OBJECTIVE: To evaluate the tensile strength, elongation, and degradation of 4 monofilament absorbable suture materials that undergo degradation by hydrolysis in specimens of canine urine with various physical characteristics. SAMPLE POPULATION: 4 monofilament absorbable sutures (polydioxanone, poliglecaprone 25, polyglyconate, and glycomer 631). PROCEDURE: Voided urine was collected from 6 healthy dogs, pooled, filter-sterilized, and prepared to provide 5 media: sterile neutral (pH, 7.0), sterile acidic (pH, 6.2), sterile basic (pH, 8.8), Escherichia coli-inoculated, and Proteus mirabilis-inoculated urine. Ten strands of each suture material were immersed in each of the media for 0 to 28 days. Tensile strength and elongation of each suture material were evaluated by use of a texture analyzer on days 0, 1, 3, 7, 10, 14, 21, and 28. RESULTS: Reduction in tensile strength was detected for all materials in all urine specimens over time. Polyglyconate and polydioxanone had superior tensile strengths in sterile neutral and E. coli-inoculated urine, and polydioxanone retained the greatest tensile strength throughout the study period. All suture materials disintegrated before day 7 in P. mirabilis-inoculated urine. CONCLUSIONS AND CLINICAL RELEVANCE: Polydioxanone, polyglyconate, and glycomer 631 may be acceptable for urinary bladder closure in the presence of sterile neutral and E. coli-contaminated urine. Tensile strength of poliglecaprone 25 in urine may be unacceptable by the critical healing time for bladder tissue (14 to 21 days). During bladder surgery, exposure of suture material that degrades via hydrolysis to urine containing Proteus spp should be minimized.  相似文献

Five different suture materials (silk, monofilament nylon, polyglyconate, polyglactin 910, and chromic gut) were placed in the skin and body wall of 10 Doitsu (scaleless) koi (Cyprinus carpio). After seven days the sutures were retrieved from five of the fish in 5 mm and 6 mm punch biopsies, and after 14 days they were retrieved in the same way from the other five. The tissue reactions were evaluated by gross visual inspection and by histological examination. The total inflammatory reaction was graded on a scale from 0 (no inflammation) to 5 (severe inflammation). The synthetic suture materials generally induced a moderate inflammatory reaction that decreased after seven days. After 14 days the superficial reaction to monofilament nylon was substantial, and the tissue reactions to the organic suture materials were slightly greater than the reactions to the synthetics. The inflammatory response to silk was greater after 14 days than after seven, and chromic gut induced a moderately severe inflammatory response after seven days; the chromic gut sutures fell out before the biopsies were taken after 14 days. The organic materials induced intense inflammatory reactions which did not subside if the suture remained in the tissue.  相似文献

Polydioxanone (PDS™) synthetic absorbable suture material was evaluated in 169 dogs and cats. Sutures or ligatures were used on 446 tissues including 18 contaminated and 16 infected wounds. There was less tissue friction than with polyglycolic acid or polyglactin 910 sutures. The suture tended to kink during placement of continuous suture patterns. There was one wound dehiscence.
Polydioxanone may be used wherever surgical gut, polyglycolic acid, or polyglactin 910 sutures are normally used.  相似文献

Seven suture materials (braided polyglycolic acid [BPGA, Dexon], multifilament stainless steel, monofilament nylon, black braided silk, braided Dacron (BD, Mersilene], and plain and chromic catgut) were implanted in surgical wounds which were inoculated with 3 dilutions of Staphylococcus aureus. The degree of swelling was measured at 6, 10, 20, and 40 days. Monofilament nylon and multifilament steel produced the least amount of swelling; plain and chromic catgut produced the most. The BPGA, BD, and silk caused swellings that were intermediate in degree. In chronic implantations, inflammatory reactions to the various suture materials differed from those occurring soon after implantation. Response to steel and nylon remained minimal; that to silk was gradual, but overall became the most severe. There was slightly less response with BD. The catguts varied little from each other in their responses. Intense reaction was induced by BPGA in the early stages, but the response was mild in chronic implantations.  相似文献

