A 1.2-year-old male-intact Standard Poodle underwent transvenous placement of an Amplazter™ atrial septal occluder for correction of a large secundum atrial septal defect. Thirty-six hours post-operatively, the dog developed high-grade Mobitz type II second-degree atrioventricular block, which resolved with time and corticosteroid administration by 12 days after the procedure. This case report outlines the observation, treatment, and resolution of high-grade Mobitz type II second-degree atrioventricular block, a known complication of atrial septal occluder placement in humans, not previously reported in veterinary literature.  相似文献

The purpose of this study was to perform temporary atrial pacing and to determine the atrial strength-duration (S-D) curve which displays the minimal pulse intensity necessary to achieve atrial capture. In 7 horses, atrial pacing was applied using a temporary pacing catheter and a pacemaker as electrical pulse generator. Using the stimulus reduction method, 3 approaches for atrial threshold determination were used. With the fixed pulse width method, at several pulse widths, the corresponding minimal amplitudes to achieve capture were determined, describing an S-D curve. With the fixed amplitude method, the corresponding threshold pulse widths were determined at several fixed amplitudes. The third method proved to be the best and was a combination of both aforementioned methods to determine 2 points of the S-D curve. From these 2 points the whole S-D curve was calculated using a mathematical equation. Temporary pacing can be used to terminate atrial flutter, to induce atrial arrhythmias or to obtain more information about the electrophysiological properties of the heart, such as the atrial refractory period, atrial vulnerability and atrioventricular conduction.  相似文献

The case reported herein describes the placement of a permanent transvenous pacemaker in an older dog with a previously undiagnosed persistent left cranial vena cava (PLCVC) and recent onset symptomatic third-degree atrioventricular (AV) block. On presentation the dog was found to have atrial flutter and third-degree AV block and echocardiography demonstrated evidence of chronic valvular disease and pulmonary arterial hypertension. The persistent left cranial vena cava was discovered via angiography when difficulties were encountered with pacemaker placement. Successful right ventricular pacing necessitated passage of the lead through the coronary sinus. The attendant complications in pacemaker placement in the presence of a PLCVC are well-described in man but, to the authors' knowledge, have not been described in companion animals.  相似文献

A 14.5-kg, 13-year-old female spayed Cocker spaniel was evaluated because of episodic hind limb weakness. Results of examination were consistent with sick sinus syndrome with intermittent second-degree atrioventricular block. Transesophageal atrial pacing was successful in providing chronotropic support during permanent pacemaker implantation. Transesophageal atrial pacing appears to be a viable option for temporary atrial pacing in dogs with hemodynamically marked bradycardia without significant atrioventricular blockade.  相似文献

This case report describes transient atrioventricular block as the etiology for syncopal events in a 6-year-old male German shepherd dog with atrial fibrillation and dilated cardiomyopathy. The arrhythmia diagnosis was obtained via Holter monitoring. Medical treatment with a sustained-release preparation of theophylline, as an additive to the standard congestive heart failure treatment (benazepril, furosemide and pimobendan) may have contributed to temporary remission of the syncopal events. However, the congestive heart failure progressed and the dog was euthanized. Veterinarians should be aware of the possibility of transient atrioventricular block causing syncope in dogs with DCM and AF and should be careful in empirically lowering the ventricular response rate if these dogs present with syncopal episodes.  相似文献

Five dogs were presented to our institution for fatigue caused by an incessant supraventricular tachycardia. In all dogs, an ECG on admission showed a narrow QRS complex tachycardia with a median ventricular cycle length of 220 ms (range 180–360 ms), and a positive atrial depolarization identifiable in the ST segment following the previous QRS complex. There was a 1:1 atrioventricular conduction ratio in all but one dog, which presented with 2:1 atrioventricular block. Electrophysiologic studies identified the underlying arrhythmogenic mechanism as a right atrial macro-reentrant tachycardia with two distinct isthmic areas: right septal (RS) in three dogs and right atrial free wall (RAFW) in two dogs. Linear radiofrequency catheter ablation was performed during tachycardia in all dogs at the identified isthmic area, which acutely blocked the macroreentrant circuit. At 18-month follow-up, 3 dogs (1 with RAFW isthmus and 2 with RS isthmus) showed no recurrence of the arrhythmia on Holter monitoring. One dog with RS isthmus showed recurrence of the supraventricular tachycardia 15 days post-ablation, and 1 dog with RAFW isthmus presented with persistent atrial fibrillation 2 months post-ablation.  相似文献

