Atrial fibrillation (AF) is the most common pathological arrhythmia in horses. Although it is not usually a life-threatening condition on its own, it can cause poor performance and make the horse unsafe to ride. It is a complex multifactorial disease influenced by both genetic and environmental factors including exercise training, comorbidities or ageing. The interactions between all these factors in horses are still not completely understood and the pathophysiology of AF remains poorly defined. Exciting progress has been recently made in equine cardiac electrophysiology in terms of diagnosis and documentation methods such as cardiac mapping, implantable electrocardiogram (ECG) recording devices or computer-based ECG analysis that will hopefully improve our understanding of this disease. The available pharmaceutical and electrophysiological treatments have good efficacy and lead to a good prognosis for AF, but recurrence is a frequent issue that veterinarians have to face. This review aims to summarise our current understanding of equine cardiac electrophysiology and pathophysiology of equine AF while providing an overview of the mechanism of action for currently available treatments for equine AF. 相似文献

Recently, treatment of equine atrial tachycardia by three-dimensional electro-anatomical mapping (3D EAM) and radiofrequency catheter ablation (RFCA) has been described. Myocardial sleeves in the caudal vena cava and pulmonary veins are a potential trigger for initiation and perpetuation of atrial tachycardia and atrial fibrillation in the horse. Isolation of these myocardial sleeves by RFCA may be an effective treatment for these arrhythmias. 相似文献

We describe the diagnosis and treatment of an atrioventricular accessory pathway (AP) in a horse using 3-dimensional electro-anatomical mapping (3D EAM) and radiofrequency catheter ablation (RFCA). During routine evaluation of the horse, intermittent ventricular pre-excitation was identified on the ECG, characterized by a short PQ interval and abnormal QRS morphology. A right cranial location of the AP was suspected from the 12-lead ECG and vectorcardiography. After precise localization of the AP using 3D EAM, ablation was performed and AP conduction was eliminated. Immediately after recovery from anesthesia an occasional pre-excited complex still was observed, but a 24-hour ECG and an ECG during exercise 1 and 6 weeks after the procedure showed complete disappearance of pre-excitation. This case shows the feasibility of 3D EAM and RFCA to identify and treat an AP in horses. 相似文献

A 2‐day‐old male Quarter Horse foal was diagnosed with uroperitoneum and ruptured bladder. Intravenous fluid therapy was initiated prior to anaesthesia and repeated unsuccessful attempts were made to drain the abdomen of accumulated urine. Prior to anaesthesia the foal exhibited clinical signs of abdominal compartment syndrome. When anaesthetised the accumulated urine was drained by free flow through a small abdominal stab incision after aseptic site preparation. A few minutes later electrocardiography indicated second degree atrioventricular block which progressed into third degree atrioventricular block and ventricular asystole. The foal was resuscitated by closed chest compressions, mechanical ventilation, sympathomimetic and antimuscarinergic drugs. When anaesthetising these neonatal patients one must be prepared to handle potentially fatal cardiac arrhythmias and, prior to surgery, the danger posed by increased intra‐abdominal pressure should be weighed against the need for diluting plasma potassium. 相似文献

Background: Invasive electrophysiology study (EPS) is one of the most important adjunct tests for the evaluation of arrhythmias in human medicine. However, EPS is neither well known nor widely used in veterinary medicine. Objective: To define the values for intracardiac conduction parameters determined during invasive EPS in dogs. Animals and methods: The study included 16 admitted dogs of various breeds, sex and ages and 6 control Beagles. In the Beagles, EPS was performed twice at 6-month intervals in order to verify the reproducibility of the results. Results: No significant differences were found between the results of the baseline and repeated EPS performed in the Beagles. We found retrograde conduction in 13 (59%) out of 22 dogs and including 4 (31%) animals with concomitant ‘jumps’ in the atrioventricular node conduction curve, pointing to the presence of dual conduction in this node. The mean values of the AV and retrograde VA Wenckebach points equaled to 220 and 360 ms, respectively, suggesting that the capability of the descending route is higher than that of the ascending route. Conclusion: The values determined in this study may be helpful in early detection of abnormalities in the electrical conduction system of the heart. 相似文献

This case report describes the evaluation, surgical approach and outcome of an 8-month-old foal with severe angular limb deformities of both metatarsophalangeal joints (MTPJ). Radiography and computed tomography (CT) were used to diagnose a windswept deformity of the hindlimbs with associated bilateral physitis. The orthopaedic surgery was planned and rehearsed using three-dimensional (3D) bone models. A bilateral closing wedge ostectomy was performed on the distal MtIII, and 3.5 mm LCP ® medial distal tibia plates were used bilaterally for osteosynthesis. Even though convalescence was prolonged and complicated by partial unilateral implant failure and osteoarthritis (OA), reduction of the deformities was successful and resulted in a sound foal. 相似文献

Heart rate and rhythm can readily be monitored during exercise. Although there is considerable variation depending on the athletic discipline, exercise can be considered to be one of the most physiologically demanding times for the cardiovascular system. Assessment of heart rate during exercise typically provides information regarding fitness and the intensity of exercise, and on some occasions may provide an early indication of disease. Cardiac causes of poor performance occur relatively infrequently in comparison with disorders of the musculoskeletal and respiratory systems. Nevertheless, exercise induces cardiac hypertrophy, which predisposes athletes to valvular regurgitation and arrhythmias. Consequently cardiac murmurs and arrhythmias are frequently present in equine athletes, whereupon the clinical significance can be difficult to determine. Undertaking an exercise test to identify exercise‐induced arrhythmias is important in the assessment of poor athletic performance and the risk of sudden cardiac death during exercise. This paper describes the assessment of heart rate and rhythm during exercise. Most research has been undertaken on racehorses but, where data are available for other disciplines, they have been included. BackgroundAtrial tachycardia (AT) can be treated by medical or electrical cardioversion but the recurrence rate is high. Three‐dimensional electro‐anatomical mapping, recently described in horses, might be used to map AT to identify a focal source or reentry mechanism and to guide treatment by radiofrequency ablation. ObjectivesTo describe the feasibility of 3D electro‐anatomical mapping and radiofrequency catheter ablation to characterize and treat sustained AT in horses. AnimalsNine horses with sustained AT. MethodsRecords from horses with sustained AT referred for radiofrequency ablation at Ghent University were reviewed. ResultsThe AT was drug resistant in 4 out of 9 horses. In 8 out of 9 horses, AT originated from a localized macro‐reentrant circuit (n = 5) or a focal source (n = 3) located at the transition between the right atrium and the caudal vena cava. In these 8 horses, local radiofrequency catheter ablation resulted in the termination of AT. At follow‐up, 6 out of 8 horses remained free of recurrence. Conclusions and Clinical ImportanceDifferentiation between focal and macro‐reentrant AT in horses is possible using 3D electro‐anatomical mapping. In this study, the source of right atrial AT in horses was safely treated by radiofrequency catheter ablation. 相似文献
