Complications of cervical myelography arising from the puncture of the subarachnoid space to collect the cerebrospinal fluid and to inject the contrast medium have been described in humans and animals. In this study, 2 ultrasound-guided procedures were developed for puncture of the atlanto-occipital subarachnoid space, collection of cerebrospinal fluid, and injection of contrast medium. Myelography was performed on 6 ataxic horses using these procedures. The first attempt to puncture the subarachnoid space was successful in 5 horses and in one horse, a second attempt was necessary. Collection of cerebrospinal fluid and injection of contrast medium were achieved without difficulty. Ultrasound-guided myelography allowed reduction of potential complications associated with blind percutaneous puncture of the subarachnoid space. Methods described in this study should be tried-at least initially in an experimental setting--to collect cerebrospinal fluid from the atlanto-occipital site in standing horses where it may represent an alternative method when lumbosacral cerebrospinal fluid collection has been unsuccessful or contaminated with blood.  相似文献

In search of a safer myelographic technique, we performed myelography via the lumbosacral intervertebral space. Eight dogs, in which the backflow of cerebrospinal fluid was observed, received contrast media via the lumbosacral intervertebral space. The subarachnoid contrast columns were successfully observed in 6 dogs. During and after examination, no physiological changes or neurological signs were observed. We recommend that the lumbosacral intervertebral space be selected first before implementing the conventional lumbar myelography.  相似文献

ObjectiveTo evaluate the use of ultrasound for identifying the site for needle puncture and to determine the depth to the epidural space in obese dogs.Study designProspective study in dogs undergoing elective orthopedic surgery.AnimalsA group of seven obese Labrador male dogs aged 6.93 ± 2.56 years and weighing 46.5 ± 4.1 kg (mean ± standard deviation).MethodsThe anesthetic protocol for these dogs included epidural anesthesia. With the dogs anesthetized and positioned in sternal recumbency with the pelvic limbs flexed forward, ultrasound imaging was used to locate the lumbosacral intervertebral space. Intersection of dorsal and transverse lines about the probe identified the point of needle insertion. A 17 gauge, 8.9 cm Tuohy needle was inserted perpendicularly through the skin and advanced to the lumbosacral intervertebral space. The number of puncture attempts was recorded and needle depth was compared with skin to ligamentum flavum distance.ResultsEpidural injection was performed in all dogs at the first attempt of needle insertion. The distance from skin to epidural space was 5.95 ± 0.62 cm measured by ultrasound and 5.89 ± 0.64 cm measured with the Tuohy needle. These measurements were not different (p = 0.26). A highly significant correlation coefficient of 0.966 between measurement techniques was obtained (p < 0.001).Conclusions and clinical relevanceUltrasound imaging identified the point of needle insertion for lumbosacral epidural injection in seven obese dogs. The results indicate that ultrasound can be used to locate the lumbosacral intervertebral space and identify an appropriate point for needle insertion to perform epidural injection.  相似文献

The serotonin metabolite, 5-hydroxyindoleacetic acid (5-HIAA) and the dopamine metabolite, homovanillic acid (HVA) in the cerebrospinal fluid (CSF) of seven clinically normal horses were evaluated with reverse phase high pressure liquid chromatography and electrochemical detection. Comparisons of the neurotransmitter metabolite concentrations were made on CSF collected simultaneously from the atlanto-occipital and lumbosacral regions. There were significantly higher amounts of 5-HIAA and HVA in atlanto-occipital CSF than in lumbosacral CSF. Mean 5-HIAA concentrations in atlanto-occipital and lumbosacral CSF were 206.1±37.0 nmol/L and 68.5±12.6 nmol/L, respectively. The mean HVA concentration in atlanto-occipital CSF was 254.4±50.4 nmol/L. No HVA was detected in lumbosacral CSF. In contrast, significantly greater protein concentrations were present in lumbosacral CSF than in atlanto-occipital CSF. Lumbosacral and atlanto-occipital CSF contained 32.1±3.7 mg protein/dl and 25.0±1.3 mg protein/dl, respectively. Thus, between the atlanto-occipital and lumbosacral space of the horses, a rostrocaudal gradient existed for neurotransmitter metabolites and a caudorostral gradient existed for protein.  相似文献

