A two-year-old neutered/male mixed-breed dog had received partial maxillectomy for fibrosarcoma. An oronasal fistula occurred as a complication of the surgical procedure. An island palatal mucoperiosteal flap was developed and rotated to repair the oronasal fistula. Acute (1-month) and long-term (8-months) follow-up indicated appropriate healing of the transposed island palatal mucoperiosteal flap with resolution of clinical signs indicative of oronasal fistula.  相似文献

Oronasal fistula is a relatively common complication associated with maxillary canine tooth extraction, problematic healing of maxillectomy, and repair of secondary cleft palate in small animals. Regardless of the clinical scenario associated with oronasal fistula, therapy requires surgical treatment. Principles for surgical repair of oronasal fistula include development of mucosal flaps with excellent vascular supply to transpose over the defect to restore continuity of the nasal and oral cavities. The specific surgical technique may vary but includes either single or double mucosal flaps. Oronasal fistula refractory to multiple attempts at surgical repair may be obturated by using a prosthodontic device.  相似文献

Two surgical attempts using mucosal advancement flaps and single layer closure failed, likely due to inadequate blood supply that was potentially exacerbated by excess tension and trauma on the graft.  相似文献

There are few published reports of equine enterocutaneous fistulae, and they are almost always related to umbilical hernias. Colocutaneous fistula as a result of a penetrating wound has not been reported in horse. In this case report, surgical treatment of a traumatic colocutaneous fistula in an 8-year-old stallion is described. Under inhalation anesthesia, the fistula was removed completely, and the resultant defect on the left ventral colonic wall was closed. The defect of abdominal wall was closed routinely. The horse regained his previous performance, and no surgical complication was observed at 1 year’s follow-up.  相似文献

An 11-year-old, female neutered Labrador retriever was presented with a micro-invasive differentiated papillar adenocarcinoma at the colorectal junction. A colorectal end-to-end anastomosis stapler device was used to perform resection and anastomosis using a transanal technique. A rectovaginal fistula was diagnosed two days later. An exploratory laparotomy was conducted and the fistula was identified and closed. Early dehiscence of the colon was also suspected and another colorectal anastomosis was performed using a manual technique. Comparison to a conventional manual technique of intestinal surgery showed that the use of an automatic staple device was quicker and easier. To the authors' knowledge, this is the first report of a rectovaginal fistula occurring after end-to-end anastomosis stapler colorectal resection-anastomosis in the dog. To minimise the risk of this potential complication associated with the limited surgical visibility, adequate tissue retraction and inspection of the anastomosis site are essential.  相似文献

A 3-month-old Schnauzer was presented with congenital defects of the secondary palate. On the clinical examination, coughing, sneezing, drainage of nasal discharge from the external nares and poor growth were found. Vital signs and results of blood examination were within normal ranges. Thoracic radiography revealed mild pneumonia in the right lung lobes. In a puppy suffering from cleft palates, a palatal prosthesis was applied to the hard palate in order to protect the surgical wound, because a routine surgery was not successful. A palatal prosthesis was applied and held in place using the instant glue and plastic bands to protect the surgical wound following the third repeated surgery. Although a small oronasal fistula still remained, there was no functional defect. This prosthesis was easy to apply and helpful to protect the surgical wound. In addition, this implant could be placed or adjusted without or sedation/anesthesia.  相似文献

Multiple ligament injuries of the canine and feline stifle joint which result in luxation are uncommon. Two cats and one dog, that had sustained such a joint injury were surgically treated. A rupture of the cranial and caudal cruciate ligaments, and at least one collateral ligament was observed in all of the three animals. Prosthetic reconstruction was used, as previously described, in combination with a novel technique of intraoperative placement of a temporary trans-articular pin (TTP) to maintain intra-operative anatomical reduction. TTP placement facilitated maintenance of joint alignment during surgical reconstruction and aided appropriate tensioning of the prosthetic sutures, preventing collapse of femorotibial joint compartments. The TTP was removed prior to closure of the joint allowing immediate post-operative joint mobilisation. Based on assessment by their owners, all the animals made a complete recovery. TTP was considered a relatively simple and effective adjunctive aid for surgical treatment of traumatic luxation of the stifle joint.  相似文献

