Arthrodesis of the right stifle was performed in a 21 kg German shepherd dog that had sustained a comminuted fracture of the lateral femoral condyle as a result of a gunshot wound. The arthrodesis was stabilised with a circular external skeletal fixator. A simplified technique to facilitate creation of the osteotomies of the femur and tibia, and positioning of the stifle at the desired angle, is described. Compression of the subchondral bone surfaces of the distal femur and proximal tibia was achieved using tensioned coiled transarticular Kirschner wires. Arthrodesis of the stifle was confirmed radiographically 21 weeks following surgery, and the fixator was removed. After union of the arthrodesis and removal of the fixator, the dog was fully weightbearing when standing or walking, and intermittently weightbearing or non-weightbearing at faster gaits.  相似文献

A 13-year-old neutered male miniature dachshund suffered ~30% total skin loss following an attack by another dog. After numerous failed attempts at wound management and closure, the wound was successfully healed by epithelialization using tilapia skin grafts. At each tilapia skin graft placement, the wound bed appeared pink, clean, and healthy with excellent progression of epithelialization at all edges. With use of the tilapia grafts, epithelialization occurred at a rate of 1.76 mm/day. As a result, the wound reached complete closure by epithelialization with no evidence of wound contracture in 102 days.Key clinical message:Tilapia skin grafts were successfully used for management of a large bite wound in a dog and may promote accelerated epithelialization in full thickness skin wounds.  相似文献

A 9-year-old Thoroughbred broodmare presented with a mass rostral to the medial canthus of the left eye. The mass was removed under general anaesthesia and the wound bed infiltrated with carboplatin emulsion before partially closing using a Z-plasty. The mass recurred and subsequently was resected twice. It has not recurred 68 weeks post-treatment. The second case describes removal of a granulomatous mass from the metatarsus of a yearling Thoroughbred colt. Skin expanders and a rotational flap were used to allow tension-free closure. This required two separate surgeries to firstly insert the skin expanders and secondly to remove the mass, the expanders and to close the defect. The wound healed with good cosmesis. There have been few reports on the use of skin flaps in equine reconstructive surgery in the last 30 years. This report details two cases of wound closure where skin flaps were used to achieve primary intention wound healing.  相似文献

The treatment of large skin defects of the distal limbs of dogs and cats can be difficult due to the lack of skin available for primary wound closure or the creation of local skin flaps. Distant pedicle skin flaps provide an acceptable alternative for wound closure with full thickness skin and provide a cosmetic result. Fourteen animals (10 dogs and four cats) that had distant pedicle skin flaps are reviewed. The technique allowed 100 per cent wound coverage in 10 of 14 animals and greater than 95 per cent skin flap survival occurred in 12 of 14 animals. Complications associated with the procedure were most commonly wound infection and partial suture line dehiscence, however these did not detract from the final outcome. Flap release was in one or two stages, however the outcomes did not appear to be affected by the release protocol.  相似文献

The purpose of this study was to compare postoperative wound healing in canine ovariohysterectomy following the use of an absorbable monofilament poliglecaprone 25 suture in 2 different skin closure techniques, the buried continuous subcuticular (BCS) suture pattern and the simple interrupted (SI) suture pattern. These 2 skin closure techniques were evaluated against a nonabsorbable polypropylene monofilament suture in an SI pattern. Wounds were assessed by using a semiquantitative scoring system at 18 to 24 hours and 10 to 14 days, postoperatively. Results indicated that the BCS closure using poliglecaprone 25 demonstrated a higher rate of tissue reactivity initially (18-24 hours postoperatively), as compared with the SI closure using either suture material. By 10 to 14 days postoperatively, poliglecaprone 25 used in a BCS closure was associated with significantly lower wound scores than was the same material used in an SI closure. It was concluded that the BCS closure may effect a better cosmetic appearance to the skin closure in a canine ovariohysterectomy at the time of the recheck appointment. Furthermore, by obviating the need for suture removal, use of the BCS pattern may eliminate the requirement for this return appointment.  相似文献

