A 3‐year‐old Arabian mare presented with diarrhoea and depression. During hospitalisation, the mare developed septic thrombophlebitis associated with a jugular catheter. One week later, she developed secondary bilateral tarsocrural joint septic arthritis. Following surgical intervention and intensive medical therapy, the mare recovered successfully. The case highlights an unusual and previously unreported complication of septic thrombophlebitis.  相似文献

OBJECTIVE: To determine the rate of development of septic arthritis after elective arthroscopy and evaluate associations between various factors and development of this complication in horses. DESIGN: Retrospective case series. ANIMALS: 682 horses that underwent arthroscopic procedures at the University of Illinois Veterinary Teaching Hospital from 1994 to 2003. PROCEDURES: Information pertaining to signalment, joints treated, whether antimicrobials were administered, and development of postoperative septic arthritis was collected from medical records. Horses with a primary problem of septic arthritis or wounds involving joints were excluded. The following factors were evaluated to determine their roles in joint sepsis: breed, sex, joint, and preoperative and intra-articular administration of antimicrobials. Telephone interviews with clients were used to determine whether unreported septic arthritis had developed. RESULTS: 8 of 932 (0.9%) joints in 7 of 682 (1.0%) horses that underwent arthroscopy developed postoperative septic arthritis. Follow-up information after discharge from the hospital was available for 461 of the 682 horses, and of those, 8 of 627 (1.3%) joints in 7 of 461 (1.5%) horses developed septic arthritis. Breed and joint treated were significant risk factors for development of postoperative septic arthritis, with draft breeds and tibiotarsal joints more likely than others to be affected. Sex, preoperatively administered antimicrobials, and intra-articularly administered antimicrobials were not associated with development of postoperative septic arthritis. CONCLUSIONS AND CLINICAL RELEVANCE: Results can be used for comparison with data from other institutions and surgical facilities. Additional precautions should be undertaken when arthroscopic surgery involves draft breeds and tibiotarsal joints.  相似文献

A 3-month-old Quarter Horse colt sustained a penetrating injury over the coronary band on the right forelimb resulting in an articular bone sequestrum and septic arthritis of the distal interphalangeal joint. Despite aggressive treatment for septic arthritis, severe osteoarthritis and ongoing sepsis resulted in persistent lameness. Facilitated ankylosis of the distal interphalangeal joint was performed using a combination of open surgical debridement, placement of an autogenous cancellous bone graft and stabilisation within a transfixation cast. Bony ankylosis of the joint was observed radiographically 7 weeks following surgery. Evaluation of the horse 30 months following surgery revealed complete fusion of the middle and distal phalanges, and the distal sesamoid bone. The horse is pasture sound and is used as a breeding stallion. This report describes the surgical approach and case management employed to achieve facilitated ankylosis as a treatment for unresolved septic arthritis of the distal interphalangeal joint. This is the first report of successful facilitated ankylosis of the distal interphalangeal joint in a foal.  相似文献

In the neonatal foal septic arthritis is an important cause of morbidity that may limit future athletic performance. This is often despite a good prognosis for resolution of joint infection. This article presents a review of the diagnosis and treatment of septic arthritis in foals less than 2 months of age, including an overview of our current understanding of inflammatory joint disease in the neonate. Management options are described, with emphasis on tailoring therapy to the individual foal based on accurate staging and monitoring of the disease process. Special considerations relevant to the immature animal are addressed.  相似文献

The objective of this retrospective study was to determine the occurrence of joint-related complications after elective arthroscopy of the tibiotarsal joint (TTJ) in 329 horses, and the association with specific clinical parameters. Data were collected from medical records of horses undergoing elective tibiotarsal joint arthroscopy for fragment removal. Exact conditional univariate regression was used to determine significant risk factors for joint-related post-operative complications.Of 485 joints, 2 (0.4%) developed surgical site infection, 4 (0.8%) developed septic arthritis, 1 (0.2%) developed synovial fistula. There was a significantly increased odds of having septic arthritis as height and length of the distal intermediate ridge of the tibia (DIRT) lesion increased. The median height and length of the DIRT fragments in affected cases was 13.5 mm and 18.0 mm, respectively. For each unit (1 mm) increase in height, there was a 42% increase in the risk of septic arthritis occurrence (P = 0.0042), and a 15% increase for each unit increase in length (P = 0.035). Horses were significantly less likely to develop septic arthritis when suture smaller than USP 0 was used.Horses with larger osteochondritis dissecans lesions of the DIRT region have an increased risk of developing septic arthritis following fragment removal.  相似文献

