Objective Examine prognostic factors that may indicate when surgical lens removal is indicated to prevent vision‐threatening complications in patients presented following traumatic perforating corneal laceration with associated lens capsule disruption. Procedures Seventy‐seven patients (10 cats and 67 dogs) were evaluated with this injury; of these, 47 were presented acutely and treated surgically and/or medically. Successful outcome was defined as functional vision in the treated eye. Results The 47 patients with acute injuries were divided into 3 treatment groups A‐C for comparison; A ‐ corneal repair/lens removal (n = 15), B ‐ corneal repair/no lens removal (n = 9), C – medical management (n = 23). Groups A and B showed a significantly greater rate of vision loss compared to Group C that was most obvious greater than 18 months post‐injury (P = 0.029 and 0.0097, respectively). Cox proportional hazards regression analysis found a significantly higher increased rate of vision loss in Group A (HR = 4.5; P = 0.023) and a higher but nonsignificant increased rate of vision loss in Group B (HR = 3.0; P = 0.23) compared to Group C after controlling for age and cause of injury. The length of the corneal laceration and time interval from injury to referral were also relevant prognostic factors. Conclusions Medical management is an appropriate therapy for patients of all ages suffering perforating corneal injuries with associated lens capsule disruption. Patients with corneal injuries requiring surgical repair or managed by lens removal following corneal repair suffered vision‐threatening complications approximately 3 to 4.5 times the rate of patients treated by medical management, respectively. Favorable prognostic signs for treatment by medical management include good corneal wound apposition and a formed anterior chamber without uveal prolapse or continued aqueous leakage.  相似文献

A modified one-stage surgical repair was performed on eight mares that suffered from third-degree rectovestibular lacerations. The rectovestibular septum was reconstructed by three lines of sutures using polydioxanone. Primary healing was achieved in seven mares. Four mares got pregnant after the surgery, and no further injury was noticed at subsequent delivery. Complications of the currently used technique included pneumorectum (in one mare) and rectovestibular fistula (in another mare). The obtained results indicated that mares with third-degree rectovestibular lacerations are candidates for this modified one-stage technique with satisfactory outcome for subsequent fertility.  相似文献

Horses with third-degree perineal lacerations require surgical treatment. To reduce the risk of postoperative dehiscence of the surgical site caused by tension, bacterial contamination, and mechanical abrasion, a preoperative and postoperative fasting up to 14 days was performed. A complete blood cell count, concentrations of electrolytes, urea, creatinine, glucose, lactate, cholesterol, triglycerides, total protein, and the activities of aspartate aminotransferase, glutamate dehydrogenase, γ-glutamyl transferase, creatine kinase, and alkaline phosphatase were determined. Fasting resulted in a marked decrease in intestinal motility and an increase in the concentrations of triglycerides and cholesterol. The activity of the glutamate dehydrogenase increased three-fold during the presurgical period. The other biochemical values showed few fluctuations. Based on these data, we concluded that presurgical and postsurgical fasting causes no severe imbalances in the metabolism in mares.  相似文献

The objective of this on-farm study was to develop an effective and labour-extensive method to cure forage Lablab purpureus ‘Highworth’ for resource-constrained smallholder farmers in the Goromonzi district, Zimbabwe in 2014. In a completely randomised factorial design, the drying rate of forage subjected to three conditioning treatments, viz. pressing with a plain water-filled 200 L steel drum, pressing with a water-filled 200 L steel drum covered with 14-gauge wire mesh, and chopping to 5?cm length with a machete, was compared with that of unconditioned forage. The conditioning treatments were subjected to three natural aeration methods, viz. Aframe, raised platform, and dry ground. Replicated four times, conditioned forage was cured at a swath density of 25?kg m?2 to cover 3?m2?per treatment over 28 d. Conditioning of forage had a significant (p < 0.05) effect on drying rate. However, conditioning did not have a significant (p > 0.05) effect on crude protein content. Aeration treatments did not have a significant effect (p > 0.05) on drying rate or crude protein content. Moisture loss rate was higher for forage conditioned with a wired drum and machete. Conditioning with a machete was most labour intensive compared with drum conditioning treatments. Laceration and pressing is an effective and labour-extensive conditioning method for resource-constrained smallholder farmers.  相似文献

In this article, 3 cases of an infrequent complication of lower limb trauma that presented as a cellulitis and deteriorating comfort a few days after lower limb laceration are described. All 3 horses sloughed the hoof capsule 10 days after initial trauma to that particular limb. Development of subsequent cellulitis/lymphangitis contributed to deterioration of distal limb perfusion. The exact pathophysiological mechanisms remain unknown but clinicians should be aware of this unusual but major complication following limb trauma.  相似文献

