In dogs, balloon valvuloplasty is considered the treatment of choice for severe pulmonary valve stenosis, and this technique is currently performed routinely in specialist referral practices with low morbidity and mortality. Stent angioplasty has also been recently proposed as a viable treatment option. The present case series describes the clinical course of four dogs with severe pulmonary valve stenosis, treated with balloon valvuloplasty or stent angioplasty at four different institutions, which developed non-cardiogenic pulmonary oedema perioperatively after apparently successful dilation of the pulmonary valve. In three cases, there was evidence of some degree of pulmonary hypertension before ballooning. Despite intensive care, the complication proved fatal in three cases. Clinicians should therefore be aware of this life-threatening complication, previously undescribed in dogs. 相似文献

Four young dogs with severe pulmonic stenosis were treated by percutaneous balloon valvuloplasty. Pressure gradients between the right ventricle and pulmonary artery were markedly reduced by the procedure resulting in dramatic improvements in growth rate and clinical condition. Case 1 involved a 4-month-old intact male Somali cat in which peripheral pulmonary artery
stenosis (PPS) was recognized after a cardiac murmur remained following patent ductus
arteriosus ligation. Case 2, which involved a 1-year-old neutered male Norwegian Forest
cat, and Case 3, which involved a 6-month-old intact female American Curl cat, were
referred, because of cardiac murmurs. Grades III to IV/VI systolic heart murmurs were
auscultated at the left heart base in all 3 cats. All cases showed bilateral pulmonary
artery stenosis, although there were no associated clinical signs. In Cases 1 and 2, the

Single right coronary artery (CA) associated with pulmonic stenosis was found in 3 English Bulldogs and a Boxer, suggesting a genetic predisposition for the associated anomalies. The left main coronary branch arose from the single right CA, encircled the pulmonic root over hypoplastic pulmonic valves at the level of the obstruction, and appeared to be the primary cause of underlying pulmonic stenosis. Patch-graft surgery to relieve pulmonic stenosis caused death in 1 dog when the unrecognized anomalous CA was served during the procedure. The anomalous left CA was detectable by use of angiocardiography in all 4 dogs and was recognized before surgery in 2 of them. In 1 dog, a right ventricle-to-pulmonary trunk-bridging conduit was implanted to improve outflow from the right side of the heart. 相似文献

Introduction/objectivesPulmonary artery dissection is a rare complication following balloon valvuloplasty for pulmonic stenosis. We sought to report the rate of this complication in dogs and describe the demographic, clinical, procedural, and outcome data in affected dogs.Animals, materials and methodsMedical records at a single academic institution between 2002 and 2021 were reviewed for dogs with pulmonic stenosis treated by a balloon valvuloplasty. Dogs were included for evaluation if there was evidence of pulmonary artery dissection on echocardiography or necropsy following balloon valvuloplasty. The demographic, clinical, surgical, and follow-up information were then recorded.ResultsSix dogs were included from 210 balloon valvuloplasty procedures for pulmonic stenosis giving a 3.9% rate of pulmonary dissection. There was a variety of signalment, pulmonary valve morphologies, and balloon catheter types used in each dog. All dogs had severe pulmonic stenosis (median pressure gradient of 208 mmHg, range 94–220 mmHg) with 5/6 dogs having a pressure gradient >144 mmHg. The median balloon to pulmonary valve annulus ratio was 1.35 (range 1.25–1.5). Three dogs died perioperatively, and three dogs were alive at follow up 3.3, 4.0, and 4.1 years postoperatively.ConclusionPulmonary artery dissection is a rare complication following balloon valvuloplasty for pulmonic stenosis. Extreme elevations in preoperative pulmonary valve flow velocity were common. Prognosis is variable, with a potential 50% perioperative survival rate, but extended survival times were noted in those patients discharged from hospital. 相似文献

