OBJECTIVE: To describe and evaluate the placement of a new design of polyurethane jugular catheter in cats using a modification of the technique of Seldinger. The maintenance and use of these catheters for repeated blood sampling over several days in healthy and diabetic cats is reported. PROCEDURE: Thirty polyurethane jugular catheters were placed in 10 clinically healthy cats and 10 cats with diabetes mellitus using the modified Seldinger technique. Catheters were placed while the cats were under general anaesthesia. RESULTS: The catheters remained in place for a range of 2 to 14 days (median 5 days) with no major complications. In all cats patency of the catheters was maintained until removal. All cats examined had patent jugular veins when assessed 4 or more weeks after the catheters were removed. CONCLUSIONS: These polyurethane jugular catheters, when placed by a modified Seldinger technique, are effective and safe in cats, and jugular patency returns after the catheters are removed.  相似文献

Venous anomalies of the jugular vein are uncommon in the horse. Jugular venous aneurysms (congenital or pseudoaneurysm) in horses usually present as a compressible asymptomatic cervical mass enlarging on manual congestion of the jugular vein. They can be classified into primary (congenital) and acquired lesions. Primary venous aneurysms are true venous aneurysms because in these lesions the venous wall is intact, whereas false or pseudoaneurysm has a disruption of the inner layers of the venous wall. Two types of jugular vein aneurysm can be identified, fusiform or saccular, the former being the most common in people. Doppler ultrasound imaging confirms the diagnosis. Surgical treatment, aneurysmectomy with venorraphy or venoplasty is indicated in case of progressive aneurysmal expansion. This clinical commentary describes a 4-month-old foal presented with a primary aneurysm of the right jugular vein in the midcervial region. Treatment consisted of partial aneurysmectomy and venorraphy. Four years later, the horse had a patent jugular vein with a normal diameter and was trained without clinical signs of venous distension.  相似文献

Objective To compare and correlate right atrial pressure, which represents central venous pressure (CVP) to jugular vein pressure (JVP) in laterally recumbent horses under anesthesia. Study design Retrospective clinical trial. Animals Seven adult healthy horses (411 ± 8.7 kg). Methods Horses were sedated with IV xylazine and anesthesia was obtained with IV ketamine and diazepam. Anesthesia was maintained with sevoflurane in oxygen. All horses were positioned in left lateral recumbency. An 8F catheter introducer was inserted into the right jugular vein to measure JVP. An 8F catheter introducer was inserted into the left jugular vein to be used as the port for a 7F 110 cm catheter that reached the right atrium to measure CVP. Both, CVP and JVP were measured simultaneously with a water calibrated aneroid manometer using the sternum as the 0 cmH2O reference point. Measurements were compared using Spearman correlation and the Bland‐Altman plot. Results Twenty paired samples were obtained over a period of 2 hours. The CVP ranged from 7 to 31 cmH2O, while the JVP ranged from 5 to 30 cmH2O. The Spearman correlation coefficient indicated that CVP and JVP had a strong correlation with r = 0.88. The Bland‐Altman plot showed a bias of 0.7 cmH2O. Conclusion and clinical relevance Jugular vein pressure showed a strong correlation with CVP in healthy, euvolemic, laterally recumbent anesthetized adult horses. Thus, JVP cannot replace CVP but it may be used clinically to monitor CVP in laterally recumbent horses.  相似文献

Objective – To compare the placement feasibility and amount of bone trauma induced by 3 intraosseous (IO) access techniques in cats: an automatic impact penetration device (A), an automatic rotary insertion device (B), and a manual IO needle (C). Design – Prospective ex vivo study. Setting – University. Animals – Eighteen adult mixed breed feline cadavers. Interventions – Cadavers provided 72 total IO insertion locations divided equally between the right and left humerus and tibia. The 3 IO techniques were randomly allocated to these locations. Time to successful insertion, ease of insertion, and success rate were recorded. Each insertion site was analyzed for the number of bone fragments and defect diameter by computed tomography. Measurements and Main Results – Device B had lower time of insertion (P=0.01) compared with devices A and C. Device B had better ease of insertion scores (P<0.01) compared with devices A and C. No differences were detected between insertion sites (tibia versus humerus). No differences in the number of bone fragments, defect diameter, or success rate were detected among devices (P=0.06, 0.31, and 0.14, respectively). Conclusions – All 3 IO access methods evaluated yield acceptable results. Device B is significantly faster and easier to place in cat cadavers when compared with other methods.  相似文献

