Objectives : To determine the significant risk factors for medial meniscal injury in naturally occurring cranial cruciate ligament rupture and to quantify the risk using multivariate analysis. Methods : A retrospective case control study was performed of dogs that had undergone surgery for cranial cruciate ligament rupture. Data recorded included patient signalment (age, breed and sex), the duration of the lameness, the extent of the cranial cruciate ligament rupture (complete or partial) and the condition of the medial meniscus. Logistic regression was used to analyse the relationship between these variables and tears in the medial meniscus. Results : One hundred and sixty-one of 443 stifles (36·3%) in 366 dogs had a medial meniscal tear. The risk of a medial meniscal tear was increased by 12·9 times in association with complete cranial cruciate ligament rupture (OR 12·9; 95% CI 6·8 to 24·2), by approximately 2·6% for each additional week of lameness (OR 1·026; 95% CI 1·009 to 1·043) and by approximately 1·4% for each additional kilogram of bodyweight (OR 1·014; 95% CI 1·000 to 1·028). Golden retrievers and Rottweilers were at increased risk and West Highland white terriers were at reduced risk of medial meniscal tears compared with Labrador retrievers. Clinical Significance : To minimise the risk of medial meniscal tears, surgical stabilisation should not be unnecessarily delayed.  相似文献

Objective : To determine the effect of signalment on the incidence and presentation of patients suffering from cranial cruciate ligament disease. Methods : Data relating to 426 dogs (44 breeds) that met specific selection criteria were obtained from the hospital archive (2002 to 2008). Cases were followed up for 2 years. Results : The breeds most commonly presented with cranial cruciate ligament disease were Labrador retriever (16%), Rottweiler (15%), golden retriever (12%) and boxer (9%). Rottweilers were significantly more likely (69%; P=0·05) to develop and present with (50%; P=0·03) bilateral cranial cruciate ligament disease. Rottweilers presenting with cranial cruciate ligament disease were significantly younger (median 977 days; P<0·0001) than other breeds; golden retrievers being significantly older at presentation (median 1994 days; P=0·004). Neither sex nor neutered status significantly affected the incidence of developing (P=0·77 and P=0·30, respectively) or presenting with (P=0·62 and P=0·35, respectively) bilateral cranial cruciate ligament disease. Entire dogs were significantly younger than neutered dogs at presentation (P=0·0004). Entire female dogs presented significantly younger than neutered females (P=0·0002), entire males (P=0·01) and neutered males (P=0·0001). Clinical Significance : Breed affects the incidence of developing and presenting with bilateral cranial cruciate ligament disease. Breed and sex both affect the age that patients present with cranial cruciate ligament disease.  相似文献

Eight dogs presented with chronic hindlimb lameness associated with cranial cruciate ligament rupture. Seven were small terriers. A caudal deformity of the proximal tibial shaft, originating at the proximal tibial physis, and an excessive caudal slope of the tibial plateau were present bilaterally in all dogs. The deformity was thought to be responsible for the cranial cruciate ligament failure and poor response to conservative management. Tibial plateau angles were in excess of 26 degrees in all dogs. The lameness was bilateral in three dogs. There was complete cranial cruciate ligament rupture in seven stifles and partial rupture in four. There were no meniscal injuries. Surgical correction resulted in a significant improvement (P<0.0001) in all dogs, with a mean follow-up of 12 months (range three to 24 months). There were no complications.  相似文献

Periarticular histiocytic sarcoma (PAHS) is the most common synovial tumour in dogs and is characterized by aggressive local disease with a high rate of distant metastasis. Previously, an association between PAHS and prior joint disease has been demonstrated in the Bernese Mountain Dog breed and suggested in the Rottweiler. We hypothesized that this association would be present in other breeds and investigated this via a retrospective, case‐controlled analysis. Cases were dogs diagnosed with PAHS of the stifle or elbow. Controls were age, breed and sex‐matched dogs without a diagnosis of histiocytic sarcoma. Diagnosis of prior joint disease was determined based on review of medical records and direct veterinarian and owner communications. Data were evaluated using logistic regression, 2‐sampled t tests, and chi‐squared analysis. Our study population consisted of 28 cases and 46 controls, including Flat‐Coated, Golden and Labrador Retrievers, Rottweilers, English Bulldogs, Shih Tzus, Australian Shepherds, Staffordshire Terriers and mixed breed dogs. Dogs with PAHS were more likely to have prior joint disease in the tumour‐affected joint compared with the control population (odds ratio [OR] = 13.42, P < .0001, 95% confidence interval [CI] = 4.33‐48.63). A total of 88.2% of dogs with stifle PAHS had prior joint disease in their tumour‐affected joint, most commonly cranial cruciate ligament rupture. This study confirms that the previously noted association between prior joint disease and PAHS in Bernese Mountain Dogs also applies to other breeds. Additional studies are needed to further investigate for a causal relationship.  相似文献