Septic arthritis in horses is a serious disease which can become life-threatening. In case the infection can be eliminated before irreversible joint damage occurs, complete recovery is possible. Osteomyelitis and/or bone sequestra are commonly reported in equids following traumatic wounds. This report describes the clinical management and outcome following an arthrotomy of the temporomandibular joint of a 1-year-old Quarter Horse gelding with septic osteomyelitis and arthritis. Computed tomography was used for further assessment of the lesions within the temporomandibular joint and helped guide surgical planning.  相似文献

OBJECTIVE: To provide information on the clinical features, diagnosis and treatment of bacterial septic arthritis in dogs. DESIGN: A retrospective study examining case records of all dogs diagnosed with bacterial septic arthritis at Murdoch University Veterinary Hospital between 1988 and 1997. RESULTS: Nineteen dogs were diagnosed with bacterial septic arthritis, which most commonly occurred after surgery involving the stifle joint. Haematogenous infection occurred in only five dogs. Diagnosis was based on clinical signs, joint fluid analysis, radiography, microbiology and/or response to treatment. Chronic lameness was the most common problem at presentation. Analysis of joint fluid invariably revealed large number of nucleated cells, which consisted primarily of neutrophils. In all but one case the neutrophils were nondegenerate. Culture of joint fluid was frequently successful. Staphylococcus spp were the most common bacteria isolated. Treatment involved antimicrobial drugs only in five dogs. Other dogs received antimicrobial drugs in combination with surgical procedures such as joint lavage and removal of nonabsorbable suture material (eight), arthrodesis (two) or amputation (one). Two dogs were euthanased. Most dogs responded well to treatment and were free of signs of septic arthritis at follow-up. CONCLUSION: Bacterial septic arthritis may often be mild and manifest as chronic lameness. Analysis of joint fluid will detect an inflammatory arthropathy but the presence of toxic neutrophils should not be relied on as an indicator of sepsis. Culture of infected joint fluid is likely to be successful if antimicrobials are not given prior to collection and if the sample is inoculated into enrichment broth. Treatment should involve antimicrobial drugs, open-joint lavage and removal of joint prostheses if the infection is associated with previous surgery.  相似文献

Diagnosis of septic arthritis in the distal interphalangeal joint (SADIJ) is often difficult, particularly when arthrocentesis or probing of the joint space are inconclusive. The aim of this prospective study was to investigate the usefulness of ultrasonography in reinforcing a tentative clinical diagnosis of SADIJ in 60 Holstein–Friesian cows with septic arthritis in one distal interphalangeal joint (DIJ). The diagnosis of septic arthritis was based on the results of clinical examination and radiographic images, as well as surgical findings. The DIJ of the partner claw of the same foot and the normal DIJs of 18 healthy Holstein–Friesian cows were used as controls. Visualisation, echogenicity and flow phenomena of the synovial fluid were assessed ultrasonographically. The size of the dorsal pouch of the distal interphalangeal joint (DPDIJ) was determined by measuring two defined distances.The DPDIJ could be imaged in all cows with SADIJ, although the quality of the image varied from good (63%) or moderate (30%) to poor (7%). The echogenicity of the synovial fluid in joints with SADIJ did not differ from those of the partner claws. Flow phenomena were seen in only 30% of the DPDIJs with septic arthritis and in none of the DPDIJs of the partner claws. The DPDIJ was larger in DIJs with septic arthritis than in those of the partner claws and controls (P < 0.05). The dorsopalmar/plantar distance of the DPDIJ indicated a suitable sensitivity (0.97) and specificity (1.0) for the diagnosis of septic arthritis when a threshold value of 6 mm was used. Ultrasonographic examination of the DIJ appeared to be a reliable method for confirming a tentative clinical diagnosis of septic arthritis in cattle.  相似文献

