In the neonatal foal septic arthritis is an important cause of morbidity that may limit future athletic performance. This is often despite a good prognosis for resolution of joint infection. This article presents a review of the diagnosis and treatment of septic arthritis in foals less than 2 months of age, including an overview of our current understanding of inflammatory joint disease in the neonate. Management options are described, with emphasis on tailoring therapy to the individual foal based on accurate staging and monitoring of the disease process. Special considerations relevant to the immature animal are addressed.  相似文献

A case of infectious arthritis of a cervical vertebral articulation and vertebral osteomyelitis in a foal caused by Salmonella is described. Ultrasound images of the cervical facet joints were suggestive of septic arthritis and ultrasound facilitated needle placement for arthrocentesis and joint lavage. Due to the uncommon location of the infection, diagnosis of septic arthritis was not immediate and, despite aggressive intra‐articular lavage and instillation of appropriate intra‐articular antibiotics after hospitalisation, we were unable to resolve the infection in this foal. Radiographs did not show evidence of osseous involvement until later in the course of the infection. Septic arthritis should be suspected in cases with similar clinical and laboratory findings and radiographic changes may not be present early in the course of infection. Ultrasound imaging provided the most useful diagnostic information regarding infection of the cervical joints, facilitating both diagnosis and treatment.  相似文献

Elbow luxation is a common orthopedic injury in the ferret. The injury is usually caused by trauma but can also occur spontaneously. The most successful treatment involves a combination of surgical and medical intervention. Surgical treatment includes open reduction of the elbow joint and stabilization with orthopedic implants. Medical treatment includes external coaptation, analgesia, and prevention of infection. Owner compliance also plays an important role in return to full function of the luxated ferret elbow joint.  相似文献

Canine immune-mediated polyarthritis (IMPA) is a diagnosis of exclusion based predominantly on clinical signs, characteristic joint fluid analysis, and elimination of potential joint infection. Ultimately, an appropriate and sustained response to immunosuppressive therapy may become the final diagnostic criterion used. Identifying associated disease processes, including breed-specific syndromes, remote infection, inflammation, drug exposure, vaccine exposure, or neoplasia, as well as initial response to therapy, is often an important contributor to prognosis. This review article is the second of a two part series and focuses on the diagnosis and treatment of immune-mediated polyarthritis. The first article in this series, published in the January/February 2012 issue, concentrated on the pathophysiology of IMPA.  相似文献

This report describes the diagnosis and treatment of synovial sepsis of the infraspinatus and intertubercular (bicipital) bursae. The origin of infection was considered to be haematogenous spread from an ipsilateral foot abscess. The horse responded well to treatment and returned to full athletic function.  相似文献

Open drainage was used to treat 26 horses with persistent or severe septic arthritis/tenosynovitis. Infected synovial structures were drained through a small (3 cm) arthrotomy incision that was left open and protected by a sterile bandage. Joint lavage was performed in all 26 horses. In addition to systemic antibiotics, 23 of these horses were also treated with intra-articular antibiotics; amikacin (17 horses), gentamycin (2 horses), cefazolin (2 horses), and 2 horses were injected at different times with gentamycin and amikacin. The infection was eliminated from the involved synovial structures in 25 of 26 horses; 24 survived and were released from the hospital. The arthrotomy incisions healed by granulation in 16 horses; in 9 horses the arthrotomy incision was sutured closed once the infection was eliminated. Seventeen horses returned to soundness and resumed athletic function. Open drainage was an effective method of achieving chronic drainage from a joint or tendon sheath. It is indicated in horses that have established intra-synovial infections or in horses that do not respond to joint lavage through needles.  相似文献

ABSTRACT

Aims: To compare the outcome, in terms of lameness score or return to athletic function, of horses with acute vs. chronic digital lameness that underwent magnetic resonance imaging (MRI) of the distal limb and to compare the proportion of horses that received intra-articular therapy of the distal interphalangeal (DIP) joint and pattern of diagnostic analgesia in these groups.