Objectives : To determine the prevalence of adverse food reactions (AFRs) in dogs with dermatological signs presented to the referral dermatological clinic of the University of Milan. Methods : The medical records of dogs with dermatological signs were reviewed. Prevalence of AFRs was calculated. Owner and clinician pruritus scores were compared. Breed, sex and age predisposition were statistically tested, as was the association between AFR and selected clinical features. Results : The prevalence of AFRs in dogs with dermatological signs was 12% (16 of 130). AFR was diagnosed in 26% of dogs with allergic disease and 48% of those subjected to a dietary trial. There was a significant association between AFRs and early onset of clinical signs (<1 year) (OR=3·8; P=0·0221, 95% CI=1·27 to 11·16). There was a significant association between AFRs and both otitis externa (OR=5·9; P=0·0015, 95% CI=2 to 17·9) and perianal fistula (OR=26·1; P=0·0058, 95% CI=2·52 to 269·4), although all dogs with perianal fistulas were German shepherd dogs. Clinical Significance : The prevalence of AFRs in the study population was higher than most reported values. Further studies are warranted to investigate the true prevalence of AFR and its possible association with perianal fistula and other potential markers.  相似文献

OBJECTIVE: To describe clinical features of dogs < 2 years old with rupture of the cranial cruciate ligament (CCL) and to evaluate breed, sex, and body weight as risk factors. DESIGN: Case-control study. ANIMALS: 201 dogs < 2 years old with rupture of the CCL and 804 age-matched control dogs. PROCEDURE: Medical records were reviewed for breed, sex, and body weight, and results were compared with results of age-matched control dogs. RESULTS: Breed predisposition was detected for Neapolitan Mastiff, Akita, Saint Bernard, Rottweiler, Mastiff, Newfoundland, Chesapeake Bay Retriever, Labrador Retriever, and American Staffordshire Terrier. Increased risk was detected for neutered males and neutered females, compared with sexually intact males and sexually intact females, respectively. Differences in prevalence of rupture of the CCL were not detected between all males and females, sexually intact males and sexually intact females, or neutered males and neutered females. Body weights of dogs with ruptured CCL were significantly greater than those of control dogs. CONCLUSIONS AND CLINICAL RELEVANCE: Several large breeds of dogs are predisposed to rupture of the CCL at a young age.  相似文献

Objective— To compare rates of contralateral cranial cruciate ligament rupture (CCLR) in Labradors based on age and weight at initial rupture, sex, and tibial plateau angle (TPA) and to determine whether Labradors that rupture their initial cranial cruciate ligament (CCL) at an earlier age (<4 years) are more likely to rupture their contralateral side within a certain period of time. Study Design— Case series. Animals— Labradors (n=94) that had tibial plateau leveling osteotomy (TPLO). Methods— Two groups: no contralateral rupture (NR) and contralateral rupture (CR) were compared for significant (P<.05) differences in percentage of subsequent cruciate tears using a Wilcoxon's rank‐sum tests for continuous variables and Fisher's exact test for sex. Adjusted odds ratios for likelihood of subsequent cruciate tears (yes/no) were estimated using logistic regression. Associations of these characteristics with time to subsequent rupture were assessed using Kaplan–Meier survival analysis estimation. Predictors of presentation with bilateral ruptures (BR) versus single rupture were also evaluated using Wilcoxon's rank‐sum tests and a generalized Fisher's exact test. Results— Subsequent CCLR occurred in 45 dogs (48%), and BR on admission were identified in 10 dogs (10.6%). Comparing NR and CR dogs, there were no significant differences between age or weight at initial rupture, sex or TPA; however there were associations toward longer time to CR for dogs older than the median age and female dogs (intact and spayed). There were no significant differences in age, sex, weight, or TPA of dogs with bilateral CCL ruptures compared with initial unilateral ruptures; however, there was a trend toward dogs presenting at an older age and with lower TPA's in the BR group. Among the 84 NR/CR dogs, the median time to rupture of the contralateral CCL was 5.5 months (95% CI 5.2–5.7). Conclusions— Age and weight at initial rupture, sex, and TPA does not affect likelihood or rate of contralateral CCL rupture or presentation with bilateral CCL ruptures. Clinical Relevance— Approximately 50% of Labradors will rupture the contralateral CCL within 5.5 months of the initial rupture but age, weight, sex, and TPA cannot be used as predictive features.  相似文献