Background – This study was performed to document the frequency and severity of limb injuries that occur when dogs come in contact with metal landscape edging. A retrospective study from 1997 to 2007 was performed at Colorado State University veterinary teaching hospital. Sixty dogs were admitted to hospital for traumatic limb injuries resulting from metal landscape edging identified by a computerized medical records search. Key Findings – Most dogs were young, large breed dogs. All 60 dogs suffered traumatic pedal lacerations when contacting metal landscape edging, the majority of which occurred on the forelimbs. Eighty‐five percent required surgical repair. All 60 dogs were discharged from the hospital and 30 were available for long‐term follow‐up. While most dogs healed with a perfect cosmetic outcome, 3 dogs with tendon involvement developed residual digital deformities. Significance – Dogs are at risk of injury when exposed to metal landscape edging. Severity of pedal injury determines treatment protocol and prognosis for recovery.  相似文献

Cataracts can be developmental or acquired lesions and cause minimal or significant visual impairment. The ability to identify cataracts and determine the likely effect on the visual potential of the horse is an important part of an equine ocular examination. Cataract surgery is the only treatment for those cataracts that cause significant visual impairment. Understanding appropriate patient selection and potential post operative complications is important so that owners can be appropriately advised of their options.  相似文献

Presently, intraocular lenses (IOLs) are not routinely implanted after equine cataract surgery. Subsequently, horses are visual but markedly farsighted (hyperopic). This report describes the surgical results and visual status after phacoemulsification and implantation of IOLs in mature horses with spontaneous cataracts. Six eyes of 5 mature horses underwent phacoemulsification and implantation of a +14 diopter (D) foldable IOL. Recheck ocular examinations were performed at 1, 4 and 24 weeks post operatively. Refractive error was recorded at 4 weeks post operatively. Visual status, refractive error and anterior chamber depth were recorded 24 weeks post operatively: 5 of 6 operated eyes remained visual and the average refractive error was +0.4 ± 1.1D. There was a significant difference between the 24 week post operative refractive error and the population mean of +10D (P<0.0001) for aphakic horses. The average post operative anterior chamber depth was 7.89 ± 1.55 mm. One globe was enucleated 2 months post operatively. Phacoemulsification with IOL implantation resulted in a significant reduction in post operative refractive error and restored vision to within 0.4D of emmetropia in 5 of 6 operated eyes. Implantation of a +14D IOL ameliorated the hyperopia documented in aphakic horses and improved the post operative visual acuity.  相似文献

A 5‐month‐old colt sustained a mandibular degloving injury and penetrating cheek wound after catching his cheek on a metal hook. Avulsion of the lower left lip and cheek exposed parts of the underlying labial surface, interalveolar margin and body of the mandible. The injury prevented the colt from closing his mouth completely and food accumulated between the degloved mandible and avulsed soft tissues. Surgical repair included 7 stented, transmandibular mattress sutures placed over the length of the interalveolar margin, closing the dead space and repositioning the avulsed soft tissues. The horse recovered without complications and functional as well as cosmetic outcome were excellent.  相似文献

A retrospective analysis of 15 cases of flexor tendon lacerations managed with a fetlock support brace between 2004 and 2008 was performed. Information was gathered concerning exact nature of the injury, treatment details and outcome. Limbs involved included 2 forelimbs and 13 hindlimbs. Eight of fifteen horses (53%) presented with a dropped fetlock, elevated toe or both when bearing weight on the affected limb. General anaesthesia was performed on 7/15 cases to further evaluate and treat the wound, 8/15 cases were managed with local anaesthesia and/or sedation only. The brace was applied within 14 days of presentation and maintained for a mean of 106 days. Sixty percent of cases were sound and used as intended and an additional 20% were sound but at a lesser level of work. Horses with flexor tendon lacerations can be managed with a fetlock support brace alone and have a similar outcome with shorter hospitalisation than horses managed by other methods.  相似文献

Two horses, one 15‐year‐old Arabian gelding and one 10‐year‐old Quarter Horse gelding, presented with a history of marked subcutaneous emphysema. The first case exhibited no external wound, although there was a depression noted on the ventral neck. The second case had a laceration on the ventral aspect of the neck over the trachea. Endoscopic examination revealed both horses to have concurrent dorsal and ventral perforations of the trachea. The horses were managed by placing a short, cuffed, J‐shaped tracheostomy tube in the ventral perforation, while the dorsal perforation healed. The dorsal perforation in the first horse was allowed to heal by second intention, whereas sutures were placed in the dorsal perforation in the second case to reduce the healing time. Both horses were maintained on oral antimicrobial and nonsteroidal anti‐inflammatory medications throughout treatment. The dorsal perforation healed after 13 days in the first horse, and 22 days in the second horse. The ventral perforation healed in both horses by second intention following tracheostomy removal, giving a cosmetically acceptable result. In addition to facilitating tracheal healing, the tracheostomy tubes prevented the progression of subcutaneous emphysema, and promoted its resolution.  相似文献