Controlled mobilization after the surgical repair of multiple disrupted ligaments is considered to be essential for return to normal function. This study compared the outcome after post-surgical mobilization without any protection to mobilization with two transarticular external skeletal fixator hinge prototypes after surgical repair of experimental injuries to multiple stifle ligaments in 15 hounds. The repair was left unprotected (NP: n=5), protected with a self-centering hinge (SH: n=5) and with a conventional hinge (CH: n=5) for four weeks after surgical repair. Outcome measurements included: orthopaedic examination, goniometric and thigh circumference (TC) measurements, total tibial translation (TT), radiographs, and kinetic gait analysis up to 120 days post-operatively. A significant effect of treatment controlling for time for medial collateral stability, TC, TT, osteophyte scores, peak vertical force (PVF) and vertical impulse (VI) was not found. There was a significant difference between time points for subjective lameness scores, TC, PVF, VI, TT and osteophyte scores within treatment. Stifle extension was significantly decreased in CH dogs compared to NP dogs on day 28. Stifle flexion was significantly decreased in CH and SH dogs on day 28 compared to NP dogs. Stifle flexion was normal in all dogs by day 42. Both hinges compromised stifle flexion initially after hinge removal, but range of motion normalized within two weeks. Hinges were not indicated for adjunct treatment after repair of multiple experimentally induced ligamentous stifle injuries.  相似文献

The present report describes the surgical treatment of a congenital lateral patellar luxation in a 6-month-old female donkey foal. The foal was presented with a slight crouched position, muscle atrophy, moderate lameness, reluctance to flex the right hindlimb at a walk and slight effusion of the right stifle joint. The foal responded painfully to the stifle flexion test. Irreducible lateral luxation of the right patella was confirmed on physical examination and radiography. The animal was treated surgically by recession trochleoplasty and imbrication of the medial joint capsule. The animal gradually improved after surgery, and good clinical results were reported after 6 months post-operatively. In conclusion, congenital lateral luxation of the patella should be considered as a congenital cause of lameness in donkeys and surgical repair by recession trochleoplasty and medial imbrication of the joint capsule can be helpful to treat this problem.  相似文献

CASE DESCRIPTION: A 9-month-old domestic shorthair cat was evaluated after being struck by a car. CLINICAL FINDINGS: The cat had a fractured tibia and avulsion of the tail base. Motor and deep pain sensation were absent from the tail. The fractured tibia was repaired 2 days after the trauma. On the third day, the cat developed tachypnea, dyspnea, high serum urea nitrogen and total bilirubin concentrations, epistaxis, persistent hypotension, and oliguria. The cat recovered with supportive care but developed extensive necrosis of the skin on the dorsum by 9 days after the initial trauma. TREATMENT AND OUTCOME: The skin was debrided from the caudal portion of the scapula to the anus and down each pelvic limb to the level of the distal portion of the femur. The tail was amputated. Wet-to-dry bandages were applied to the wound for 3 days. Approximately 50% of the wound underwent delayed primary closure, and the remainder was managed with vacuum-assisted closure. A healthy granulation bed was quickly established. Vacuum-assisted closure was also applied after graft application. Graft acceptance was 100%, and use of the vacuum-assisted closure bandage was not associated with the complications associated with the traditional bandage. CLINICAL RELEVANCE: Vacuum-assisted closure is a useful, easily applicable technique for open and grafted wounds, even when wounds are in challenging anatomic locations.  相似文献

A 14-yr-old female eastern black rhinoceros (Diceros bicornis michaeli) presented with progressive suppurative osteomyelitis in her left hind lateral toe. beta-Hemolytic Streptococcus sp. was isolated. The animal was treated with multiple systemic antibiotics, and topical wound cleansing. Repeated debridements and nail trimmings were performed for 5 mo prior to electing amputation. The toe was surgically amputated under general anesthesia between the first and second phalanges. Analgesia was diffused into the wound topically via a catheter and elastomeric pump. The open amputation site was covered with adherent drapes and a negative-pressure wound therapy device provided vacuum-assisted closure (V.A.C.) for 72 hr. Three months later this animal developed a deep dermal ulcer on the lateral aspect of the right hind limb, at the level of the stifle. Methicillin-resistant Staphylococcus aureus was isolated. The wound was managed by initial daily lavage, followed by 1 mo of V.A.C. therapy, with 72 hr between dressing changes. Clinically, this therapy expedited the formation of healthy granulation tissue and overall healing was accelerated. The animal tolerated the machine and bandage changes well via operant conditioning. The use of negative-pressure wound therapy appeared to shorten time to resolution of slow-healing wounds in black rhinoceros.  相似文献

