Seven horses were used to compare the Gambee, the crushing, and a 2-layer inverting suture pattern composed of a simple continuous layer in the mucosa oversewn with a continuous Lembert pattern in the seromuscular layer. Horses were evaluated at 30 days for adhesion formation, lumen diameter, and quality of healing at the anastomotic sites. One horse was euthanatized 9 days after surgery after 24 hours of ileus and colic; necropsy revealed septic peritonitis and widespread adhesions. One horse had no adhesions. The remaining horses had adhesions associated with 50% of the Gambee and 50% of the crushing anastomoses. There were no adhesions related to the 2-layer inverting techniques in these 6 horses. There was no significant difference in percentage reduction of lumen diameters between the 3 techniques, and there was no evidence of chronic obstruction resulting from any of the anastomotic techniques. Histologically, the inflammatory response and fibrosis were minimal in the single layer patterns, but there was increased fibrosis and suture tract inflammation in the 2-layer inverting technique.  相似文献

Intravenous administration of sodium fluorescein appears useful but further investigation of the method is required. Signs of mild colic, intermittent lethargy, and weight loss of 6 weeks' duration in a 2-year-old Quarter Horse gelding were attributed to persimmon (Diospyros virginiana) phytobezoar formation. Diagnosis of the phytobezoar was facilitated by gastric endoscopy. Signs of gastrointestinal tract obstruction were associated with a large phytobezoar in the lumen of the stomach, gastric ulceration, and obstruction of the small intestine (as a consequence of fragmentation of the primary bezoar). Conservative treatment, using mineral oil and dioctyl sodium sulfosuccinate, was unsuccessful. A celiotomy was performed, and gastric impaction and partial obstruction of the small intestine associated with phytobezoar formation and fragmentation were identified. The horse made a complete recovery following removal of all phytobezoars. Persimmon phytobezoar should be considered in the fall and winter as a possible cause of lethargy, colic, and weight loss in horses allowed access to persimmon fruit.  相似文献

This reports discusses a perforating metallic wire that was suspected of causing chronic weight loss, inappetance and pyrexia in a 15‐year‐old Standardbred gelding, due to the fact that during the previous 18 months, 6 horses coming from the same yard were referred for recurrent or acute colic related to the ingestion of metallic wires. Splenomegaly was detected ultrasonographically and confirmed during surgery. During necropsy, 2 metallic wires 0.2 mm in diameter and 3–4 cm long were found in a markedly enlarged spleen with several nonencapsulated abscesses. Metallic wire perforation and migration through the lower alimentary tract may involve different abdominal quadrants (intestine, abdominal wall, spleen, liver) and lead to different clinical syndromes as acute or recurrent colic and weight loss. A clinical diagnosis is challenging as the clinical signs are often nonspecific and prognosis is generally considered poor.  相似文献

The microvascular anatomic features of the small intestine was described by correlating results of microangiography, light microscopy, gross studies, and scanning electron microscopy of vascular replicas in 14 horses. After heparinization, the horses were euthanatized, a length of jejunum was transected, and blood was flushed free of the circulation, using isotonic NaCl solution. In six horses, the circulatory system was perfused with a modified radiopaque medium and evaluated radiographically. These sections were then evaluated by standard histologic methods. Sections from 8 horses were perfused with 1 of 2 types of plastics and studied grossly or by scanning electron microscopy. The marginal arterial arcade gives rise to vessels that enter the jejunum at the mesenteric angle. These vessels penetrated either directly, by branching and entering on both sides of the mesenteric angle, or supplying only 1 side of the mesenteric angle. All these vessels continued in the submucosa branching extensively, forming a submucosal plexus. This submucosal plexus supplied the tunica muscularis, tunica serosa, and the mucosa. Vessels within the 2 muscle layers ran parallel to the muscle fibers and, consequently, perpendicular to each other. The arterial supply to the mucosa penetrated the muscularis mucosae and branched to supply 2 mucosal capillary networks. An eccentrically placed arteriole penetrated the base of the villus and spiralled to the tip where it "fountained" into a mesh-like capillary network, which descended peripherally in the villus to drain via 1 to 3, but most commonly 2 venules.
