Images generated during the course of patient evaluation and management are an integral part of the medical record and must be retained according to local regulations. Digital Imaging and Communications in Medicine (DICOM) makes it possible for images from many different imaging modalities to be distributed via a standard internet network to distant viewing workstations and a central archive in an almost seamless fashion. The DICOM standard is a truly universal standard for the dissemination of medical images. Picture Archive and Communication System (PACS) refers to the infrastructure that links modalities, workstations, the image archive, and the medical record information system into an integrated system, allowing for efficient electronic distribution and storage of medical images and access to medical record data. This paper discusses the important elements to a successful PACS implementation in a practice, including how it interacts with other practice computing systems.  相似文献

Digital Imaging and Communication in Medicine (DICOM) is a communication protocol that imaging devices use to communicate. The universal acceptance of the DICOM standard by the major medical vendors means that the digital transition in veterinary medicine should be relatively smooth provided DICOM is used. DICOM service objects, roles, service classes, and conformance standards are discussed. The authors strongly encourage the use of image acquisition software and image archive systems that support the DICOM standard.  相似文献

Ultrasonography has gained increased utility to diagnose pelvic fractures in horses; however, internal pelvic contours can be difficult to appreciate from external palpable landmarks. We developed three‐dimensional (3D) simulations of the pelvic ultrasonographic examination to assist with translation of pelvic contours into two‐dimensional (2D) images. Contiguous 1 mm transverse computed tomography (CT) images were acquired through an equine femur and hemipelvis using a single slice helical scanner. 3D surface models were created using a DICOM reader and imported into a 3D modeling and animation program. The bone models were combined with a purchased 3D horse model and the skin made translucent to visualize pelvic surface contours. 3D models of ultrasound transducers were made from reference photos, and a thin sector shape was created to depict the ultrasound beam. Ultrasonographic examinations were simulated by moving transducers on the skin surface and rectally to produce images of pelvic structures. Camera angles were manipulated to best illustrate the transducer–beam–bone interface. Fractures were created in multiple configurations. Animations were exported as QuickTime movie files for use in presentations coupled with corresponding ultrasound videoclips. 3D models provide a link between ultrasonographic technique and image generation by depicting the interaction of the transducer, ultrasound beam, and structure of interest. The horse model was important to facilitate understanding of the location of pelvic structures relative to the skin surface. While CT acquisition time was brief, manipulation within the 3D software program was time intensive. Results were worthwhile from an instructional standpoint based on user feedback.  相似文献

The conventional anatomical study of specimens requires cutting processes which destruct the limited specimens. A non-destructive method, namely an ultrasonography, can be used to assess the anatomical organ information of those specimens. The aim of this research is to analyse the macroanatomy of the female reproductive organ in the Sunda porcupine (Hystrix javanica), using ultrasonographical imaging. In this study, four formaldehyde-fixed reproductive organ specimens of the Sunda porcupine were used. A 10–12 MHz linear ultrasound transducer was utilized to provide an imaging format of both longitudinal and transversal views. Photographic images were then used as comparison with a sonographic image. The results show that the ultrasound image of the Sunda porcupine reproductive organ soft tissue was hypoechoic, the lumen and antrum follicles were anechoic, while atretic follicles and the mons pubis were hyperechoic. Generally, the size of the organ was not significantly different between photographical and ultrasonographical imaging (p > .05). In conclusion, ultrasound images can be utilized for anatomical studies of the Sunda porcupine reproductive organs without destructing the specimen.  相似文献

Computed tomography (CT) is commonly used in veterinary practice to evaluate dogs with suspected brain disease, however contrast resolution limitations and artifacts may reduce visualization of clinically important anatomic features. The purpose of this study was to develop an optimized CT protocol for evaluating the canine brain. The head of a 5‐year‐old Springer Spaniel with no neurological signs was imaged immediately following euthanasia using a 4‐slice CT scanner and 282 protocols. Each protocol used a fixed tube voltage of 120 kVp and 10 cm display field of view. Other acquisition and reconstruction parameters were varied. For each protocol, four selected images of the brain were reconstructed, anonymized and saved in DICOM format. Three board‐certified veterinary radiologists independently reviewed each of the four images for each protocol and recorded a numerical quality score for each image. The protocol yielding the lowest total numerical score was defined as the optimal protocol. There was overall agreement that the optimal protocol was the one with the following parameters: sequential mode, 300 mAs, 1 mm slice thickness, 1 s tube rotation time, medium image reconstruction algorithm and applied beam hardening correction. Sequential imaging provided optimal image resolution. The thin‐sliced images provided a small blur due to partial volume artifacts. A high tube current resulted in a relatively low noise level. Use of a medium frequency image reconstruction algorithm provided optimal contrast resolution for brain tissue. Use of a proprietary beam hardening correction filter (Posterior Fossa Optimization) markedly reduced beam‐hardening artifact.  相似文献

