Objective To compare the fresh gas flow requirements of the ‘Maxima’ and Jackson‐Rees modified Ayre's T‐piece (JRMATP) in spontaneously breathing anaesthetized in cats. Study design Prospective randomized clinical study. Animals or sample population Fifteen adult cats (6 male, 9 female, 3.1 ± 0.4 kg [x? ± SD]). Materials & methods After pre‐anaesthetic medication with acepromazine and pethidine, anaesthesia was induced using thiopentone and the trachea was intubated with a cuffed endotracheal tube. This was attached to either a ‘Maxima’ or a JRMATP breathing system; allocation was randomized. Anaesthesia was maintained with halothane delivered in a 1 : 1 oxygen : nitrous oxide mixture. Initial total fresh gas flow (FGF) was set at 600 mL kg?1 min?1. After 20 minutes, FGF was reduced in increments of 200 mL min?1 until rebreathing (inspired CO2 concentration >0.2%) occurred. At this point, FGF was increased to 600 mL kg?1 and the process was repeated with the other breathing system. The respiratory rate and airway pressure at the endotracheal tube connector were monitored throughout anaesthesia. Results The mean fresh gas flow that prevented rebreathing with the Maxima system (164 ± 39 mL kg?1) was significantly less (p < 0.0001) than that required in the modified T‐piece (455 ± 0.77 mL kg?1). Respiratory rates and airway pressures at the endotracheal tube connector were not significantly affected by breathing system employed. Conclusions In terms of the gas flow requirements that prevent rebreathing, the ‘Maxima’ breathing system is more efficient than the modified Ayre's T‐piece in spontaneously breathing cats anaesthetised with halothane.  相似文献

A horse was presented for soft palate thermocautery and surgical advancement of the larynx under general anaesthesia, following a history of respiratory noise and poor athletic performance. Physical examination prior to surgery was unremarkable. The horse was anaesthetised and a 20 mm cuffed endotracheal tube (ETT) used to intubate the trachea via the right nostril. The cuff was deflated at the end of the procedure and the tube secured in place around the head of the horse for the recovery. The horse was assisted to standing and during this process the end of the nasotracheal tube broke and was observed hanging from the head collar. The remainder of the tube was aspirated into the trachea of the horse. The remaining length of endotracheal tube was removed using a modified endoscopic technique. The horse recovered with no further problems. An unusual complication of a nasotracheal tube left in situ for recovery in a horse is reported and possible reasons for this complication are discussed.  相似文献

Objective: To evaluate the ability of capnography to document proper placement of nasoesophageal (NE) and nasogastric (NG) feeding tubes. This study was conducted in 3 phases. Phase I of this study was designed in order to test the efficacy of capnography to distinguish placement of a feeding tube in the alimentary tract versus the respiratory tract. Phase II was designed in order to document that carbon dioxide (CO2) could be measured through a polyvinyl chloride (PVC) feeding tube. Phase III was performed in order to evaluate the technique of continuous monitoring during insertion of the feeding tube into the esophagus and stomach as would be performed during a clinical‐tube placement. Design: Prospective study. Setting: Research laboratory. Animals: 24 adult dogs. Interventions: In Phase I, sedated dogs were instrumented with an intratracheal catheter and an 8 French feeding tube placed nasally into the distal esophagus and later advanced into the stomach. In Phase II, dogs were anesthetized and an 8 French feeding tube was placed down the endotracheal tube, then into the esophagus and later advanced into the stomach. In Phase III, sedated dogs were instrumented with an 8 French feeding tube inserted intranasally and then advanced to the level of the nasopharynx, distal esophagus and, lastly, the stomach. Fluoroscopy was used in order to determine location of the feeding tube. Measurements and main results: Phase I measurements included respiratory rate and CO2 from the trachea, esophagus, and stomach and pH of gastric fluid sample. Phase II measurements included respiratory rate and CO2 from the endotracheal tube, feeding tube in the endotracheal tube, feeding tube in the distal esophagus, and feeding tube in the stomach. Phase III data collection included respiratory rate and CO2 as the tube was passed through the nasal cavity, nasopharynx, esophagus and stomach. Phase I fluid samples were collected from 5 of the 9 dogs and had pH values from 1.68 to 4.20. In both phases, values for the respiratory rate and CO2 from the esophagus and stomach were 0 ± 0, significantly lower (P < 0.001) than the values from the trachea. In Phase II, there was no significant difference between the respiratory rates (P = 0.886) and CO2 (P = 0.705) readings obtained from the endotracheal tube compared to readings from the feeding tube in the endotracheal tube. In Phase III, there was a significant difference (P < 0.001) between the respiratory rates and CO2 readings obtained from the nasal cavity and the nasopharynx when compared to those readings obtained from the esophagus and stomach. Measurement of CO2 and respiratory rate resulted in a reading of 0 every time the feeding tube was in the esophagus or stomach. Conclusions: Capnography may be used in order to detect airway placement of NE and NG tubes.  相似文献

