An 11-week-old, domestic shorthair cat was presented for evaluation of a congenital right tarsal deformity. The cat was non-weightbearing lame on the right hindlimb. There was severe tarsal hyperextension and concurrent spasticity/contracture/shortening of the gastrocnemius muscle-tendon. The cat was otherwise healthy. Radiographically there was luxation of the proximal intertarsal joint and talocrural hyperextension. Treatment with 10 mu/kg botulinum toxin A (Botox; Allergan) intramuscular injections directly into the gastrocnemius muscle and passive physiotherapy marginally improved the range of motion in the talocrural joint. Definitive surgical treatment with partial tarsal arthodesis produced an excellent clinical outcome. This case report demonstrates the successful treatment of a severe congenital tarsal abnormality and also documents the first clinical use of botulinum toxin in a feline patient.  相似文献

Seventeen cats with injuries involving the distal tibia, talocrural joints, tarsus and metatarsus were treated with contoured connecting bar transhock external fixators as either primary or secondary fixation. The assemblies stabilised all the injuries successfully. Fracture union was achieved in 11 out of 12 cases. Excellent results were achieved in eight cats and the outcome was satisfactory in a further five. Those cats with injuries involving talocrural joint luxation with displacement and significant periarticular soft tissue disruption had poorer long-term results. Complications occurred in seven cases.  相似文献

A 2-year-old dwarf rabbit was referred for right forelimb lameness of 1-week duration. Physical examination and radiographic findings were consistent with an elbow luxation. Manual reduction under anesthesia was unsuccessful in correcting the luxated joint. A surgical approach was recommended to repair the rabbit elbow luxation. Transcondylar, transradial, and transulnar tunnels were drilled from lateral to medial with 2 suture prostheses being placed, 1 from the radius to the humerus and 1 from the humerus to the ulna. Analgesia was provided and the patient was reevaluated on days 2, 7, and 14 after the procedure. The luxation was reduced and no recurrence was noted a year following the surgical procedure. Circumferential suture prostheses, recently described with no reported postoperative complications in cats, can be used successfully in rabbits as evidenced in this case report.  相似文献

OBJECTIVE: To investigate sacroiliac luxation repair with positional screw insertion from the ventral surface of the sacral wing via a ventral abdominal approach in cats. ANIMALS: 18 European shorthair cats. PROCEDURES: All cats underwent clinical examination including orthopedic and neurologic examination and assessment of lameness and pain before and immediately after surgery and 6 and 16 weeks after surgery. All sacroiliac luxations were stabilized with a single positional 2.4-mm cortical titanium self-tapping screw. The pelvic floor was also repaired in selected cats. Screw entry points and angles determined in a prior study of cadavers were used. Radiographs were taken before surgery and during follow-up evaluations to assess postoperative sacroiliac luxation reduction, implant placement, and repair stability. RESULTS: All implants were placed correctly. Iatrogenic sciatic nerve injuries occurred in 2 cats. Median time to ambulation was 1.5 days for cats with sacroiliac luxation as the sole injury. Radiographic outcome of sacroiliac luxation repair was excellent in 15 of 17 repairs, good in 1 of 17 repairs, and poor in 1 of 17 repairs. Clinical outcome was excellent in 11 of 15 cats and good in 4 of 15 cats. CONCLUSIONS AND CLINICAL RELEVANCE: Insertion of a positional screw across the sacroiliac joint via a ventral abdominal approached can be an alternative to conventional techniques of sacroiliac luxation repair in cats. This novel technique allowed repair of bilateral sacroiliac luxation, repair of pelvic floor fractures, and treatment of soft tissue injuries of the abdominal cavity or abdominal organs with a single approach.  相似文献

A 2-week-old Miniature Horse foal was referred for evaluation and treatment of a luxated right tarsometatarsal joint. Treatment consisted of closed reduction and internal fixation using two partially threaded Steinmann pins placed in normograde fashion through the tuber calcis into the proximal third metatarsus. Traumatic luxation has been reported to occur in the tarsocrural, proximal intertarsal and tarsometatarsal joints within the equine tarsus. Treatment for luxation of the distal intertarsal joint has not been documented. The treatment method most commonly suggested for tarsal luxation is closed reduction and cast immobilisation. Internal fixation using lag screws and plating has also been described. A combination of internal fixation and external coaptation is thought to achieve maximal stability and allow faster convalescence in cases of tarsal luxation. This case report describes for the first time a technique using two Steinmann pins to achieve successful internal fixation of a traumatic tarsometatarsal joint luxation in a 2-week-old Miniature Horse foal.  相似文献