A 36-day-old, 100-kg Thoroughbred colt was presented with a sudden onset of abnormal head carriage and pyrexia. Radiography revealed an osteomyelitis of the vertebral body of the second cervical vertebra. Ultrasonography revealed abscessation in the region of the atlantoaxial articulation on the left side, and fluid aspirated from the abscess confirmed suppurative inflammation. Surgical management was not feasible on economic grounds, and therefore this case was managed with aggressive medical therapy based on the most likely pathogens involved. To provide prolonged antibiotic therapy within the client’s budget, the antibiotic flucloxacillin (Staphlex, 500 mg) was used at a dose of 10 mg/kg by mouth, three times daily. Thirty-eight days after first presentation, the foal was euthanized because of chronic pain and deterioration in neurologic signs. A diagnosis of chronic osteomyelitis of the body of the second cervical vertebra (axis) was made with concurrent septic arthritis of the atlantoaxial joint involving the articular surface of the first cervical vertebra (atlas) and cervical myelopathy of the spinal cord within the first two vertebrae. No pathogens were isolated post mortem. This is the first report to describe the use of flucloxacillin in a foal. Although the outcome was fatal, the medical therapy provided could be considered successful at sterilizing the bony lesion.  相似文献

The objective of this retrospective article was to describe the use of, and to determine long-term outcome of, tibiotarsal arthroscopy in dogs. The medical records of 20 client-owned dogs with tibiotarsal joint disease with arthroscopic treatment were reviewed. Long-term follow-up evaluation of lameness, force plate gait analysis, and radiographs to assess progression of degenerative joint disease (DJD) were performed. Arthroscopy was utilized in the diagnosis of talar osteochondritis dissecans (OCD), collateral ligament injury, septic arthritis, immune mediated arthritis, and a distal talar fragment. Sixteen joints with OCD treated resulted in 10/14 dogs with lameness after exercise only, progression of DJD in most cases, and chronic lameness when comparing operated to unoperated limbs with force plate evaluation at a mean follow-up of 35 months. Following treatment, three dogs with collateral ligament injury had reduced weight bearing on the operated limb, radiographic progression of DJD, and minimal lameness at a mean follow-up of 27 months. Tibiotarsal arthroscopy can be successfully used to help diagnose, and often to treat: OCD, collateral ligament injury, fractures, septic and non-septic arthritis in the dog. The minimally invasive nature of arthroscopy preserved joint stability while allowing complete examination of the articular cartilage. In most cases long term tibiotarsal DJD advancement was the rule.  相似文献

This case report describes the successful management of a chronic multidrug resistant infection of the antebrachiocarpal joint with a synovial‐cutaneous fistula 7 weeks after initial trauma in a 14‐year‐old Icelandic stallion. Resolution of the multiresistant Enterobacter cloacae (extended‐spectrum β‐lactamase) infection from the joint and complete closure of the fistula was achieved by arthroscopic lavage under general anaesthesia repeated on 3 occasions, surgical debridement, ultrasound assisted wound therapy and vacuum assisted closure. Lavage of the wound with the vacuum assisted closure in place was facilitated via the novel VeraFlo instillation system consisting of specially designed therapeutic regulated accurate care pad tubing. The combination of these therapies resulted in resolution of the septic arthritis and fistula. Six weeks after admission the horse was discharged from the hospital with a completely healed wound and was not lame at a walk.  相似文献