REASONS FOR PERFORMING STUDY: The acceptance of skin grafts in horses is unpredictable and the final cosmetic result can be disappointing. Besides movement and infection, graft failure is often caused by chronic inflammation, inherently present during second intention healing of limb wounds in horses. In human burns affected by infection and inflammation, the acceptance of the island skin grafts of the modified Meek technique appeared to be better than meshed sheet skin grafts. HYPOTHESIS: The percentage take of Meek micrografts is higher than of other techniques; and rates of both wound contraction and epithelialisation are increased. METHODS: Large traumatic limb wounds of 13 horses healing by second intention were grafted using the modified Meek technique. Photographs of the wounds were taken at set intervals. Wound areas, and areas of acceptance and rejection were determined using a digital image post processor (Scion Image). The percentages of take, wound contraction and epithelialisation were calculated. RESULTS: The initial mean wound area was 7500 mm2. Graft acceptance was mean +/- s.d. 93.7 +/- 5.9%. Wound closure was due to contraction (55.2 +/- 11.1%) and epithelialisation (44.8 +/- 11.1%) and resulted in a 96.7 +/- 3.6% reduction of the initial wound area 29.1 +/- 6 days after grafting. All wounds showed functional and cosmetic healing. CONCLUSIONS: The method for skin grafting in horses achieved higher percentages of take than reported previously and consistent cosmetic and functional results. The grafts increased not only the rate of epithelialisation but also had a strong positive effect on wound contraction, resulting in rapid closure and smaller scars. POTENTIAL RELEVANCE: The modified Meek technique proved to be a novel technique for skin grafting equine wounds in clinical practice, which can be performed easily. The molecular background of the increase of wound contraction by the grafts may provide a clue in the search for medicinal stimulation of wound contraction during second intention healing.  相似文献

N-butyl-cyanoacrylate tissue adhesive was evaluated as a skin closure material in a population control programme. Fifty-two female cats underwent ovariohysterectomy: skin closure of 25 of those cats was undertaken with nylon (3-0) and in 27 with the adhesive. Twenty-three cats spent the postoperative period confined in cages and 29 were free within a 50-square-metre fenced area. The parameters used for evaluation were: dehiscence or secretion and presence and extension of erythema at the surgical site. It was observed that the time required for closure of the surgical site with the adhesive was shorter than with nylon, saving approximately 1 min per animal. The adhesive or conventional suture allowed similar healing, and independent of material used for skin closure or the postoperative exercise, there was no statistical difference in wound cicatrization. As the adhesive eliminates the need for suture removal and decreases surgery time, without additional risk to the cat, it may be indicated for surgical population control programmes.  相似文献

Objective: Evaluate the effect of marker placement on kinematics of the canine stifle in 3 distinct hindlimb models. Study Design: In vivo biomechanical study. Animals: Normal adult mixed‐breed dogs (n=5). Methods: Ten retroreflective markers were affixed to the skin on the right rear leg of each dog to establish normal stifle kinematics. Four additional markers were placed around the greater trochanter (GT), 2 cm cranial, caudal, dorsal, and ventral to evaluate single marker placement variability on kinematic model data. Dogs were walked and trotted 5 times through the calibrated space. Sagittal flexion and extension angle waveforms were acquired during each trial with 3 models that were produced simultaneously during each gait. The GT marker was reassigned to 1 of the 4 additional locations (cranial, caudal, dorsal, and ventral) to alter the kinematic model. Comparison of sagittal flexion and extension angle waveforms was performed with Generalized Indicator Function Analysis. Results: Each model provided consistent equivalent sagittal flexion–extension data. Analysis revealed statistically significant differences between all GT locations. The differences were greatest in the cranial and caudal locations for all models. Conclusions: Deviation of the GT marker in the cranial/caudal direction from an anatomically normal position produces a greater degree of difference than deviation in a dorsal/ventral direction.  相似文献

A 3 · 5‐year‐old whippet was presented 4 weeks after sustaining a traumatic injury to the right stifle joint. A penetrating injury had resulted in patellar ligament (PL) rupture. Magnetic resonance imaging revealed a normal appearing proximal half of the PL and absence of normal fibres within the distal half of the ligament. Surgical repair involved reconstruction of the defect using an autogenous graft harvested from the medial third of the ipsilateral PL. This repair was augmented with a pedicle hamstring graft. A transarticular external skeletal fixator was used to protect the repair for 4 weeks. Seven months after surgery, the dog was able to exercise normally without lameness. To the author's knowledge, this is the first veterinary report of PL grafting of a PL defect, and the first report of surgical augmentation using a hamstring graft.  相似文献