Objective: To report repair of a coxofemoral joint luxation in an Alpaca using a toggle‐pin technique. Study Design: Case report. Animals: An 11‐month intact male Alpaca with luxation of the right coxofemoral joint. Methods: The Alpaca was anesthetized and an open repair and reduction of the luxation was performed using a toggle‐pin technique. Results: The luxation was successfully reduced. An Ehmer sling was used for the initial 3 days after surgery and the Alpaca was discharged 7 days postoperatively without complications. Follow‐up examinations confirmed maintained reduction of the coxofemoral joint, as well as no evidence of lameness or muscle atrophy. Conclusions: Coxofemoral joint luxations in Alpacas can be successfully repaired using a toggle‐pin technique alone, without the need for other techniques such as capsular reconstruction or greater trochanter transposition. Use of an Ehmer sling for the immediate postoperative period provided additional protection to the repair and was tolerated well.  相似文献

Patellar luxation and associated pelvic limb lameness were treated with polyethylene sulcal ridge augmentation prostheses in four cats. In all cases, the lameness resolved within weeks of a single surgery. No important complications were encountered. On radiographs obtained at least 2 years postoperatively, three cases showed radiographic progression of degenerative joint disease, and one showed evidence of screw loosening, but these were not reflected in apparent lameness and no case required revision.  相似文献

OBJECTIVE: To determine hip, stifle, and tarsal joint ranges of motion (ROM) and angular velocities during swimming and walking in healthy dogs and dogs with surgically corrected cranial cruciate ligament (CCL) rupture. DESIGN: Prospective clinical study. ANIMALS: 13 healthy dogs and 7 dogs with CCL rupture. PROCEDURE: Dogs with CCL rupture were enrolled in a postoperative aquatic rehabilitation program and evaluated 21 to 35 days after surgery. Dogs were filmed while swimming in a pool and while walking at a fast (1.3 m/s) or slow (0.9 m/s) pace on a treadmill. Maximal angles of extension and flexion, ROM, and angular velocities were calculated. RESULTS: In healthy dogs, swimming resulted in a significantly greater ROM in the hip joint than did walking, but in dogs with CCL rupture, ROM of the hip joint did not vary with swimming versus walking. For dogs in both groups, swimming resulted in significantly greater ROM of the stifle and tarsal joints than did walking, primarily because of greater joint flexion. Stifle joint ROM was significantly lower in dogs with CCL rupture than in healthy dogs, regardless of whether dogs were swimming or walking. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggested that following surgical management of a ruptured CCL in dogs, swimming resulted in greater ROM of the stifle and tarsal joints than did walking. This suggests that if ROM is a factor in the rate or extent of return to function in these dogs, then aquatic rehabilitation would likely result in a better overall outcome than walking alone.  相似文献

Epidural morphine is widely used in veterinary medicine, but there is no information about the anti-hyperalgesic and anti-inflammatory effects in acute inflammatory joint disease in horses. The analgesic, anti-hyperalgesic and anti-inflammatory effects of epidural morphine (100mg/animal or 0.17±0.02mg/kg) were therefore investigated in horses with acute synovitis. In a cross-over study, synovitis was induced in the talocrural joint by intra-articular lipopolysaccharide (LPS). The effect of epidural morphine was evaluated using physiological, kinematic and behavioural variables. Ranges of motion (ROM) of the metatarsophalangeal and talocrural joints were measured, clinical lameness scores and mechanical nociceptive thresholds (MNTs) were assessed and synovial fluid inflammatory markers were measured. The injection of LPS induced transient synovitis, resulting in clinical lameness, decreased ranges of motion in the talocrural and metatarsophalangeal joints, decreased limb loading at rest and increased composite pain scores. Epidural morphine resulted in a significant improvement in clinical lameness, increased ROM and improved loading of the LPS-injected limb at rest, with no effects on synovial fluid inflammatory markers. Morphine prevented a decrease in MNT and, hence, inhibited the development of hyperalgesia close to the dorsal aspect of inflamed talocrural joints. This study showed that epidural morphine provides analgesic and anti-hyperalgesic effects in horses with acute synovitis, without exerting peripheral anti-inflammatory effects.  相似文献

