OBJECTIVE: To compare clinical outcome, healing, and effect of tracheostomy in conventional incisional and carbon dioxide (CO2) laser techniques for resection of soft palates in brachycephalic dogs. DESIGN: Prospective randomized trial. ANIMALS: 20 adult brachycephalic dogs. METHODS: Dogs were randomly allocated into 4 groups, and 1 of the following was performed: palate resection by use of a CO2 laser; incisional palate resection and closure with suture; and palate resection by use of a C02 laser or incision with tracheostomy. A clinical score for respiratory function was assigned to each dog at 0, 2, 8, 16, and 24 hours. Biopsy specimens of incision sites obtained at days 0, 3, 7, and 14 were examined. Data were analyzed to determine the effects of technique on clinical and histologic outcome. RESULTS: Mean surgical time for laser (309 seconds) was significantly shorter than for sharp dissection (744 seconds). Surgical technique significantly affected clinical scores at 3 of the 5 postoperative time points, but differences were not clinically apparent. Tracheostomy significantly affected clinical scores at 3 of 5 postoperative time points. After tracheostomy tube removal, clinical scores were similar to those of dogs without tracheostomies. Inflammation, necrosis, and ulceration were evident in all groups at day 3; these lesions had almost resolved by day 14. Most complications were associated with tracheostomy. CONCLUSIONS AND CLINICAL RELEVANCE: Clinical outcomes appear to be similar with the laser and incisional techniques. Regarding surgical time and ease, laser resection of the soft palate appears advantageous. Tracheostomy is not warranted in dogs that have uncomplicated surgeries and recoveries.  相似文献

CASE DESCRIPTION: A 7-year-old sexually intact male Thoroughbred racehorse was evaluated because of exercise intolerance, respiratory tract noise, and coughing when eating. CLINICAL FINDINGS: A persistent dorsal displacement of the soft palate was identified during endoscopic examination of the upper portions of the respiratory tract. Radiography of the pharyngeal and laryngeal regions revealed a hypoplastic epiglottis that was ventral to, and not in contact with, the soft palate. The horse was anesthetized, and an oral endoscopic examination revealed a subepiglottic frenulum that had resulted in the dorsal displacement of the soft palate. TREATMENT AND OUTCOME: The frenulum was transected transendoscopically by use of a diode laser. Twenty-four hours following surgery, repeat endoscopic and radiographic examinations revealed that the epiglottis had returned to its correct anatomic position in relation to the soft palate. Four weeks after surgery, endoscopy of the upper portions of the airway revealed recurrence of the dorsal displacement of the soft palate. CLINICAL RELEVANCE: A subepiglottic frenulum should be considered as a cause of persistent dorsal displacement of the soft palate in horses. An endoscopic examination of the oropharyngeal region should be performed in horses prior to undertaking any surgical interventions to treat persistent dorsal displacement of the soft palate.  相似文献

Verrucous pastern dermatitis is a progressive inflammatory skin disease commonly involving the palmar or plantar aspects of the pasterns of horses. There are no reports of successful surgical treatment for multifocal circumscribed verrucous masses in the chronic stages of verrucous pastern dermatitis. A combination of sharp dissection and electrocautery was used to resect numerous multifocal circumscribed verrucous masses from the distal hindlimb of an 11-year-old gelding draught horse. There was no evidence of significant regrowth or complications at a 24 month postoperative examination.  相似文献

Fifty-seven Standardbred and 44 Thoroughbred racehorses and 1 Thoroughbred polo mare with primary clinical signs of exercise intolerance or respiratory tract noise or combined exercise intolerance and respiratory tract noise were referred for laser correction of epiglottic entrapment. Significantly (P less than 0.001) more Standardbred than Thoroughbred racehorses were affected, compared with the observed hospital population during the same period. At referral, 14 horses did not have evident epiglottic entrapment and were returned to exercise without development of entrapment after treatment, which consisted of 1 week of rest and administration of anti-inflammatory medication. In 88 standing horses under sedation and topical anesthesia, epiglottic entrapment was corrected transendoscopically by use of a contact neodymium:yttrium aluminum garnet laser. In these 88 horses, 98% of entrapments were persistent, 92% were thick, 97% were wide, and 45% were ulcerated. Thirty-one percent of the horses had endoscopic evidence of epiglottic hypoplasia, and 8% had deviated epiglottic axis. Complete correction was achieved in 97% of the horses, Persistent dorsal displacement of the soft palate in 1 horse and severe epiglottic hypoplasia with thick, chronic entrapping membranes in 2 horses precluded successful transendoscopic correction with the horses in standing position. Most horses were treated on an outpatient basis, and all were able to be returned to exercise after 7 to 14 days of rest and treatment with anti-inflammatory medication. Entrapment recurred in 4 horses (5%), 3 of which had hypoplastic epiglottis. Dorsal displacement of the soft palate developed after surgery in 9 horses (10%) and continued in 4 horses (5%) that had displaced soft palate before surgery. All these horses had epiglottic hypoplasia. Laser correction of epiglottic entrapment in standing horses was safe, well tolerated, and effective. Laser surgery was an alternative to conventional surgery, and eliminated the need for general anesthesia and laryngotomy. It also reduced convalescence and postoperative complications.  相似文献