Objectives : The diagnostic utility of routine faecal examinations can be greatly enhanced through an appreciation of risk factors most commonly associated with endoparasitism. Methods : From a sample of 6578 canine patients presenting to a veterinary teaching hospital between 1996 and 2006, this study used univariate and multi‐variable techniques to examine putative signalment, medical history and demographic factors predisposing dogs to intestinal parasites. Results : Age and median household income were the strongest predictors of endoparasitism. The odds of a patient being diagnosed with endoparasites were 0·82 times smaller for every 1 year increase in age (OR=0·82, 95% CI: 0·80 to 0·84) and for every $10,000 increase in median household income, the odds were 0·86 times lower (OR=0·86, 95% CI: 0·83 to 0·89). The variables gender, neuter status, month of diagnosis, admitting clinical service and primary diagnosis were not significant predictors. Animals that were presented for underlying medical conditions were less likely to have parasites and the presence of diarrhoea was associated with 76% lower odds of endoparasitism compared to healthy animals (OR=0·76, 95% CI: 0·64 to 0·90). Clinical Significance : Clinicians should maintain a high index of suspicion for parasites in younger patients that live in high population density and low income neighbourhoods.  相似文献

Canine cranial cruciate ligament rupture is often bilateral and asymmetrical, ranging from partial to complete rupture. The purpose of our diagnostic accuracy study was to assess the accuracy of 3 Tesla magnetic resonance imaging (MRI) detection of fiber loss and use of a visual analog scale in the diagnosis of complete versus partial cranial cruciate ligament rupture in 28 clinical dogs with unilateral complete rupture and contralateral partial rupture. Three Tesla MRI was performed on 56 stifles using sagittal sequences (T2‐weighted fast spin echo with fat saturation, proton density fast spin echo, and T2‐weighted 3D fast spin echo CUBE). Two MRI observers assessed the cranial cruciate ligament for fiber loss and completed a visual analog scale. The MRI data were compared to arthroscopy and clinical status. Accuracy classifying partial or complete rupture was assessed using receiver operating characteristic analysis. Compared to arthroscopy, for complete cranial cruciate ligament rupture, sensitivity, specificity, and accuracy of MRI detection of fiber loss were 0.78, 0.50–0.60, and 0.68–0.71, respectively, and, for partial tears, specificity was 1.00. An MRI visual analog scale score ≥79 was indicative of complete cranial cruciate ligament rupture (sensitivity 0.72–0.94 and specificity 0.71–0.84). Using a visual analog scale cut‐point ≥79, observers achieved good accuracy discriminating clinical status of partial or complete cranial cruciate ligament rupture (area under the curve 0.87–0.93). MRI evaluation for fiber loss and use of a visual analog scale are specific in stifles with clinically stable partial cranial cruciate ligament rupture. In stifles with clinically unstable complete cranial cruciate ligament rupture, both MRI tests are sensitive though not specific compared to arthroscopy. As a diagnostic imaging method, MRI may help guide treatment in patients with cranial cruciate ligament damage, particularly for stable partial rupture.  相似文献

Partial rupture of the cranial cruciate ligament was diagnosed in 25 dogs. In all dogs, the primary problem at the time of physical examination was hind limb lameness. The mean time from initial onset of lameness to diagnosis at exploratory surgery was 17 weeks. A cranial drawer sign was detected in 13 of the 25 dogs; in 9, the cranial drawer sign was evident only when the stifle was positioned in flexion. Of the 25 dogs, 12 had no detectable cranial drawer sign in response to manipulation of the involved stifle. In all dogs, lateral stifle arthrotomy was performed in routine manner, and the cranial cruciate ligament was found to be incompletely torn. Lesions identified during arthrotomy were rupture of the craniomedial band (n = 20 dogs), interstitial tear (n = 4 dogs), and rupture of the caudolateral band (n = 1 dog).  相似文献