Suture materials are foreign bodies which induce and prolong tissue reaction in the wound area. Cellular responses to infected wounds containing 7 suture materials (braided polyglycolic acid [BPGA, Dexon], multifilament stainless steel, monofilament nylon, black braided silk, braided Dacron [Mersilene], and plain and chromic catgut) were studied at 6, 10, 20, and 40 days. Cellular reaction varied with different suture materials. In general, neutrophils were the predominant cells in acute infection, but later, macrophages and fibroblasts predominated. Occasionally, plasma cells, lymphocytes, eosinophils, and giant cells were present. In the infected wounds with implants of plain catgut, chromic catgut, silk, and braided Dacron, there were large numbers of neutrophils even in chronic implantation, indicating persistence of local infection. With nylon, steel, and BPGA, the number of neutrophils rapidly decreased. Although BPGA induced intense acute reaction, the response was mild in chronic implantation. It appeared to be absorbed more slowly than plain or chromic catgut, and tissue reaction was minimal in the absorption stages. A reaction similar to the Splendore-Hoeppli phenomenon was seen around multifilament silk strands in chronic stages of implantation.  相似文献

The knot security of chromic gut, polyglycolic acid, polyglactin 910, polydioxanone, polypropylene, and monofilament nylon size 2-0 suture materials were tested biomechanically in vitro. Twenty reproducible knots were tied and incubated in canine serum at 37 degrees for 24 hours before testing. A "secure knot" was defined as a knot that, when tested to failure, broke rather than untied by slippage. The minimum number of throws necessary to make a secure, snug (1500 g tension) square knot was three for gut, polyglycolic acid, polyglactin 910, and polypropylene and four for polydioxanone and nylon. All throws including the first were counted. With all suture materials tested, surgeon's knots were as secure as square knots. Only gut, polyglycolic acid, and polydioxanone granny knots were as secure as square knots; no loosely tied (500 g tension) asymmetric square knots were as secure as snug square knots, and only polydioxanone and polypropylene loose square knots were as secure as snug square knots. Square knots used to start a continuous pattern required one additional throw with gut, polydioxanone, and nylon. Square knots used to end a continuous pattern required two to three additional throws with all materials tested.  相似文献

Objective: To compare the mechanical, knotting, and absorption properties of 3 synthetic absorbable suture materials (polyglactin 910, polydioxanone, poliglecaprone 25) from 2 different manufacturers (Huaiyin Medical Instruments Co. Ltd. and Ethicon Inc.). Sample Population: Suture material samples from the same manufacturing lot for each suture type and manufacturer. Methods: Part 1: 10 samples of each suture were incubated in bovine serum and tested for ultimate load and stiffness at days 0, 1, 7, 14, 21, and 28. Part 2: 10 knotted samples were similarly evaluated, without incubation. Results: Huaiyin polydioxanone had a greater (P<.001) ultimate load on days 0, 1, and 7; and a consistently greater stiffness for the duration of the study, compared with the Ethicon polydioxanone. Ethicon poliglecaprone 25 had a greater ultimate load on days 0, 1, 7 (P<.001); on day 21 Huaiyin had a greater ultimate load. Ethicon poliglecaprone had greater stiffness on days 0, 1, 7 (P<.001). Ethicon polyglactin 910 had a greater (P<.001) ultimate load at all times and a greater stiffness (P<.001) at days 0, 1, 7, and 14 compared with Huaiyin polyglactin 910. Huaiyin polydioxanone and polyglactin 910 had greater knot breaking strengths than Ethicon equivalents (P<.001). Conclusion: Differences in mechanical properties exist between nominally identical suture materials from different manufacturers.  相似文献

The synovial tissue response to three different suture materials was assessed in nine dogs. Stifle joint arthrotomies were sutured with either chromic gut, teflon coated polyester suture, or polypropylene suture; all joint capsule layers were penetrated, exposing the suture material to the joint cavity. The synoviocytic and subsynviocytic layers at the arthrotomy site were examined histologically one, two, and three weeks following surgery. Chromic gut caused the greatest cellular inflammatory response and polypropylene the least. Teflon coated polyester and chromic gut were associated with nearly the same degree of tissue fibrosis. Polypropylene was associated with much less fibrosis than the other suture materials.  相似文献