ObjectivesDescribe the clinical, electrocardiographic (ECG), radiographic and echocardiographic findings in cats with isolated pulmonary artery stenosis. Assess the usefulness of systolic and diastolic Doppler measurements at predicting stenosis severity.BackgroundPulmonary artery stenosis is an infrequent congenital cardiac defect in humans that has not been reported in cats. In humans, pulmonary artery stenosis is usually seen in conjunction with other cardiac defects and may lead to clinical signs if severe.Animals, materials and methodsSeven cats with pulmonary artery stenosis were retrospectively evaluated. Medical records, radiographs, ECGs, echocardiograms and angiocardiograms were reviewed. Severity of stenosis was assessed by two-dimensional and color Doppler echocardiographic evaluation and clinical findings. Peak systolic and diastolic gradients across the stenosis, and systolic and diastolic pressure decay half-times were graded using echocardiography. In addition, the duration of antegrade flow during diastole was subjectively assessed. Univariate analyses were performed to assess the best variable to predict stenosis severity.ResultsConcurrent congenital defects were not identified. Only cats with severe obstruction showed clinical signs including exertional dyspnea and lethargy. Diastolic Doppler measurements were superior to systolic measurements at predicting severity of stenosis. Antegrade flow throughout diastole and/or a diastolic pressure half-time of >100 ms indicated severe obstruction. The prognosis for pulmonary artery stenosis appears to be good regardless of severity.ConclusionAmong cats with pulmonary artery stenosis, clinical signs are uncommon and prognosis is good. Doppler assessment of diastolic flow appears to be superior to systolic flow at predicting severity. 相似文献

After laryngeal surgery, cranial glottic stenosis secondary to laryngeal web formation was diagnosed as the cause of upper airway obstruction in 4 dogs. Initial treatment of 3 dogs by peroral laryngeal web excision resulted in rapid recurrence of clinical signs. Subsequently, these 3 and the fourth dog were treated by scar excision, mucosal flap coverage, and placement of a silicone intraluminal laryngeal stent for 14 days. By 5 months (average) after surgery, on the basis of clinical evaluation of exercise intolerance, respiratory distress, and stridor, all dogs had alleviation of upper airway obstruction. 相似文献

Hybrid balloon valvuloplasty for management of severe mitral stenosis in four symptomatic,adult dogs
Four adult dogs weighing <10 kg presented for the evaluation of severe mitral valve stenosis with clinical signs. Owing to the size of the dogs, a hybrid surgical and interventional approach was utilized for balloon valvuloplasty. A left lateral thoracotomy was performed to allow direct entry through the left atrial wall. Transesophageal echocardiography was utilized for the entirety of the procedure in all dogs, and fluoroscopy was additionally used in two dogs. One dog had mild to moderate intra-operative bleeding from the left atrial wall during the procedure, but no other intra-operative complications were observed. No dogs developed a clinically relevant amount of worsened mitral regurgitation. Based on mitral leaflet mobility and transmitral flow profiles, there was perceived improvement in all four dogs. One dog died 6 h after extubation due to respiratory arrest. The remaining dogs survived to discharge and had resolution of clinical signs at home and discontinuation of heart failure medications. One dog died of an unknown cause at five months and another developed atrial fibrillation, and the owners elected to euthanize at ten months after the procedure. One dog continues to do well six months after the procedure as of the time of this writing. Hybrid balloon valvuloplasty can be a viable management option for small breed dogs with severe mitral stenosis exhibiting clinical signs, and both transesophageal echocardiography and fluoroscopy can be used intra-operatively to assist in successful procedural outcomes. 相似文献

Nasopharyngeal stenosis secondary to chronic upper respiratory tract infection is described in four cats. The stenosis is caused by a thin but tough inflammatory membrane which develops across the proximal nasopharynx, partly or completely occluding it. Diagnosis is aided by use of a dental mirror for caudal rhinoscopy, together with test passage of a fine catheter through the ventral nasal meatus. All four cases were treated successfully by surgical resection of the membrane through an incision in the soft palate. Nasopharyngeal stenosis should be considered in the differential diagnosis of all chronic upper respiratory disease in cats. 相似文献