Transvenous electrical cardioversion was attempted in a horse with drug refractory atrial fibrillation. A temporary pacing catheter and two defibrillation catheters were inserted transvenously into the right ventricular apex, the right atrium and the pulmonary artery, respectively. Under general anaesthesia 100, 200, 300 and 360 J monophasic shocks were delivered between both defibrillation catheters but sinus rhythm could not be restored. Immediately after the 200, 300 and 360 J shock, transient third-degree atrioventricular block occurred for a period of, respectively, 15, 40 and 55 s. These periods of profound bradycardia were corrected by temporary right ventricular pacing until spontaneous conduction resumed. It is concluded that temporary right ventricular pacing should be available during electrical cardioversion of atrial fibrillation in horses.  相似文献

This report describes a dog with recurrent atrial fibrillation and atrial flutter associated with chronic gastritis. Although no underlying structural heart disease was apparent antemortem or on gross post-mortem examination, chronic atrial myocarditis was seen on histopathological examination. Atrial myocarditis is a recognised cause of atrial fibrillation in human beings with presumed lone fibrillation, and an association between supraventricular tachyarrhythmias and infectious agents has been made.  相似文献

A nine-and-a-half-year-old dog was referred for investigation of bradycardia after being hit by a car. Electrocardiography revealed a third degree atrioventricular (AV) block. The dog died during a syncopal attack. Post mortem examination revealed an atrial septal tear with haemorrhage and myocardial degeneration affecting the AV bundle.  相似文献

A two-year-old mixed breed dog was referred to the University of Florida Veterinary Medical Teaching Hospital for the management of post traumatic ventricular arrhythmias. Electrocardiography revealed third degree atrioventricular (AV) block. An echocardiogram demonstrated an echogenic, pedunculated mass arising from the caudoventral aspect of the atrial septum. The mass was believed to represent a haematoma or a thrombus associated with trauma to this region of the atrial septum. The patient was managed conservatively; third degree AV block was still pre- sent five days after trauma. The owner of the dog refused specific therapy of the arrhythmia. The patient was discharged and, unfortunately, lost to follow-up. Third degree AV block has not previously been reported following accidental, blunt thoracic trauma in the dog.  相似文献

Conversion of atrial flutter was accomplished by the combined therapy of quinidine and digitalis. Neither drug used alone was successful. The case history and method of treatment is discussed, along with the action of these two drugs, when used in combination. Electrocardiograms are presented to show the sequence of events during and after therapy.
Résumé. Une conversion du flutter auriculaire a été effectuée par un traitement combiné par la quínidine et la digitale. Aucun des deux médicaments utilisé seul, n'a donné de résultat. Cette observation et le mode de traitement ont été discutés ainsi que l'action de l'association des dew produits. On présente les électrocardiogrammes montrant l'évolution clinique pendant et après le traitement.
Zusammenfassung. Die Behandlung des Vorhofflatterns wurde erfolgreich mit der kom-binierten Anwendung von Chinidin und Digitalis vorgenommen. Keines der beiden Medi-kamente brachte bei alleiniger Anwendung den Erfolg. Die Krankengeschichte und die Behandlungsmethode werden besprochen, zugleich mit der Wirkungsweise der beiden Medikamente bei kombinierter Anwendung. For obtaining the preliminary data on the pathogenesis of sudden death in calves naturally heavily infected with Strongyloides papillosus, we monitored 8 Holstein calves experimentally infected with the larvae on electrocardiographic and pneumographic changes. Six calves died suddenly on days 11 to 17 after infection. Sinus tachycardia had been recorded continuously since 1 to 6 days before death. Heart rates increased gradually until death. Since 1 or 2 days before death, various patterns of tachyarrhythmia and bradyarrhythmia had been observed among patterns of sinus tachycardia. Arrhythmias included serious ventricular premature beat, paroxysmal ventricular tachycardia, complete atrioventricular block and so on. The terminal pattern observed suddenly in all of the cases was ventricular arrhythmias consisting of serial ventricular tachycardia, flutter and fibrillation, which were followed by respiratory arrest. Abnormal pneumograms were not obtained before the terminal ventricular fibrillation. Two of 8 calves recovered from the infection, only one of which showed sinus arrest and the second degree of atrioventricular block transiently. We concluded that calves heavily infected with the larvae died due to sudden cardiac arrest.  相似文献