Objective— To assess the ability of an operator to differentiate intact from transected canine and feline elbow joint collateral ligaments (CL) using a reported manipulative test (Campbell's test) and to determine the potential for elbow joint luxation in canine and feline elbows with intact, transected, and surgically stabilized CL. Study Design— In vitro biomechanical study. Sample Population— Canine (n=6) and feline cadavers (n=3). Methods— Thoracic limb specimens were mounted on a custom‐built jig with the elbows and carpi fixed in 90° of flexion. Angles of pronation and supination were recorded after applying rotational forces to the manus. Attempts were made to manually luxate each elbow with intact CL. Constructs were re‐evaluated after sequential sectioning of the medial (MCL) and lateral (LCL) collateral ligaments and after insertion of a new CL prosthesis. Results— Mean (±SD) angles of rotation in dogs increased from 27.3±8° (range, 16.7–41.3°) in pronation to 58.9±9.2° (range, 38–88.3°) after sectioning the MCL and from 45.5±10.8° (range, 30.7–67.3°) in supination to 68.9±17.2° (range, 45–94°) after sectioning the LCL. Angles of pronation and supination were subject to significant interanimal variability, with a strong correlation between increasing animal weight and smaller angles of rotation. Elbow luxation in dogs was not possible unless at least the LCL was transected. In cats, mean angles of rotation increased from 49.8±14.9° (range, 30.7–70°) in pronation to 99.1±17.6° (range, 79–111.7°) after sectioning the MCL and from 128.7±18.8° (range, 108.3–151.7°) in supination to 166.7±13.1° (range, 157.3–181.7°) after sectioning the LCL. Luxation in cats was not possible unless both CL were cut. Use of the ligament prosthesis without primary CL repair reliably prevented reluxation in all canine and feline elbows. Conclusions— Campbell's test allowed reliable differentiation of intact, transected and surgically stabilized canine and feline elbow joint CL in a cadaveric model. Luxation could not be performed by application of rotational forces to specimens with intact CL. Clinical Relevance— Clinical examination findings, specifically Campbell's test, can be used to determine elbow CL integrity in dogs and cats. The contralateral elbow should be used as a control, because of interanimal variability in angles of rotation.  相似文献

Three horses and three ponies were treated for traumatic luxation of the proximal intertarsal or tarsometatarsal joint. In each case, there were fractures of one or more tarsal bones. Three of the animals had disruption of one of the collateral ligaments. Five animals were treated by closed reduction and external coaptation. One pony was treated by open curettage, a cancellous bone graft, and cast immobilization. No internal fixation was used in any of these animals. From this series of cases, it would appear that closed reduction and external coaption alone is adequate treatment for most cases of tarsal luxation.  相似文献

Both cats and dogs belong to animals with the same type of limb support but have different nature of movement. Despite belonging to digitigrade animals, cats and dogs have a different nature of motion. While moving, the medial joint surface in cats and lateral surface in dogs carry the larger pressure. The aim of the study was to compare the similar surfaces of the cat's and dog's tarsal joint capsule and to detect differences in its histostructure and vascularisation. For the study, we used the capsule of the tarsal joint of five cats and five dogs dissected with accordance to anatomical surfaces. Sections of the capsule joint were stained with haematoxylin–eosin. The sections were examined with a microscope at magnification ×250 and ×400. The statistical analysis of the results was done using Student's t test. During the research, a difference in histostructure and vascularisation of tarsal joint capsule in cats and dogs on respective surfaces were found. The medial surface of the tarsal joint capsule was the most saturated with hemomicrocirculatory bed structures in cats, whereas this was the lateral surface in dogs. The most active metabolic processes also take place in these areas of the joint capsule, which is important to know when prescribing therapeutic procedures and determining an optimal surgical access.  相似文献

Talocalcaneal luxation in dogs was studied by anatomic dissection of the talocalcaneal joint in cadavers and review of five clinical cases. The integrity of the talocalcaneal joint was maintained by two strong ligaments traversing the tarsal sinus between the two bones. The joint was found to be a low motion joint. Luxation in clinical cases was not always apparent on standard radiographic views. Three dogs were treated surgically with a screw inserted in lag fashion from talus to calcaneus. One luxation was treated surgically with figure-of-eight orthopedic wires and one was treated with external coaptation. Four dogs returned to their previous levels of function without clinically detectable lameness.  相似文献