OBJECTIVE: To report use of mandibular condylectomy for treatment of advanced degenerative joint disease of the temporomandibular joint (TMJ) caused by sepsis. DESIGN: Clinical case report. ANIMALS: Two-year-old Noriker filly. METHODS: Computed tomography (CT) was used to confirm advanced degenerative joint disease of the left TMJ and for follow-up after mandibular condylectomy and removal of necrotic meniscus. RESULTS: Painful swelling (3 weeks duration) with drainage located just caudal to the left lateral canthus of the eye was associated with atrophy of the left masseter muscle, masticatory problems, and inappetence. There was incisor malocclusion with a 1.4 cm resting lateral mandibular deviation to the right and inability to open the mouth more than a few centimeters. Left mandibular condylectomy combined with meniscectomy and thorough debridement of the septic left TMJ resulted in resolution of clinical signs and complete return of function. Streptococcus zooepidemicus, intermediately resistant to penicillin and subsequently to enrofloxacin, was isolated from the wound. Regeneration of a mandibular "pseudocondyle" was evident on CT at 4 months. At 4 months and 1 year the filly had maximal mouth opening >10 cm and no malocclusion. CONCLUSION: Mandibular condylectomy can be used to facilitate management of septic TMJ arthritis. There was no radiologic or clinical evidence of TMJ ankylosis >1 year after meniscal removal. CLINICAL RELEVANCE: Mandibular condylectomy, including meniscectomy, is an acceptable treatment option for advanced TMJ septic arthritis and allowed return of normal masticatory function.  相似文献

The aim of this study was to develop a specific myeloperoxidase (MPO) activity assay in the synovial fluid of horses and investigate whether MPO activity is increased in different forms of joint diseases. Synovial fluid samples were taken from affected joints from horses with osteoarthritis, chronic non-septic arthritis and septic arthritis, and from healthy control horses. MPO activity was measured using a specific modified o-dianisidine-assay containing 4-aminobenzoic acid hydrazide as a potent and specific inhibitor of the MPO. This assay is characterized by high reproducibility. The results reveal only a slight elevation of MPO activity in the synovial fluid of horses with osteoarthritis and chronic non-septic arthritis. However, in the cases of septic arthritis a significant increase in MPO activity was found when compared to the controls. In conclusion the first field study suggests that synovial fluid MPO may be used as a marker for septic arthritis in horses.  相似文献

A retrospective study was made of 58 horses with open joint injuries admitted to the veterinary teaching hospital. Twenty-five (43%) were admitted within 24 hours of injury, 15 (26%) were examined 2 to 7 days after injury, and 18 (31%) were evaluated a week or more after the initial injury. The joints of the lower portions of the limbs most commonly were affected, with injuries that varied from puncture wounds to severe lacerations with soft tissue deficits. Diagnosis was made on the basis of clinical signs and results of synovial fluid analysis and radiography. Radiography was found to be an important diagnostic tool, indicating joint involvement in 80% of horses on which it was done. Sixteen horses were euthanatized on the day of admission; of the horses treated, 53% that were examined within the first 24 hours developed septic arthritis, and the overall survival was 65%. Ninety-two percent of horses examined within 2 to 7 days of injury developed septic arthritis, with 38.5% surviving; all horses evaluated a week or more after joint injury had septic arthritis, and 50% survived. The prognosis for return to function was best in horses that were examined within the first 24 hours. Horses examined more than 24 hours after injury had a significantly (P less than 0.05) higher chance of developing septic arthritis, and thus, were significantly (P less than 0.0014) less likely to survive the injury.  相似文献

Equine neonatal septicaemia was confirmed in 24 foals hospitalised at the Rural Veterinary Centre between 1989 and 1992 with suspected septicaemia. Septicaemia was confirmed by culture of bacteria from blood of live foals and tissues obtained at necropsy of foals that died or were euthanased. Pathogenic bacteria isolated were predominantly Enterobacteriaceae (including Escherichia coli and Salmonella serovars) and Actinobacillus equuli. Clinical manifestations of septicaemia included signs of depression, dehydration, abnormalities in body temperature and manifestations of localised infection including diarrhoea, pneumonia, and septic arthritis. Most common haemato-logical abnormalities were neutropenia and increase of circulating band neutrophils. Survival rate of foals with confirmed septicaemia was 70.8%. Survival was found to be less likely in the presence of pneumonia, severe signs of depression, marked haematological changes or septic arthritis at the time of admission. Seven foals were confirmed to have septic arthritis without concurrent septicaemia. Of these, 4 had multiple joint involvement. Bacteria isolated from infected joints were predominantly Salmonella serovars. Four foals with septic arthritis failed to survive, due to multiple joint infection, which was unresponsive to treatment. The clinical and haematological abnormalities present in foals with confirmed septicaemia and septic arthritis were consistent with those observed in other studies. The bacterial isolates from foals with confirmed septicaemia were similar to those isolated in other studies. In contrast, the bacteria isolated from foals with septic arthritis without concurrent septicaemia were different from other studies.  相似文献