Swelling and inflammation along the incision line were observed after elective ovariohysterectomy in 22 of 66 cats in a retrospective study. In a prospective study of 99 feline abdominal incisions closed with surgical gut, polyglactin 910, or polydioxanone, with and without subcutaneous closure, the least inflammation occurred when the linea alba was sutured with polyglactin 910 and the subcutaneous tissues were not sutured. Histologically, reactions in the linea alba of 12 other cats progressed from purulent to fibromononuclear to fibrous within 14 days after closure with gut, polyglactin 910, and polydioxanone. Microscopic evidence of seroma formation in 9 of 12 animals in which the subcutaneous tissue was not sutured suggested that surgical closure of subcutaneous dead space was indicated. The inflammatory reaction did not appear to be related to any one suture material as reactions were seen with all of them.  相似文献

The effects of fine suture materials on corneal wound healing in the dog were studied. In 20 dogs, standardized perforating perilimbal clear corneal wounds were made and closed with either monofilament Polyglactin 910 or nylon. Five dogs each were euthanized 8,12,16, and 21 days postoperatively. Results of gross, biomicroscopic, and histologic examinations at 8,12,16, and 21 days showed nylon and Polyglactin 910 to cause similar inflammatory responses. Epithelialization and suppuration around the suture tracts were observed more frequently when Polyglactin 910 was used. Both materials were associated with a foreign body (granulomatous) response. There was no loss of wound integrity with either material. Strengths of the incised corneal tissues with and without the sutures intact were determined. At day 16 postsurgery, there was a statistically significant difference in tissue strengths between cases in which sutures were and were not intact. This difference was not apparent at 21 days postsurgery, which suggested that the suture dependent phase ends between the 16th and 21st postsurgical days. In addition, at day 16 there was no statistical difference between the tissue strength of wounds sutured with nylon or Polyglactin 910. Corneal suture materials studied should remain in place for at least 16 days, and absorbable material is only appropriate if it retains tensile strength for 16 days.  相似文献

OBJECTIVE: To determine in vitro effects of PBSS, milk, and bacteria-contaminated milk (BCM; contaminated by Streptococcus agalactiae) on properties of 3 synthetic absorbable suture materials. SAMPLE POPULATION: 3 types of synthetic absorbable suture materials (poliglecaprone 25, polyglycolic acid, and polydioxanone). PROCEDURES: Suture materials were tested to determine breaking strength and elasticity before (day 0) and after incubation in 3 media (PBSS, milk, and BCM) for 7, 14, and 21 days. A loop of suture material was elongated at a rate of 60 mm/min until it reached the breaking point. Tensile properties were statistically analyzed among media and incubation times. RESULTS: Incubation in milk and BCM significantly decreased breaking strength and elasticity of poliglecaprone 25, compared with results for incubation in PBSS. Incubation in BCM significantly decreased tensile properties of polyglycolic acid suture, compared with results for incubation in PBSS and milk. After incubation for 21 days, tensile properties of polydioxanone did not differ significantly among the media but were significantly decreased from values on day 0. CONCLUSIONS AND CLINICAL RELEVANCE: On the basis of this study, poliglecaprone 25 is an inappropriate suture material for use in teat surgery. Polyglycolic acid suture should be avoided in teats of cattle with mastitis. Of the suture materials tested, polydioxanone was best suited for use in teat surgery, as determined on the basis of material testing after incubation in milk, even when the milk was contaminated with bacteria.  相似文献

