The case history, radiographic and scintigraphic findings of two horses with pelvic limb navicular bone fractures are presented. In both cases the fractures were of traumatic origin. One horse had a bilateral fracture of the navicular bone, distal border, the other horse had a fracture of the proximal articular border in one pelvic limb navicular bone.  相似文献

Radiography was used to evaluate 32 horses presented for puncture wounds into the navicular bursa. At the initial radiographic examination, 21 horses had no radiographic signs of osteomyelitis, while 11 did. The mean-time interval between injury and radiographic examination was 20.6 days for horses without evidence of osteomyelitis and 59.3 days for those with evidence of osteomyelitis. Radiographic signs of osteomyelitis of the navicular bone included flexor cortical destruction and irregularity of the flexor surface. Other abnormalities included pathologic fracture of the navicular bone, subluxation of the distal interphalangeal joint, or secondary joint disease. The lateromedial and palmaroproximal-palmarodistal oblique projections were most useful for identifying abnormalities of the navicular bone. There was a high correlation between horses that had positive presurgical radiographic signs and surgical findings. However, approximately 50% of horses that had negative radiographic findings initially had positive surgical findings. Ten of 21 horses with negative radiographic findings on initial examination had evidence of bony lesions when reevaluated from 3 weeks to 6 years later. Nine of 11 horses with radiographic signs of osteomyelitis on initial radiographic examination were euthanatized or had an unsatisfactory outcome. Seven of 10 horses with radiographic signs on reexamination were euthanatized. Thirteen of 20 horses with positive surgical findings for navicular bone infection were euthanatized or had an unsatisfactory outcome.  相似文献

Computed tomography (CT) was performed on 12 Finnhorse cadaver forefeet with known radiographic changes in the navicular bone (poor corticomedullary junction, irregular appearance of the flexor central eminence, uneven or unequal thickness of the flexor cortex, and/or irregular outline of the proximal or distal flexor margin). The purpose was to confirm the radiographic findings and to investigate if further information of the flexor aspect of the bone could be gained with CT. In CT, the midsagittal outline as well as the internal structure of the bones varied greatly. Different combinations of trabecular and compact bone were seen within the flexor central eminence. Lucencies within the compact bone were commonly present in the proximal half of the eminence, but in five bones lucencies were also identified in the distal half. Due to partial overlapping of the bone and varying bony composition of the eminence, accurate radiographic evaluation of the shape and internal structure of the flexor central eminence was often found to be difficult. The flexor cortex usually appeared to be thinner in CT than in conventional radiographs. Medullary sclerosis and poor flexor corticomedullary junction were commonly overinterpreted radiographically. New bone formation on the proximal flexor margin of the navicular bone was generally visualized in radiographs, but CT allowed also the evaluation of the internal structure of the bone. In one navicular bone, an avulsion fragment on the distal flexor margin was seen in CT images; radiographically this fragment could not be visualized. It was concluded that the flexor aspect of the navicular bone may be difficult to assess reliably with conventional radiography.  相似文献

A macroscopic, arteriographic and histological study of the development and the arterial anatomy of the navicular bone of 33 foetuses and 55 young horses is described. After 125 days of gestation the blood supply consists of two routes: one situated in the superficial layer of the fibrocartilage and the other similar to the blood supply of the navicular bone of the normal mature horse. After 270 days gestation, the blood vessels in the fibrocartilage gradually regressed and retracted until they have disappeared at six months after birth. At two months after birth the first macroscopic thinning of the fibrocartilage was noticed. From seven months to one year about 45 per cent of the navicular bones showed a slight thinning of the fibrocartilage. A positive correlation was found between radiographic abnormalities (ie enlargement of the nutrient foramina) and the frequency of thinning of the fibrocartilage. Radiographic abnormalities were first recognised 14 days after birth, whereas the arteriogram showed the first changes such as fewer or no arteries entering distally at the distal extremities at 10 weeks after birth. At four weeks after birth the first arterial wall changes were found, ie intimal thickening with or without splitting of the internal elastic membrane. From that age onward, the number of navicular bones with arterial wall changes gradually increased. Starting at five months after birth only 6 to 20 per cent of the arteries in the navicular bones without radiographic abnormalities showed arterial wall changes. However, the navicular bones with radiographic abnormalities showed arterial wall changes in 25 to 80 per cent of the arteries.  相似文献