The goals of this study were to evaluate techniques for collection of peritoneal fluid from calves, establish reference ranges for fibrinogen in peritoneal fluid during the 1st month of life, and determine if abomasal puncture would alter peritoneal fluid or hematologic variables. Twenty-two healthy Holstein calves underwent 3 peritoneal fluid collections on day 1, day 15, and day 30 of age. Fibrinogen concentration in peritoneal fluid was 0.20 g/dL and 0.10 g/dL (P < .05) for day 1 and day 30, respectively, and 0.10 at day 15 (P > .05) for calves without abomasal puncture. Plasma fibrinogen concentration was 0.60 g/dL and 0.70 g/ dL (P < .05) for days 15 and 30, respectively, in calves without abomasal puncture. There were no significant differences (P < or = .05) in peritoneal fluid and peripheral blood total protein and fibrinogen concentrations, specific gravity, total and differential cell count, or erythrocyte counts between calves with or without abomasal puncture. We concluded that the reference ranges established for fibrinogen and total protein concentration are important for accurate evaluation of peritoneal fluid in calves for further comparison with similar-aged animals with gastrointestinal-tract or abdominal-cavity disease. Additionally, accidental abomasal puncture does not alter values of fibrinogen, total protein, and nucleated cell count in peritoneal fluid and does not cause apparent clinical abnormalities.  相似文献

Cerebrospinal fluid analyses are important for diagnosis of neurologic problems in rabbits and for translational research projects using rabbits as models. Blind puncture of the cisterna magna is the current standard technique for sampling cerebrospinal fluid in this species. However, the complexity and small size of the cisterna magna and surrounding structures are limitations of this technique. Aims of this prospective, anatomic, pilot study were to (1) describe the normal anatomy of the atlanto‐occipital region, (2) describe ultrasonographic anatomic landmarks, and (3) develop and evaluate a technique for ultrasound‐guided puncture of the cisterna magna for cerebrospinal fluid sampling in rabbits. Thirty healthy rabbits were included and the study was conducted in three stages. Three rabbit cadavers were used for the first stage of the study. Then, the second stage was completed using 13 rabbit cadavers. Finally, the third stage was completed in 14 live rabbits. The ultrasound‐guided puncture performed in 13 cadavers was successful at the first attempt in 10 cases, and at the second attempt in the remaining three cases. In the in vivo study, the ultrasound‐guided puncture was successful in all 14 cases, without signs of complications. Findings supported the use of ultrasound‐guided puncture of the cisterna magna as a safe technique that may be used routinely or when the sample of cerebrospinal fluid cannot be obtained with the blind technique in rabbits.  相似文献

Standing myelography in the horse has been previously described. In that study, metrizamide was used and significant complications were reported. In recent years, the introduction of less-toxic nonionic contrast media has reduced the incidence of complications. This study was undertaken to determine whether standing myelography using a nonionic contrast medium could provide a diagnostic study and be performed safely in the equine patient. Standing myelography was performed in eight horses. The contrast medium used was iohexol. In five horses a myelogram of diagnostic quality was achieved; in one horse contrast flowed only to the level of C6 and in two horses contrast medium did not reach the cervical subarachnoid space. Owing to the difficulty in achieving good flow of the contrast medium in some horses, this procedure may be of limited utility. However, if puncture of the lumbosacral subarachnoid space can be achieved easily and quickly, standing myelography may be a clinically useful procedure. It may be attempted in cases in which the economic value of the patient makes myelography under general anesthesia impractical. In patients presenting for evaluation of ataxia it may be possible to perform a standing myelogram at the time of CSF sample collection from the lumbosacral space.  相似文献

Gurltia paralysans is a rare metastrongylid nematode of domestic cats that is found mainly in the veins of the spinal cord subarachnoid space and parenchyma. Endemic regions for G. paralysans mainly include Chile and Argentina. The ante mortem diagnosis of gurltiosis is difficult and based primarily on neurological signs, epidemiological factors, and the exclusion of other causes of feline myelopathies. The purpose of this retrospective case series was to describe clinical, imaging, and pathologic characteristics in nine domestic cats naturally infected with G. paralysans. Imaging tests included radiography, myelography, computed tomographic myelography (myelo‐CT), and magnetic resonance imaging (MRI). Neurological signs included paraparesis, paraplegia, pelvic limb ataxia and proprioceptive deficits, pelvic limb tremors, lumbosacral hyperesthesia, and tail trembling or atony. Complete blood count findings included a decrease in the mean corpuscular hemoglobin concentration value in eight cats. Eosinophilia in peripheral blood was observed in three cats, and thrombocytopenia was observed in three cats. Cerebrospinal fluid analysis revealed mononuclear pleocytosis in five cases. Myelo‐CT showed diffuse enlargement of the spinal cord at the midthoracic, lumbar, and sacral regions in all cats. Magnetic resonance image findings in the thoracic and lumbar region demonstrated multiple small nodular areas of T2 hyperintensity in the periphery of the spinal cord parenchyma. Localized intraparenchymal areas of increased T2 intensity were also observed in the thoracolumbar spinal cord and lumbosacral conus medullaris. In conclusion, G. paralysans should be considered as a differential diagnosis for domestic cats in endemic regions that have this combination of clinical and imaging characteristics.  相似文献