OBJECTIVE: To compare the effects of postoperative pain after ovariohysterectomy by harmonic scalpel-assisted laparoscopy (HALO) and traditional ovariohysterectomy (OVH) in dogs. STUDY DESIGN: A randomized, blinded, prospective study. SAMPLE POPULATION: Sixteen, purpose-bred, intact female, Beagle dogs. METHODS: Dogs were divided into 2 groups: Group 1 (8 dogs), which had OVH by HALO, and Group 2 (8 dogs), which had traditional OVH. Physiologic data, abdominal nociceptive threshold scores, and University of Melbourne pain scores (UMPS) were recorded at 2, 6, 12, 24, 48, and 72 hours after surgery. Blood samples for measurement of plasma cortisol, glucose, and creatine phosphokinase (CPK) concentrations were collected at the time of the incision, and 2, 6, 12, 24, 48, and 72 hours after surgery. RESULTS: No significant surgical complications occurred. The HALO mean surgical time was significantly longer (55.7 minutes) than traditional OVH (31.7 minutes). No significant differences were observed between groups for the pain measures of heart rate, respiratory rate, temperature, CPK, and glucose concentrations. The OVH group had significantly higher mean plasma cortisol levels at hour 2 after surgery than the HALO group (P=.0001). The mean UMPS were significantly higher in OVH than the HALO group at all postoperative times (P=.0001). The mean nociceptive threshold measurements revealed significantly higher tolerated palpation pressures in HALO than OVH at all postoperative times, except hour 72 (P=.0002). CONCLUSIONS: Dogs appeared to be in less pain with HALO than OVH. The harmonic scalpel coagulated ovarian and uterine vessels completely with minimal collateral damage to surrounding tissues. CLINICAL RELEVANCE: HALO is a safe alternative to OVH and offers a minimally invasive and less painful method of surgery.  相似文献

A submucosal abscess, located on the dorsal surface of the epiglottis, was diagnosed in 2 Thoroughbred racehorses by use of endoscopy. Both horses had exercise intolerance. One horse had intermittent dorsal displacement of the soft palate, coughed while eating and galloping, and made an abnormal respiratory noise. Both abscesses were drained transendoscopically by use of a contact neodymium:yttrium aluminum garnet laser. Eleven days after surgery, the surgical sites appeared to have healed. Clinical signs resolved permanently, and both horses returned to successful racing careers.  相似文献

OBJECTIVE: To compare postoperative signs of discomfort and complications associated with use of CO2 laser for onychectomy with those of the scalpel technique in cats. DESIGN: Prospective, randomized, masked clinical trial. ANIMALS: 20 client-owned cats. PROCEDURE: Forelimb feet (right, left) were randomly assigned to laser and scalpel treatment groups. Signs of discomfort (lameness and signs of pain) and complications (hemorrhage, swelling, and discharge) were assessed on days 0, 1, and 7. Surgeries were performed by 1 experienced surgeon. Evaluations were performed by 2 individuals without knowledge of treatment group. Signs of discomfort and complications were scored on scales of 0 to 8 and 0 to 9, respectively. RESULTS: Onychectomy did not result in high discomfort or complication scores 1 day after surgery, regardless of technique used, although the laser-treated group had significantly lower scores for signs of discomfort and complications. Seven days after surgery, significant differences were not detected between groups for signs of discomfort or complications. CONCLUSIONS AND CLINICAL RELEVANCE: The CO2 laser can be an excellent tool for onychectomy in cats, with excellent hemostasis and minimal postoperative discomfort and complications. Differences in discomfort and complications between groups treated via scalpel versus CO2 laser were not clinically relevant and were only observed 1 day after surgery.  相似文献