An 8-month-old thoroughbred colt presented with sudden onset right forelimb lameness. A radiographic series of the right carpus was performed, and it revealed a slab fracture of the fourth carpal bone and fracture of the proximal part of the third metacarpal bone. Arthroscopically guided repair of the slab fracture of the fourth carpal bone with a 3.5 mm cortex screw and lag screw fixation of the fracture of the proximal part of the third metacarpal bone were performed. The horse started to race at 32 months old and started in 65 races over three years without any trouble associated with the right carpus.  相似文献

A collection of 69 isolated navicular bones, normal or affected with navicular disease, was examined radiographically using dorsopalmar, lateromedial and palmaroproximal-palmarodistal views. Radiographic findings on each view were recorded. A radiographic diagnosis of normality or navicular disease was made, first on each view separately and afterwards based on the combination of the 3 views. Forty-four navicular bones were considered radiographically normal and 25 navicular bones had distinct signs of radiographic navicular disease. The dorsopalmar and the lateromedial views of each navicular bone were assessed together and compared with the palmaroproximal-palmarodistal view. In not a single bone was the palmaroproximal-palmarodistal view considered indispensable to make the final diagnosis.  相似文献

RADIOGRAPHIC APPEARANCE OF THE NAVICULAR BONE IN SOUND HORSES   总被引:2,自引:0,他引:2  
Radiographs of the navicular bone in 523 sound horses were reviewed. Detailed evaluation criteria were used. The incidence of radiographic changes and variations in normal horses were recorded. Results were tested for right-left limb difference and for age relationship. Variation in shape and bone structure was commonly seen in the navicular bone. Radiographic changes with an incidence of less than 2% included flexor cortex defects and calcification on the flexor surface. Fragments at the distal navicular bone border, calcification in the impar ligament and enthesiophytes at the proximal border were radiographic findings with an incidence of 2% to 10%. Abnormal canals at the distal border were found in 11% horses. Elongation of the lateral proximal extremity was commonly found. The mean width of the flexor cortex was 3.6 mm.  相似文献

The clinical, radiographic, arteriographic, scintigraphic and histological effects of experimental occlusion of the Ramus navicularis (R. Navicularis) and its branching arteries are evaluated. Occlusion of the R. navicularis and its branching arteries creates changes, arteriographically and histologically, which resemble those of navicular disease. The increased bone remodelling, the shift in arterial pattern, the formation of collaterals and the increased connective tissue in the synovial membrane and nutrient foramina, as a reaction to the reduction of the distal arterial supply of the navicular bone, are also pathological features in the navicular bones of horses with navicular disease. The results of the present study support the theory that a reduced distal arterial supply of the navicular bone is important in the pathogenesis of navicular disease.  相似文献

Radiographic measurements from the lateromedial projection of the equine foot were compared in three groups of horses. Group 1 consisted of 143 normal horses, group 2 were 60 horses with clinical navicular disease and group 3 were 161 horses with clinical and radiographic navicular disease. Several measurements tended to be larger in group 3 than group 1. An enlargement of the navicular bone was observed in proximodistal and dorsopalmar directions. Partial enlargement of the pedal bone was observed in groups 2 and 3. Few differences were observed between age classes. All horses aged four years and over had an increased length of the hoof in the dorosopalmar direction and a decrease of the cranial angle of the hoof. Enlargement of the navicular bone fits well into the concept of osteoarthrosis. The pedal bone was partly engaged. These findings may be an expression of a regional acceleratory phenomenon.  相似文献

Five cats with Y-T fractures of the humeral condyle were reviewed. Breeds presented included domestic shorthair (four cats) and Maine Coon (one cat). Age ranged from two to 16 years. All the cats were neutered males. A road traffic accident was suspected in all cases. Four of the fractures were severely comminuted and one fracture had four fragments. The fractures were repaired via combined medial and lateral approaches. Fixation of the epicondylar ridges was performed using buttress plates in four cases and neutralisation plates in one case. The intracondylar fracture was stabilised using a 2.7 mm lag screw in four cases and a 3.5 mm lag screw in the other. A corticocancellous bone graft was applied in two cases. The intracondylar fracture was accurately reduced in all cases. Complete radiographic healing was documented in two cases 6 and 11 weeks following surgery. Failure of the fixation occurred in the most severely comminuted fracture five days postoperatively. Surgical revision was not performed, and the limb was amputated. Three cats were free of lameness and had resumed outdoor activities at follow-up (five to eight months after surgery). Marked lameness due to loss of elbow movement was observed in the other case.  相似文献