Purpose The results of a recent study on accommodation in humans and baboons has revealed that lens fiber structure and organization are key components of the mechanism of accommodation. Dynamic focusing involves the controlled displacement and replacement, or realignment, of cortical fiber-ends at sutures as the mechanism of accommodation at the fiber level. This emended explanation of the mechanism of accommodation raises the following question: as the structure of crystalline lenses are only similar, not identical between species, is accommodative amplitude related to differences in the structure and organization of fibers between species? To address this question, we have quantitatively examined the structure and organization of fibers in a number of the more commonly used animal models (mice, cattle, frogs, rabbits and chickens) for lens research. Materials and methods Lenses (a minimum of 12–18 lenses/species) from mice, cattle, frogs and rabbits were used for this study. Prior to fixation for structural analysis, measurements of the gross shape of the lenses (equatorial diameter, anterior and posterior minor radii [anterior + posterior minor radius = polar axis]) were taken directly through a stereo surgical dissecting microscope equipped with an ocular reticle. Lenses were then prepared for and examined by light (LM), transmission (TEM) and scanning electron microscopy (SEM). Scale computer-assisted drawings (CADs) of lenses and lens fibers were then constructed from quantitative data as described above and from quantitative data contained in micrographs. Results The differences in fiber structure and organization that effect accommodative range arise early in development and are continued throughout lifelong lens growth. In umbilical suture lenses (avian) secondary fibers develop with almost completely tapered anterior ends (85–90% reduction of their measures of width and thickness at the equator). By comparison, in lenses with line sutures (e.g. frogs and rabbits) secondary fibers develop with just a 50–60% reduction in anterior fiber taper. In lenses with Y sutures (mice and cattle), fiber width taper is only 25–40%. However, in all cases, while the taper of the posterior end width of fibers is just slightly less (approx. 15–20%) than that of anterior ends, posterior end thickness is only reduced by one half that of anterior thickness. Conclusions In humans, the mechanism of accommodation at the fiber level involves the controlled realignment of very flattened and flared, rather than tapered fiber-ends at sutures. In this manner, the simultaneous increase in lens thickness and surface curvature in the accommodated state is the result of fiber-ends being overlapped along multiple (9–12) suture branches covering the majority of the anterior and posterior surfaces. The results of this animal study strongly suggest that accommodative range is directly related to quantitative differences in fiber structure and organization in the different suture types. The very broad accommodative range in birds is made possible, at least in part, by the almost complete tapering of fiber-ends at umbilical sutures. In contrast, the essentially negligible accommodative range of animals that have line- and Y-suture lenses is at least partially the result of the fact that these lenses have fibers with very little end taper. Thus, the blunt ends of fibers in line- and Y-suture lenses precludes any significant overlap of end segments to effect accommodation.  相似文献

A 7‐year‐old Warmblood mare presented with blood loss from a laceration of the medial digital artery sustained in the field. The mare was initially managed conservatively by pressure bandaging but the bleeding did not stop. After a blood transfusion the mare underwent arterial repair under general anaesthesia and was initially managed in a cast post operatively. Doppler ultrasonography 4 days post operatively confirmed effective blood flow distal to the arterial anastomosis and the mare was discharged from the hospital. At 6 months follow‐up the owner reported that the mare was sound at the trot with an excellent cosmetic outcome of the surgical site.  相似文献

Dorsal intratracheal laceration was diagnosed in a horse with severe subcutaneous emphysema. Due to the size of the defect and the severity of the symptoms, different methods were attempted to close the laceration. OBJECTIVE: To determine ocular dimensions (A- and B-scan ultrasound) and corneal curvature (radius of corneal diameter determined in B-scan ultrasound) in the equine eye and to calculate the appropriate dioptric power for a posterior chamber intraocular lens (IOL) necessary to achieve emmetropia in the eyes of horses undergoing lens extraction. ANIMALS: Fourteen clinically normal adult horses of various breeds. Additionally, for comparison, one American Miniature colt foal, and one 2.5-year-old Shire gelding were examined. PROCEDURE: B-scan ultrasound was performed on one eye from each horse. One eye from both the Shire and the American Miniature were examined for comparison. Data from ultrasound (globe measurements and corneal curvature), and the estimated postoperative IOL positions were entered into theoretical IOL formulas (Binkhorst and Retzlaff theoretical formulas) in order to calculate the predicted IOL strength required to achieve emmetropia after lens extraction in horses. Results: Mean axial length of globes was 39.23 mm +/- 1.26 mm, mean preoperative anterior chamber depth (ACD) was 5.63 +/- 0.86 mm, and mean lens thickness was 11.75 +/- 0.80 mm. Predicted postoperative ACD (PACD) was calculated as the ACD plus 50% of the lens thickness. Additionally, PACD 2 mm anterior and 2 mm posterior to the center of the lens were calculated in order to evaluate the effect of IOL position on its required refractive power. Required IOL strength calculated, using the three values for the predicted postoperative ACD, was 29.91 D +/- 2.50, 29 D +/- 2.52 (center of lens); 27.13 D +/- 2.27, 26.33 D +/- 2.20 (2 mm anterior to center of lens); and 33.18 D +/- 2.78, 32.24 D +/- 2.68 (2 mm posterior to center of lens) with the Binkhorst and Retzlaff theoretical formulas, respectively. CONCLUSIONS: An IOL of substantially lower diopter strength than that needed in either dogs or cats is required to achieve emmetropia after lens extraction in adult horses. IOL strength of approximately 30 D, depending on where the IOL ultimately comes to rest, will probably be required.  相似文献
