The complexity of the equine skull makes the temporomandibular joint a difficult area to evaluate radiographically. The goal of this study was to determine the optimal angle for a complementary radiographic projection of the equine temporomandibular joint based on a computed tomography (CT) cadaver study. CT was performed on six equine cadaver heads of horses that were euthanized for other reasons than temporomandibular joint disease. After the CT examination, 3D reconstruction of the equine skull was performed to subjectively determine the angle for a complementary radiographic projection of the temporomandibular joint. The angle was measured on the left and right temporomandibular joint of each head. Based on the measurements obtained from the CT images, a radiographic projection of the temporomandibular joint in a rostral45°ventral-caudodorsal oblique (R45°V-CdDO) direction was developed by placing the X-ray unit 30° laterally, maintaining at the same time the R45°V-CdDO angle (R45°V30°L-CdDLO). This radiographic projection was applied to all cadaver heads and on six live horses. In three of the live horses abnormal findings associated with the temporomandibular joint were detected. We conclude that this new radiographic projection of the temporomandibular joint provides superior visualization of the temporomandibular joint space and the articular surface of the mandibular condyle.  相似文献

Objective— To report a technique for eye enucleation in standing sedated horses and to report outcome in 40 horses.
Study Design— Retrospective study.
Animals— Horses (n=40) requiring eye enucleation.
Methods— The eye was enucleated using a transpalpebral technique in 40 horses restrained in stocks and sedated. Anesthesia of orbital structures was provided by local nerve blocks and infiltration of the surgical site with local anesthetic solution.
Results— Affected eyes were successfully enucleated with the horse standing. Short-term complications included moderate swelling (5 horses) and wound discharge (1). Long-term complications were not observed.
Conclusions— A diseased eye can be safely enucleated with a horse standing.
Clinical Relevance— Enucleating an equine eye in the standing position eliminates the risks and costs of general anesthesia.  相似文献

Objective— To assess whether there is a difference in the mechanical medial proximal tibial angle (mMPTA) measured on a tangential caudocranial (tCdCr) radiographic projection versus a straight caudocranial (sCdCr) projection before and after inducing a varus deformity in the proximal tibia. Study Design— In vitro study. Sample Population— Cadaveric canine tibiae (n=4 pair). Methods— For each tibia, a mediolateral radiographic projection was performed and the tibial plateau angle was measured. sCdCr and tCdCr radiographic projections were obtained and the mMPTA measured. A varus deformity was created in the proximal aspect of the tibia and sCdCr and tCdCr projections were repeated and mMPTA measured. Results— mMPTA for tCdCr was statistically different from mMPTA for the sCdCr projection for the varus tibiae (P<.05). There was no significant difference in the mMPTA measured on the sCdCr projections before and after creation of a varus deformity (P>.05). There was a significant difference in mMPTA measured on the tCdCr projection before and after creation of a varus deformity (P<.05). Conclusion— Varus deformity in the mMPTA was identified on tCdCr projections of the varus tibiae whereas it was not identified on sCdCr projections. Clinical Relevance— Tangential radiographic projections of the tibial plateau may be useful for evaluating varus deformities involving the articular surface of the tibia and should be performed during preoperative evaluation of angular limb deformities involving the proximal aspect of the tibia and for tibial plateau leveling osteotomy (TPLO) planning.  相似文献

Objectives— To describe and evaluate the use of a transoral, endoscope-guided technique for transection of an entrapping epiglottic fold in sedated standing horses.
Study Design— Retrospective study.
Animals— Horses (n=16) with epiglottic fold entrapment (EFE).
Methods— Medical records (2005–2006) of 16 horses with EFE were reviewed to determine history, physical and endoscopic examination findings, postoperative complications, and outcome after axial division of EFE using a hooked bistoury, under an endoscopic-guided, transoral approach.
Results— EFE was confirmed by endoscopy. Axial division was successfully performed in 15 sedated, standing horses. One horse had to be anesthetized to complete the procedure. None of the horses made abnormal respiratory noise after surgery and all returned to their intended use. After surgery, 1 horse had a short, deformed epiglottis.
Conclusions— EFE can be axially divided safely and effectively using an endoscope-guided, transoral approach, with the horse sedated and standing.
Clinical Relevance— Endoscopic-guided, transoral axial division of EFE in sedated standing horses is an alternative choice to performing this procedure under general anesthesia.  相似文献

