Most emerging infectious diseases are zoonotic, yet recent commissions have highlighted deficiencies in their surveillance. We conducted a survey to understand the needs of state agencies for zoonotic disease surveillance. The Ontario Animal Health Network (OAHN) is an innovative disease surveillance program created to enhance preparedness, early detection, and response to animal disease in Ontario. Laboratory data and, where available, abattoir condemnation data and clinical observations submitted by practicing veterinarians form the core of regular discussions of the species-sector networks. Each network is comprised of government veterinarians or specialists, epidemiologists, pathologists, university species specialists, industry stakeholders, and practicing veterinarians, as appropriate. Laboratorians provide data for diseases of interest as determined by the individual network, and network members provide analysis and context for the large volume of information. Networks assess data for disease trends and the emergence of new clinical syndromes, as well as generate information on the health and disease status for each sector in the province. Members assess data validity and quality, which may be limited by multiple factors. Interpretation of laboratory tests and antimicrobial resistance trends without available clinical histories can be challenging. Extrapolation of disease incidence or risk from laboratory submissions to broader species populations must be done with caution. Disease information is communicated in a variety of media to inform veterinary and agricultural sectors of regional disease risks. Through network engagement, information gaps have been addressed, such as educational initiatives to improve sample submissions and enhance diagnostic outcomes, and the development of applied network-driven research. These diverse network initiatives, developed after careful assessment of laboratory and other data, demonstrate that novel approaches to analysis and interpretation can result in a variety of disease risk mitigation actions.  相似文献

Effective prevention of deaths due to human rabies is currently hampered by a lack of understanding of the scale of the problem, and the distribution of both animal and human cases across countries, regions and continents. Unfortunately, despite the severity of the disease, accurate data on which to assess these questions and to prioritize and direct public health interventions are not available for many parts of the world. This survey sought to understand the current global situation regarding the surveillance of human rabies. Data were collected from 91 countries across all continents and all categories of human rabies risk, generating the most complete and representative global data set currently available. Respondents were asked key questions about whether human rabies was a notifiable disease, how the surveillance system for human rabies operated and whether the respondent considered that the surveillance system was working effectively. Across the 91 countries from which data were collated, human rabies was a notifiable disease in all but eight. Despite international guidance, surveillance systems were very varied. Even where rabies is a notifiable disease, many countries had surveillance system judged to be ineffective, almost all of these being high and moderate rabies risk countries in Africa and Asia. Overall, 41% of the population covered by this survey (around 2.5 billion people) live in countries where there is no or ineffective rabies surveillance. The lack of robust surveillance is hindering rabies control efforts. However, whilst worldwide rabies surveillance would be improved if rabies were notifiable in all countries, many other challenges to the implementation of effective global human rabies surveillance systems remain.  相似文献

Zoonotic diseases concern human and animal populations and are transmitted between both humans and animals. Nevertheless, surveillance data on zoonoses are collected separately for the most part in different databases for either humans or animals. Bearing in mind the concept of One Health, it is assumed that a global view of these data might help to prevent and control zoonotic diseases. In following this approach, we wanted to determine which zoonotic data are routinely collected in Germany and whether these data could be integrated in a useful way to improve surveillance. Therefore, we conducted an inventory of the existing data collections and gathered information on possible One Health surveillance areas in Germany by approaching experts through a scoping survey, personal interviews and during a workshop. In matching the information between the status quo for existing data collections and the possible use cases for One Health surveillance, this study revealed that data integration is currently hindered by missing data, missing pathogen information or a lack of timeliness, depending on the surveillance purpose. Therefore, integrating the existing data would require substantial efforts and changes to adapt the collection procedures for routine databases. Nevertheless, during this study, we observed a need for different stakeholders from the human and animal health sectors to share information to improve the surveillance of zoonoses. Therefore, our findings suggest that before the data sets from different databases are integrated for joint analyses, the surveillance could be improved by the sharing of information and knowledge through a collaboration of stakeholders from different sectors and institutions.  相似文献