Objective To compare the success rates of central venous catheter placement (CVCP) in dogs using electrocardiograph (ECG)‐guided and external landmark (‘blind’) techniques. To report success rates determined retrospectively of CVCPs in dogs using external landmarks at a tertiary referral institution. Study design Prospective blinded comparison of techniques. Retrospective analysis of case records. Animals Adult Beagles weighing 11.9 ± 2.6 kg were used in the experimental group (n = 38). Various breeds of dogs were in the retrospective clinical group (n = 33). Methods Laboratory dogs were anesthetized and CVCPs were placed using a modified Seldinger technique. Catheter tip position was first based on external landmarks and then the catheter was repositioned using an ECG‐guided placement. The ECG‐guided technique used the V‐lead with the positive electrode attached to the guide wire. Catheter placement was determined by moving the catheter cephalad and caudad to the point of maximum p‐wave amplitude and then withdrawing the catheter 1–2 cm from this point. Catheter position with each technique was determined using a lateral thoracic radiograph. Retrospective data were collected from the medical records of dogs that had CVCPs using anatomical landmarks and corresponding thoracic radiographs. Results The number of successful CVCP attempts was the same for both prospective groups (21/38). There was no statistically significant difference in success between the ECG‐guided technique and the blind technique. From the retrospective investigation 10/33 of the cases that fit the criteria had correct CVCPs. Conclusions and clinical relevance The odds of correctly placing a central venous catheter by ECG‐guidance were the same as the external landmark technique. The ECG‐guided technique may be useful in situations where external landmarks are not readily available.  相似文献

A 10-year-old, 7 kg, male mixed breed dog was presented, following a dog fight, with open fractures of the radius and ulna and extensive skin and muscle lesions. Using electric nerve stimulation, an indwelling catheter was positioned in the axillary space and 2 mg kg(-1) bupivacaine was injected. Complete sensory and motor blocks were obtained allowing amputation distal to the non-traumatized tissue and debridement of the limb. The injection of bupivicaine was repeated 5 hours after the first bolus and a similar result was obtained in term of muscle relaxation and pain alleviation. Subsequent injections were not successful in providing the same effect, most likely as a result of catheter displacement and alternate means of analgesia were used.  相似文献

ObjectiveTo evaluate the accuracy of epidural catheter placement at different levels of the spinal cord guided solely by electrical nerve stimulation and resultant segmental muscle contraction.Study designProspective, experiment.AnimalsSix male and two female Beagles, age (1 ± 0.17 years) and weight (12.9 ± 1.1 kg).MethodsAnimals were anesthetized with propofol and maintained with isoflurane. An insulated epidural needle was used to reach the lumbosacral epidural space. A Tsui epidural catheter was inserted and connected to a nerve stimulator (1.0 mA, 0.1 ms, 2 Hz) to assess positioning of the tip at specific spinal cord segments. The catheter was advanced to three different levels of the spinal cord: lumbar (L2–L5), thoracic (T5–T10) and cervical (C4–C6). Subcutaneous needles were previously placed at these spinal levels and the catheter was advanced to match the needle location, guided only by corresponding muscle contractions. Catheter position was verified by fluoroscopy. If catheter tip and needle were at the same vertebral body a score of zero was assigned. When catheter tip was cranial or caudal to the needle, positive or negative numbers, respectively, corresponding to the number of vertebrae between them, were assigned. The mean and standard deviation of the number of vertebrae between catheter tip and needle were calculated to assess accuracy. Results are given as mean ± SD.ResultsThe catheter position in relation to the needle was within 0.3 ± 2.0 vertebral bodies. Positive predictive values (PPV) were 57%, 83% and 71% for lumbar, thoracic and cervical regions respectively. Overall PPV was 70%. No significant difference in PPV among regions was found.Conclusion and clinical relevancePlacement of an epidural catheter at specific spinal levels using electrical nerve stimulation was feasible without radiographic assistance in dogs. Two vertebral bodies difference from the target site may be clinically acceptable when performing segmental epidural regional anesthesia.  相似文献

Objective

To assess the reaction of client-owned dogs to intravenous (IV) catheter placement after applying a local anaesthetic (EMLA) or placebo cream for either 30 or 60 minutes.

Study design

Prospective, randomized, blinded, placebo-controlled, clinical trial.

Animals

A total of 202 client-owned dogs of various breeds.

Methods

With owner consent, dogs were randomly allocated to one of four treatment groups: EMLA 60 minutes, EMLA 30 minutes, Placebo 60 minutes and Placebo 30 minutes. After the cream was applied for the allocated time, an IV catheter was placed and the behavioural reaction of the dog was scored. The reaction score was analysed using a Kruskal–Wallis test followed by Mann–Whitney U tests of the multiple pairwise comparisons, with Bonferroni correction.