The aim of this study was to monitor the postnatal radiographic development of the proximal and distal double contours and the modelling of the shape of the proximal articular border. In mature horses, the proximal and distal contours of the navicular bone on dorsopalmar dorsoproximal-palmarodistal oblique (upright pedal) radiographs are commonly visualised as 2 lines, one being the articular border and the second representing the border of the cortex facing the deep digital flexor tendon (flexor border). The shape of the proximal articular border may be concave, undulating, straight or convex in the mature animal. These shapes have been found to be hereditary and to constitute a predisposing factor in the pathogenesis of navicular disease. This predisposing role may result from a shape dependent distribution of the biomechanical forces exerted on this region. There is no agreement in the literature with respect to the moment when the navicular bone takes its mature radiographic appearance. Upright pedal radiographs of the left front foot of 19 Dutch Warmblood foals were made at age 1 month and subsequently at intervals of 4 weeks, until the age of 11 months. The distal double contour developed soon after birth and the radiographic visibility of the articular border improved from ill-defined at 1 or 2 months to clear manifestation at 3 or 4 months. The proximal double contour developed later. The articular border became usually visible at age 3 or 4 months and was clearly visible from age 9 months. The mature shape of the proximal articular border usually became recognisable from age 7 months and was always obvious between 9 and 11 months. This development was associated with a gradual modelling of the lateral and medial extremities of the navicular bone. It was concluded that the navicular bone adopts its mature radiological appearance during the first year postpartum. Considering this early manifestation of the mature shape of the proximal articular border and its previously demonstrated inheritance, a force-dependent development of this shape, as predicted by the trajectional theory/Wolffs law, is improbable. The predisposing role of this shape in the pathogenesis of navicular disease may therefore be explained by a shape-dependent distribution of the biomechanical forces exerted on the navicular bone. Considering the potential application of these findings, from age 1 year shape determination enables identification of the individual and breed susceptibility for the development of navicular disease.  相似文献

Our objective in this study was to examine the effect of gelatin hydrogel (GH) sheets containing basic fibroblast growth factor (bFGF) on healing of proximal sesamoid bone transverse fractures in the horse. Ten healthy adult Thoroughbreds were used. The lateral proximal sesamoid bone of the left forelimb and the medial proximal sesamoid bone of the right forelimb were osteotomized, while the horses were under general anesthesia, and subsequently repaired by lag screw fixation using a single 4.5-mm cortical screw. A GH sheet containing 100 μg of bFGF was then sutured to the synovial membrane adjacent to the osteotomized proximal sesamoid bone. In the control group, the fracture was fixed with a lag screw, and the articular capsule was sutured. Fracture healing was assessed by radiographic examination once a week for 16 weeks after the operation. Radiographic examination of bone healing revealed significantly lower demineralization of the fracture line in the GH sheet-treated group than in the control group. The rate of demineralization of the fracture line in the GH sheet-treated group was significantly lower than that in the control group at 2, 4, and 8 weeks after the operation. In this study, we demonstrated that the use of a GH sheet containing bFGF promotes healing of proximal sesamoid bone fracture in the horse. Therefore, it is believed that this treatment strategy would be useful for quick recovery from bone fracture in the horse.  相似文献

Dual-energy x-ray absorptiometry was used to measure bone mineral density of four regions in healed femora of nine dogs after fracture fixation with a leg-lengthening plate. Six to 85 months (mean, 46 months) after surgery, the bone mineral density of healed femora was not significantly different from the contralateral uninjured femora ( P >.05; power = 0.8 at Δ= 15%). Radio-lucencies around the proximal screws, apparently associated with screw loosening, were seen on radiographic views of the healed femora of three dogs. In one of these dogs, one screw in the proximal metaphysis had broken. Force-plate analysis of gait was also performed on dogs at the time of bone mineral density measurement. Peak vertical force was decreased in the pelvic limb with the healed fracture compared with the contralateral unoperated limb ( P < 0.05). Clinically apparent lameness in three dogs did not appear to be associated with altered bone mineral density and may have been caused by hip osteoarthritis, a nondisplaced hairline diaphyseal fracture, and screw loosening in conjunction with extensive post-traumatic soft tissue injury.  相似文献