The sites of possible atrial ectopic foci are discussed in relation to the anatomy of the atrial chambers and the orientation of the P vectors associated with atrial ectopic beats.

The clinical findings in a dog with sinoatrial arrest, atrioventricular block and multifocal idioventricular escape beats are described. The clinical findings were associated with a primary myocarditis involving the sinoatrial node, the left and right atrium and atrioventricular node. A permanent, transvenous, atrioventricular, sequential pacing system was implanted successfully in a 7-year-old Quarter Horse gelding with complete heart block. The transvenous atrial and ventricular electrodes were placed in their respective positions in the heart, using real-time, 2-dimensional echocardiography. An asymptomatic nine-year-old Dobermann Pinscher underwent a screening for dilated cardiomyopathy. Echocardiography revealed left ventricular eccentric hypertrophy and systolic dysfunction; the rest of the echocardiographic parameters were within normal limits. Holter monitoring demonstrated sinus rhythm as the dominant cardiac rhythm during the first hours of the recording. Then, during a period of physiologically enhanced vagal tone (sleep), spontaneous development of atrial flutter (AFL) associated with variable ventricular response was documented. Alternation between AFL and paroxysmal atrial fibrillation was also observed. Subsequently, during a period of physiological increase of sympathetic tone (physical activity/excitement), spontaneous conversion of AFL to sinus rhythm occurred. In light of these findings, a presumptive diagnosis of vagal AFL was made. The images here described allow us to study the onset, behavior and termination of this intriguing electrocardiographic entity.  相似文献

A Labrador retriever, presented for a routine yearly wellness exam, shows on ECG a normal sinus rhythm with first degree atrioventricular blockade and atrial dissociation (AD). A detailed cardiologic examination was performed including thoracic radiography, echocardiography and blood pressure measurement, all of which were considered normal. The dog described in this current report was re-examined several times over the subsequent 2 years and no clinical signs of heart failure, heart disease or other diseases were recognized, the AD appears to have been benign.  相似文献

A collie-cross dog with syncopal attacks associated with total atrioventricular heart block was successfully fitted with a programmable, ‘physiological’ pacemaker, employing atrial sensing and ventricular pacing leads. Transvenous placement of leads under sedation and local analgesia, followed by temporary pacing, then general anaesthesia for the fitting of a permanent pacemaker, offers advantages over epicardial pacing. The use of a ‘tined’ ventricular lead appears to reduce the risk of lead displacement.  相似文献

The objective of this study was to assess the type and frequency of cardiac dysrhythmias occurring after routine ovariohysterectomy or orchidectomy in young, healthy dogs by using 2 anesthetic protocols (group I: propofol and isoflurane; group II: thiopental and halothane). Fifty dogs under 2 years of age, judged to be clinically normal by physical examination and standard electrocardiography, were evaluated by using 24-hour ambulatory electrocardiography. The most common dysrhythmias in the postoperative period were 2nd degree atrioventricular block (44%), ventricular premature complexes (44%), and atrial premature complexes (32%). For study purposes, more than 100 ventricular or atrial premature complexes per 24 hours, or any occurrence of R-on-T phenomenon, ventricular or atrial tachycardia were classified as clinically significant arrhythmias. Significant arrhythmias were observed in 9 dogs in the postoperative period, 5 of which were in group I and 4 in group II. All of these dogs were under 1 year of age. The R-on-T phenomenon occurred in 4 dogs in group II and 1 dog in group I. Results from this study show that significant arrhythmias, including R-on-T phenomenon, can occur in the perioperative period in young, healthy dogs undergoing routine surgeries with both protocols used.  相似文献