Annually, groups of health professionals from high‐income countries are drawn to work in low‐ and middle‐income countries (LMICs) through timed engagements intended to improve the well‐being of people in most disadvantaged communities. The existing evidence on understanding volunteer experience in LMICs often focuses on activities within the medical discipline; whereas little research has been conducted on the experiences of volunteers in other disciplines. This paper focuses on understanding veterinary and public health professional's experiences conducting disease surveillance work in Northern Uganda. Forty US‐based health care professionals were recruited to complete multiple‐choice and open‐ended questions to understand prior and current experiences working in resource‐limited settings. Responses were coded using NVivo 10®, qualitative analysis package. Of the 44 volunteers, 50% completed the questionnaire. Responses were largely positive towards surveillance activities; they reported personal and professional gains, new cultural experiences and mutual learning environment with local colleagues. Nevertheless, respondents highlighted challenges during various stages of program implementation—some difficulty with program logistics, inadequacy of preparation materials, in addition to concerns inherent to working in a resource‐limited environment. This assessment suggests that international volunteer work could positively influence programmatic outcomes, personal experiences, furthering health organizational goals, often achieved in a limited time‐frame. To maximize volunteer impact, these assessments are critical and suggest that volunteer and institutional capacities should be seriously considered when planning placement as it can influence programmatic decision‐making, gauge training needs and volunteer readiness.  相似文献

Zoonotic diseases are endemic in the country of Georgia. Using the non‐linear canonical correlation (NCC) method, the aim of this study was to examine the relationship between thirteen epidemiological risk factors and seropositivity to five zoonotic infections (anthrax, Q fever, tularemia, leptospirosis, and Crimean‐Congo hemorrhagic fever [CCHF]) among Georgian military recruits during 2014–2016. According to this multivariate statistical technique, which is suitable for the analysis of two or more sets of qualitative variables simultaneously, two canonical variables were identified. These variables accounted for 68% of the variation between the two sets of categorical variables (“risk factors” and “zoonotic infections”). For the first canonical variable, there was a relationship among CCHF (canonical loading, which is interpreted in the same way as the Pearson's correlation coefficient, [cl] = 0.715), tick bites (cl = 0.418) and slaughter of animals (cl = 0.351). As for the second canonical variable, Q fever (cl = ?0.604) and leptospirosis (cl = ?0.486) were related to rodents inside and outside home (cl = ?0.346) and sweeping in or around home (cl = ?0.317). The NCC method allows researchers to obtain additional insights into the complex relationship between epidemiological risk factors and multiple zoonotic infections.  相似文献

AIMS: To describe results of a relative validation exercise using the three simulation models of foot-and-mouth disease (FMD) in use by the quadrilateral countries (QUADS; Australia, Canada, New Zealand, and United States of America; USA).

METHODS: A hypothetical population of farms was constructed and, following the introduction of an FMD-like disease into a single farm, spread of disease was simulated using each of the three FMD simulation models used by the QUADS countries. A series of 11 scenarios was developed to systematically evaluate the key processes of disease transmission and control used by each of the three models. The predicted number of infected units and the size of predicted outbreak areas for each scenario and each model were compared using the Kruskal-Wallis test. Agreement among the three models in terms of geographical areas predicted to become infected were quantified using Fleiss' Kappa statistic.

RESULTS: Although there were statistically significant differences in model outputs in terms of the numbers of units predicted to become infected, the temporal onset of infection throughout the simulation period, and the spatial distribution of infected units, these differences were generally small and would have resulted in the same (or similar) management decisions being adopted in each case.

CONCLUSIONS: Agreement among the three models in terms of the numbers of premises predicted to become infected, the temporal onset of infection throughout the simulation period, and the spatial distribution of infected premises provides evidence that each of the model developers are consistent in their approach to simulating the spread of disease throughout a population of susceptible individuals. This consistency implies that the assumptions taken by each development team are appropriate, which in turn serves to increase end-user confidence in model predictions.