Six enterotomies 3 cm long, spaced at 3 cm intervals, were made in the antimesenteric border of the pelvic flexure of the ascending colon in five adult horses. Ten incisions each were sutured with a Utrecht pattern (single layer), a full-thickness simple continuous oversewn with a Cushing pattern (two layer), and a mucosal simple continuous with a seromuscular simple continuous oversewn with a Cushing pattern (three layer). In all horses, chromic gut and polyglactin 910 were each used once for each pattern. On day 6, the pelvic flexure was excised and the colonic vessels were injected with a radio-opaque mass. Sections of each enterotomy were studied by light microscopy and high detail radiography. The single-layer closure led to serosa-to-mucosa healing and a thin bowel wall, and the three-layer closure resulted in distortion of the bowel wall by hematoma formation and submucosal contamination with ingesta. The two-layer closure resulted in good alignment and improved healing. There was less inflammatory reaction with polyglactin 910 than with chromic gut.  相似文献

Tensile testing of reproducible loops of size 5 braided polyester, size 2 polyglycolic acid, size 2 monofilament nylon, and size 2 polydioxanone sutures was performed in vitro and after subcutaneous implantation in horses for 7, 14, and 28 days. Maximum breaking strength, energy absorption, and percentage elongation to breakage point were measured. Size 5 braided polyester had the highest maximum breaking strength and energy absorption over 28 days. Polydioxanone had better mechanical performance over 28 days than did polyglycolic acid. On day 28, none of these parameters was measurable in polyglycolic acid. Monofilament nylon had the lowest breaking strength of the three materials tested on day 28.  相似文献

Twelve ponies were used to evaluate the reliability of an abdominal adhesion model and the efficacy of intraperitoneal infusion of sodium carboxymethylcellulose in preventing abdominal adhesions. A celiotomy was performed on each of the 12 ponies and the serosa of the distal portion of the jejunum was abraded with a dry gauze sponge at 5 locations. In addition to the serosal damage, a single 2-0 chromic gut suture was placed through the seromuscular layer of the jejunum in the center of the abraded area. After closure of the celiotomy, a 1% solution of sodium carboxymethylcellulose (7 ml/kg of body weight) was infused into the peritoneal cavity of 6 ponies. The other ponies served as untreated controls. All ponies were euthanatized 14 days after surgery. All ponies in the control group had abdominal adhesions at the time of necropsy. Four of the 6 ponies in the treatment group were free of adhesions. There was a significant (P less than 0.0001) difference in the total number of adhesions between the 2 groups.  相似文献

OBJECTIVE: To assess gross and histologic tissue responses of skin incisions closed by use of absorbable subcuticular staples, cutaneous metal staples, and polyglactin 910 suture in pigs. ANIMALS: 8 purpose-bred disease-free pigs. PROCEDURE: Pigs were randomly allocated to 1 of 4 groups from which tissues were collected after death on postoperative days (PODs) 7, 14, 21, or 42. In each pig, 4 incisions were made; 1 was closed subcuticularly with 3-0 polyglactin 910 suture, 1 was closed with metal staples, and 2 were closed with absorbable subcuticular staples. Incision sites were grossly evaluated every 3 days after closure. At necropsy, incision sites and surrounding tissues were examined histologically; a histopathologic scoring system was used to quantitate healing and tissue response directed against the closure material. RESULTS: Postoperatively, the metal staples induced a severe inflammatory response, compared with minimal inflammation associated with the suture or absorbable subcuticular staples. Histologic evaluation of incisions on PODs 7, 14, and 21 revealed less severe inflammation associated with absorbable subcuticular staples than that associated with the other materials. CONCLUSIONS AND CLINICAL RELEVANCE: Results indicated that absorbable subcuticular staples induced a less severe inflammatory response in the early stages of healing in pigs, compared with other commonly used methods of wound closure. Use of absorbable staples potentially combines the benefits of subcuticular closure with the speed and precision of staple placement.  相似文献