Implants of plaited carbon fibre have been used in the repair of ruptured Achilles tendons in ten dogs. Additionally the hock was held in extension by inserting a screw through the fibular tarsal bone and the tibia. OBJECTIVE: To investigate the effects of disk fenestration and ventral slot formation on vertebral motion unit (VMU) range of motion (ROM) and determine the effects of fenestration and ventral slot width on VMU ROM. SAMPLE POPULATION: C5-C6 VMUs from 10 skeletally mature canine cadavers. PROCEDURES: Specimens were assigned to 2 groups (5 specimens/group). Surgery was performed in which width of a fenestration and a ventral slot was 33% (group 1) or 50% (group 2) the width of the vertebral body. Flexion-extension, lateral bending, and axial torsion ROMs were measured during loading before surgery, after fenestration, and after ventral slot formation. Range of motion was compared within groups to determine effects of surgical procedure on stability and between groups to determine effects of width of fenestration and ventral slot on stability. RESULTS: For both groups, fenestration resulted in a significant increase in ROM during flexion-extension, compared with results for intact specimens. Ventral slot formation resulted in a significant increase in ROM during flexion-extension and lateral bending, compared with results for intact specimens. Ventral slot formation resulted in a significant increase in ROM only during flexion-extension, compared with results for fenestrated specimens. There were no significant differences in ROM of the intact, fenestrated, and ventral slot specimens between groups. CONCLUSIONS AND CLINICAL RELEVANCE: Analysis of these results suggests that fenestration and ventral slot procedures each affect the biomechanics of the C5-C6 VMU. Width of a fenestration or ventral slot up to 50% of the width of C5-C6 may be clinically acceptable.  相似文献

Multiple ligament injuries of the canine and feline stifle joint which result in luxation are uncommon. Two cats and one dog, that had sustained such a joint injury were surgically treated. A rupture of the cranial and caudal cruciate ligaments, and at least one collateral ligament was observed in all of the three animals. Prosthetic reconstruction was used, as previously described, in combination with a novel technique of intraoperative placement of a temporary trans-articular pin (TTP) to maintain intra-operative anatomical reduction. TTP placement facilitated maintenance of joint alignment during surgical reconstruction and aided appropriate tensioning of the prosthetic sutures, preventing collapse of femorotibial joint compartments. The TTP was removed prior to closure of the joint allowing immediate post-operative joint mobilisation. Based on assessment by their owners, all the animals made a complete recovery. TTP was considered a relatively simple and effective adjunctive aid for surgical treatment of traumatic luxation of the stifle joint.  相似文献

Lateral luxation of the patella in four femoropatellar joints of three foals was repaired by recession sulcoplasty and medial imbrication of the joint. One foal was euthanatized because of septic arthritis caused by incisional dehiscence, and the other two foals were normal 2 years after surgery. Recession sulcoplasty combined with medial imbrication of the joint capsule resulted in a functional femoropatellar joint and should be considered in foals affected with lateral patellar luxation.  相似文献

A 14-year-old Quarter Horse x Thoroughbred gelding was admitted to the teaching hospital because of inability to bear weight on its right hind limb, associated with a severe deviation of the tarsus. The provisional diagnosis was tarsal joint luxation or fracture. Radiography revealed complete luxation of the tarsocrural joint. The luxation was reduced, using minimal force. The horse was confined to a box stall and was maintained in a full-length hind-limb cast for 33 days. Box stall confinement was continued for 3 months after removal of the cast. On reexamination 18 months after the injury, the horse had only mild lameness (grade 1 of 5), but had marked reduction in the range of motion of the tarsus. Radiography revealed extensive changes indicative of severe degenerative joint disease. The horse was still used occasionally as a light pleasure riding horse and maintained itself on pasture well.  相似文献

Repair of lateral luxation of the patella in horses and cows has consistently produced poor results; reluxation, muscle atrophy, septic arthritis, and degenerative joint disease are frequent postoperative complications of the techniques previously described. A new technique for repair of congenital lateral luxation of the patella in foals and calves is described; it includes lateral release of the patella followed by imbrication of the medial aspect of the joint capsule. Postoperative evaluation, up to 1½ years, confirmed the efficacy of the technique with retention of normal patellar position and range of motion. Muscle atrophy and lameness disappeared completely in 2 animals, which showed no radiographic evidence of degenerative joint disease at the time of surgical repair. Since the preparation of this article, 3 more calves have been successfully repaired with excellent clinical results up to 12 weeks postoperatively.  相似文献