CLINICAL RELEVANCE: Relative validation is one of a number of steps that can be undertaken to increase end-user confidence in predictions made by infectious disease models.  相似文献

Inadequate financing for the delivery of animal health services in many developing countries has been blamed for lack of efficiency and effectiveness of veterinary services. There are no reports of how the delivery of veterinary services in Ghana is financed. The aim of this paper is to provide information on the funding of veterinary services in Ghana to help in decision making on resource allocation.Various indicators and measures were used in assessing the adequacy of financing and resource allocation from 1990 to 1995. These measures were the veterinary budget as proportions of the national budget, GDP and AGDP; the proportions of the veterinary budget allocated to salaries; the ratios of salaries to non-staff expenditure and of non-staff expenditure to veterinary livestock units and technical staff; coefficient of efficacy; and R- ratio. These generally declined or worsened over the period, deviating from recommended norms where such norms exist. This confirmed the paucity of financing and resource allocation for the delivery of veterinary services. Revenue generation from cost recovery over the 1993–95 period was a potential source of funding, exceeding 100% of non-staff expenditure for 1993 and 1994. However, the revenue generated was not channelled back to veterinary services but went to the national coffers. This served as a disincentive.There is an urgent need to review how veterinary services are financed in Ghana, if the delivery of services is to improve in efficiency and effectiveness.  相似文献

Blastocystosis is an interesting parasitosis, since the parasitic infection is still seriously neglected and a considerable zoonotic evidence is emerging. Blastocystis sp. infects the intestinal lumen of humans and a wide range of animals, while there is a lack of comprehensive information on Blastocystis epidemiology in cattle worldwide. Thus, the present systematic review and meta-analysis were performed by exploring four electronic databases (PubMed, Web of Science, Scopus, and Google scholar) for relevant published papers up to 7th November 2020, utilizing a random-effects model to pool estimations and assign 95 % confidence intervals (CIs). Results of 28 studies (29 datasets) on cattle showed a 24.4 % (95 % CI: 16.9–33.9 %) prevalence for Blastocystis infection. Also, 16 out of 26 reported subtypes (STs) were isolated from cattle, with ST10 (18 datasets) as the highest-reported [32.3 % (95 % CI: 21.6–45.3)] as well as ST24 and ST25 (one study each) as the lowest-reported STs [1.4 % (95 % CI: 0.2–9.1)]. Additionally, among nine well-known zoonotic STs (ST1-ST8 and ST12), all STs except for ST8 were reported from cattle worldwide, demonstrating this animal species as a potential reservoir for human infections. Meanwhile, the overall prevalence of Blastocystis in various subgroups (publication year, WHO regions, countries, continents, and age groups) was analyzed separately. The finding of the present review article highlights the cattle as a significant source of zoonotic transmission of Blastocystis infection to humans, which must be considered for preventive measures.  相似文献

Melatonin and its receptors play a crucial role in the regulation of the animal reproductive process, primarily in follicular development. However, the role that melatonin performs in regulating hormones related with reproduction remains unclear. Melatonin and its receptors are present both in female and male animals’ organs, such as ovaries, heart, brain and liver. Melatonin regulates ovarian actions and is a key mediator of reproductive actions. Melatonin has numerous effects on animal reproduction, such as protection of gametes and embryos, response to clock genes, immune‐neuroendocrine, reconciliation of seasonal variations in immune function, and silence or blockage of genes. The growth ratio of reproductive illnesses in animals has raised a remarkable concern for the government, animal caretakers and farm managers. In order to resolve this challenging issue, it is very necessary to conduct state‐of‐the‐art research on melatonin and its receptors because melatonin has considerable physiognomies. This review article presents a current contemporary research conducted by numerous researchers from the entire world on the role of melatonin and its receptors in animal reproduction, from the year 1985 to the year 2017. Furthermore, this review shows scientific research challenges related to melatonin receptors and their explanations based on the findings of 172 numerous research articles, and also represents significant proficiencies of melatonin in order to show enthusiastic study direction for animal reproduction researchers.  相似文献

