Chronic orocutaneous, oronasal, or orosinus fistulae can be challenging to repair. This report describes placement of a dental bridge as a treatment for oral fistulae and outcome in two horses. A 12-year-old, Westphalian gelding was presented for nasal discharge because of an oromaxillary fistula after dental repulsion of the Triadan 209 4 years earlier. The second case, a 5-year-old Oldenburg mare was presented for an orocutaneous fistula 7 weeks after dental repulsion of the Triadan 208. Both were treated by placing an 8-shaped cerclage wire, inserted through the rostral and caudal interdental spaces and crossing at the level of the missing tooth. Polymethylmethacrylate (PMMA) was then used to seal the fistula. No significant complications occurred during or after the surgery. In both cases clinical signs did not recur. In the first case the dental bridge is currently (5 years after the intervention) in place. In Case 2 the construction was removed after 1.5 years and the fistula had healed completely. This case report suggests that placing a dental bridge composed of cerclage wire and PMMA should be considered as an easy, noninvasive and efficient way to manage large oromaxillary or orocutaneous fistulae.  相似文献

A 3-year-old Standardbred gelding was referred for evaluation of a fluctuant swelling that developed over the dorsum of the carpus subsequent to arthroscopic surgery on that carpus. A synovial hernia was diagnosed and surgically repaired. Although complications secondary to arthroscopy are rare, synovial herniation can develop after arthroscopic surgery. The condition must be differentiated from the more common true bursitis or false bursitis that is commonly referred to as a hygroma.  相似文献

A 9‐year‐old Paint pony gelding presented for signs of left carpal swelling of 1–2 weeks' duration. Radiographic, ultrasonographic and arthroscopic evaluation of the left carpus was consistent with synovial osteochondromatosis. This presumptive clinical diagnosis was confirmed histopathologically. Arthroscopic removal of the osteochondral bodies resulted in resolution of the carpal effusion and return to previous athletic activity by 4.5 months post operatively. Arthroscopic removal of osteochondral bodies is the treatment of choice in cases of suspected synovial osteochondromatosis.  相似文献

A four-year-old gelding was lame owing to a chronic septic common digital extensor tendon and sheath. The horse had been treated by open surgical lavage but the sepsis had recurred after three months. Physical, ultrasonographic, cytological and histological examinations confirmed chronic septic tenosynovitis and tendonitis. The entire intrathecal component of the common digital extensor tendon was resected under general anaesthesia and the synovial lining of the sheath was ablated. Postoperatively the horse regained good limb function and became sound.  相似文献

A 13‐year‐old Quarter Horse gelding was presented for repair of a central tarsal bone fracture. Radiographs showed a single large nondisplaced sagittal slab fracture; however, computed tomography (CT) revealed an additional small, displaced central tarsal bone fragment. The complex fracture was repaired, under the same anaesthetic period as the CT examination, using the CT hard copies images, intraoperative fluoroscopy and radiographs. The horse recovered well and after rehabilitation showed no residual lameness at work. Computed tomography was instrumental in achieving precise screw placement and successful fracture repair.  相似文献

Pharmacokinetic parameters were established for enantiomers of the nonsteroidal anti-inflammatory drug (NSAID) ketoprofen (KTP) administered as the racemic mixture at a dose of 2.2 mg/kg and as separate enantiomers, each at a dose of 1.1 mg/kg to a group of six horses (five mares and one gelding). A four-period cross-over study in a LPS-induced model of acute synovitis was used. After administration of the racemic mixture S(+)KTP was the predominant enantiomer in plasma as well as in synovial fluid. Unidirectional inversion of R(-) to S(+)KTP was demonstrated but the inversion was less marked than previously reported. It is suggested that this reduction could be because of the influence of the inflammatory reaction on hepatic metabolism. The disposition of KTP enantiomers after administration of the racemic mixture was similar to those observed after administration of S(+) and R(-)KTP. The S(+) and R(-)KTP concentrations in synovial fluid were low and short lasting. After administration of R(-)KTP significant concentrations of the optical antipode were detected in synovial fluid.  相似文献

OBJECTIVE: To describe repair of chronic palatine defects in cats, with free cartilage graft harvested from either the pinna (scapha) or vertical ear canal (annular cartilage). STUDY DESIGN: Retrospective study. ANIMALS: Cats (n=5) with chronic oronasal fistula. METHODS: Cartilage was harvested from either the scapha or annular cartilage and epithelium removed. After preparation of the edges of the palatal defect by separation of the oral and palatal mucosa circumferentially, the graft was inserted between the epithelial layers and secured without tension. The graft acted as a scaffold for migration of granulation tissue and epithelialization. RESULTS: Oronasal fistulae were successfully repaired in 5 cats (3 pinna grafts; 2 annular cartilage grafts); 1 cat required a second graft after the first graft was dislodged. CONCLUSIONS: Auricular cartilage provides a reliable framework for repair of oronasal fistulae in cats. CLINICAL RELEVANCE: Chronic oronasal fistulae refractory to conventional repair can be treated by use of auricular free graft (pinna or annular cartilage) without disfigurement. Use of annular cartilage to support palatal repairs offers surgeons an additional option when other methods have failed. We recommend using conchal cartilage as the initial treatment approach for repair of small oronasal fistulae in cats.  相似文献