Investigation of temporomandibular joint (TMJ) disease requires a clear diagnostic image, which can be challenging to obtain using conventional radiography. The aim of this study was to compare five different oblique radiographic views with the head in lateral recumbency, assessing the clarity of visualization of the normal TMJ anatomy. The views under investigation were the laterorostral-laterocaudal oblique at a 10° and 20° rotation of the head ("nose-up" view), laterorostral-laterocaudal oblique with a rostrocaudal X-ray beam angulation of 10° and 20°, and a parallax view with the beam centered over C2 and collimated to include the TMJ region, using the divergence of the X-ray beam to project the TMJs separately on the radiograph. The views were performed on both TMJs of thirty canine cadavers and were graded independently by experienced and inexperienced observers. Grading was performed on the mandibular fossa, condylar process, joint space, retroarticular process, and the overall TMJ, and was based on a four-point scale. Mean grades for each component and for the overall joint were compared for each observer and each projection. Mean grades were significantly (P < 0.05) higher for the "Nose-up" projections than the angled beam or parallax projections, as was interobserver agreement, and both observers showed significantly higher (P < 0.05) mean grades for the 20(o) "Nose-up" angulation than the 10(o) "Nose-up" angulation. These results suggest that a latero 20(o) rostral-laterocaudal oblique gives the best representation of the anatomy of the TMJ of the dog of the projections assessed, and should be considered when investigating clinical cases of TMJ disease.  相似文献

Objective —To describe incomplete oblique sagittal dorsal cortical fractures of the equine third metacarpal bone, their surgical repair, and subsequent performance of the horses.
Study Design —Retrospective examination of medical records and racing performance.
Animal Population —Six Thoroughbred race horses, 2 to 4 years of age.
Methods —Radiographic confirmation of all fractures preceded general anesthesia and surgical correction. Three fractures were treated by intracortical compression using screws placed in lag fashion, and five fractures were treated by osteostixis. Race records were reviewed for each horse to determine performance after surgery.
Results —Fractures were best observed on palmarodorsal radiographic projections. Three horses treated by intracortical compression returned to racing, but fracture recurred in one horse and was treated by osteostixis. This horse and the other three horses treated by osteostixis raced after surgery.
Conclusions —Horses with incomplete oblique sagittal fractures of the dorsal cortex of the third metacarpal bone can race after surgical management of the fracture by screws placed in lag fashion or osteostixis. The authors' preferred surgical procedure for managing this fracture is osteostixis.
Clinical Relevance —Palmarodorsal radiographic projections of the third metacarpal bone are recommended in young Thoroughbred race horses suspected of having dorsal metacarpal stress fractures.  相似文献

Objective— To describe and evaluate an endoscope-guided balloon sinuplasty technique for dilation of the equine nasomaxillary opening (NMO).
Study Design— Experimental study.
Animals— Equine cadaver heads (n=5); Quarter Horses (n=4).
Methods— A custom rigid balloon introducer was passed into position within the NMO at the caudal recess of the middle meatus. Under endoscopic guidance, a balloon catheter was passed via the introducer into the NMO and inflated to 6 atmospheres, for 30 seconds 3 times, to dilate the NMO. Drainage rates after sinuplasty were evaluated for the caudal maxillary sinus in 5 cadaver heads and balloon sinuplasty was performed in 4 live horses. All skulls were examined to assess the efficacy of NMO dilation.
Results— Positioning of the balloon catheter was readily achieved in all heads. Dilation was observed endoscopically and confirmed on necropsy in all heads. Drainage rates from the caudal maxillary sinus increased significantly (∼1.5 ×) in all 5 cadaver heads after sinuplasty ( P =.001).
Conclusions— Dilation of the NMO improved sinus drainage in all 5 cadaver heads. Endoscopic evaluation of the ostium revealed marked dilation of the NMO in live horses and indicates potential clinical use in standing horses.
Clinical Relevance— Balloon sinuplasty of the NMO opening is an easily performed, clinically applicable technique for use in standing horses. Clinical studies will be necessary to evaluate the efficacy of this technique in diseased sinuses.  相似文献

Lifting the contralateral forelimb (unipodal stance) is often used as a method of restraint in horses. This experimental one group pretest, posttest study was conducted to evaluate the effects of unipodal stance on quantitative radiographic parameters in equine forefeet. Seven nonlame horses were randomly selected. Lateromedial (LM) and dorsopalmar (DP) projections were acquired for both forefeet, squarely placed on blocks, using two X‐ray generators. Radiographs of each foot were acquired first in a bipodal stance, immediately followed by the same radiographic projections obtained in a unipodal stance. The following measurements were recorded for each stance: distal interphalangeal joint (DIPJ) space width on both projections; mediolateral joint balance as the difference between lateral and medial DIPJ space widths on DP projections; extensor process‐to‐middle phalangeal condyle distance; and deep digital flexor tendon angle on LM projections. A matched pairs design and Student's t‐test with a 95% confidence level were used to test for statistical significance. Compared to a bipodal stance, lateral DIPJ space width was significantly reduced on unipodal DP views, whereas mediolateral joint imbalance and to a lesser extent medial DIPJ space width were significantly increased. On unipodal LM views, there was a significant higher degree of DIPJ flexion. These findings suggest that stance should be carefully taken into consideration when measuring radiographic parameters in equine forefeet, especially if assessing foot balance and conformation, as unipodal stance significantly affects the mediolateral balance of the DIPJ on DP radiographs and significantly alters the phalangeal axis on LM radiographs.  相似文献