A five-month-old, male German shepherd dog was presented with a left hindlimb deformity. Wearing a cast for a prolonged period to treat left metatarsal fractures at eight weeks of age had resulted in physeal closure of the calcaneus, metatarsals and phalanges. Hyperextension of the phalanges and ulceration of the plantar skin proximal to the main tarsal pad were seen. The left foot was 40 mm shorter than the right. At seven months of age, a modified circular external skeletal fixator (CESF) was used to perform distraction osteogenesis of the metatarsal bones for 28 days. The CESF apparatus was removed after eight weeks when there was radiographic consolidation of regenerate bone. At follow-up evaluations four and 12 months after surgery, the phalangeal hyperextension and plantar skin ulceration had resolved. No lameness was detected.  相似文献

This report describes the successful closure of five chronic feline axillary wounds. The aetiology was known to be forelimb entrapment in a neck collar in three cases and was suspected in the others. Each cat underwent a single surgical procedure during which the wound was debrided, the normal structure of the elbow skin fold was restored and the remaining skin defect was closed primarily. None of the cats had undergone any previous reconstruction attempts. No postoperative complications were observed and the wounds healed uneventfully. Cats have well-developed elbow skin folds, allowing a wide range of limb motion to occur. Having a forelimb trapped in a neck collar not only creates a wound in the axilla but also disrupts the normal anatomy of the skin fold. This report demonstrates that restoring the elbow skin fold before closing the wound may improve the chances of a successful reconstruction at the first surgical intervention.  相似文献

Avulsion of the insertion of the triceps tendon, which had a pre-existing tendinopathy, is described in a cat. The tendon was re-attached to the olecranon and the repair was immobilised using a type la trans-articular external skeletal fixator. The treatment was successful, however, mild and intermittent lameness persisted. Although it is an uncommon condition in small animals, the diagnosis should be based on careful clinical and radiographic examination.  相似文献

This report describes the successful management of peristomal tissue necrosis following prepubic urethrostomy in a cat. The novel technique of temporary urethral ligation was used in combination with temporary tube cystostomy and vacuum assisted closure to allow for wound management prior to performing wound closure by utilization of a flank fold skin flap then definitive prepubic urethrostomy. Eleven month follow-up indicated excellent outcome with the cat having returned to normal behaviour apart from having adapted its posture to urinate.  相似文献

Three dogs were presented for the management of disease processes resulting in large skin defects over the dorsal lumbosacral region. One had severe dog bite wounds, one had a large burn sustained from a heating pad and one had a large myxosarcoma in the region. In each case, the extent and location of the resulting skin defect were assessed as factors likely to prevent reconstruction using simple tension-relieving techniques alone or in combination with established reconstructive techniques, such as axial pattern flaps or skin stretching devices. Bilateral skin fold rotation-advancement flaps (SFRAFs) based on the flank folds were mobilised dorsally and allowed complete wound closure in two dogs and subtotal closure in the other dog. All wounds healed without major complications and an acceptable cosmetic outcome was achieved in each case. Minor flap debridement was required in two dogs. The use of bilateral SFRAFs is a useful technique alone or in combination with other reconstructive techniques for the closure of large dorsal lumbosacral skin defects when existing techniques are not sufficient. Small flank folds, such as those of obese dogs, may yield unexpectedly large SFRAFs.  相似文献

Locations of 3 instantaneous centers of joint rotation (ICR) were found in the stifles of 6 healthy dogs. A novel algorithm, verified by computer simulation, was used to determine ICR from radiographs. Comparisons of ICR locations were made between flexion and extension, between left and right limbs, and between living and dead dogs, using analysis of variance. All stifles were examined for evidence of lesions, but all were normal. The normal location of the ICR in the stifle is near the joint surface, indicating that there is a predominantly rolling type of motion. As the limb flexes, the ICR moves caudally; as the limb extends, the ICR moves cranially. Significant differences in ICR position were not found between left and right limbs or between living and dead dogs. The average ICR position was significantly more caudal during extension than it was during flexion. Knowing the normal ICR position is a preliminary to diagnosing joint abnormalities, using the ICR.  相似文献