Uroperitoneum as a sequela to urethral calculus in an adult gelding was successfully managed by use of subischial urethrotomy and abdominal drainage. Necrosis of bladder mucosa was seen endoscopically, but a tear or rupture was never identified. Peritonitis developed but was controlled with antibacterial treatment. Although uroperitoneum is usually a sequela to bladder rupture and the ideal treatment is surgical repair, conservative management may be warranted in selected cases.  相似文献

CASE DESCRIPTION: A 12-year-old Standardbred gelding was referred for swelling of the right metacarpophalangeal joint. CLINICAL FINDINGS: Ultrasonography of the right metacarpal area revealed hypoechoic areas in the right digital sheath and metacarpophalangeal joint consistent with synovial effusion. Radiography of the right metacarpophalangeal joint revealed lysis of the axial border of the proximal sesamoid bones. Aspergillus fumigatus was detected on fungal culture of synovial fluid. TREATMENT AND OUTCOME: Regional limb perfusion (150 mg of amikacin in 60 mL of saline [0.9% NaCl] solution perfused for 30 minutes) was performed 2 and 4 days after admission. Itraconazole (5 mg/kg [2.27 mg/lb], PO, q 24 h) was administered for approximately 9 weeks. Joint lavage with amikacin (500 mg) in 1 L of saline solution was performed 4 times. Three months after discharge, the owner reported that the horse was mildly lame during trotting but was moving freely and comfortably during all gaits and had gained a considerable amount of weight. Because the osteoarthritis was not expected to improve and because it was recommended that the horse not return to purposeful exercise, the owner decided to retire the horse from racing. CLINICAL RELEVANCE: Various diagnostic imaging methods and fungal cultures are useful for diagnosing fungal osteomyelitis of the axial borders of the proximal sesamoid bones in horses. Fungal osteomyelitis of the sesamoid bones and erosive arthritis should be considered as a differential diagnosis for horses in which corticosteroids have been administered intra-articularly.  相似文献

The case records of 47 mares with third-degree perineal lacerations or rectovestibular fistulae were examined to evaluate their fertility following surgical repair. Of 32 mares bred, 24 became pregnant, suggesting that surgical repair is indicated in any mare with sufficient genetic potential. Perineal trauma after surgical repair was recorded in 3 of 20 mares at subsequent parturition.  相似文献

OBJECTIVE: To describe the surgical treatment of a dorsal diaphragmatic hernia in an adult horse using thoracic rib resection aided by thoracoscopy and a flank incision. STUDY DESIGN: Case report. ANIMAL POPULATION: One client-owned horse. RESULTS: A six-year-old Dutch warmblood gelding was admitted for colic-associated colonic incarceration in a diaphragmatic hernia. Seven weeks after the initial colic surgery, the gelding underwent surgery to repair the defect. Thoracoscopy and a flank incision were used to identify the location of the hernia and the subsequent site of rib resection. The stomach was adhered to the edges of the defect, leaving only a small residual hole. Resection of the 14th rib provided excellent exposure of the defect and closure of the remaining hernia. CONCLUSION AND CLINICAL RELEVANCE: Thoracic rib resection can provide access to diaphragmatic hernias in adult horses. Thoracoscopy or a flank incision, or both, may aid in determining which rib is best resected.  相似文献

A 12-year-old American Saddlebred gelding was referred to a veterinary teaching hospital for evaluation of a chronic lameness problem in the right radiocarpal joint. The horse had been treated for osteoarthritis of the right radiocarpal joint with multiple injections of cortisone during the past 3 years. The horse was severely lame on the right forelimb at a trot. Radiography and computed tomography revealed a 3 x 2-cm lytic defect in the distal portion of the radius and periarticular bone proliferation around the right radiocarpal joint. Ultrasonography of the distal portion of the radius revealed a soft tissue mass in the palmarolateral aspect of the joint. Proliferative synovium with a large amount of fibrin was observed in the dorsal and palmar aspects of the joint via arthroscopic examination of the right radiocarpal joint. Histologic examination of synovial biopsy specimens revealed proliferative granulomatous synovitis with giant cells. Mycobacterium avium complex was cultured from the synovial fluid. Infection with M avium complex should be considered in horses with chronic recurring arthritis associated with granulomatous synovitis.  相似文献