Objectives : To identify clinical risk factors for seizures in dogs with intracranial neoplasia. Methods : A cross‐sectional retrospective study of 68 dogs with histopathologically confirmed primary or secondary intracranial neoplasia, complete clinical history and magnetic resonance imaging of the brain was conducted. Signalment and clinical history were retrieved from clinical records and magnetic resonance images of the brain were re‐evaluated. Prevalence of findings was compared between dogs with and without seizures. Results : Forty‐two dogs had tumour‐related seizures, the remaining 26 were seizure‐free. Tumour types included meningioma (23 dogs with and 5 without seizures), glioma (9 dogs with and 6 without seizures), choroid plexus tumour (2 dogs without seizures), neuroblastoma (1 dog with seizures) and metastatic/invasive tumours including lymphoma (9 dogs with and 13 without seizures). On the basis of multi‐variable logistic regression analysis, risk factors for seizures associated with intracranial neoplasia were magnetic resonance imaging findings consistent with the presence of neoplastic tissue in frontal lobe [odds ratio (OR) 9·61; 95% confidence interval (CI) 2·59 to 35·61], marked gadolinium enhancement (OR 10·41; 95% CI 2·07 to 52·30) and magnetic resonance imaging findings of subfalcine and/or subtentorial herniation (OR 3·88; 95% CI 1·10 to 13·71). Clinical Significance : Dogs with primary or secondary intracranial neoplasia are at risk of seizures, particularly those with tumours that affect the frontal lobe, enhance markedly with gadolinium, or cause subfalcine and/or subtentorial herniation.  相似文献

The patellar tendon reflex is a clinical parameter commonly used to assess neurological status. Factors such as quadriceps muscle fiber composition, femur length, and patient age have been reported to influence total and fractionated reflex times in human beings. The purpose of this blinded, cross-sectional study was to examine the effect of age on the patellar reflex in the dog. Eighty-six dogs without evidence of neurological impairment or orthopedic disease of the stifle (eg, patellar luxation or cranial cruciate ligament rupture) were assigned to 1 of 2 groups on the basis of age: group 1, <10 years old (n = 72); or group 2, > or =10 years old (n = 14). Patellar reflexes were elicited in both pelvic limbs by a reflex hammer while the dog was in lateral recumbency. The reflex was scored as present or absent by a blinded individual. Two dogs in group I had reflexes absent in both limbs, 3 dogs in group 2 had reflexes absent in both limbs, and 1 dog in group 2 lacked a response in 1 limb. The proportion of dogs with absent patellar reflexes in group 2 (4 of 14, 95% CI, 8-58%) was significantly higher than the proportion of dogs with absent patellar reflexes in group 1 (2 of 72, 95% CI, 0-10%) (P < .006). Furthermore, dogs lacking at least 1 patellar reflex were older than those having both reflexes present (P = .04). Weight was not related to the presence of both reflexes (P = .49). These findings suggest that neurologically normal dogs may have an age-dependent decline in patellar reflex magnitude or a prolongation of total reflex time (TRT).  相似文献

O bjectives : To report the signalment, history, clinical features, and outcome in dogs weighing greater than 15 kg, treated surgically and non-surgically for patellar luxation. Risk factors for the development of patellar luxation, postoperative complications, and outcome were evaluated.
M ethods : Details regarding signalment, bodyweight, breed, aetiology, unilateral or bilateral luxation, duration of lameness, grade of luxation, direction of luxation, grade of lameness at presentation, concomitant cranial cruciate ligament rupture, method of treatment, surgical technique, surgeon, and complications were obtained from the medical records. Outcome was graded as excellent, good, fair, or poor, according to the degree of lameness.
R esults : Seventy dogs (45 males and 25 females) were included. Thirty-five had bilateral luxations (105 limbs). Mean age was two years, and mean weight was 30 kg. The relative risk for Labrador retrievers was 3·3 (P<0·001). All luxations were developmental. Luxations were medial in 102 stifles and lateral in three. Fourteen stifles had concomitant cranial cruciate ligament rupture. As the grade of patellar luxation increased, so did the grade of lameness (P<0·001). Surgery was performed in 70 stifles, and outcome was excellent/good in 94 per cent and fair/poor in 6 per cent of stifles. Complications occurred in 29 per cent of stifles, and increasing bodyweight was found to be a risk factor (P=0·03). Thirty-five stifles were managed non-surgically, and outcome was excellent/good in 86 per cent and fair/poor in 14 per cent of stifles.
C linical S ignificance : In view of the potential risk of postoperative complications, all surgically treated cases of patellar luxation in large breed dogs should be managed with a femoral trochleoplasty, a tibial tuberosity transposition (stabilised with K-wires and a tension band wire), and soft tissue releasing and tightening procedures.  相似文献