During anaesthesia in the horse, muscle blood flow has been found to be reduced, possibly leading to hypoxia or ischaemia in the muscle. The aim of this study was to use the muscle biopsy and microdialysis techniques to determine whether long-term inhalation anaesthesia in laterally recumbent horses induces metabolic changes in gluteal muscle indicative of anaerobic metabolism. Muscle biopsies and plasma samples were taken from seven horses at the start and end of halothane anaesthesia. In six isoflurane-anaesthetised horses, given three pharmacological provocations (dobutamine, detomidine, acepromazine), repeated blood samples and microdialysis was performed during anaesthesia and muscle biopsies were taken before and at the end of anaesthesia. Adenosine triphosphate (ATP), adenosine diphosphate, adenosine monophosphate, inosine monophosphate (IMP) creatine phosphate and lactate concentrations did not differ between dependent and non-dependent muscles at either sampling time. Creatine phosphate decreased in both the halothane (-38%) and isoflurane (-28%) group. In the halothane group, ATP was decreased (-15%) at the end of anaesthesia, while IMP was increased (+32%). Lactate in muscle and plasma increased in both groups. Lactate in dialysate increased after induction and remained elevated above plasma concentrations. These results show that long-term inhalation anaesthesia in horses is associated with an anaerobic metabolic response within the muscle and that microdialysis can be used to detect metabolic changes within the muscle during equine anaesthesia.  相似文献

OBJECTIVE: To compare the cardiorespiratory effects and incidence of gastroesophageal reflux with the use of a laryngeal mask airway (LMA) or endotracheal tube (ET) in anesthetized cats during spontaneous (SV) or controlled ventilation (CV). STUDY DESIGN: Prospective randomized experimental trial. ANIMALS: Thirty-two adult crossbred cats, weighing 2.7 +/- 0.4 kg. METHODS: The cats were sedated with intramuscular (IM) methotrimeprazine (0.5 mg kg(-1)) and buprenorphine (0.005 mg kg(-1)), followed 30 minutes later by induction of anesthesia with intravenous (IV) thiopental (12.5-20 mg kg(-1)). An ET was used in 16 cats and an LMA in the remaining 16 animals. Anesthesia was maintained with 0.5 minimum alveolar concentration (0.6%) of halothane in oxygen using a Mapleson D breathing system. Cats in both groups were further divided into two equal groups (n = 8), undergoing either SV or CV. Neuromuscular blockade with pancuronium (0.06 mg kg(-1)) was used to facilitate CV. Heart and respiratory rates, direct arterial blood pressure, capnometry (PE'CO2) and arterial blood gases were measured. Gastric reflux and possible aspiration was investigated by intragastric administration of 5 mL of radiographic contrast immediately after induction of anesthesia. Cervical and thoracic radiographs were taken at the end of anesthesia. Data were analyzed using anova followed by Student-Newman-Keuls, Kruskal-Wallis or Friedman test where appropriate. RESULTS: Values for PaCO2 and PE'CO2 were higher in spontaneously breathing cats with the LMA when compared with other groups. Values of PaO2 and hemoglobin oxygen saturation did not differ between groups. Gastroesophageal reflux occurred in four of eight and two of eight cats undergoing CV with ET or LMA, respectively. There was no tracheal or pulmonary aspiration in any cases. CONCLUSIONS AND CLINICAL RELEVANCE: The use of an LMA may be used as an alternative to endotracheal intubation in anesthetized cats. Although aspiration was not observed, gastric reflux may occur in mechanically ventilated animals.  相似文献