Nineteen pregnant cows were experimentally infected with bovine viral diarrhoea virus (BVDV) between day 74 and 81 of pregnancy. All cows became infected and developed serum antibodies. Sixteen of the cows delivered persistently infected (PI) offspring, whereas the remaining three gave birth to calves with detectable serum antibodies and free from BVDV. The 16 cows with PI foetuses developed higher levels of antibodies in serum during pregnancy than did their three peers carrying non-PI calves. Multivariate analysis showed that the antibody levels in these two groups of cows were significantly different from day 135 of pregnancy. Foetal fluid was successfully collected from 18 of the 19 infected cows and from five uninfected control cows between 10 and 24 days before delivery by use of a percutaneous, blind puncture technique. No negative effects were observed in the cows or their offspring. BVDV was isolated and detected with an immunoperoxidase test in foetal fluid from 13 of the 16 cows carrying PI foetuses, and from 15 of the cows when a quantitative fluorescent polymerase chain reaction (PCR) technique was used. The negative sample in the PCR assay was positive for BVDV antibodies. The number of viral copies per microlitre in foetal fluids varied between 103 and 1080 in the positive samples. All samples taken from the cows carrying non-PI foetuses were negative for BVDV in both assays. In this experiment, examination of either serum or foetal fluids could identify the cows carrying a PI foetus. Examination of serum for BVDV antibodies was a reliable indicator of a PI foetus if the serum was collected during the last 2 months of pregnancy. For examination of foetal fluids, both viral and serological analyses should be performed. For viral analysis, PCR should be the test of choice. High levels of BVDV antibodies in conjunction with a negative result in the PCR may be indicative of a false-negative virus result. Further experience with the method of collection of foetal fluids is necessary for evaluation of its safety. Investigation of pregnant cows in order to discover a PI offspring before it is born could be a useful tool in control and eradication of BVDV.  相似文献

OBJECTIVE: To describe ultrasonographic landmarks for use in collection of CSF from the lumbosacral region in equids. DESIGN: Prospective study. ANIMALS: 37 equids (27 with neurologic disease and 10 with nonneurologic disease). PROCEDURES: Standing equids (n = 17) were sedated with detomidine hydrochloride (0.006 to 0.01 mg/kg [0.003 to 0.005 mg/lb], IV) followed by butorphanol tartrate (0.01 mg/kg, IV) and restrained with a nose twitch for collection of CSF. The CSF was collected from 20 laterally recumbent equids (10 sedated and 10 immediately after euthanasia). Anatomic landmarks were identified ultrasonographically. Height at the dorsal point of the shoulders, body weight, depth of the spinal needle, number of attempts to collect CSF, and cytologic evaluation of CSF were recorded. RESULTS: Lumbosacral puncture cranial to the cranial border of the most superficial location of both tuber sacrale along the midline was consistently successful for CSF collection (35/37 equids). Two horses had anatomic abnormalities that precluded CSF collection. Mean number of attempts to collect CSF per animal was 1.1. Height and body weight were strongly correlated with needle depth for CSF collection. Pelvic and sacral displacement was observed in several laterally recumbent animals, which resulted in discrepancies of the midline between the cranial and caudal aspects of the vertebral column. In most equids, the spinal needle was aligned on the midline of the caudal aspect of the vertebral column. CONCLUSIONS AND CLINICAL RELEVANCE: Ultrasonography was a useful aid for collection of CSF from the lumbosacral space and decreased the risk of repeated trauma and contamination in equids.  相似文献