Many chemotherapy drugs used in human patients are discarded after single use or within 24 h of reconstitution, as per the manufacturer's product label recommendations. This can be wasteful and costly to veterinary clients. This report reviews the published stability and storage data for 19 injectable chemotherapy drugs commonly used in veterinary medicine. Based on these data, storage procedures are presented, assuming aseptic technique and a closed system drug transfer device (CSDTD) are used for drug preparation and handling. Further studies on the risk of microbiological contamination of chemotherapeutics using a CSDTD, and validated high quality drug assays such as stability‐indicating high‐performance liquid chromatography, are required. The authors' intent is not to supersede product label recommendations, but to suggest that longer storage without significant loss of drug efficacy may be possible, thus reducing the costs of chemotherapeutics to some veterinary clients.  相似文献

Chagas disease is a parasitic infection that can result in a progressive dilated cardiomyopathy. Here, we present the epidemiologic details of a suspected locally acquired transmission case originating from the southern United States. This is the first published report of Chagas disease in a young, healthy United States veteran with repeat triatomine exposures in Arizona. Serum creatinine concentration, the classical biomarker of chronic kidney disease (CKD) in cats, has important limitations that decrease its value as a biomarker of early CKD. Recently, serum symmetric dimethylarginine concentration was introduced as a novel glomerular filtration rate biomarker for the early detection of CKD in cats. However, data on its specificity are still limited. The limitations of conventional biomarkers and the desire for early therapeutic intervention in cats with CKD to improve outcomes have prompted the discovery and validation of novel renal biomarkers to detect glomerular or tubular dysfunction. Changes in the serum or urinary concentrations of these biomarkers may indicate early kidney damage or predict the progression of kidney before changes in conventional biomarkers are detectable. This review summarizes current knowledge on renal biomarkers in CKD in cats, a field that has progressed substantially over the last 5 years.  相似文献

Reasons for performing study: Geriatric horses (aged ≥15 years) represent a substantial proportion of the equine population, yet very few studies have investigated the prevalence of diseases within the UK equine geriatric population. Objectives: To describe the provision of routine preventive health care measures, prevalence of clinical signs of disease and the prevalence of owner reported diseases. Additionally, the effect of increasing age on the provision of preventive health care and the presence or absence of clinical signs and disease was assessed. Methods: A cross‐sectional study was conducted, surveying a randomly selected sample of veterinary registered owners with horses aged ≥15 years, using a self‐administered postal questionnaire. Results: As geriatric horses increased in age, there was a reduction in the provision of preventive health care measures, such as vaccination, farrier care and routine veterinary checks. Only 68.7% of horses had received a routine veterinary visit within the previous 12 months. Owners frequently observed clinical signs in their animals, with 77% reporting at least one clinical sign of disease. Increasing age was associated with increased reporting of many clinical signs of disease. Over half (58%) of horses had at least one episode of disease within the previous 12 months, yet only 31% of owners reported that their animal currently suffered from a known disease or disorder. Conclusions and potential relevance: Although owners frequently observed clinical signs in their aged horse, there may be incorrect or under recognition of many diseases and health problems. Reduced frequency of routine preventive health care measures, along with suboptimal owner recognition of health and welfare problems may lead to compromised welfare in the geriatric population.  相似文献

There are few reports describing hypertrophic osteopathy (HO) in horses and no reports describing this in the Irish equine population. This case series outlines the clinical, radiological and pathological features of HO in horses in Ireland and compares and contrasts the findings to previous reports. Overall, the prevalence of HO in Irish horses was found to be low with clinical signs similar to previous reports. Diagnosis of the primary lesion was low, largely due to the limited diagnostic evaluation in a large proportion of the horses evaluated. The horses had a low average age and a high prevalence (35.5%) of mandibular and facial involvement.  相似文献