OBJECTIVES: To compare tibial plateau angle (TPA) among Labrador Retrievers, Rottweilers, Boxers, and German Shepherd Dogs that had undergone tibial plateau leveling osteotomies (TPLO) for cranial cruciate ligament disease, and to determine if higher TPAs are associated with increased risk for concurrent meniscal injury. METHODS: This was a retrospective study including 275 client-owned dogs of one of the four aforementioned breeds that received TPLO radiographs between 1999 and 2005 prior to undergoing the TPLO procedure. The TPA measured by the original surgeon was compared among four breeds and analyzed with respect to presence or absence of meniscal injury diagnosed via arthroscopy or arthrotomy at the time of surgery. RESULTS: Labrador Retrievers, Rottweilers, Boxers, and German Shepherd Dogs had mean TPAs of 25.9 degrees , 26.2 degrees , 25.9 degrees , and 28.2 degrees , respectively. The TPA of German Shepherd Dogs was significantly higher than that of the other breeds of dogs examined. TPA higher than the mean for dogs in this study did not significantly increase the risk for concurrent meniscal injury. CLINICAL SIGNIFICANCE: German Shepherd Dogs with cranial cruciate ligament disease had a significantly higher TPA compared to the other breeds examined. Higher TPA did not increase the likelihood of meniscal tears in this study. However, several variables that were not assessed, including the degree of cranial cruciate ligament integrity at the time of surgery and the time between original CrCL deficiency and surgical assessment, could have affected this analysis.  相似文献

This retrospective study identified complications associated with tibial plateau leveling osteotomy (TPLO) and predisposing factors for these complications in a large population of dogs from a metropolitan area with cruciate ligament deficiency. There were 943 dogs that underwent unilateral TPLO and 288 with staged bilateral TPLO for a total of 1519 procedures. There were 47 cases with at least 1 major complication and 126 cases with at least 1 minor complication but no major complications. The total complication rate (major or minor) was 11.4% [95% confidence interval (CI) estimate: 9.8%, 13.2%]; the major complication rate was 3.1% (95% CI: 2.3%, 4.1%); and the minor complication rate was 8.3% (95% CI: 7.0%, 9.8%). Factors associated with development of complications included being a German shepherd dog [odds ratio (OR): 3.2], tibial plateau angle > 30° (OR: 1.6), and heavier weights (for every 4.5 kg increase in body weight the OR increased by 1.10). Tibial plateau leveling osteotomy is a common treatment for dogs with cruciate ligament deficiency and has a low complication rate.  相似文献

Low‐field magnetic resonance imaging (MRI) is commonly used to evaluate dogs with suspected cranial cruciate ligament injury; however, effects of stifle positioning and scan plane on visualization of the ligament are incompletely understood. Six stifle joints (one pilot, five test) were collected from dogs that were scheduled for euthanasia due to reasons unrelated to the stifle joint. Each stifle joint was scanned in three angles of flexion (90°, 135°, 145°) and eight scan planes (three dorsal, three axial, two sagittal), using the same low‐field MRI scanner and T2‐weighted fast spin echo scan protocol. Two experienced observers who were unaware of scan technique independently scored visualization of the cranial cruciate ligament in each scan using a scale of 0–3. Visualization score rank sums were higher when the stifle was flexed at 90° compared to 145°, regardless of the scan plane. Visualization scores for the cranial cruciate ligament in the dorsal (H (2) = 19.620, P = 0.000), axial (H (2) = 14.633, P = 0.001), and sagittal (H (2) = 8.143, P = 0.017) planes were significantly affected by the angle of stifle flexion. Post hoc analysis showed that the ligament was best visualized at 90° compared to 145° in the dorsal (Z = ?3.906, P = 0.000), axial (Z = ?3.398, P = 0.001), and sagittal (Z = ?2.530, P = 0.011) planes. Findings supported the use of a 90° flexed stifle position for maximizing visualization of the cranial cruciate ligament using low‐field MRI in dogs.  相似文献