The clinical and pathologic features of 4 dogs with malignant histiocytosis were evaluated. The most common clinical signs were weight loss, lethargy, lymphadenopathy, hepatosplenomegaly, and anemia. Neoplastic histiocytic infiltrates most commonly were found in the spleen, bone marrow, liver, or lymph nodes. Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), or Bland-White-Garland syndrome, is a rare congenital malformation described in children and adults. In this condition, the left coronary artery, which normally originates from the left coronary sinus in the aorta, instead originates from the pulmonary trunk, which results in retrograde flow of blood away from the myocardium into the lower-pressure pulmonary artery. Myocardial hypoxic-ischemic injury results in cardiac dysfunction, failure, and eventually in patient death if not surgically repaired. This report describes gross and microscopic findings in 4 beef calves with ALCAPA. All the calves had a history of being found dead with few or no premonitory signs, 2 shortly after sudden strenuous exercise. Gross necropsy lesions suggestive of heart failure included cardiomegaly with atrial and ventricular dilation and/or ventricular hypertrophy, and hepatomegaly. Dissection of each heart revealed the origin of the left coronary artery arising in the pulmonary trunk above the anterior cusp of the pulmonic valve. No degeneration; mineralization; and fiber loss, with replacement by fibrous connective tissue, predominantly in the left ventricular papillary muscle and the interventricular septum. Changes observed in the liver and lungs, including hepatomegaly, sinusoidal congestion, centrilobular fibrosis, and pulmonary congestion, edema, and intra-alveolar pigment-laden macrophages were consistent with heart failure. 相似文献

Eighteen dogs with degenerative lumbosacral stenosis were presented to the University of Queensland Small Animal Clinic (UQSAC) over a three-year period. Presenting clinical signs included lumbosacral pain (89 per cent), hindlimb paresis and proprioceptive deficits (56 per cent), lameness (49 per cent), flaccid tails (22 per cent), and urinary dysfunction (16 per cent). All 18 dogs were treated by decompressive laminectomy. Two dogs were also treated by a pin fixation-fusion technique. The major compressive lesion was a type II disc protrusion (72 per cent). Seventeen dogs (94 per cent) showed improvement postoperatively with minimal complications. Confirmation of diagnosis is difficult in that many aged dogs without clinical signs show radiographic signs compatible with stenosis. 相似文献

A seven-year-old castrated male domestic shorthair cat was diagnosed with hypertrophic cardiomyopathy (HCM) and suspected mitral stenosis (MS) based on electrocardiography, thoracic radiographs and echocardiographic findings. Post-mortem examination of the heart revealed morphological features consistent with HCM. In addition, there was marked fibrous deposition on the surfaces of the chordae tendineae extending to both mitral valve leaflets, which caused total chordal fusion into pillars of fibrous tissue and fusion of the commissures. The present case indicates that acquired MS can occur in association with HCM in the cat. 相似文献

Pulmonary hypertension (PH) is an important predictor of poor outcomes in dogs with mitral regurgitation (MR). The feasibility of radiography to predict PH in dogs with MR is unknown. This retrospective, observational, and analytic study aimed to identify a radiographic parameter to predict PH in dogs with MR. A total of 302 dogs diagnosed with MR on echocardiography were enrolled. Medical record and radiographic findings such as the size of the main pulmonary artery, left atrium, left ventricle, and right chamber, and cranial and caudal pulmonary arteries and veins were evaluated according to the presence of PH. The diameters of the cranial and caudal pulmonary vessels were compared to the fourth rib and the ninth rib, respectively, and the ratio of the pulmonary artery to the corresponding vein (CdPA/CdPV) was calculated. Pulmonary hypertension was diagnosed in 77 dogs (25.5%) and the prevalence of PH increased with MR grade. The CdPA/CdPV was significantly higher (P < 0.001) in the presence of PH. Multivariate analysis showed that the CdPA/CdPV was the only independent radiographic parameter that had a significant association with PH in dogs with MR (P = 0.028). The cut-off value of the CdPA/CdPV = 1.10 showed 90.6% specificity and 31.1% sensitivity for detecting PH in dogs with MR. In dogs with MR, PH can be predicted with high specificity when the caudal pulmonary artery is 1.1 times larger than the corresponding vein on radiographs. 相似文献

Cutaneous hybrid cysts were recognized in 4 dogs. The lesions were solitary, asymptomatic, and occurred over the head, neck, proximal thigh, and lumbar region. These cysts were not accompanied by other disease processes, and did not recur following surgical excision. Histologically, the hybrid cyst is characterized by epidermal differentiation in its upper portion, and trichilemmal differentiation in its lower portion. 相似文献