Matrix metalloproteinases (MMPs) are important enzymes found in connective tissues and thought to be involved in cartilage degradation. They are detectable in bovine synovial fluid and may play a destructive role in bovine septic arthritis. The MMP gelatinase enzymes were detected by gelatin zymography using image analysis of the gels. The active gelatinase levels were determined by a gelatin degradation enzyme-linked immunosorbent assay (ELISA). Increased concentrations of MMP-9 activity were found in the synovial fluids of cows with septic arthritis (P < 0.001) in comparison with fluids from normal joints. Using the gelatin degradation ELISA the net active gelatinases were measured, and significant increases were found in gelatinase bioactivities in synovial fluids from septic joint disease cases (P < 0.001). Increased concentrations of MMP-2 activity were found in the synovial fluids of cows with aseptic arthritis, which appeared to be playing an important role in degradation of articular cartilage in joint disease. An eight-year old, Rottweiler was presented with a chronic non-weight bearing right pelvic limb -lameness and a discharging sinus at the right stifle joint. Fifteen weeks previously a diagnosis of bacterial infective arthritis had been made, presumed attributable to haematogenous spread. Failure to respond to appropriate surgical and medical management resulted in severe joint dysfunction. The poor prognosis for restoration of limb function resulted in limb amputation. Subsequent exploration of the sinus tract and the stifle joint revealed a stick within the craniolateral aspect of the joint. This penetrating stick injury had been responsible for the persistent bacterial infective arthritis.  相似文献

OBJECTIVES: To characterise the presenting signs and pathological changes of canine bacterial infective arthritis in 31 dogs, and to document the response to different treatment regimens. Risk factors that may predispose joints to bacterial infective arthritis and influence the success of treatment were also investigated. METHODS: A retrospective review of cases of bacterial infective arthritis that were presented to three university veterinary referral hospitals over a five-year period (January 1997 to January 2002) was performed. RESULTS: The elbow joint (38 per cent) and stifle joint (44 per cent) were most commonly affected. Radiographic changes consistent with pre-existing osteoarthritis were identified in 14 joints, which had no history of previous surgery (articular or periarticular) or penetrating wound. No significant difference (P = 0.78) was identified between the outcome of combined surgical and medical management, and medical management alone. There were trends for poorer outcomes with increased bodyweight of the dog, longer duration of lameness and a higher nucleated cell count of the affected joint fluid at presentation. The overall infection rate for articular surgical procedures at one institution was 1-3 per cent. CLINICAL SIGNIFICANCE: Medical and/or surgical management were usually successful in resolving infection (94 per cent). The medical records of 12 horses with septic arthritis of a distal interphalangeal joint were reviewed to determine clinical features and response to treatment. Sepsis was caused by trauma or an injection that resulted in an open or contaminated distal interphalangeal joint. All horses were severely lame. Treatment included broad-spectrum parenterally administered antimicrobial drugs (ten horses), percutaneous through-and-through joint lavage (eight horses), indwelling drains (three horses), immobilization of the limb in a cast (three horses), intraarticular injection of sodium hyaluronate (one horse), intraarticular injection of antimicrobial drugs (five horses), curettage of the distal phalanx (one horse), and cancellous bone grafting to promote fusion (one horse). Five horses were euthanatized. Ankylosis of the affected joint developed in five horses, four of which are pasture sound. Two horses treated medically are sound although one underwent subsequent palmar digital neurectomy for treatment of navicular syndrome.  相似文献