Objective: To test single cycle to failure tensile strength characteristics of 6 suture material–pattern combinations in equine superficial digital flexor (SDF) tenorrhaphy, specifically to compare a 10‐strand modification of the Savage core suture technique with the 3‐loop pulley technique. Study Design: Ex vivo mechanical experiment comparing 3 different suture patterns with 2 different materials. Sample Population: Forelimb and hindlimb SDF tendons (n=48) harvested from adult Thoroughbred and Standardbred horses of mixed age and gender. Methods: Six suture material–pattern combinations were evaluated: (1) 10‐strand Savage, size 2 polydioxanone (PDS); (2) 10‐strand Savage, size 2 polyglactin 910 (PG910); (3) 10‐strand Savage, size 2 PDS with Lin‐locking epitenon suture, size 2–0 PDS; (4) 10‐strand Savage size 2 PG910 with Lin‐locking epitenon suture, size 2–0 PDS; (5) 3‐loop pulley, size 2 PDS; and (6) 3‐loop pulley, size 2 PG910. Maximum load at failure (N), gap at failure (mm), and mode of failure (suture breakage or pull through) were evaluated for each of the 6 suture material–pattern combinations and underwent statistical analysis to determine significance of differences and interactions of the measured data. Results: The 10‐strand Savage technique failed at a mean load of 872 N (804–939, 95% CI). The 10‐strand Savage with Lin‐locking failed at a significantly greater mean load of 998 N (930–1065, 95% CI). The 3‐loop pulley pattern failed with a mean load of 337 N (270–405, 95% CI). There were significant interactions between the technique and suture material used. Conclusion: The 10‐strand Savage technique for repair of transected cadaver SDF tendons has superior strength characteristics with or without the epitenon suture when compared to the 3‐loop pulley. Suture material and pattern interactions were observed with PG 910 conferring higher strength for the 10‐strand Savage whereas PDS did so for the 3‐loop pulley.  相似文献

In an attempt to determine the best method for surgical removal of devitalized small colon lesions, 12 horses underwent a double small colon resection and end-to-end anastomosis. In 4 horses (study 1), an appositional single-layer (APP-1) suture pattern was compared with an inverting 2-layer (INV-2) suture pattern. In 8 horses (study 2), an appositional 2-layer (APP-2) suture pattern was compared with the INV-2 suture technique. Polydioxanone suture (size 1-0), was used. Horses were evaluated at necropsy 3, 10, 14, 28, or 56 days after surgery. Postoperative complications (peritonitis, impaction, or excessive adhesions) were encountered in 100, 42, and 13% of the APP-1, INV-2, and APP-2 anastomoses, respectively. Postmortem evaluation of the small colon revealed dehiscence of the anastomotic site, diffuse peritonitis, and adhesion formation in 3 of the 4 horses in which the resection line was closed with the APP-1 pattern. With the INV-2 and APP-2 techniques, more intestinal inversion was present in the nontaenial than in the taenial portion of the small colon. More postoperative impactions were found with the INV-2 (n = 5) anastomoses than with the APP-2 (n = 1) technique; this appeared to be the result of excessive tissue inversion. Chemical stability and in vitro bactericidal efficacy of 0.9% enrofloxacin‐compounded solutions were evaluated following storage at room temperature for 28 days. Chemical stability of enrofloxacin was determined by high‐performance liquid chromatography (HPLC) in five compounded solutions, including sterile water. Bactericidal efficacy was determined by spiral plating serial 10‐fold dilutions of bacteria and solutions followed by colony counts. Tris–EDTA [TrizEDTA® (TE)], Tris–EDTA and 0.15% chlorhexidine [TrizChlor® (TC)], 2.5% lactic acid, 0.1% salicylic acid and 0.1% parachlorometaxylenol [Epi‐Otic (EO)], and 0.1% free salicylic acid, 0.1% parachlorometaxylenol and 0.5% EDTA [Epi‐Otic Advanced (EA)] were used. High‐performance liquid chromatography was carried out with one‐step liquid/liquid extraction to detect and quantify enrofloxacin stability. Mean recoveries for compounded samples run in triplicate at 28 days were 97.7% (TE), 99.9% (TC), 98.1% (EO) and 97.8% (EA). Kruskal–Wallis analysis showed no significant difference in the percentage recovery (H = 0.0539, df = 3, P = 0.9967). American Type Culture Collection strains of Staphylococcus pseudintermedius and Pseudomonas aeruginosa were used to evaluate in vitro efficacy following 30 min incubation on days 0, 14 and 28. Consistent in vitro bactericidal efficacy of all compounded solutions, indicated by killing >2.3 × 107 colony‐forming units/mL, was seen; however, bactericidal efficacy decreased for compounded TC on day 14. Pseudomonas aeruginosa was more sensitive to the ear cleaners and enrofloxacin than S. pseudintermedius. The HPLC and in vitro data suggest that 0.9% enrofloxacin compounded with sterile water, TE, EO and EA maintains chemical stability and bactericidal efficacy for 28 days.  相似文献