Laryngotracheal injury associated with nasotracheal intubation in the horse   总被引:2,自引:0,他引:2  
Laryngotracheal damage following short-term nasotracheal intubation was studied in 7 healthy horses. A flexible fiberoptic endoscope was used to examine the upper respiratory tract of each horse before nasal intubation with a cuffed silicone endotracheal tube and again at 1 hour, 24 hours, and 48 hours after extubation. Any abnormalities still evident at 48 hours were evaluated at 7 days after extubation. Mucosal damage involved the nasal meatus (5 of 7 horses), the arytenoid cartilages (5 of 7 horses), the trachea (5 of 7 horses), the dorsal pharyngeal recess (4 of 7 horses), the vocal folds (3 of 7 horses), and the entrance to the guttural pouch (3 of 7 horses). Laryngeal injury was attributable to tube pressure on the arytenoid cartilages and vocal folds. Tracheal damage appeared to be a function of pressure exerted by the inflated cuff on the tracheal mucosa.  相似文献

为评价七氟醚对心脏功能的影响。并与氟烷和异氟醚作比较,对动物医院就诊、心肺功能正常、拟作手术治疗的8例患犬进行麻醉试验,所有犬均以l-美散痛和安定和为麻醉前给药,分别以1.5%七氟醚、1.0%七氟醚与66.7%笑气、1.0%氟烷、1.5%异氟醚维持手术麻醉,并作人工呼吸,各组均未观察到心律异常,手术麻醉期间,七氟醚组犬心率降幅(4.73%)低于异氟醚组(9.74%)或氟烷组(10.23%)。七氟醚组、氟烷组和异氟醚组的收缩压/舒张压轻度升高(七氟醚3.38%/11.93%氟烷6.09%/9.09%,异氟醚4.82%/6.25%)。而七氟醚与笑气组的收缩压/舒张压则降低(6.84%/5.73%)。证实七氟醚应为临床首选的吸入麻醉剂。  相似文献

A total of 103 anaesthetic inductions were performed in horses for a variety of elective procedures. All cases were premedicated with acepromazine maleate (0.02 to 0.05 mg/kg body weight [bwt] intramuscularly [im]). In 50 cases (Group A) anaesthesia was induced by a single intravenous (iv) bolus of thiopentone sodium (11.1 mg/kg bwt or 1 g/90 kg bwt) followed immediately by a bolus of suxamethonium chloride (0.1 mg/kg bwt). In 53 cases (Group B) anaesthesia was induced using iv guaiacol glycerine ether (GGE) (approximately 50 mg/kg bwt) followed by a bolus of thiopentone at half the usual dose rate (5.6 mg/kg bwt or 1 g/180 kg bwt). Induction of anaesthesia was uneventful in both groups although in Group B it was particularly smooth. Following endotracheal intubation anaesthesia was maintained with halothane in oxygen administered via a circle system. The duration of anaesthesia was comparable between the two groups; however, the mean (+/- sd) time to standing in Group B, 35 +/- 22 mins, was significantly shorter than in Group A, 48 +/- 25 mins. OBJECTIVE: To evaluate effects of medetomidine on anesthetic dose requirements, cardiorespiratory variables, plasma cortisol concentrations, and behavioral pain scores in dogs undergoing ovariohysterectomy. DESIGN: Randomized, prospective study. ANIMALS: 12 healthy Walker-type hound dogs. PROCEDURE: Dogs received medetomidine (40 micrograms/kg [18.2 micrograms/lb] of body weight, i.m.; n = 6) or saline (0.9% NaCl) solution (1 ml, i.m.; 6) prior to anesthesia induction with thiopental; thiopental dose needed for endotracheal intubation was compared between groups. Ovariohysterectomy was performed during halothane anesthesia. Blood samples were obtained at various times before drug administration until 300 minutes after extubation. Various physiologic measurements and end-tidal halothane concentrations were recorded. RESULTS: In medetomidine-treated dogs, heart rate was significantly lower than in controls, and blood pressure did not change significantly from baseline. Plasma cortisol concentrations did not increase significantly until 60 minutes after extubation in medetomidine-treated dogs, whereas values in control dogs were increased from time of surgery until the end of the recording period. Control dogs had higher pain scores than treated dogs from extubation until the end of the recording period. CONCLUSION AND CLINICAL RELEVANCE: Administration of medetomidine reduced dose requirements for thiopental and halothane and provided postoperative analgesia up to 90 minutes after extubation. Dogs undergoing ovariohysterectomy by use of thiopental induction and halothane anesthesia benefit from analgesia induced by medetomidine administered prior to anesthesia induction. Additional analgesia is appropriate 60 minutes after extubation.  相似文献