The medical records of 171 dogs with traumatic hip luxations were examined. German shepherds, mixed breeds, and poodles were significantly overrepresented. The mean age of the patients at the time of diagnosis was 4.4 years (range 5 months to 14 years). Vehicular trauma caused 83% of the 133 luxations where the etiology was known; 53% had at least one other diagnosed traumatic injury. Long-term results were obtained from owner questionnaires (25 dogs) or clinical and radiographic examinations (11 hip joints). The short-term recurrence rate after closed reduction (47.3%) was higher than that after surgical reduction using any of the three surgical techniques most frequently used here (9.5, 12.5, and 14.3%), even when the operation was performed after failure of a closed reduction (11.8%). Excluding dogs that had a femoral head ostectomy, 68% (17 of 25) of the dogs had a normal gait, 16% had only a mild lameness, and 16% had more severe lameness when evaluated on an average of 31 months after repair. Of 11 hip joints radiographed on an average of 25 months following repair, five had no radio-graphic abnormalities related to the luxation and six showed one or more of the following: periosteal new bone formation, femoral head subluxation, partial resorption of the femoral head, periarticular osteophytes, and subchondral bone erosion. Closed reduction should be the initial treatment of choice in uncomplicated luxations even though the rate of reluxation is high, because it avoids the need for surgery in approximately one half of affected dogs and does not increase the recurrence rate following subsequent surgical reduction.  相似文献

This article reports a case of a one-year-old Quarter Horse filly with an enterocutaneous fistula resulting from an umbilical hernia since birth, treated successfully by en bloc resection of the hernial sac with the fistula and closure of the ileum with a modified Heineke-Mikulicz technique. This consisted of closing the intestinal wound transversely to the long axis after excision of the fistula to help preserve a sufficient intestinal lumen and prevent potential stricture formation after longitudinal closure. Umbilical hernias are reported to have an incidence of 0.5–2.0% usually resolve spontaneously. Reducible hernias do not represent a surgical emergency whereas incarcerations should always undergo surgery as soon as possible. Enterocutaneous fistulae occur uncommonly as a result of congenital umbilical herniae that developed spontaneously into Richter's herniae or Littré herniae or they are induced traumatically or iatrogenically after therapeutic attempts. In most cases, enterocutaneous fistulae do not require immediate surgery. However, the excision of the fistula should be scheduled within a few days after diagnosis. In the current case, the owner reported a healthy development of the filly after 1 year without any functional problems. This case represents an example of successful use of the Heineke-Mikulicz technique to establish an alternative to standard end-to-end anastomosis.  相似文献

The case records of 20 cows with either a third-degree perineal laceration or rectovestibular fistula were reviewed to ascertain the signalment, history, treatment, and long-term result of treatment. Fifteen cows, including 10 first-calf cows, had third-degree perineal lacerations that occurred at calving. Surgery was done in 14 of 15 cows; 10 of the 14 (71%) remained fertile. The cow that was not treated surgically was culled after 24 months because of infertility. Five cows had a rectovestibular fistula; 4 of these were treated surgically. The cow that did not have surgery healed by second intention and remained fertile, and 3 of the 4 cows in which surgery was performed were fertile. None of the cows that produced calves after the initial injury suffered a perineal laceration at subsequent calvings. Single-stage surgical repair of third-degree perineal laceration or rectovestibular fistula appeared to have a good prognosis for subsequent fertility in cows.  相似文献

OBJECTIVE: To report the diagnosis and surgical treatment of a case of intraventricular tension pneumocephalus in a dog after a transfrontal craniectomy for removal of a falx cerebri meningioma. STUDY DESIGN: Case report. ANIMAL: A 12-year-old spayed English springer spaniel. RESULTS: Intraventricular air and a fistula between the craniectomy site and ventricular system were identified by magnetic resonance imaging. Prompt repair of the dural defect using prosthetic dura mater resulted in immediate regression of the neurological signs and cerebral spinal fluid rhinorrhea. Magnetic resonance imaging repeated 8 weeks after surgery showed complete resolution of pneumocephalus. CONCLUSIONS AND CLINICAL RELEVANCE: Tension pneumocephalus is an uncommon but life-threatening complication of craniectomy that requires urgent diagnosis and treatment.  相似文献