Objective— To compare racing performance before and after sternothyroideus myotomy, staphylectomy, and oral caudal soft palate photothermoplasty in Thoroughbred racehorses with intermittent dorsal displacement of the soft palate (DDSP).
Study Design— Retrospective study.
Animals— Thoroughbred racehorses (n=102) with DDSP.
Methods— Retrieved data included signalment, primary complaint, and upper respiratory tract endoscopic finding. Lifetime race records were compared for earnings per start before and after surgery, days to 1st start, and races won postoperatively.
Results— Comparing mean earnings per start for 3 races before ($2792) and after ($3806) surgery, racing performance improved significantly after surgery in 63% (46/73) of horses that competed in at least 1 race before and after surgery ( P =.02). Mean and median days to 1st start after surgery were 109 and 69 days, respectively. Of horses that raced postoperatively 65% (60/92) won at least 1 race, and 77% (71/92) raced in >5 races after surgery.
Conclusions— Sternothyroideus myotomy, staphylectomy, and oral caudal soft palate photothermoplasty significantly improved racing performance in experienced Thoroughbred racehorses with performance limiting DDSP, and overall had a similar outcome to other reported surgical techniques for treatment of DDSP.
Clinical Relevance— Sternothyroideus myotomy, staphylectomy, and oral caudal soft palate photothermoplasty should be considered as a surgical approach to correction of DDSP in Thoroughbred racehorses; however, it is possible that staphylectomy may not be necessary to achieve a desirable outcome.  相似文献

Dorsal displacement of the soft palate is a common cause of upper airway obstruction in racehorses and is of unknown aetiology. To determine whether the palate may displace for structural reasons, knowledge of the normal soft palate is required. The present study aimed to describe, qualitatively and quantitatively, the structure of the normal equine soft palate using histomorphometry. In soft palates from 12 Thoroughbreds, glandular tissue predominated (ca. 40% of total area), located mainly in the rostral and ventral regions. Rostrally, muscles attached to a tendinous aponeurosis located dorsal to the glandular tissue. Muscle was most abundant in the dorsal mid region and decreased caudally. The oral mucosa consisted of a non-keratinised stratified squamous epithelium whereas the nasopharyngeal mucosa was pseudostratified, columnar and ciliated. Elastin fibres were observed in the nasopharyngeal submucosa, becoming more prevalent caudally. The palates were bilaterally symmetrical although the proportion of tissue types varied considerably between individuals.  相似文献

An 18-month-old Welsh Cob was referred to a university teaching hospital for investigation of abnormal respiratory noise at rest. Video-endoscopy revealed persistent displacement of the soft palate with bilateral 2–3 cm diameter raised areas on either side of the midline. Neither trans-nasal nor oral approaches allowed sufficient visualisation of the caudal soft palate for diagnosis or treatment. Latero-lateral radiographs showed a blunted, thickened caudal edge of the soft palate consistent with a palatal cyst. Two palatal cysts were thermally ablated with a transendoscopic diode laser via temporary tracheotomy. The horse recovered uneventfully and resolution of the displacement was evident on follow-up endoscopy 11 months later. This report documents a novel surgical approach to the caudal aspect of the soft palate.  相似文献

OBJECTIVE: To evaluate short-term postoperative forelimb function after scalpel and laser onychectomy in cats. DESIGN: Randomized, prospective study. ANIMALS: 20 healthy adult cats. PROCEDURES: Cats were randomly assigned to the laser (n = 10) or scalpel (10) onychectomy group. Unilateral left forelimb onychectomy was performed. In the scalpel group, a tourniquet was used during surgery and a bandage was applied after surgery. Pressure platform gait analysis was performed prior to and 1, 2, 3, and 12 days after onychectomy. Peak vertical force (PVF), vertical impulse, and the ratio of the PVF of the left forelimb to the sum of the remaining limbs (PVF ratio) were used as outcome measures. RESULTS: The laser onychectomy group had significantly higher ground reaction forces on days 1 and 2 and significantly higher PVF ratio on day 12, compared with the scalpel group. Similarly, significant differences were found in change in ground reaction forces on days 1 and 2 and the PVF ratio on day 12, compared with day -1. No cats required rescue analgesia during the course of the study. One cat in the laser group had signs of depression and was reluctant to walk on day 2 after surgery, had physical examination findings consistent with cardiac insufficiency, and was euthanized. CONCLUSIONS AND CLINICAL RELEVANCE: Cats had improved limb function immediately after unilateral laser onychectomy, compared with onychectomy with a scalpel, tourniquet, and bandage. This improved limb function may result from decreased pain during the 48 hours following unilateral laser onychectomy.  相似文献

Transendoscopic neodymium:yttrium-aluminum-garnet (Nd:YAG) laser was used to treat 12 standing horses with epiglottic entrapment (EE) or dorsal displacement of the soft palate (DDSP), or both. In four horses, transendoscopic laser staphylectomy was performed. The most common presenting complaints were respiratory stridor, cough, and exercise intolerance. Ten horses with EE healed without epiglottic complications; in one horse, partial adhesion of the aryepiglottic fold to one side of the epiglottis was corrected surgically through a laryngotomy incision. One horse with DDSP had no further signs, one continued to have continual DDSP, and two had induced DDSP. Transendoscopic Nd:YAG laser proved to be a feasible means of correcting EE and selected cases of DDSP.  相似文献