BackgroundThe equine temporomandibular joint (TMJ) has a complex anatomical structure that makes diagnosis of TMJ disorders difficult. Computed tomography (CT) is now available in equine medicine; hence, TMJ evaluation has become more convenient.ObjectivesThe objectives of this study were to describe the CT features of the TMJ in Jeju horses and to compare these features with those of Thoroughbreds.MethodsIn this report, the TMJs of 10 Jeju horses (mean age: 4.5 ± 1.9 yr; mean body weight: 282.6 ± 40.3 kg) and 6 Thoroughbreds (mean age: 7.3 ± 1.6 yr; mean body weight: 479.7 ± 44.0 kg) were examined using CT. After CT scanning, the Hounsfield units (HU) and height to width ratio (H:W) of the mandibular condyle were measured.ResultsThe mean H:W in Jeju horses was significantly lower than that in Thoroughbreds. The mean HU in Jeju horses was lower than that in Thoroughbreds; however, the difference was not significant. The most frequent CT finding was an irregular medial margin of the mandibular condyle in both breeds.ConclusionsIn this study, the shape of the mandibular condyle in Jeju horses was flatter than that in Thoroughbreds. This report could be useful in evaluating the TMJ in Jeju horses. Moreover, CT could be a pragmatic choice for the examination of the TMJ in horses.  相似文献

Objective – To describe the clinical and pathological findings in 2 adult horses with documented increases in intra-abdominal pressure (IAP), and to describe the direct puncture technique used to measure this pressure.
Series Summary – Two adult horses developed increases in IAP secondary to large-volume abdominal effusion. A 9-year-old Quarter Horse cross gelding was presented for evaluation of urinary and neurologic signs. Abdominal ultrasonographic examination showed a hepatic abscess along with abdominal effusion. A 4-year-old Quarter Horse gelding was presented for evaluation of castration complications. A castration site infection extended into surrounding tissues, resulting in peritonitis, abdominal effusion, and severe cellulitis of the limbs. IAP measured in both horses was increased relative to reported equine reference values. Changes in hemodynamic parameters in both horses, notably increased central venous pressure, were consistent with those seen in other species in which intra-abdominal hypertension (IAH) and its sequellae have been documented.
New or Unique Information Provided – Extensive research and clinical literature guides management of humans with IAH and abdominal compartment syndrome. Knowledge of these conditions in companion animal and large domestic species is less well developed. Recent research has established reference values for standing, sedated and recumbent, anesthetized horses. Detailed reports of equine clinical cases of IAH have not been reported in the literature. This report provides information on the clinical, hemodynamic, and pathologic characteristics of 2 horses with measured increases in IAP, and describes the direct puncture technique used to perform these measurements.  相似文献

Involvement of the ventral conchal sinus (VCS) is an important diagnostic and prognostic feature in cases of the equine sinus disease. The authors aimed to ascertain if the caudo-dorsal extension of the VCS, the ventral conchal bulla (VCB) is identifiable on plain radiographs of cadaver skulls without sinus disease. Bilateral frontonasal sinus flaps were made in 10 equine cadaver skulls. Plain lateral, lateral oblique and dorso-ventral radiographs were then obtained followed by the same views taken with stainless steel wire outlining the caudal border of the VCB. Plain radiographs were randomised and blindly evaluated by two observers who marked where they believed the VCB to be positioned. This was then correlated with the true position of the VCB using radiographs with wires in place. The ease of identification of the VCB was classified as 'easy' or 'difficult'. The VCB was correctly identified in 70 per cent of lateral radiographs, but only 45 per cent of lateral oblique radiographs and 17 per cent of dorso-ventral radiographs. If a clinician was confident that he or she could identify the VCB, they were usually correct. Conversely if the clinician judged VCB identification as 'difficult', they usually identified it incorrectly. In the authors' clinical experience, the VCB of horses with sinusitis involving this compartment is more radiologically evident than in clinically normal horses. Knowledge of the normal radiographic anatomy of this structure should aid clinicians in identifying horses with sinusitis affecting the VCS.  相似文献