The purpose of this case report was to describe the surgical correction of a cholecystocutaneous fistula in a dog. A 6 yr old Vizsla presented with a 2 mo history of a chronic draining wound on the right ventral thorax. Diagnostics revealed numerous fistulous tracts opening at a single site on the right ventrolateral chest wall, extending caudodorsally through the chest wall and diaphragm to the region of the right medial liver lobe. Exploratory laparotomy revealed the apex of the gallbladder adhered to the diaphragm with a tract of fibrous tissue extending along the diaphragm laterally to the right thoracic wall. Cholecystectomy was performed. The fistulous tract was incised to expose the lumen of the fistula, and the fistula was omentalized. Twenty-eight months after surgery, the dog had had no recurrence of the fistulous tract. Exploratory laparotomy allowed excellent visualization of the intra-abdominal path of the fistula and facilitated the ease of resection of the source. Cholecystectomy resulted in rapid and complete resolution of the fistula without the need for excision of the fistula. Although rare, gallbladder disease should be a differential for chronic fistulous tracts.  相似文献

A six-year-old, female Tibetan terrier was referred for investigation of a non-healing wound on the left caudal thorax. A subcutaneous swelling had initially developed on the chest wall, followed by a draining tract from which seropurulent fluid drained for two months. There had been no response to antibiotic treatment. Following radiographic and ultrasonographic examinations, a bone sequestrum from a fractured rib or a foreign body was suspected. Surgical exploration of the wound identified a sinus tract and a wooden foreign body (an ice-lolly stick) was located in subcutaneous tissues. Partial wound dehiscence of the surgical site occurred postoperatively, but healed after 10 days. One month later, fluid began to discharge from the area again. Further surgical exploration confirmed a gastrocutaneous fistula. Dissection of the fistula and surgical closure of the stomach, body wall and skin led to resolution of all signs.  相似文献

A Thoroughbred gelding was referred for treatment of a parotid duct fistula. This had developed after the referring veterinarian had excised a portion of the duct, and ligated the proximal end, as a means of treating a recurring sialocoele. This report describes the successful surgical reconstruction of the excised 15 cm portion of the parotid duct using an interposition polytetrafluoroethylene tube graft.  相似文献

OBJECTIVE: To describe repair of chronic palatine defects in cats, with free cartilage graft harvested from either the pinna (scapha) or vertical ear canal (annular cartilage). STUDY DESIGN: Retrospective study. ANIMALS: Cats (n=5) with chronic oronasal fistula. METHODS: Cartilage was harvested from either the scapha or annular cartilage and epithelium removed. After preparation of the edges of the palatal defect by separation of the oral and palatal mucosa circumferentially, the graft was inserted between the epithelial layers and secured without tension. The graft acted as a scaffold for migration of granulation tissue and epithelialization. RESULTS: Oronasal fistulae were successfully repaired in 5 cats (3 pinna grafts; 2 annular cartilage grafts); 1 cat required a second graft after the first graft was dislodged. CONCLUSIONS: Auricular cartilage provides a reliable framework for repair of oronasal fistulae in cats. CLINICAL RELEVANCE: Chronic oronasal fistulae refractory to conventional repair can be treated by use of auricular free graft (pinna or annular cartilage) without disfigurement. Use of annular cartilage to support palatal repairs offers surgeons an additional option when other methods have failed. We recommend using conchal cartilage as the initial treatment approach for repair of small oronasal fistulae in cats.  相似文献

The records of 33 dogs that had perianal fistula disease treated with en bloc surgical excision and bilateral anal saculectomy, and that were perioperatively administered an exclusive white fish and potato diet, were reviewed to determine outcome. By 1 year after surgery, 87.9% of the dogs had complete or near-complete resolution of visible fistula disease, while only 20.7% continued to have mild intermittent clinical signs. Fecal incontinence was not reported in any dog. Overall, complications were considerably less in both severity and frequency when compared with previous reports.  相似文献