Reasons for performing study: Criteria for the radiographic evaluation of navicular bones in horses have been published to standardise classification of radiographic signs. However, intra‐ and interobserver agreement have not been established. Objective: To determine intra‐ and interobserver agreement in the evaluation of radiographic and computed tomographic (CT) navicular changes. It was hypothesised that: 1) intraobserver agreement would be better than interobserver agreement; 2) agreement would be better for CT than for radiography; and 3) pathological changes would be recognised with greater certainty with CT. Methods: Radiographs and CT scans of 60 cadaver navicular bones were evaluated by 3 observers using published criteria. A subset of 30 studies was evaluated twice by one observer. Agreement was tested using the kappa statistic. Certainty about pathological changes was evaluated by giving the observers the option to choose ‘not sure’. Results: Agreement varied from poor to almost perfect for radiographic evaluation and from poor to substantial for CT evaluation. For radiographic evaluation mean interobserver agreement was fair, as it was for CT evaluation. For radiographic evaluation mean intraobserver agreement was moderate as it was for CT evaluation. Pathological changes were evaluated with greater certainty on CT scans compared to radiographs; however, this was not associated with improved agreement. Conclusions: Variations in classification of navicular lesions in radiographic and CT studies were considerable between and within observers and challenge the use of such studies for diagnostic and prognostic purposes. Potential relevance: The results of this study allowed the identification of evaluation criteria with sufficient precision to be useful for navicular bone evaluation.  相似文献

REASONS FOR PERFORMING STUDY: Prepurchase examinations of horses are used increasingly as a means of evaluating future soundness. Data is lacking in the literature of the radiographic findings and results of the lameness examination of comprehensive prepurchase examinations. OBJECTIVE: To summarise the clinical and radiographic findings of prepurchase examinations and determine if radiographic findings correlated with the lameness examination and sale outcome. METHODS: Records of 510 cases were reviewed, radiographs evaluated and grades assigned the navicular bone, distal phalanx, and tarsus. Follow-up information on the horse status was obtained by telephone interviews for 173 horses. RESULTS: Thoroughbred geldings represented the most common breed and sex, mean age 8 years, mean asking price 12,439.40 dollars, and 52.8% were lame. Radiography was the most common diagnostic procedure performed (61.6%), with views of the front feet requested most often (86.6%) followed by the tarsi (68.1%). Grade 1 was most common for the navicular bone while Grade 2 predominated for the distal phalanx. The number of sound horses decreased as grades became more severe. For the tarsi, Grades 0 and 1 were most common for the proximal intertarsal and distal intertarsal/metatarsal joints, respectively. Horses with significant tarsal changes were still able to compete at their expected level. With respect to the radiographic examination, the mean +/- s.d. grade of the horses which were not lame at follow-up was 1.2 +/- 0.9 for the navicular bone and 15 +/- 0.8 for the third phalanx. The mean +/- s.d. grade of sound horses for the distal intertarsal joint was 0.7 +/- 0.6 and 1.14 +/- 0.8 for the tarsometatarsal joint. Horses for which owner follow-up was available and which had a Grade 3 score were also evaluated. For the navicular bone, 17/31 with a Grade 3 remained in active use at follow-up and for the distal phalanx 21/27 were in active use. For the distal intertarsal and tarsometatarsal joints, 20/21 with a Grade 3 were still in active use. CONCLUSIONS: Prepurchase examinations can have a significant effect on the outcome of the sale. For the navicular bone and distal phalanx, higher grades were associated with lameness. In contrast, higher grades in the tarsus were less likely to be associated with lameness. Warmbloods tended to have more extensive changes in the navicular bone and distal phalanx relative to Thoroughbreds but were not as lame. POTENTIAL RELEVANCE: Radiographic changes detected in the navicular bone, distal phalanx and tarsus should be interpreted with consideration to the clinical examination.  相似文献

Horses with navicular disease have an increased load on the navicular bone in early stance. This has been suggested to be a response to pain in the heel region. Seven horses with clinical, radiographic and scintigraphic signs of navicular disease underwent forceplate and kinematic analysis before and after desensitisation of the heel region with a bilateral palmar digital nerve block. The compressive force exerted on the navicular bone during stance, and stride kinematics, were determined in each state. After regional analgesia of the palmar digital nerves (PDNB) the compressive force on the navicular bone was lower throughout stance. The mean +/- s.d. peak force at the beginning of stance was 7.05+/-1.10 N/kg before, and 6.46+/-1.15 N/kg after PDNB (P = 0.01) and at the end of stance the mean peak values were 5.00+/-2.05 N/kg before, and 4.39+/-1.65 N/kg after PDNB (P = 0.05). We explained this finding as indicating that the horse responds to heel pain (including pain in the navicular region) by contracting the deep digital flexor muscle to unload the heels. This increases the compressive load on the navicular bone, which may cause remodelling and, in some horses, damage to the overlying flexor cartilage, which is then painful and identified as navicular disease. This mechanism identifies navicular disease as a possible end point for a variety of heel related conditions.  相似文献