The effects of different arterial carbon dioxide tensions (PaCO2) on cerebrospinal fluid pressure (CSFP) and intraocular pressure (IOP) were studied in 6 male halothane-anesthetized horses positioned in left lateral recumbency. Steady-state anesthetic conditions (1.06% end-tidal halothane concentration) commenced 60 minutes following anesthetic induction with only halothane in oxygen. During atracurium neuromuscular blockade, horses were ventilated, and respiratory rate and peak inspiratory airway pressure were maintained within narrow limits. The CSFP and IOP were measured at 3 different levels of PaCO2 (approx 40, 60, and 80 mm of Hg). The PaCO2 sequence in each horse was determined from a type of switchback design with the initial PaCO2 (period 1), established 30 minutes after the commencement of steady-state anesthesia, being repeated in the middle (period 3) and again at the end (period 5) of the experiment. Measurements taken from the middle 3 periods (2, 3, and 4) would form a Latin square design replicated twice. The interval between each period was approximately 45 minutes. Data from periods 2, 3, and 4 indicated that CSFP (P less than 0.05) and mean systemic arterial pressure increased significantly (P less than 0.05) with high PaCO2. Mean central venous pressure, heart rate, and IOP did not change significantly during these same conditions. Measurements taken during periods 1, 3, and 5 were compared to assess the time-related responses to anesthesia and showed a significant increase in CSFP, a significant decrease in mean central venous pressure, and a small (but not statistically significant) increase in mean systemic arterial pressure.  相似文献

The effects of sevoflurane on anesthesia induction, recovery, ventricular pressures, heart rate, ventricular pH, blood gas values, and electrolytes were evaluated in desert tortoises (Gopherus agassizii). Tortoises were orotracheally intubated while awake and ventilated manually with 3-7% sevoflurane in oxygen (1 L/min) to achieve desired expired sevoflurane concentrations. Data, consisting of induction time, recovery time, systolic, diastolic, and mean ventricular pressures, heart rate, ventricular pH, blood gas values, and electrolytes, were collected prior to anesthesia and sequentially at 2.50% and 3.75% expired sevoflurane as measured at the junction of the endotracheal tube and the breathing circuit. Blood pressure was measured and blood samples were collected through a 25-ga needle passed through a cardiac access port that was placed while the tortoises were in dorsal recumbency. Mean (+/-SE) induction time was 2.55+/-0.55 min, recovery time was 27.58+/-7.55 min, and duration of anesthesia was 105+/-12 min. Mean (+/-SD) values for systolic, diastolic, and mean ventricular pressures in awake tortoises were 28+/-3 mm Hg, 22+/-2 mm Hg, and 24+/-2 mm Hg, respectively. Sevoflurane (2.5% expired) significantly decreased systolic (14+/-3 mm Hg), diastolic (12+/-1 mm Hg), and mean (13+/-1 mm Hg) ventricular pressures compared with those of awake tortoises. Ventricular pressures did not decrease further with increasing depth of anesthesia. Heart rate (32+/-4 beats/min) did not change significantly under sevoflurane anesthesia. Sevoflurane administration increased ventricular PO2 but did not change Na+, K+, or iCa++ concentrations. Sevoflurane appears to provide safe and effective anesthesia with rapid induction and recovery.  相似文献

Using two types of infrared anaesthetic analysers (Capnomac, Datex, Finland and Anaesthetic Gas Monitor [AGM] 1304, Bruel & Kjaer, Denmark) simultaneous measurements were made of halothane and methane concentrations in the expired gases from nine anaesthetised horses. Methane concentrations varied from 232 to 1770 ppm. The Capnomac gave halothane readings 0.35 to 4.5 per cent higher than those given by the other analyser. A highly significant linear relationship (r = 0.87) was found between this difference and the concentration of methane in the anaesthetic circuit. This linear relationship was confirmed by an in vitro study using methane-air mixtures containing approximately 500 to 2,500 ppm of methane and analysed in both the halothane and isoflurane measuring modes of the analysers. The AGM 1304 was totally unaffected by methane. We concluded that monitors with absorption in the upper end of the infrared spectrum are preferable for measuring the concentration of inhalation agents in low-flow anaesthetic systems.  相似文献

