普通骨板内固定治疗玩具犬桡尺骨骨折的回顾性分析   总被引:1,自引:1,他引:0

本试验旨在评估普通骨板内固定治疗玩具犬桡尺骨骨折的临床效果及并发症。回顾了中国农业大学动物医院使用普通骨板(圆洞骨板或兽医可剪裁骨板)开放式复位和内固定治疗桡尺骨骨折的玩具犬的病历记录,并对这些病例进行回访。入选病例满足以下条件:体重不超过7 kg;回访时间大于12个月;病例信息记录完整。结果显示:共纳入63只犬的64例桡尺骨骨折,49例(76.6%)术后无跛行,7例(10.9%)术后勉强可见到跛行,6例(9.3%)存在轻度跛行,2例(3.1%)存在中度跛行。严重并发症的发生率是6.3%,轻微并发症的发生率是27%。试验表明,普通骨板内固定能有效治疗玩具犬桡尺骨骨折,该方法临床效果良好,且严重并发症发生率低。  相似文献

OBJECTIVE: To compare drilling, tapping, and screw-insertion torque, force, and time for the 4.5-mm AO and 6.5-mm Acutrak Plus (AP) bone screws, and to compare the mechanical shear strength and stiffness of a simulated complete lateral condylar fracture of the equine third metacarpal bone (MC3) stabilized with either an AO or AP screw. STUDY DESIGN: In vitro biomechanical assessment of screw-insertion variables, and shear failure tests of a bone-screw-stabilized simulated lateral condylar fracture. SAMPLE POPULATION: Eight pairs of cadaveric equine MC3s METHODS: Metacarpi were placed in a fixture and centered on a biaxial load cell in a materials-testing system to measure torque, compressive force, and time for drilling, tapping, and screw insertion. Standardized simulated lateral condylar fractures were stabilized by either an AO or AP screw and tested in shear until failure. A paired t test was used to assess differences between screws, with significance set at P < .05. RESULTS: Insertion and mechanical shear testing variables were comparable for AO and AP insertion equipment and screws. CONCLUSION: The 6.5-mm tapered AP screw can be inserted in equine third metacarpal condyles and is mechanically comparable with the 4.5-mm AO screw for fixation of a simulated lateral condylar fracture. CLINICAL RELEVANCE: Considering the comparable mechanical behavior, the potential for less-persistent soft-tissue irritation with the headless design, and the ability to achieve interfragmentary compression by inserting the screw in one hole drilled perpendicular to the fracture plane, the 6.5-mm tapered AP screw may be an attractive alternative for repair of incomplete lateral condylar fractures in horses.  相似文献

External immobilization and traditional pinning techniques for radius and ulna fractures in immature animals are associated with complications. Prolonged splinting and casting risks postural hyperextension. Pinning techniques may injure growing bones and joints. Eight cases are presented in which fractures of the radius and ulna in immature animals were treated with plates and without external splints. Ail fractures healed without iatrogenic injury to the growing bones and joints and without hyperextension problems of fracture disease. Plating proved a highly effective method of treatment for these fractures in growing animals, avoiding complications and the problems associated with external splinting and traditional pinning techniques.  相似文献

Objective —This study evaluates the technique of medial bone plating in the repair of radius fractures in dogs and cats.
Study Design —A retrospective study was made of patients diagnosed with fractures of the radius that were stabilized by application of a bone plate to the medial side of the bone.
Animal Population —Sixteen client-owned dogs and 4 client-owned cats. The animals' ages varied from 5 months to 8 years, and their weight varied from 1.3 to 49 kgs.
Methods —Animals were evaluated using radiography or client assessment, or both.
Results —Medial plate application was technically easier to do than cranial plate application, the technique avoided the extensor tendons, and permitted greater versatility in the selection of smaller plates for the fixation of distal radius fractures. All fractures repaired by this method healed with no postoperative complications.
Conclusions —Medial plate application may be used for osteosynthesis of distal or middiaphy-seal radius fractures. Further studies are needed to evaluate the use of this technique in morphologically complex fractures. In patients with fractures of the proximal radius, cranial plate application is considered more appropriate.
Clinical Relevance —Medial plating of radial fractures has significant advantages compared with conventional plate application; it provides the surgeon with an alternative method of fixation for selected radius fractures in dogs and cats.  相似文献

Objective— To compare stiffness and strength of a dynamic condylar screw plate combined with dorsal broad dynamic compression plate (DCS–bDCP) fixation with double broad dynamic compression plate (dbDCP) fixation used to repair oblique distal fractures of adult equine radii. Study Design— Experimental. Sample Population— Adult equine radii (n=10 pair). Methods— An unconstrained three‐dimensional loading–measurement system was used to determine stiffness of a 50 mm long intact, and then DCS–bDCP or dbDCP‐plated osteotomized/ostectomized segment of radii when subjected to a nondestructive sequence of compression, torsion, and lateral‐to‐medial (LM), medial‐to‐lateral (ML), cranial‐to‐caudal (CrCa), and caudal‐to‐cranial (CaCr) bending. Uniform load over the entire length of construct identified its weakest characteristics during torsion and LM and CrCa bending to failure. Results— No difference was observed between osteotomized/ostectomized DCS–bDCP and dbDCP construct stiffness for all 6 loading modes, and strength for all 3 failure loads. Ostectomized DCS–bDCP and dbDCP construct stiffness was significantly lower than osteotomized radii, the latter approaching intact for axial, LM, and CrCa bending. Most frequent failure was bone fracture through exit site of a screw located adjacent to osteotomy/ostectomy. Conclusions— DCS–DCP and dbDCP constructs had comparable strength and stiffness when repairing osteotomies/ostectomies in equine adult radius bone. Fracture reduction increased stiffness that approached intact bone for loads that placed the unplated side in compression. Clinical Relevance— DCS–bDCP and dbDCP constructs are comparable in stiffness and strength when applied to oblique distal diaphyseal osteotomies/ostectomies in equine radius bone. However, the DCS's localized effect on distal epiphyseal structure because of additional bone removal remains to be investigated under in vivo articular loading conditions.  相似文献