Five dogs with rectovaginal fistula and atresia ani that had been treated by surgical correction of the malformations were studied retrospectively. Ages at presentation varied from 1 to 3 months and weight from 350 g to 7.5 kg. The histories included voiding of feces through the vulva, with or without tenesmus, usually observed after weaning. Atresia ani, presence of feces in the vaginal canal, abdominal distention, and discomfort on abdominal palpation were observed during clinical examination. Also, 3 dogs had partial tail agenesis. In all dogs, the rectovaginal fistula was isolated and transected, the vulvar and rectal defects were closed separately, and the atresia ani was repaired. Normal defecation was restored, but 1 dog had fecal incontinence that subsequently resolved. One dog died 2.5 months postoperatively, and follow-up was done on the others for periods ranging from 1.6 year to 7.7 years. Surgical correction in dogs with rectovaginal fistula and atresia ani may result in a favorable outcome, if it is done early.  相似文献

Recurrent laryngeal neuropathy usually results in left‐sided laryngeal hemiparesis or hemiplegia and is one of the most common problems of the upper respiratory tract in equids. There are multiple methods of management, and the laryngoplasty procedure is still the most common surgical treatment. Different nonabsorbable suture materials and techniques are used for laryngoplasty. Post surgical complications are not uncommon. Of these complications, coughing is the most common. The following case report describes diagnosis and treatment of 2 horses with chronic coughing after laryngoplasty due to infection at the surgical site and laryngeal fistula formation. In both horses an elastic material (probably Lycra) had been used as part of the prosthesis implant. This material had deteriorated into multiple small pieces making removal difficult, and therefore use of this material for a laryngoplasty procedure should be avoided.  相似文献

A 24-yr-old, male western lowland gorilla (Gorilla gorilla gorilla) was diagnosed with congestive heart failure using transesophageal and transthoracic echocardiology. New York Heart Association (NYHA) Class III was assigned to the severity of the condition. Over 16 mo, this progressed to NYHA Class IV despite increasing medical therapy. Repeated evaluations suggested that implantation of a cardiac resynchronization therapy device with a defibrillator (CRT-D) could benefit this animal based on clinical signs and underlying evidence of dyssynchrony and suspected fibrotic myocardial disease. Surgical implantation of leads into the right atrium, right ventricle, and left ventricle was accomplished. The CRT-D device was placed under the thoracic pectoral muscles during an initial surgical procedure. Improvement in the gorilla's clinical condition after implantation of the CRT-D device was immediate and dramatic. Subsequent scanning of the device was accomplished through operant conditioning. The data from these device interrogations included stored and real-time cardiac data, which were used to minimize recognized environmental stressors and change device settings. Over 4 yr, case management was critical to successful device use in treatment of the clinical disease. This involved medications, training for device interrogation, exercise to increase activity and improve body condition, and phlebotomy attempts. Dietary management was necessary to manipulate caloric and sodium intake and encourage medication compliance. Cardiac resynchronization therapy device implantation, although requiring specialized equipment and surgical skill, appears to be a viable option for treatment of fibrosing cardiomyopathy with systolic dysfunction in gorillas refractory to medical management. In addition to treatment, this device provides cardiovascular data at rest that could allow for early diagnosis and treatment of gorillas with this and other cardiac conditions in the future. This describes the comprehensive medical, husbandry, and training techniques necessary to successfully manage this intense clinical case in conjunction with intracardiac device therapy.  相似文献

Clinical and radiological features of 16 horses with fractures of the lateral malleolus of the tibia are reported. The paper describes surgical techniques used, results obtained and discusses justification for removal. Fourteen fractures were unilateral and two bilateral. There was no left:right disparity. The history included a known traumatic incident in 14 cases. All animals had a tarsocrural joint effusion and 10 had palpable thickening of the lateral collateral ligaments. Crepitus was also palpable in 10 horses. The fracture was identified in all dorsoplantar and 14 of 18 dorsomedial-plantarolateral oblique radiographic projections. Nine fractures were simple and 9 comminuted. All fractures were removed via a tarsocrural arthrotomy. Approaches were dorsolateral in 14 joints, plantarolateral in 3 and dorsolateral and plantarolateral in 1. After an ascending exercise programme horses returned to work 6 months after surgery: 15 horses were free of lameness after 17-62 months, with 13 animals performing at a level similar to pre-injury standard.  相似文献