The frequency of gastro-oesophageal reflux (GOR) in sheep anaesthetized with halothane was reduced by withholding food and water for 24 hours. The total reflux volume increased. The effect of body position on GOR was studied by inclining the operating table at angles to the horizontal and positioning the head up or down on sand bags. The operative positions investigated were: right and left lateral recumbency with head down, dorsal recumbency with head down and right lateral recumbency with head up. Least GOR occurred when the sheep was in right lateral recumbency with a head up tilt and the body inclined at 20° from the horizontal. A cuffed oesophageal drainage tube increased the incidence of GOR but prevented the chances of the aspiration of rumen material.  相似文献

This study evaluated two methods of endotracheal tube selection using 28 fresh canine carcasses of various ages, weights, and genders. The two selection methods were 1) nasal septal width pairing with outer diameter of an endotracheal tube, and 2) digital palpation of the tracheal outer diameter to determine the endotracheal tube size. All dogs were dolichocephalic breeds. Results of this study showed that the canine nasal septal width method of endotracheal tube selection was correlated with the size of the tracheal internal (r=0.72) and outer (r=0.73) diameters. However, evidence shows that the digital palpation method is slightly more effective than the nasal width method in selecting the best-fitting endotracheal tube. The percentage of the best-fit tube selection for the nasal septal width method was 21%, while the digital palpation method was 46%. With these two methods, selecting an endotracheal tube that is too small is possible, especially when the tube internal diameter is > or =7 mm.  相似文献

Epinephrine-induced arrhythmias were studied in 4 cats (group A), using a 4 X 4 Latin square design. Each cat was anesthetized 4 times, 1 week apart, with halothane (1.5% end expired), isoflurane (2.0% end expired), and halothane or isoflurane preceded by ketamine administered IM (8.8 mg/kg). Lead II of the ECG and femoral artery pressure were recorded. Epinephrine was infused in progressively doubled rates (initial rate = 0.125 micrograms/kg/min) for a maximum of 2.5 minutes or until at least 4 ventricular premature depolarizations occurred within 15 s of each other. The arrhythmogenic dose of epinephrine (ADE; micrograms/kg) was calculated as the product of infusion rate and time to arrhythmia. The ADE (means +/- SD) during anesthesia with halothane alone and with ketamine-halothane anesthesia were 1.33 +/- 0.65 and 1.37 +/- 0.59 micrograms/kg, respectively; during anesthesia with isoflurane alone and ketamine-isoflurane anesthesia, the ADE were 9.34 +/- 1.29 and 16.16 +/- 3.63 micrograms/kg, respectively. The ADE was significantly greater (P less than 0.05) during isoflurane anesthesia and ketamine-isoflurane anesthesia than during halothane anesthesia. The percentages of change in systolic blood pressure (means +/- SD) at the ADE during halothane, ketamine-halothane, isoflurane, and ketamine-isoflurane were 31 +/- 34, 41 +/- 17, 127 +/- 27, and 148 +/- 57, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Objective To compare the incidence of arrhythmias in cats receiving either acepromazine or diazepam for pre-anaesthetic medication prior to halothane anaesthesia.
Study design A blinded, randomized clinical study.
Animals Forty-six healthy cats undergoing surgery.
Methods Animals were allocated to one of two groups for pre-anaesthetic medication. Group 1 received diazepam (0.2 mg kg−1). Group 2 received acepromazine (0.02 mg kg−1). The trial drug was administered intramuscularly in combination with buprenorphine (0.01 mg kg−1) 30 minutes prior to induction of anaesthesia with propofol (approximately 5 mg kg−1). Anaesthesia was maintained using halothane: delivered concentration was 1–2% carried in oxygen and nitrous oxide via an endotracheal tube attached to an Ayre's T-piece (with Jackson-Rees modification) breathing system. The incidence of cardiac arrhythmias was determined by continuously monitoring the electrocardiogram from the time of induction until recovery occurred. Demographical group characteristics were compared using analysis of variance. The incidence of cardiac arrhythmias was compared by the Chi squared test. Statistical significance was set at the 5% level.
Results The two groups were similar in weight, age, length and type of procedure undertaken. The incidence of arrhythmias was the same in each group (3/23 cases) ( p = 1.0).
Conclusions The incidence of cardiac arrhythmias in this study did not appear to be influenced by the nature of pre-anaesthetic medication.
Clinical relevance The incidence of cardiac arrhythmias under halothane anaesthesia was 13% in this study. Acepromazine did not appear to exert an anti-arrhythmic effect. This may not be the case in a larger scale study.  相似文献