Results

A large number of dogs, even in the placebo groups, did not react to IV catheter placement. However, the Kruskal–Wallis test showed an overall difference between treatment groups (χ2 = 11.029, df = 3, p = 0.012). The pairwise comparisons showed a lower overall reaction score in the EMLA 60 group than in the EMLA 30 and Placebo 60 groups (adjusted p = 0.018 and adjusted p = 0.044, respectively).

Conclusions and clinical relevance

This study shows that EMLA cream applied for 60 minutes reduces the behavioural reaction of dogs to IV catheter placement; therefore, this intervention can be advocated for routine use in veterinary medicine to enhance the welfare of dogs undergoing IV catheter placement.  相似文献

This case series describes a technique to treat guttural pouch tympany that may be useful to equine practitioners. The objective of this work is to describe the treatment technique, outcome, possible complications and benefits. Data from a retrospective case series obtained by records review and follow‐up with owners and referring veterinarians were used. Foley catheters were placed into the guttural pouch using endoscopic guidance via the nasal passage, the balloon inflated and the catheter sutured to the external nares under tension. Foals were treated successfully with this method and complications were minor. This treatment method is a viable, low‐cost option for guttural pouch tympany in foals.  相似文献

OBJECTIVE: To evaluate the feasibility of ultrasound guided vascular access in pigs by comparing central venous and arterial cannulation techniques. ANIMALS: Twenty-two healthy female Pietrain pigs, 14-18 weeks old and weighing 51.1 +/- 4.3 kg (mean +/- SD). STUDY DESIGN: Comparative animal trial. MATERIALS AND METHODS: After induction of general anaesthesia, cannulation of the external jugular vein and internal carotid artery was attempted using real-time ultrasound guidance. The quality of the ultrasound picture was assessed on an analogue scale from 1 (excellent) to 5 (insufficient). Vessel size, cannulation success rate, number of puncture attempts and time from first puncture attempt until insertion of the Seldinger wire were recorded. RESULTS: Cannulation was successful in all but one animal in which a cut-down technique was performed. The arteries were significantly smaller than the veins (p < 0.001) resulting in a significantly prolonged cannulation time (p = 0.032) for insertion of arterial catheters without differences in success rate. In 89% of attempted cannulations, the Seldinger wire was inserted within 5 minutes. CONCLUSIONS AND CLINICAL RELEVANCE: In anaesthetized pigs undergoing instrumentation for biomedical research, ultrasound-guided vascular access is a simple and rapid alternative to surgical cut-down. In veterinary anaesthesia, the technique might be useful in sedated or anesthetized pigs in which arterial or central venous access is required.  相似文献

Objective— To evaluate 4 methods of cholecystostomy catheter placement and to report on laparoscopic (Lap) cholecystostomy for the management of extrahepatic biliary obstruction (EHBO) in 3 dogs.
Study Design— Experimental study and clinical report.
Animals— Cadaveric dogs (n=20); 2 canine and 1 feline patient.
Methods— Pigtail cholecystostomy catheters were inserted in 20 canine cadavers using ultrasound (US) or Lap guidance. Insertion routes were either transperitoneal or transhepatic. Methods studied included Lap-transperitoneal, US-transperitoneal, US-transhepatic, and US-Seldinger (n=5 dogs/group). Insertion success, pleural penetration, and insertion site leakage (Lap-transperitoneal group) were evaluated. Three clinical EHBO cases were treated by Lap-transperitoneal technique.
Results— Insertion success was 100% by Lap-transperitoneal but 0% with US-transperitoneal and US-Seldinger methods. US-transhepatic yielded 3 of 5 successful placements. The pleura was penetrated in all US-transhepatic and US-Seldinger insertions. Leakage pressure for Lap-transperitoneal catheters averaged 75 cm H2O (±20 cm H2O). Lap-transperitoneal cholecystostomy resulted in marked improvement in 2 dogs, but the catheter became obstructed in the cat. One dog spontaneously regained common bile duct patency and the remaining 2 animals had successful cholecystoenterostomy.
Conclusions— In cadaver testing, the Lap-transperitoneal cholecystostomy method was superior based on high insertion success with no pleural penetration. In 2 clinical cases, Lap-transperitoneal placement successfully provided biliary drainage for patient stabilization.
Clinical Relevance— The role for temporary cholecystostomy has yet to be established, but may aid patient stabilization and mortality reduction in EHBO.  相似文献