Objectives : This study aims to investigate lifestyle risk factors for the development of progesterone‐related diabetes mellitus in female elkhounds. Methods : Owners of 48 diabetic elkhounds and 58 healthy elkhounds were interviewed by phone concerning lifetime diet and exercise routines. A logistic model was developed to assess the impact of diet and exercise on diabetes diagnosis. The agreement between lifetime owner‐perceived body condition score (BCS) and veterinary‐perceived BCS at inclusion was estimated in healthy control dogs using the Kappa statistic. Results : The model showed that diabetic dogs had increased odds for having been overweight (before diagnosis) compared with controls (OR=2·8, 95% confidence interval 1·1–7·5, P=0·034). Although feeding other food than commercial dog feed was associated with diabetes case status, the effect was not significant after BCS was entered into the model. The overall agreement between lifetime owner‐ and veterinary‐perceived BCS at inclusion in the study was 75% and had a Kappa statistic of 0·16 (P=0·12). Clinical Significance : This study indicates that a high owner‐perceived lifetime BCS is associated with progesterone‐related diabetes in elkhounds.  相似文献

O bjectives : To investigate whether West Highland white terriers exhibit significant variation in the position of the medial fabella compared to both small and large breed dogs.
M ethods : Criteria for the normal location of the medial fabella on a caudocranial radiograph were established. A retrospective analysis was undertaken of a consecutive series of bilateral caudocranial stifle joint radiographs from 70 West Highland white terriers, 100 small and 100 large control dogs by three reviewers. Medial fabellar location, cruciate ligament disease and medial patellar luxation were examined within and between groups.
R esults : Abnormal medial fabellar location was identified in 70 per cent, 9 per cent and 0 per cent of West Highland white terriers, small dogs and large dogs, respectively. In the vast majority of the affected dogs, the fabella was found in a mediodistal location. Presence of concurrent cranial cruciate ligament disease or medial patellar luxation and body weight was excluded as confounding factors.
C linical S ignificance : West Highland white terriers appear predisposed to, and have a high prevalence of, an abnormal mediodistal location of the medial fabella. The authors suggest this is an incidental finding and should not be confused with true pathological fabellar displacement.  相似文献

Objective— To describe a method for radiographic measurement of proximal and distal mechanical joint angles of the canine tibia. Normal ranges were established for a population of dogs, and specifically, Labrador retrievers, with cranial cruciate ligament rupture.
Study Design— Retrospective study.
Sample Population— Tibiae (n=105) of dogs evaluated for cranial cruciate ligament rupture; 70 tibiae were from Labrador retrievers, 35 tibiae were from other breeds.
Methods— Anatomic landmarks were established and craniocaudal radiographs were used to measure the mechanical medial proximal (mMPTA) and distal (mMDTA) tibial angles in the frontal plane.
Results— Means (±SD) for all tibiae were mMPTA, 93.30±1.78°, and mMDTA, 95.99±2.70°. For Labrador retrievers, means were mMPTA, 93.38±1.81°, and mMDTA 96.34±2.51°. No significant differences were detected between Labrador Retrievers and non-Labrador retrievers with respect to mMPTA (power=0.5) or mMDTA (power=0.342). Labrador Retrievers were significantly younger than non-Labradors ( P =.003).
Conclusion— A method for measurement of the mechanical joint angles of the canine tibia in the frontal plane was established and reference ranges for a population of dogs and Labrador Retrievers with cranial cruciate ligament rupture are reported.
Clinical Relevance— The established method of measurement and references ranges can be used to aid in diagnosis, determining indications, and surgical planning for angular limb deformities of the tibia, especially when affected bilaterally. The methodology and reference values may also be used for postoperative critique.  相似文献