Equine temporomandibular joint (TMJ) diseases are increasingly recognized as a problem for the well‐being and performance of horses. Diagnosis is confounded by overlap of clinical signs associated with pathology of the oral cavity, poll, and cervical vertebrae. Arthrocentesis for intra‐articular analgesia, sampling of synovial fluid, and medication is needed for diagnostic and therapeutic purposes. Ultrasound features of the normal TMJ and a blind arthrocentesis technique have been described, but a systematic approach to ultrasound‐guided (USG) arthrocentesis has not been reported. Ultrasound guidance allows visualization of the TMJ that may prove beneficial in cases when pathology, abnormal anatomy, or clinician inexperience make blind arthrocentesis difficult. We hypothesized that USG arthrocentesis would result in fewer needle repositions than blind arthrocentesis. We also aimed to assess synovial fluid parameters for normal equine TMJs. A prospective randomized method comparison with crossover experimental design compared the number of needle positionings required for accurate injection of the TMJ using each technique. Arthrocentesis technique and operator experience were tested using cadavers and two operators. Injection success was confirmed using CT. The radiologist then applied both techniques in normal live horses. No statistically significant difference was noted between arthrocentesis techniques or operators (P > .05). No complications were observed in live horses following either technique. Synovial fluid parameters were largely within the normal range expected for other synovial joints. Either blind or USG arthrocentesis of the equine TMJ can be performed with minimal prior operator experience. Ultrasound‐guided arthrocentesis is an alternative method and can be considered in cases with altered anatomy.  相似文献

Horses with intracranial lesions and severe ataxia are not good anesthesia candidates; however, only one method to obtain cerebrospinal fluid (CSF) from the cervical region in a standing horse has been reported. This method is not performed routinely due to the difficulty for sample acquisition. Our hypothesis is that standing cervical centesis can be performed in horses without complication. Ultrasound‐guided centesis of the CSF between C1 and C2 in 11 clinically normal horses and two horses with neurologic signs were performed. Horses were sedated and ultrasound was used to identify the subarachnoid space and spinal cord between C1 and C2. With ultrasound guidance, a needle was introduced into the dorsal aspect of the subarachnoid space using a lateral approach. Ten milliliters of CSF was obtained and analyzed. Two normal horses in this study had moderate red blood cell contamination in the CSF (940 and 612 RBC/μl). One horse had 11 RBC/μl and the remaining horses had <4 RBC/μl. The total procedure time was approximately 2 min. No reaction was observed and no complications were detected up to 48 h after the procedure. Ultrasound‐guided centesis between C1 and C2 is a rapid procedure that causes minimal to no reaction in standing, sedated horses used in this study. The use of ultrasound to guide a standing C1–2 centesis of the subarachnoid space provides an additional route to obtain CSF for analysis in the equine patient.  相似文献

Background — Delayed analysis of blood samples may be caused by restricted access to laboratories. Artifactual changes may occur in the measured analytes as a consequence of delayed analysis and may complicate interpretation of the data.
Objective — The purpose of this study was to characterize artifactual changes in equine blood, due to storage, using the Advia 120 hematology analyzer.
Methods — Samples of blood from 5 horses were analyzed using the Advia 120 soon after collection and again after 24 and 48 hours of storage at either 4°C or ambient laboratory temperature (∼24°C).
Results — Delayed analysis of equine blood samples resulted in increased numbers of normocytic hypochromic RBCs, increased numbers of macrocytic hypochromic RBCs, misclassification of granulocytes as mononuclear cells using the basophil reagent method, and pseudothrombocytosis, due to misclassification of ghost RBCs as platelets. The latter artifact was corrected by an amended version of the software. Many of the artifactual changes were identified by morphology flags.
Conclusion — Characteristic changes in cytograms produced by the Advia 120 allowed recognition of artifactual changes in stored equine blood samples. These changes were less pronounced in samples stored at 24°C than at 4°C.  相似文献

Radiographic evaluation of the entire adult equine thorax in the standing position requires a minimum of four overlapping lateral radiographs. Small horses could often be studied with fewer radiographs. This basic examination provided the optimal radiographic opportunity for detecting lung diseases in the horse. An understanding of normal gross and radiographic anatomy is an absolute necessity if one is to avoid the potential diagnostic pitfalls inherent in radiographic interpretation of the equine thorax. Of equal importance is an appreciation for the technic required to produce thoracic radiographs. The advent of rare-earth intensifying screen and highspeed film systems has enhanced the opportunity for obtaining diagnostic standing lateral equine thoracic radiographs.  相似文献

Contrast paranasal sinusography was a simple procedure that enhanced evaluation of the paranasal sinuses in the standing horse. Two techniques of positive-contrast paranasal sinusography were developed using cadavers and sedated horses. For both techniques, 37% organically bound iodine was used. For the first technique, the conchofrontal sinus was injected with 30 ml of contrast medium followed by an additional 100 ml and 70 ml. For the second technique, the rostral maxillary sinus was injected with 20 ml followed by an additional 50 ml. Each technique enhanced visualization of different paranasal sinuses, and each was necessary for complete positive-contrast radiographic evaluation of the paranasal sinuses. The techniques of positive-contrast radiographic evaluation of the paranasal sinuses are described.  相似文献