Reasons for performing study: The significance of distal border fragments of the navicular bone is not well understood. There are also no objective data about changes in thickness and proximal/distal extension of the palmar cortex of the navicular bone. Objectives: To describe the distribution of distal border fragments and their association with other radiological abnormalities of the navicular bone and describe the shape of the navicular bone in sound horses and horses with foot‐related lameness, including navicular pathology. Methods: Sound horses had radiographs acquired as part of a prepurchase examination. Lame horses had forelimb lameness abolished by palmar nerve blocks performed at the base of the proximal sesamoid bones. Diagnosis was assigned prospectively based on results of local analgesia and all imaging findings. The thickness of the palmar cortex of the navicular bone and size of proximal/distal extensions were measured objectively. Other radiological abnormalities were evaluated subjectively and each navicular bone graded. Results: Fifty‐five sound and 377 lame horses were included. All measurements were larger in lame compared with sound horses except the size of the distal extension of the palmar cortex. Fragments were observed in 3.6 and 8.7% of sound and lame horses respectively and in 24.1% of horses with a diagnosis of primary navicular pathology. There was an association between fragments and overall navicular bone grade, radiolucent areas at the angles of the distal border of the navicular bone and number and size of the synovial invaginations. Conclusions and potential relevance: The palmar cortex of the navicular bone was thicker in lame compared with sound horses. Distal border fragments were most frequent in horses with navicular pathology. Evaluation of changes in shape of the navicular bone may also be important for recognition of pathological abnormalities of the bone.  相似文献

The medical records of 38 horses with puncture wounds of the navicular bursa were reviewed. Only 12 horses had a satisfactory outcome (breeding or riding). Of the remaining 26 horses, 19 were euthanized, five were sold due to persistent severe lameness, one died, and one was lost to long-term follow-up. Different combinations of conservative management prior to surgical debridement and drainage of the navicular bursa were unsuccessful in resolving the condition. Horses that were treated surgically within 1 week of the injury and had a hind leg affected had the best chance of a satisfactory outcome. Additional wound debridement was necessary in 15 horses after initial surgical treatment. The most common complications encountered were navicular bone osteomyelitis and sepsis of the deep digital flexor tendon. Thirteen of 14 horses that had rupture of the deep digital flexor tendon and subluxation of the distal interphalangeal joint had an unsatisfactory outcome. One mare subsequently developed ankylosis of the distal interphalangeal joint and was a useful brood mare. Two horses that had biaxial palmar digital neurectomy because of persistent lameness were later euthanized because of navicular bone fracture and rupture of the deep digital flexor tendon. Results from limited numbers of bacterial cultures and antibiotic sensitivities suggest that penicillin and an aminoglycoside antibiotic should be used as initial antibiotic therapy. Immediate surgical debridement and appropriate antibiotic treatment are recommended as the minimum therapy for penetrating wounds of the navicular bursa in horses.  相似文献

Evaluation of foot pain in the standing horse by magnetic resonance imaging   总被引:1,自引:0,他引:1  
The records of 41 horses with previously undiagnosed foot pain that had been examined by standing magnetic resonance imaging were reviewed and follow-up information was obtained from their owners two years after the examination. A range of soft tissue and osseous abnormalities were identified, with multiple lesions frequently occurring. Deep digital flexor tendonitis was recorded in 12 of the horses, and distension of the distal interphalangeal joint was identified in 15, but it was not always associated with lameness; in contrast, distension of the navicular bursa was always associated with lameness in the seven affected horses. Navicular bone lesions were identified in 13 of the horses, often in the absence of radiographic changes. Follow-up information was obtained for 35 of the horses, 27 of which were alive; of these, 16 had returned to their previous level of performance. Of the five horses with navicular bursal changes, four had been euthanased owing to lameness and the other had returned to work at a reduced level.  相似文献