Ocular photographs are an increasingly important method of documenting lesions for inclusion in medical records, teaching purposes, and research. Availability of affordable high-quality digital imaging equipment has allowed for enhanced capture of desired images because of immediate on-camera viewing and editing. Conversion of the standard digital camera sensor to one that is sensitive to infrared light (i.e. > 760 nm < 1 mm wavelength) can be done inexpensively. In the equine eye, advantages of infrared digital photography include increased contrast of anterior segment images, identification and monitoring of pigmentary changes, and increased visualization of the anterior segment through a cloudy or edematous cornea.  相似文献

Despite the increasing use of magnetic resonance imaging (MRI), ultrasound remains a valuable tool to diagnose injuries that cause distal extremity lameness in the horse. The key to a successful examination is a strong knowledge of anatomy in combination with proper ultrasonographic technique and the patience and dedication to learn these skills. Similar to all imaging modalities, it is equally important to recognize and consider the limitations of ultrasound in this region so that findings can be interpreted appropriately. Ultrasound can be used to diagnose injuries to the deep digital flexor tendon (DDFT), straight distal sesamoidean ligament and branches of the superficial digital flexor tendon using standard pastern ultrasonographic technique. The addition of newer techniques to image the DDFT at the level of P2, the navicular bursa and the collateral sesamoidean ligament can enhance the diagnostic utility of ultrasound in horses with distal extremity lameness. Although visibility is limited, ultrasound can be used to diagnose collateral ligament injuries of the coffin joint in many affected horses. Transcuneal imaging may be useful in some horses to detect abnormalities of the distal sesamoidean impar ligament and navicular bone, but evaluation of the DDFT is limited. Ultrasound should be considered in all horses with distal extremity lameness, regardless of the ability to perform advanced imaging procedures. Information gained is often complementary to other imaging modalities and may provide the basis for recheck examination purposes.  相似文献

Ultrasonography and MRI have become valuable tools for imaging of tendon injuries. The current study examines the histopathologic basis for the imaging abnormalities. Five injured equine forelimbs and two normal contralateral limbs were studied with high resolution real time ultrasound and MRI. Histologic sections were made and correlated with the diagnostic images. All lesions were readily seen by both modalities. Lesions characterized by hemorrhage, edma, and cellular infiltration were sonolucent on ultrasound and bright on MRI images. MRI returned to normal as fibrogenesis ensued. Ultrasound images remained abnormal until fibrillar reallgnment occured with completion of the healing process. High resolution real time ultrasound and MRI both accurately reflect the tissue abnormalities in acute tendon injuries. The injured horse does appear to be an effective model for correlative imaging studies of tendon injuries.  相似文献

The utility of magnetic resonance (MR) imaging in the evaluation of equine solar foot penetrations is well established. The objective of this pilot study was to evaluate the utility of MR imaging in assessment of equine distal limb wounds excluding solar penetrations. Low-field MR images of 23 horses that had previously sustained distal limb wounds were reviewed in consensus by two ECVDI diplomats. Structures (bone; synovial structure; subcutaneous tissue and skin; and ligament/tendon) were identified as normal or abnormal on MR images, radiographs and ultrasound images and reports. All abnormalities were described. The presence of artefacts and their effect on image interpretation were also noted for each modality. Comparisons were made between imaging modalities, and it was noted if MR imaging influenced case management. Abnormalities of the bone were identified in 26% of horses on MR images and 17% of horses on radiographs; there were no osseous abnormalities identified on radiographs that were not identified on MR images, and additional features and better characterisation of lesions were noted on MR images. Tendon/ligament abnormalities were identified in 57% horses on MR and 47% of horses on ultrasound images. Magnetic susceptibility artefacts compromised MR image interpretation in 17% of horses. MR imaging of equine distal limb wounds allowed identification of both osseous and tendon/ligament abnormalities in more cases than either radiography or ultrasonography, and altered case management in 20/23 horses. Although MR imaging should not replace conventional imaging, this study highlights that MR imaging of equine distal limb wounds can yield information not detected on conventional imaging which may direct treatment and affect prognostication.  相似文献