Methods: This is a retrospective study of horses (n?=?95) with acute (≤12 weeks; n?=?46) or chronic (>12 weeks; n?=?49) digital lameness that underwent MRI of the distal limb from 2009–2016, at two equine referral centres in the USA. Criteria for inclusion in the study were that a majority of lameness localised distal to the fetlock, and that lameness assessments for ≥12 months following MRI could be obtained from the medical record or the owner could be interviewed regarding their horse's athletic function. Outcome was characterised by an improvement score where 2?=?return to work at a previous or higher level or lameness improved by one grade or more, 1?=?return to work at a lower level or lameness improved by less than one grade, and 0?=?did not return to work or lameness grade worsened. Whether horses had received intra-articular therapy of the DIP joint and the pattern of diagnostic analgesia prior to MRI was also obtained from medical records or by interviewing the owner.

Results: There was a difference (p?=?0.004) in the proportion of horses assigned to improvement scores of 0, 1 and 2 between horses with acute or chronic lameness. There was no evidence of a difference in the likelihood of having received intra-articular therapy of the DIP joint prior to MRI between horses with chronic or acute lameness (p = 0.085). Similarly, there was no evidence of a difference in the pattern of diagnostic analgesia prior to MRI between the two groups (p = 0.94). Eighty-two percent of owners of horses with acute and 62% of those with horses with chronic lameness had a positive opinion of the utility of MRI as a diagnostic modality.

Conclusion: In a population of horses with digital lameness undergoing MRI, a difference in the outcome, in terms of lameness score or return to athletic function was identified between horses with acute lameness compared to those with chronic lameness.

Clinical relevance: Horses with digital lameness that undergo MRI when the lameness is acute may have an improved prognosis due to accurate diagnosis and earlier application of appropriate therapy.  相似文献

The purpose of this paper was to identify factors that would positively or negatively affect the short-term survival rate of foals with septic arthritis. Medical records of 81 foals (≤seven months of age) with a clinical diagnosis of septic arthritis, referred to the equine hospital at Cornell University Hospital for Animals, between 1994 and 2003 were reviewed. Signalment, age at presentation, number of affected joints, joint fluid parameters, bacterial agents, treatment modalities and year of treatment were compared between survivors and nonsurvivors. Sixty-two of 81 foals (77%) were discharged from the hospital and classified as 'survivors'. Multiple joint involvement and detection of intra-articular Gram-negative, mixed bacterial infection and degenerate neutrophils were negatively associated with short-term survival. Initiation of treatment within 24 hrs of onset of clinical signs and combination of treatment modalities were positively correlated with survival. Further investigation is needed to determine if these two factors have a similar influence on athletic performance.  相似文献

Objective To investigate predictors of survival and athletic function in adult horses with infection of a synovial structure. Hypotheses Increasing duration from contamination to referral, bone or tendon involvement and positive microbial culture decreases short‐term survival. Synovitis and/or sepsis at 5 days post‐admission and involvement of Staphylococcus spp. decreases long‐term athletic function. Design Retrospective study. Methods Records over 4 years of adult horses with synovial sepsis were reviewed. A two‐tailed Fisher's exact test, Mann‐Whitney U test or t‐test was used to examine whether variables were predictive of short‐term survival and long‐term athletic function. Results During the study period 75 horses underwent treatment for infection of 93 synovial structures. Short‐term survival was 84% (63/75) and 54% (30/56) of horses returned to athletic function. Of the variables measured at admission, duration from contamination to referral did not affect survival, whereas evidence of bone or tendon involvement decreased survival and athletic function. Of the variables available during treatment, abnormal synovial fluid at 4–6 days post‐admission and positive microbial culture reduced athletic function. Staphylococcal infection was associated with persistent sepsis. Conclusions Of the variables available at presentation, only evidence of bone or tendon involvement negatively affected survival and athletic function. During treatment of synovial sepsis, analysis of synovial fluid at 4–6 days and bacterial culture results have prognostic value.  相似文献

