spinous processes was evaluated using microangiography and correlated histology. There was no apparent change in vascular architecture associated with plate application, nor with surgery alone, at any time interval. There did appear to be a time dependent loss of bone associated with plate application, indicating a change in the relative balance of osteoblastic and osteoclastic activity, and altered bone turnover.  相似文献

The surgical treatment of impingement of dorsal spinous processes (DSPs) at 2 equine veterinary hospitals following a similar diagnostic and surgical protocol is described with special reference to the diagnosis and case selection. The diagnosis was made from clinical examination and results of radiography and diagnostic analgesia of the affected sites. Scintigraphy was also used as an aid to diagnosis at one hospital. Surgical treatment was generally recommended when conservative treatment, which included 3-9 months rest, intralesional corticosteroids and physiotherapy, had failed. The DSPs were approached through a midline incision in the skin and supraspinous ligament. They were resected using an oscillating saw. Postoperatively, horses were returned to lungeing work after 2 weeks and riding commenced at 3 months, in most cases, and by 6 months in all successful cases. From one to 6 DSPs between thoracic vertebra (T) 11 and lumbar vertebra (L) 3 were resected in individual horses. T15, 16 and 17 were the spines most commonly treated. Long-term follow-up information was obtained in 209 horses. Seventy-two percent of these horses returned to full work and a further 9% improved sufficiently to be used for some athletic work.  相似文献

A 10-year-old neutered male Persian cat and a 4-year-old spayed female domestic shorthair (DSH) cat were evaluated for acute-onset severe lateralising tetraparesis and hemiplegia, respectively. Both cats also had left-sided Horner's syndrome. Neurological examination of the cats localised the lesion to cranial to C5 in the Persian and the left cervical intumescence (C6-T2) in the DSH. Physical examinations were otherwise generally unremarkable. Routine laboratory tests and spinal radiography were normal for the Persian cat and were not performed for the DSH cat. A cerebrospinal fluid (CSF) tap was attempted for the Persian cat but aborted because of gross blood contamination, and was not performed for the DSH cat. Magnetic resonance imaging (MRI) of the Persian cat revealed a lesion within the spinal parenchyma at segments C1 to C3 (slightly more left-sided) which was iso- to hypointense on T1-weighted scans and hyperintense on T2-weighted scans, and which enhanced slightly with gadolinium. MRI of the DSH cat revealed a lesion within the spinal parenchyma at segment C7 (predominantly left-sided) which was hypointense on T1-weighted scans and hyperintense on T2-weighted gradient echo scans. Contrast was not administered. The MRI findings in both cases were highly suggestive of acute spinal cord infarction, based upon comparison to human cases. Both cats made full neurological recoveries with supportive treatment only. This paper describes two cases of suspected acute spinal cord infarction in the cat, demonstrates the potential diagnostic value of MRI, and discusses the clinical syndrome of this condition with a brief review of published cases.  相似文献

A 23-yr-old black bear (Ursus americanus) was examined because of paralysis of unknown duration. The precise onset of clinical signs was unknown as a result of seasonal torpor. The bear was immobilized and transported to a university veterinary teaching hospital for further evaluation and treatment. Radiography revealed increased mineral opacity and ventral bridging across vertebral segments T8-11. Magnetic resonance imaging demonstrated dorsal and ventral compression of the spinal cord at T8-9. Given the bear's advanced age, the unknown duration of spinal cord compression, unknown presence of deep pain perception, and thus an unknown prognosis for surgical success, euthanasia was elected. Postmortem examination revealed severe spondylosis deformans from T7 to L3 and dorsal extradural extruded disc material in the area of T8-9. Histopathology demonstrated the dorsal horns of the spinal cord at T9 were replaced by foamy macrophages extending into the dorsal and lateral funiculi of the white matter compatible with focal, severe, chronic myelomalacia. This is the first report of intervertebral disc disease and myelomalacia diagnosed using MRI in a large carnivore.  相似文献

A 6-month-old male-castrated domestic shorthair cat was presented shortly after being bitten on the neck by a large breed dog. On presentation the cat was non-ambulatory tetraparetic with preserved deep pain perception. Plain radiographs of the neck did not show any abnormalities. Computed tomography was performed and showed a right-sided depressed fracture of the dorsal lamina of the atlas. A partial dorsal laminectomy was performed to alleviate the spinal cord compression. The cat made a full recovery within 12 weeks of surgery.  相似文献

A 10-year-old female mongrel cat with back pain was brought to the Nihon University Animal Medical Center. Palpation demonstrated a mass in the back region. Radiography revealed partial destruction of the processus spinosus and the arch of the T8 and T9 vertebrae. On magnetic resonance imaging, the mass was found to have compressed the spinal cord and extended to the outside of the spinal canal. We performed extirpation of the mass, and confirmed that it arose from the spinal canal. Histopathologically, the mass was a malignant peripheral nerve sheath tumor.  相似文献

This is the first report of feline solitary plasmacytoma of bone. We describe the clinical, clinico-pathological, radiographic and pathological findings of two successfully treated cats with long-term follow-up. The first case presented with spinal pain and neurological deficits. Radiographs demonstrated sclerosis of lumbar vertebra L6 and a myelogram confirmed interference to flow of contrast in the L4-7 region. A biopsy of L6 revealed neoplastic plasma cell infiltration. There was no evidence of paraproteinaemia on serum protein electrophoresis. The cat underwent hypofractionated megavoltage radiotherapy. Clinical signs resolved completely and 4 years after diagnosis the cat remains well and has no electrophoretically detectable paraproteinaemia. The second case presented with neurological deficits of the tail and spinal radiographs revealed extensive osteolysis of the sacrum. A biopsy of sacral bone demonstrated neoplastic plasma cell infiltration. The animal was normoglobulinaemic. The cat improved clinically with induction chemotherapy (melphalan and methylprednisolone). The same chemotherapeutics were continued at maintenance doses for 4.3 years, at which time there was recurrence of neurological deficits and a palpable sacral mass. Cytological examination of a fine needle aspirate confirmed recurrence of plasma cell neoplasia. A low concentration monoclonal paraproteinaemia was detected. Vincristine was administered resulting in resolution of neurological deficits and a palpably smaller sacral mass. Eighteen months into vincristine therapy, there was recurrence of clinical signs and the cat was euthanased, more than 6 years after the initial diagnosis.  相似文献