Objective – To describe the different forms of digital radiography (DR), image file formats, supporting equipment and services required for DR, storage of digital images, and teleradiology. Background – Purchasing a DR system is a major investment for a veterinary practice. Types of DR systems include computed radiography, charge coupled devices, and direct or indirect DR. Comparison of workflow for analog and DR is presented. Summary – On the surface, switching to DR involves the purchase of DR acquisition hardware. The X‐ray machine, table and grids used in analog radiography are the same for DR. Realistically, a considerable infrastructure supports the image acquisition hardware. This infrastructure includes monitors, computer workstations, a robust computer network and internet connection, a plan for storage and back up of images, and service contracts. Advantages of DR compared with analog radiography include improved image quality (when used properly), ease of use (more forgiving to the errors of radiographic technique), speed of making a complete study (important for critically ill patients), fewer repeat radiographs, less time looking for imaging studies, less physical storage space, and the ability to easily send images for consultation. Conclusions – With an understanding of the infrastructure requirements, capabilities and limitations of DR, an informed veterinary practice should be better able to make a sound decision about transitioning to DR.  相似文献

The “gold standard” for verification of patient positioning before linear accelerator‐based stereotactic radiation therapy is kilovoltage cone‐beam computed tomography (kV‐CBCT), which is not uniformly available or utilized; planar imaging is sometimes used instead. The primary aim of this study was to determine if the position of the bony skull, when used as a surrogate for isocenter verification, is different when orthogonal megavoltage (MV) portal or kilovoltage (kV/kV) radiographs are used for image guidance, rather than kV‐CBCT. A secondary aim was to determine the influence of intra‐observer variability, body size and skull conformation on positioning, as determined using these three imaging modalities. Dogs and cats receiving radiotherapy of the head were recruited for this prospective analytical study. Planar (MV portal and kV/kV images) and volumetric (kV‐CBCT) images were acquired before treatment, and manually coregistered with reference images. Differences in skull position when matched based on MV portal, kV/kV images and kV‐CBCT were compared. A total of 65 subjects and 148 unique datasets were evaluated. The Wilcoxon rank‐sum test was used to evaluate effects of transitioning between imaging modalities. When comparing magnitude of shifts in MV to kV‐CBCT, MV to kV/kV and kV/kV to kV‐CBCT, there were statistically significant differences. Results were not measurably impacted by body size, skull conformation or interobserver differences. Based on shift magnitude and direction, an isotropic setup margin of at least 1 mm should be incorporated within the planning target volume when MV or kV planar imaging is used for position verification.  相似文献

Endoscopic examination of the upper gastrointestinal (GI) tract in dogs and cats is a relatively recent development. It was just over 20 years ago, in 1978, that the first series report of GI endoscopy in dogs and cats was presented. In those initial days, endoscopy was truly a novelty, and the primary uses for endoscopes were to retrieve foreign objects or visualize the esophagus and stomach. It was not until the mid-1980s that technology and training made gastroduodenoscopy standard practice. The evolution in endoscopic technology has also mirrored the evolution in computers and imaging modalities. The original fiberoptic endoscopes, although still available, are giving way to video endoscopy and digital imaging capabilities that provide spectacular images and an ability to generate hard copy images for medical records, teaching purposes, and research data that was previously not possible.  相似文献

The complete archive of the New Zealand Veterinary Journal went online on 1 January 2003 on SciQuest (www.sciquest.org.nz), an innovative new e-publishing website developed by the New Zealand Veterinary Association (NZVA). This follows hot on the heels of the CD-Rom version released from the same platform in June last year. Subscribers in 2003 will be able to search and retrieve, online, the full text of any article published in the journal since its inception in 1952. To date, few veterinary science journals are able to offer such a complete and comprehensive resource. SciQuest's mission is to provide high-speed access to science and continuing education articles to scientists and veterinarians in a fully indexed and searchable format, both online (via the internet) and offline (via CD-Rom). Searching and browsing the archive online is free and abstracts are available for all articles published from 1972 onwards. Access to full text, which is presented in PDF format exactly as published, is restricted to journal subscribers. The site will be updated with each issue of the journal as it is published. The online format will largely suit institutional users and casual enquiries, whereas the offline version is designed to be loaded onto computer hard drives for immediate access, any-time, and is better suited to frequent users and veterinary practitioners. A major advantage of the offline resource is its speed and immediacy, returning search results and full text much faster than the online version, without the need for internet access. The search engine has been designed to suit specific problem-oriented enquiries, and great effort has been expended to ensure the archive is both complete and accurate. The same search engine and interface has been provided on both versions, which allows for fast and highly specific searching of titles and full text of all articles for the occurrence of any word or combinations of words, as well as indexes of author, volume, year, article type, animal type and subject. Search results can be instantly sorted by author and date of publication, and powerful secondary search features such as 'by the same author', 'in the same issue' and 'same subject' links add power and flexibility. The offline version will be updated with annual releases and is available for purchase to subscribers and members of the NZVA.  相似文献