OBJECTIVE: To provide information on the clinical features, diagnosis and treatment of bacterial septic arthritis in dogs. DESIGN: A retrospective study examining case records of all dogs diagnosed with bacterial septic arthritis at Murdoch University Veterinary Hospital between 1988 and 1997. RESULTS: Nineteen dogs were diagnosed with bacterial septic arthritis, which most commonly occurred after surgery involving the stifle joint. Haematogenous infection occurred in only five dogs. Diagnosis was based on clinical signs, joint fluid analysis, radiography, microbiology and/or response to treatment. Chronic lameness was the most common problem at presentation. Analysis of joint fluid invariably revealed large number of nucleated cells, which consisted primarily of neutrophils. In all but one case the neutrophils were nondegenerate. Culture of joint fluid was frequently successful. Staphylococcus spp were the most common bacteria isolated. Treatment involved antimicrobial drugs only in five dogs. Other dogs received antimicrobial drugs in combination with surgical procedures such as joint lavage and removal of nonabsorbable suture material (eight), arthrodesis (two) or amputation (one). Two dogs were euthanased. Most dogs responded well to treatment and were free of signs of septic arthritis at follow-up. CONCLUSION: Bacterial septic arthritis may often be mild and manifest as chronic lameness. Analysis of joint fluid will detect an inflammatory arthropathy but the presence of toxic neutrophils should not be relied on as an indicator of sepsis. Culture of infected joint fluid is likely to be successful if antimicrobials are not given prior to collection and if the sample is inoculated into enrichment broth. Treatment should involve antimicrobial drugs, open-joint lavage and removal of joint prostheses if the infection is associated with previous surgery.  相似文献

The diagnosis of equine endometritis should generally begin with a history of reproductive failure followed by demonstration of inflammatory changes in the endometrium. Only then should the results of uterine cultures be considered in establishing an etiological diagnosis.

In therapy, the first consideration should be an attempt to correct the conditions which made the mare susceptible to the disease in the first place. Then advantage should be taken of measures to enhance the mare's natural resistance, perhaps sometimes even allowing her to clear herself of infection without antimicrobial therapy. Finally appropriate antimicrobial therapy should be selected based upon a knowledge of the conditions existing in the uterus and the results of culture and sensitivity.  相似文献

Successful management of anaerobic infection first requires an accurate diagnosis. Cytologic examination of wound exudates and inspection for characteristic clinical clues greatly facilitates an accurate initial diagnosis of anaerobic infection. Knowledge of antimicrobial activity against specific anaerobic pathogens is essential, since antibiotic susceptibility information is not routinely available. Whenever possible, antimicrobial and surgical therapy should be combined in managing anaerobic infections. Chloramphenicol, clindamycin, and metronidazole provide the most consistently reliable activity against pathogenic anaerobes, including Bacteroides. Penicillins are also generally effective, except for treatment of infections caused by penicillinase-producing strains of Bacteroides. Cephalosporins are not considered drugs of choice for anaerobic infections, although cefoxitin may in some instances be useful as monotherapy of mixed infections containing obligate anaerobes and coliform bacteria. Aminoglycosides and sulfonamides are ineffective and should be avoided for treatment of anaerobic infection.  相似文献

In this paper on the diagnosis and treatment of arthritis in horses, both degenerative arthritis and septic arthritis are considered. Diagnosis should be made on the combination of clinical examination together with the use of diagnostic aids such as radiology, intra-articular local anaesthesia and synovial fluid analysis. Intra-articular therapy appears to be the most effective in the treatment of degenerative arthritis. Excellent responses to therapy have been reported with corticosteroids, sodium hyaluronate, orgotein and synovial fluid transfer, where joints showed an absence of degenerative changes on radiographs. In septic arthritis, systemic treatment with the appropriate antibiotic, following bacterial culture and sensitivity testing, can produce good results if prompt diagnosis is made.  相似文献

Osteomyelitis is a challenging infectious disease affecting humans and animals. It is difficult to diagnose because, in many cases, symptoms are non-specific and, for example in implant-related cases, can appear long time after surgery. In addition to this, it is also difficult to treat due to the need to find the appropriate antibiotic regime and delivery system to reach the site of infection and to avoid development of bacterial resistance. The central purpose of this review is to compare the microbiological aspects of osteomyelitis in human and veterinary medicine, with the aim of improving the microbiological diagnosis and treatment of this infection in animals. Furthermore, the study of osteomyelitis in animals may help to improve the development of animal models for testing new treatments in humans. Host factors and underlying conditions have been studied mainly in humans, although aspects as immunodeficiency have been described in some veterinary cases. Even when Staphylococcus aureus is still considered the most prevalent causing microorganism, this prevalence should be reviewed using molecular diagnostic techniques, and this could affect treatment options. New approaches to treatment include local delivery of antibiotics using different biomaterials, antimicrobial photodynamic therapy, and new antimicrobial compounds. We would like to remark the need of large, high-quality clinical trials and of the development of guides for the diagnosis and treatment of osteomyelitis in different animal species.  相似文献