The time to loss of interdigital reflex, duration of action, duration of muscle relaxation and cardiorespiratory effects were compared after epidural block using lignocaine, bupivacaine or a combination of lignocaine and bupivacaine in dogs. Dogs were pre-medicated with methotrimeprazine, anaesthesia was induced with thiopentone, in order to facilitate epidural puncture, and a lumbosacral epidural block was performed. Body temperature fell after pre-medication, thiopentone and epidural anaesthesia. Minimal changes were observed in arterial O2 saturation, end tidal CO2, respiratory rate, heart rate and mean arterial blood pressure. The combination of bupivacaine with lignocaine produced a shorter time to loss of interdigital reflex than bupivacaine alone, longer analgesia than lignocaine alone and longer muscle relaxation than either lignocaine or bupivacaine. The combination appeared to be the best choice when surgical time is prolonged.  相似文献

Survey radiographic studies of the lumbosacral region for 93 normal dogs and for 26 dogs with confirmed degenerative lumbosacral stenosis were reviewed. Normal dogs were divided into 9 groups based on age and body weight. For normal dogs, increasing age and body weight were associated with a decreased ability to extend the lumbosacral joint and with increased incidence and severity of spondylosis. Transitional lumbosacral vertebrae and evidence of lumbosacral disc space collapse were very infrequent findings, and the pivot point for lumbosacral motion was consistently centered over the lumbosacral disc space. Relative to an age/weight matched sub-population of normal dogs, dogs with degenerative lumbosacral stenosis had similar mean normalized lumbosacral vertebral canal height, larger mean neutral lumbosacral angle, decreased extension of the lumbosacral joint, increased flexion of the lumbosacral joint, reduced lumbosacral range of motion, increased lumbosacral dynamic malalignment, higher incidence and severity of spondylosis, higher incidence of transitional vertebrae, and higher incidence of lumbosacral disc space collapse. A logistic model based strictly on radiographic parameters was able to discriminate normal from affected dogs with an overall accuracy rate of 86%.  相似文献

Management of degenerative lumbosacral stenosis in military working dogs more frequently utilizes core conditioning exercise programs. Future research on the effectiveness of these programs may benefit from an improved understanding of relationships between paraspinal muscle size and lumbosacral stability. The aim of this retrospective, secondary analysis, cross‐sectional study was to test the following hypotheses related to CT measures: (a) transverse paraspinal muscle area ratios differ between hip flexion and extension, (b) lumbosacral angle and lumbosacral range of motion differ by lumbosacral angle measurement technique, and (c) transverse paraspinal muscle area ratios are correlated with CT measures of lumbosacral stability (parasagittal and parasagittal oblique foraminal area changes) regardless of hip position and with lumbosacral range of motion within each hip position. Lumbosacral CT scans in hip flexion and extension were retrieved from a previous prospective study of military working Labrador Retrievers. A single observer performed triplicate measurements of transverse paraspinal muscle area ratios, parasagittal foraminal area, and parasagittal oblique foraminal area for each hip position and another observer independently performed triplicate measurements of lumbosacral angle and lumbosacral range of motion at L7‐S1 using two published techniques. Thirty‐nine dogs were analyzed and significant differences were identified between hip flexion and extension for all mean transverse paraspinal muscle area ratio values (P ≤ .05). Mean lumbosacral angles also significantly differed between the published techniques in both hip flexion and extension. When comparing mean lumbosacral range of motion values, one of the published techniques produced significantly smaller values. No significant correlation was found between transverse paraspinal muscle area ratios and parasagittal foraminal area changes, parasagittal oblique foraminal area changes, or lumbosacral range of motion. These results should be considered when designing studies using these CT measures in military working dogs.  相似文献

Standing myelography was conducted in six neurologically normal adult horses. Myelograms were performed without general anesthesia or tranquilization. Four horses were premedicated orally with 2.2 milligrams/kilogram (mg/kg) of phenobarbital 16 hours and 4 hours prior to myelography to raise their seizure threshold. One-hundred milliters (ml) of cerebral spinal fluid (CSF) was withdrawn from the lumbosacral space prior to injection. Approximately 100 ml of technical grade metrizamide * was injected into the subarachnoid space via a lumbosacral tap. After injection the horse's head was lowered for approximately 10 minutes to facilitate cranial movement of the metrizamide. Lateral cervical radiographs were taken in a standing position. Filling of the subarachnoid space in the cranial cervical region was good to excellent in five horses. In one horse there was poor definition of the cranial cervical region. There was good to excellent filling of the subaranoid space in the caudal cervical region in all six horses. Reactions to metrizamide injection varied. The most severe reaction was generalized seizure.  相似文献