A 5-month-old female chinchilla (Chinchilla lanigera) was evaluated for failure to thrive, difficulty drinking, and respiratory signs. Physical examination demonstrated a thin body condition score, purulent nasal and ocular discharge, and an abnormal facial appearance. Endoscope-guided oral examination and computed tomography (CT) scan demonstrated a cleft soft palate, absent nasal turbinates, and bilateral otitis media. The animal was euthanized and necropsy confirmed the diagnosis of palatoschisis (cleft palate) of the soft palate. Other relevant postmortem findings included otitis media, interna, and externa, bony bullae proliferative changes, absent nasal turbinates and rhinosinusitis. Congenital abnormalities are rarely reported in chinchillas. While cleft palates have been reported in many other domestic animals, there are no published reports of this condition in chinchillas to the best of the authors’ knowledge.  相似文献

Eighteen dogs were used to compare histopathological findings following excision of the soft palate using either a bipolar sealing device or a carbon dioxide laser. Histopathological comparisons were done at 48 and 96 hours after soft palate resection. Mean depths of tissue injury at 96 hours were 3.5 and 3.33 mm for bipolar sealing device and carbon dioxide laser, respectively. Control of hemorrhage was excellent in all dogs, and none of the dogs developed signs of respiratory compromise after soft palate resection. Using the bipolar sealing device for soft palate resection was significantly faster than using the carbon dioxide laser, although both techniques were fast.  相似文献

Palmar digital neurectomy is typically performed as a treatment for palmar foot pain that is unresponsive to medical treatment and therapeutic shoeing. Although it is useful for the elimination of pain related to navicular disease or wing fractures of the distal phalanx, a number of potential complications can occur after this procedure, including painful neuroma formation, persistence of skin sensation, reinnervation, and rupture of the deep digital flexor tendon. Over the years, surgical techniques have been developed to minimize complications associated with digital neurectomy, including sharp dissection (the guillotine method), epineural capping, and cryosurgical treatment of the nerve, using a double freeze-thaw cycle. Alternatively, lasers may be used to divide the palmar digital nerve with coagulation of the nerve endings. The thermal effects of the laser theoretically help to minimize the potential for painful neuroma formation and reinnervation by sealing the cut surface of the nerve endings, preventing adhesion formation with surrounding soft tissues, and delaying the reanastomosis of the proximal and distal nerve segments.  相似文献

Dorsal displacement of the soft palate associated with dysphagia and aspiration pneumonia was diagnosed in 2 young foals. The displacement appeared to be congenital. Clinical signs associated with the condition were resolved after staphylectomy. Dorsal displacement of the soft palate is usually recognized in adult animals as an upper respiratory tract disease, but may be a contributing factor to dysphagia and lower respiratory tract disease in young animals.  相似文献

Review of medical records of 78 horses admitted to the George D. Widener Hospital for Large Animals with dorsal displacement of the soft palate revealed 94% of these horses to have evidence of an intermittent abnormal "gurgling" respiratory noise at the time of exercise. Sternothyrohyoideus myectomy was used as a primary treatment for 17 of these horses, with a success rate of 58%. Anatomic dissection of 30 horses indicated that the midcervical region is the optimal site for sternothyrohyoideus myectomy to alleviate dorsal displacement of the soft palate.  相似文献

Palatal sclerotherapy using sodium tetradecyl sulfate has been suggested as a treatment for dorsal displacement of the soft palate in young Standardbred horses. The present study evaluated histological and biomechanical changes in the equine soft palate following trans-endoscopic treatment with a low dose of this compound. Two horses were euthanased and examined at 2 weeks and at 1, 2, 4 and 6 months post-sclerotherapy, while two further horses served as untreated controls. The technique was easily performed in all cases without major complications. On histological examination there was no evidence of palatal necrosis, inflammation or fibrosis in any of the treated or control animals. There was no variation in the density of palatal connective tissue between individuals, and on biomechanical assessment no significant difference in the stiffness of the palatal tissue was found between treated and control horses at any time. The lower dose of sodium tetradecyl sulfate used in this study relative to previous reports, might explain the absence of tissue alterations. This method of sclerotherapy did not alter the morphology or biomechanical properties of normal equine soft palates.  相似文献

A 10-year-old Thoroughbred mare was referred to Tennessee Equine Hospital for evaluation and treatment of a subepiglottic mass. Physical and laboratory examinations revealed no other abnormalities. The tumour was excised through a laryngotomy site using transendoscopic laser and sharp excision. Histological evaluation revealed a poorly differentiated sarcoma with features of a leiomyosarcoma, a rarely identified neoplasm in the oropharynx of the horse. The mare developed intermittent dorsal displacement of the soft palate (iDDSP) after surgery but was able to resume full training. There was no evidence of regrowth 8 months post-operatively.  相似文献