In small animal practice, prostatic diseases are increasingly encountered. All dogs may experience prostatic disease, but particular care should be addressed to breeding dogs, in which prostatic affection may lead to decrease in semen quality and fertility. The most common prostatic disease is the benign prostatic hyperplasia (BPH) followed by prostatitis, prostatic neoplasia and prostate squamous metaplasia. These diseases do not have pathognomonic symptoms, therefore, making a correct diagnosis may not be easy. An accurate clinical examination and a correct diagnostic protocol are essential in order to begin the most appropriate treatment, and also to do a good prophylaxis where it is possible. BPH therapy is usually recommended when mild‐severe signs are present or if symptoms disturb the patient. New therapeutic approaches, both medical and surgical, allow to maintain fertility in most animals with prostatic disorders. Prostate cancer is relatively infrequent. Elective therapy is the surgical one, but it is considered palliative and can result in important post‐operative complications. The aim of this paper is to lay down the most appropriate diagnostic process describing the aetiologies of prostatic disease, their symptoms, the right investigative tools and therapy.  相似文献

Septic arthritis in horses is a serious disease which can become life-threatening. In case the infection can be eliminated before irreversible joint damage occurs, complete recovery is possible. Osteoarthritis (OA) is one of the most common musculoskeletal pathologies in horses and is a disease capable of halting their athletic career. Clinically, OA can be a difficult problem to deal with, particularly when there is no longer a positive response to corticosteroids or rest. In order to avoid further articular tissue degeneration, which could lead to the loss of the joint function, novel therapies are focusing not only on controlling inflammation and pain but also on tissue healing and repair. Autologous platelet concentrate (PC) growth factors are known to have anabolic and angiogenic properties and a positive effect on synovial epithelium, cartilage, and pain. The aim of this study was to analyze the clinical outcome of horses with OA treated with PC. Selected cases were treated only with PC, and they all belong to the same sport activity. We show here the method for PC preparation, follow-up evaluation, and outcome of 20 cases with refractory OA treated exclusively with autologous PC. After a 1-year follow-up, 80% of patients were able to resume work. According to our results and observations, PC can be used as a safe and low-cost intra-articular therapy for refractory OA in the horse.  相似文献

Two intact female dogs were examined for refractory pollakiuria and stranguria. One of these two dogs also exhibited urinary incontinence. Ectopic ureteroceles were diagnosed via ultrasonography and cystoscopy in both dogs. Both dogs were taken to surgery for ureterocele resection and neoureterostomy, resulting in complete resolution of clinical signs. Although uncommonly reported in the veterinary literature, ectopic ureteroceles can be associated with recurrent lower urinary tract infection with and without urinary incontinence. With a correct diagnosis and appropriate treatment, prognosis for ectopic ureteroceles can be good provided there is not a concurrent significant loss of renal function.  相似文献

Rhodococcus equi is a soil borne bacterium that causes severe morbidity and death in young foals. The economic costs of the disease include loss of life, treatment expenses, veterinary monitoring expenses and, perhaps most importantly, potential reduction in future athletic performance in horses that suffer severe lung abscessations caused by R. equi. Current standard of care for pneumonia caused by R. equi is treatment with a macrolide antimicrobial and rifampicin. However, the hallmark of pneumonia caused by R. equi is severe formation of pyogranulomas and a walling off effect that can prevent systemic antibiotics from reaching antimicrobial concentrations in lung tissues. It is hypothesized that streptolysin O (SLO) used as an adjunct therapy with antibiotics will reduce the duration and severity of disease caused by R. equi pneumonia compared to antibiotic therapy alone. Addition of SLO to the antibiotic enhanced clinical responses compared to the other groups, including the antibiotic alone group. Of particular significance were lower bacterial counts in the lungs and longer survival time in those foals treated with SLO and antibiotics.  相似文献