This study investigated the potential for nephrotoxicity of gentamicin in cats by measuring marker enzyme concentrations, [Na], [K], osmolality, and pH of the urine, and blood urea nitrogen (BUN) levels. Gentamicin was administered i.m. at 4.4 mg/kg once daily (s.i.d.) or twice daily (b.i.d.) for 7 days. Concentrations of lactic dehydrogenase (LDH), lysozyme (LZM), alkaline phosphatase (AP), and glutamate dehydrogenase (GD) were measured as total 24-h excretions. The s.i.d. regimen produced only a slight increase in LDH excretion after 5 days, whereas the b.i.d. regimen caused an increase in the excretion of all enzymes. The greatest elevations were observed for LZM and LDH. Of the enzymes studied, these appeared to be the most appropriate to monitor for potential nephrotoxicity, except that urinary concentrations did not correlate well with duration of gentamicin administration. Only slight elevations in BUN were observed for either regimen. Single daily administration increased urine osmolality slightly, but b.i.d. treatment caused a marked and immediate decrease in urine osmolality, [Na], and total Na excretion. Urinary [K] was also depressed, as was total K excretion after 6 days. Urine pH was not substantially affected. This study showed that the recommended daily dose of 4.4 mg/kg produced little if any evidence of nephrotoxicity as indicated by the parameters measured. Twice daily dosing, however, produced elevations in urine enzyme concentrations, and markedly decreased urine osmolality and Na and K excretion. Compared to other species studied, the cat appears particularly sensitive to urine concentrating alterations resulting from repeated gentamicin administration.  相似文献

OBJECTIVE: To determine failure strength, stiffness, and failure mode of 4 suture materials in equine linea alba. STUDY DESIGN: Randomized complete block design. SAMPLE POPULATION: Linea albas collected from 12 adult horses (mean weight, 475 kg; mean age, 10 years). METHODS: The ventral abdominal fascia, including the linea alba, was collected and bisected along the linea alba into right and left halves. Each half was divided into four, 5-cm sections extending from the umbilicus cranially, and randomly assigned to 1 of 4 suture materials: 2 polydioxanone (2 PD), 3 polyglactin 910 (3 PG), 6 polyglactin 910 (6 PG), and 7 polydioxanone (7 PD). A single cycle to failure test was performed on each specimen at a distraction rate of 100 mm/min. Differences in failure strength and stiffness for the materials were evaluated using a mixed linear model with significance set at P<.05. RESULTS: In 94 of 96 test sections, constructs failed by suture failure. There were significant differences in failure strength (P<.0001) and stiffness (P<.001) among the suture/tissue constructs. 7 PD had the highest breaking strength (316.8 N) followed by 6 PG (281.3 N), 3 PG (229.9 N), and 2 PD (193.0 N). Six PG had the largest stiffness (14 N/mm) followed by 3 PG (12.7 N/mm), 7 PD (10.1 N/mm), and 2 PD (7.2 N/mm). Suture breaking strength and stiffness were not affected by linea or fascia thickness, individual horse, half of the linea alba, or abdominal wall position. Eighty-five (90.4%) suture loops failed adjacent the knot. CONCLUSIONS: When tested in single cycle to failure, suture material was more likely to fail than the linea alba. Larger suture materials had higher breaking strengths than smaller suture materials and stiffness was also affected by suture material and size. CLINICAL RELEVANCE: Given its high initial breaking strength and its relatively longer in vivo strength retention, 7 PD seems an appropriate choice of suture material for closure of the equine linea alba when maximal short-term failure strength is desired.  相似文献