Subarachnoid lumbar puncture is used commonly in the dog for cerebrospinal fluid collection and/or myelography. Here in we describe the percutaneous ultrasound anatomy of the lumbar region in the dog and a technique for ultrasound‐guided lumbar puncture. Ultrasound images obtained ex vivo and in vivo were compared with anatomic sections and used to identify the landmarks for ultrasound‐guided lumbar puncture. The ultrasound‐guided procedure was established in cadavers and then applied in vivo in eight dogs. The anatomic landmarks for the ultrasound‐guided puncture, which should be identified on the parasagittal oblique ultrasound image are the articular processes of the fifth and sixth lumbar vertebrae and the interarcuate space. The spinal needle is directed under ultrasound‐guidance toward the triangular space located between the contiguous articular processes of the fifth and sixth lumbar vertebrae and then advanced to enter the vertebral canal. Using these precise ultrasound anatomic landmarks, an ultrasound‐guided technique for lumbar puncture is applicable in the dog.  相似文献

German shepherd dogs are overrepresented in the group of dogs with cauda equina compression syndrome due to degenerative lumbosacral stenosis. A congenital predisposition for early degeneration of the lumbosacral intervertebral disc has been suspected. Our aims were to assess the morphologic appearance of the lumbosacral intervertebral disc and the lumbosacral junction in healthy German shepherd dogs compared to other breeds and to evaluate for an early onset of degenerative changes. The lumbosacral spine of 110 clinically sound German shepherd dogs and 47 healthy dogs of other large breeds was examined using magnetic resonance (MR) imaging. The degeneration of every intervertebral disc was graded using an established classification system. Signal intensity of the entire lumbosacral disc and the nucleus pulposus was determined independently. Lumbosacral malalignment was assessed according to a previously described method. The findings for the German shepherd dogs were compared to those of the other breeds. Although most dogs were younger than 18 months at the date of examination, significantly higher grades of degeneration were detected for the lumbosacral intervertebral disc of German shepherd dogs (P < 0.003). Degeneration of the lumbosacral intervertebral disc was independent from findings in the other lumbar discs. We conclude that the German shepherd dog has a predisposition for degenerative changes in the lumbosacral intervertebral disc.  相似文献

Lumbar myelography, intraosseous caudal vertebral venography, and epidurography were performed in 12 normal, mature mixed-breed dogs. The radiographic appearance of the lumbosacral region was evaluated in both the lateral and ventrodorsal projections. These three radiographic contrast procedures were repeated in the same dogs after the introduction of a silicone mass into the spinal canal at the lumbosacral junction. The radiographic findings were compared with postmortem findings to determine which contrast procedure was most useful in detecting the mass in the lumbosacral spine. None of the procedures evaluated consistently produced good-quality studies in the normal dogs. After introduction of the silicone mass, lumbosacral epidurography yielded the largest number of positive correlations. However, the sensitivity of lumbosacral epidurography was less than 50% in the lateral projection and less than 20% in the ventrodorsal projection. Though none of the procedures were consistently helpful in the diagnosis of the lumbosacral masses, lumbosacral epidurography has the most potential to give consistently good-quality studies and thereby a greater probability of detecting an abnormality.  相似文献

The pH, pCO2, and pO2 values and the concentrations of sodium, potassium, calcium, magnesium, chloride, phosphorus, bicarbonate, base excess, protein, glucose, as well as the activity of alkaline phosphatase and malate dehydrogenase were determined in the venous blood and uterine fluid of control and EDS'76 virus-infected fowl. Moreover, the pH of the mucosa of different parts of the oviduct was measured. Hens were examined in the period from 10 to 24 days following infection; blood and uterine fluid samples were collected approximately 14 hours after oviposition, provided a plumped egg was present in the uterus. Examination of blood and pH measurement of oviduct mucosa did not yield significant differences between infected and noninfected hens. In comparison with noninfected control birds, the mean sodium concentration of the uterine fluid of infected hens producing soft shelled or shell-less eggs had evidently increased, while the mean concentration of potassium, calcium, magnesium and glucose had decreased. Similar differences were also observed between infected hens producing normally shelled eggs and infected hens producing abnormally shelled eggs. No significant differences between infected and not infected hens were observed concerning the other values determined in the uterine fluid. It is concluded that functional disturbances which account for shell aberrations following EDS'76 virus infection are located in the surface epithelial cells of the uterine mucosa. These disturbances are very probably initiated by a depressed function of the sodium pump. All changes observed in the uterine fluid of infected hens could be explained by this depressed function.  相似文献
