Proteoglycan degradation is central to the development of degenerative joint disease. Proteoglycans may be degraded by lysosomal enzymes from chondrocytes, synoviocytes or leucocytes. Collagen and matrix degradation occurs either by direct damage or due to degrading enzymes released into synovial fluid. Once the pathological sequence has begun it continues in a cyclic manner unless arrested by the ability of chondrocytes to synthesise sufficient matrix components. REASONS FOR PERFORMING STUDY: The centrodistal (CD) and tarsometatarsal (TMT) joints are often injected individually with a corticosteroid to resolve lameness caused by osteoarthritis (OA). There are no data available regarding diffusion of methylprednisolone (MP) from the TMT joint to the CD joint. HYPOTHESIS: A therapeutic concentration of MP diffuses into the CD joint after methylprednisolone acetate (MPA) is administered into the TMT joint. OBJECTIVE: To measure the concentration of MP in the CD joint after MPA was administered into the TMT joint. METHODS: MPA was administered into a TMT joint of 16 horses. At different times, the ipsilateral CD joint of these horses was injected with a small amount of saline and recovered saline was measured for concentration of MP using high performance liquid chromatography. RESULTS: Six hours after administration of MPA into the TMT joint, a therapeutic concentration of MP was found in all 10 CD joints sampled at this time. CONCLUSIONS: Horses with pain arising from the distal 2 joints of the hock can be treated by administering MPA into the TMT joint alone. POTENTIAL RELEVANCE: Administering MPA into the TMT joint only, to treat OA of the distal 2 hock joints, reduces the difficulties and risks associated with centesis of the CD joint.  相似文献

OBJECTIVE: To determine whether analysis of net joint moments and joint powers is a suitable technique for evaluation of mechanics and energetics of lameness in horses and to measure effects of superficial digital flexor tendinitis. ANIMALS: 6 sound horses. PROCEDURE: Horses were evaluated before (sound evaluation) and after (lame evaluation) induction of superficial digital flexor tendinitis in 1 forelimb by injection of collagenase. Recordings were made with an optoelectronic system and a force plate as horses trotted. Net joint moments and joint powers in the sagittal plane at each joint in the forelimbs during the stance phase were determined. Peak values were determined, and mechanical energy absorbed and generated at each joint was calculated. Comparisons were made between contralateral limbs during sound and lame evaluations. RESULTS: Lame limbs had significant reductions in peak values for net joint moments on the palmar aspect of metacarpophalangeal (fetlock), carpal, and humeroulnar joints. Total energy absorbed was significantly lower at every joint in lame limbs, compared with compensating limbs. CONCLUSIONS AND CLINICAL RELEVANCE: Horses with superficial digital flexor tendinitis had significant differences between lame and compensating limbs for net joint moments and joint powers at all joints, indicating that the gait of horses with superficial digital flexor tendinitis is energetically inefficient. Assessment of net joint moments and joint powers is a useful tool in evaluating equine lameness.  相似文献