A 13-year-old, 370-kg, Appaloosa gelding with mild colic of 4 days' duration was found to have a near circumferential retroperitoneal rectal tear. Accumulation of feces at the site of the repair had created a large perirectal cavity. Removal of feces and cleansing of the perirectal cavity were performed daily. Sutures were placed to stabilize, but not reappose, the torn ends of the rectum. The perirectal cavity filled with granulation tissue, and the rectal tear healed.  相似文献

A 3-year-old Standardbred gelding (Case 1) and a 2-year-old Thoroughbred gelding (Case 2) were referred for surgical evaluation of a left radial carpal bone (RCB) fracture, sustained during training. Clinical findings at the time of initial examination included a palpable effusion within the left middle carpal joint in both horses and marked signs of pain and reduced range of motion on flexion of the affected carpus. In both horses, the RCB fracture was evident on the following radiographic views of the carpus: dorsolateral–palmaromedial oblique (30° off lateromedial) and flexed lateromedial. An additional loose wedge-shaped osteochondral fragment at the proximal articular surface of the RCB could be seen in Case 2. Both horses underwent surgical reduction and repair of the fracture between 1 and 2 days following the initial injury, which consisted of arthroscopic removal of any intra-articular osteochondral fragments, and arthroscopic assisted-interfragmentary compression via a standard dorsomedial and dorsolateral approach to the antebrachiocarpal joint (ACJ) and middle carpal joints (MCJ). The two horses returned to function as racehorses, 6 months (Case 1) and 16 months (Case 2) after surgery. The RCB is a relatively uncommon site for large carpal fractures in horses. The clinical presentation and findings from this report were similar to that of third carpal bone (C3) slab fractures, confirming that surgical repair is indicated in selected cases of RCB fractures.  相似文献

A 1‐year‐old Dutch Warmblood gelding was presented to the University of Florida’s Large Animal Hospital (UF‐LAH) for correction of ectropion of the right lower eyelid. The ectropion was the result of a lower eyelid laceration. A primary repair was performed by the referring veterinarian; however, the horse prematurely removed the sutures and the wound healed with inversion of the eyelid margin. Surgical correction of the entropion, with removal of tissue from the lower eyelid, resulted in cicatricial ectropion. During the initial evaluation at UF‐LAH, a corneal ulcer was noted in the right eye because of exposure from the anatomically nonfunctional lower eyelid. A modified Kuhnt–Szymanowski procedure was performed to correct the ectropion and repair the eyelid margin. The surgical site healed appropriately with an acceptable cosmetic and functional result. No recurrences of corneal ulcers in the right eye were reported in the 18 months following surgical correction of the ectropion.  相似文献

Two months following the surgical repair of an acute, open, comminuted, and articular fracture of the left olecranon, a 15-year-old American Saddlebred gelding presented with nonweight-bearing lameness. A fungus cultured from the cubital joint was identified as Scedosporium prolificans by sequencing of the D2 large subunit ribosomal DNA region and subsequently confirmed by phenotypic methods. Therapy with systemic fluconazole, terbinafine and intra-articular voriconazole was attempted, but was unsuccessful.  相似文献

Head injuries occur frequently in horses and a successful outcome after repair depends upon the severity of the initial trauma, the anatomical structures involved and the extent of tissue loss. Often these injuries involve facial bones and enter into the nasal passages or paranasal sinus system. When these injuries occur in conjunction with substantial tissue loss, the resulting defect may be difficult to close and naso/sinocutaneous fistulae can develop. These defects can be challenging and time consuming to repair and an appropriate reconstructive technique must be selected. We describe the use of reverse periosteal flaps as a technique in repairing large nasocutaneous fistulae in 2 horses after previous attempts to close the defects by other means had failed.  相似文献

A case of paranasal sinus fibromatosis in an 11‐year‐old Thoroughbred gelding is reported. At admission the gelding underwent a unilateral frontal sinusotomy and a paranasal sinus mass was extensively debulked. The mass was diagnosed via histopathology as a desmoid fibromatosis with bone invasion, reactive osteoclastic osteolysis and osteoproliferation. The gelding had radiographic evidence of slow recurrence of the mass 8 months post operatively. The histopathological appearance and short‐term outcome are presented for this previously undiagnosed condition of the horse. Recognition of paranasal fibromatosis and the resultant differentiation from other types of paranasal sinus neoplasia may allow for improved prognostication of equine sinus lesions.  相似文献

After becoming entangled in safety lines and being submerged in a chlorinated swimming pool, a 2-year-old Thoroughbred gelding was evaluated and treated successfully. Treatment included antimicrobial agents, anti-inflammatory drugs, bronchodilators, diuretics, humidified oxygen, and surfactant transplant. The results of follow-up examination of the gelding 120 days after hospitalization were normal.  相似文献