Quebec Landrace pigs (n = 896) were halothane tested and blood samples were taken for the determination of Phi, Po2 and Pgd phenotypes. The incidence of the halothane positive pigs was 5.3%. The frequencies of the favorable alleles PhiA, Po2S and PdgA were respectively 29.2%, 39.6% and 64.4%. The highest linkage disequilibrium was found between Hal-Phi (0.0606) followed by Hal-Pgd (0.0428) and Hal-Po2 (-0.0308). Alleles PhiB, PgdB and Po2F were associated respectively with 97%, 81% and 74% of Haln haplotypes. It was concluded that a selection in order to increase the favorable marker loci PhiA and PgdA would reduce the Haln frequency in Quebec Landrace pigs.  相似文献

The cardiopulmonary effects of a halothane/oxygen combination were studied in eight cats subjected to a 25% whole blood volume loss. Test parameters included cardiac output measured via thermodilution, heart rate, respiratory rate, arterial blood pressure (systolic, diastolic and mean) and blood gas analysis. Values for cardiac index, stroke volume and systemic vascular resistance were calculated from these data. Posthemorrhage cardiac output, cardiac index, stroke volume and measurements of arterial blood pressure were significantly decreased (p less than 0.05). Heart rate remained unchanged. Following induction of halothane anesthesia the above parameters experienced a further significant decline (p less than 0.05) from their immediate preanesthetic (i.e. posthemorrhage) values. Heart rate also significantly decreased (p less than 0.05). Thirty minutes following the cessation of halothane anesthesia these values returned to near-hemorrhage levels, being above their respective preanesthetic values. Systemic vascular resistance initially rose, peaking ten minutes into halothane anesthesia, before gradually falling to prehemorrhage values at the end of halothane anesthesia. Following hemorrhage, respiratory rate demonstrated a transient increase, associated with an arterial CO2 tension fall, before returning to initial values at the preanesthetic time. During halothane anesthesia respiratory rate remained unchanged whereas arterial CO2 tension rose significantly (p less than 0.05) and pH declined slightly from preanesthetic readings. These returned to prehemorrhage values 30 minutes following the cessation of halothane anesthesia.  相似文献

Computed tomography (CT) is used to document upper airway lesions in dogs with brachycephalic obstructive airway syndrome. The presence of an endotracheal tube during CT scanning is often required for general anesthesia. We hypothesized that the endotracheal tube placement would change the soft tissue dimensions of the upper airway. The aims of this prospective, method comparison study were to evaluate the reliability of the previously reported upper airway CT measurements with endotracheal tube placement, and to propose measurements that are minimally affected by the endotracheal tube. Twenty brachycephalic dogs were included in this study. Each dog underwent head/neck CT with an endotracheal tube, followed by a second scan without the endotracheal tube. Ten measurements of the soft palate, nasopharynx, and trachea were performed. Tracheal dimension was significantly larger with the endotracheal tube compared to without, whereas the soft palate cross‐sectional area was significantly smaller with the endotracheal tube than without the endotracheal tube. The influence of the endotracheal tube on the caudal nasopharynx cross‐sectional (transverse‐sectional) area varied with a mean proportional absolute difference of 35%. Rostral soft palate thickness, tracheal perimeter, and cross‐sectional area of the rostral nasopharynx were the measurements least affected by the endotracheal tube (intraclass correlation coefficient = 0.964, 0.967, and 0.951, respectively). Therefore, we proposed that these three measurements may be most useful for future brachycephalic obstructive airway syndrome studies that require CT scanning of intubated animals. However, with endotracheal tube placement, measurements of soft palate length, caudal nasopharyngeal cross‐sectional area, and trachea height and width may not be reliable.  相似文献