A sagittal fracture of the third carpal bone of a Standardbred racehorse was repaired with lag screw fixation. Healing of the fracture was uncomplicated, and the horse made a successful return to racing. It was concluded that selected sagittal fractures of the third carpal bone may be treated successfully with lag screw fixation. Criteria for selection of cases include fragment size and orientation of the fracture plane.  相似文献

The intention of the comparison of both low and high field was to examine which anatomical brain structures of cats were visible on low field images, as in clinical veterinary practice, 3 Tesla (T) magnets were of limited availability. The research was performed on 20 European short‐haired male and female cats, aged 1–3 years, with body weight of 2–4 kg. 0.25 T magnetic resonance images of neurocranium were acquired in all using T2‐weighted fast spin echo sequences with repetition time (TR) of 4010 ms and echo time (TE) of 90 ms in dorsal and transverse plane, and T2‐weighted fast spine echo sequences with TR of 4290 ms and TE of 120 ms in sagittal plane. Based on a detailed catalogue of feline brain structures visible at 3 T in previously published studies, it was examined which structures were visible on low field images. Anatomic structures were identified and compared to assess the reliability of diagnoses made based on low‐field magnetic resonance imaging. In low‐field scans, 92 structures were identified. Elements of auditory, visual, motor pathways, hippocampus and cerebral ventricular system were distinguished. Low‐field as well as high‐field magnetic resonance imaging support the identification of local tissue lesions, metastasis, focal ischaemia and haemorrhage, disorders associated with ventricular system dilation and hydrocephalus. It also produced accurate images of the hippocampus, which contributes to reliable diagnoses of various forms of epilepsy in cats. Due to technical limitations, a low‐field scanner is unlikely to visualize microtraumas, local inflammations, small haematomas or metastatic tumours.  相似文献

Objective— Compare the biomechanical characteristics of screw and wire fixation with and without polymethylmethacrylate (PMMA) re-enforcement for acetabular osteotomy stabilization in dogs. Animals— Pelves removed from 8 adult mixed breed dogs weighing between 25 and 30 kg. Procedure— The pubic symphysis of each pelvis was split and a central transverse acetabular osteotomy was performed. One hemipelvis from each dog was stabilized with the composite fixation (interfragmentary Kirschner wire, two screws and a figure-of-eight orthopedic wire with PMMA). The contralateral hemipelves was stabilized with an interfragmentary Kirschner wire, two screws, and a figure-of-eight orthopedic wire without PMMA. All hemipelves were tested in bending by using a materials testing machine at a cross head speed of 5 mm/min. An extensometer was placed on the dorsomedial surface of the hemipelves centered over acetabular osteotomy to record distraction of the osteotomy during loading. A load/deformation curve and a load/distraction curve was produced for each hemipelvis. The slope for the initial linear portion of the load/deformation curve and the load/distraction curve, yield load and maximum load sustained were compared between repair groups using a paired t-test with P < .05 considered significant. Results— The slope of the load/deformation curve was significantly greater (P= .001 ) for hemipelves stabilized with the composite fixation (mean ± SD: 69 ± 18 N/mm) compared with hemipelves stabilized without PMMA (mean ± SD: 39 ± 8 N/mm). There was no significant difference (P= .593 ) between repair groups in the slope of the load/distraction curves as measured on the extensometer. Yield load was significantly greater (P= .0002 ) for hemipelves stabilized with the composite fixation (mean ± SD: 184 ± 25 N) compared to hemipelves stabilized without PMMA (mean ± SD: 74 ± 12 N). Maximum load sustained was also significantly greater (P= .013 ) for hemipelves stabilized with the composite fixation (mean ± SD: 396 ± 71 N) compared to hemipelves stabilized without PMMA (mean ± SD: 265 ± 94 N). Failure of hemipelves stabilized with the composite fixation occurred primarily by ventrolateral bending of the cranial and caudal pelvic segments at the osteotomy site. Failure of hemipelves stabilized without PMMA occurred by ventrolateral bending of the cranial and caudal pelvic segments at the osteotomy site with pronounced concurrent ventrolateral rotation of the cranial pelvic segment. Conclusion— PMMA improves the mechanical characteristics of acetabular fracture fixation, at least in part by neutralization of rotational forces. The results of this study justify use of PMMA as a component of the composite fixation when repairing acetabular fractures.  相似文献