Harmonic ultrasound is a technique based on the principle of transmitting at frequency f and receiving at frequency 2f (or 1/2f). This technology has become available through the development of wide-bandwidth transducers. Microbubble contrast media produce a large amount of harmonic signal. Contrast harmonic ultrasound provides the opportunity to image patterns of high flow vasculature and overall perfusion. Regions of poor perfusion, including necrosis or infarction, can be identified with contrast harmonic ultrasound. While proportionately lower, tissues also produce harmonic signals. Tissue harmonic ultrasound sequences often improve subjective image quality compared to fundamental ultrasound in echocardiographic and abdominal examinations. This review will discuss the physical principles of harmonic ultrasound signal generation, medical and animal research applications, and an overview of current veterinary experiences.  相似文献

Scatter radiation is a source of noise and results in decreased signal-to-noise ratio and thus decreased image quality in digital radiography. We determined subjectively whether a digitally processed image made without a grid would be of similar quality to an image made with a grid but without image processing. Additionally the effects of exposure dose and of a using a grid with digital radiography on overall image quality were studied. Thoracic and abdominal radiographs of five dogs of various sizes were made. Four acquisition techniques were included (1) with a grid, standard exposure dose, digital image processing; (2) without a grid, standard exposure dose, digital image processing; (3) without a grid, half the exposure dose, digital image processing; and (4) with a grid, standard exposure dose, no digital image processing (to mimic a film-screen radiograph). Full-size radiographs as well as magnified images of specific anatomic regions were generated. Nine reviewers rated the overall image quality subjectively using a five-point scale. All digitally processed radiographs had higher overall scores than nondigitally processed radiographs regardless of patient size, exposure dose, or use of a grid. The images made at half the exposure dose had a slightly lower quality than those made at full dose, but this was only statistically significant in magnified images. Using a grid with digital image processing led to a slight but statistically significant increase in overall quality when compared with digitally processed images made without a grid but whether this increase in quality is clinically significant is unknown.  相似文献

In order to determine the effect of different image compounding functions on perceived image quality, 84 pairs of ultrasound images were collected, mixed, and reviewed by four independent observers who were asked to identify the highest quality image of each pair. Each image in a pair was made using the same orientation, transducer, frequency, and gain settings but different compounding settings. Outcomes were analyzed using logistic regression. Observers judged compound images to be better quality than noncompound images in 69% cases, the same quality in 24%, and poorer quality in 7%. Overall, compound images were considered significantly better quality than noncompound images (P < 0.001). Compound images were more likely to be considered better quality than the corresponding noncompound images when combined transmit/receive spatial compounding was used rather than receive-only spatial compounding or transmit compounding, and when the vector transducer or the curvilinear transducer were used rather than the linear transducer. Observers considered improved border definition and increased signal to noise ratio to be the properties that accounted most often for higher quality of compound images compared with noncompound images.  相似文献

One large study in medical education has reported that the choice of question format (or question content) could introduce a gender bias, with men outperforming women on questions with a true-false component or that required knowledge of anatomy or physiology. The purpose of our study was to ascertain whether this finding is also true in veterinary medical education. Two veterinary student cohorts were analyzed across four different modules over a three-year period (804 questions in total). The results of the study show that the women's and men's performance did not differ in any of the question types analyzed across any module or year. When students' (both women and men) overall average performance on different question types was compared with their performance on standard multiple-choice questions (MCQs), performance levels increased when students were asked to answer MCQs that contained an image-based prop (IMCQ) such as a photograph, X-ray image, or diagram. In contrast, students' performance was consistently lower when answering assertion-reason questions (ARQs), and this performance could not be explained by the demographic makeup of the two cohorts analyzed. When comparing standard MCQs with MCQs that contained a true-false question stem, no specific trend in the data could be determined. In conclusion, this study suggests that the short-answer question style does not bias against one gender in veterinary medical education, but that overall students do perform differently according to question type and, in particular, less well when ARQs are used in examinations.  相似文献
