Two kittens aged between four and five months were presented having sustained patellar fractures. In both cases, healing was subsequently documented radiographically; this has not been reported previously in the literature. One kitten had bilateral patellar fractures - the symptomatic right stifle was treated with a pin and tension-band-wire which later failed, at which point partial patellectomy was performed. The fracture of the left patella was minimally displaced and was treated conservatively. A radiograph of the left patella taken eleven months after initial presentation showed complete healing of the fracture. The second case was treated surgically with a circumferential wire; healing of the fracture was demonstrated radiographically at twelve weeks postoperatively. Radiographic images taken five weeks postoperatively had shown some narrowing of the fracture gap. These two cases demonstrate that bony union of patellar fractures can be documented, given a long enough duration of radiographic follow-up; circumferential wire was an effective treatment in a displaced fracture, and conservative treatment resulted in complete healing of a minimally displaced fracture.  相似文献

A sagittal fracture of the medial aspect of the patella was identified in 15 horses, 2 of which had been kicked and 12 of which had hit a fixed fence while jumping. Eight horses showed concurrent fragmentation of the base of the patella, and 2 had sustained a concurrent fracture of the distal end of the lateral trochlear ridge of the femur. A cranioproximal-craniodistal oblique radiographic view was essential to identify the site of the medial patellar fracture and to determine its configuration. Fourteen horses were treated by surgical removal of the medial patellar fracture fragment(s). Of 12 horses (83%) without evidence of pre-existing degenerative joint disease, 10 were treated successfully with return to full athletic function.  相似文献

A 1-year-old athletic golden retriever dog was presented for a non-weight-bearing right pelvic limb lameness that occurred during a canicross race. Orthopedic examination revealed pain and inflammation of the right stifle joint. Radiographs showed a traumatic comminuted patellar fracture. Surgery was elected and a midsubstance patellar ligament rupture was also identified. Surgical management consisted of a combination of techniques for patellar fracture fixation and primary patellar ligament reconstruction. All orthopedic implants were removed secondary to implant failure. Postoperative 20-month radiographic follow-up revealed patella alta and non-union of the fracture. However, an excellent clinical outcome was achieved with a complete return to a high activity level.Key clinical message:Surgical stabilization of patellar fractures is usually a significant challenge. A return to apparent normal limb function was achieved in this dog suffering complex patellar fracture with concomitant patellar ligament rupture despite postoperative fracture non-union.  相似文献

This report describes the clinical experience with internal fixation of a sagittal and a medial parasagittal patellar fracture using lag screws. Severely displaced sagittal and medial parasagittal fractures of the patella were diagnosed in 2 horses (a 10‐year‐old Thoroughbred gelding and a 5‐year‐old Thoroughbred mare) and were repaired using internal fixation. A direct approach through the fascia latae was used. In both cases, the leg was placed in full extension and the fracture was corrected by a combination of digital manipulation and application of bone reduction forceps. Articular congruency was assured by arthrotomy and digital assessment, and radiographic skyline projection. Then, the leg was placed in flexion and two 5.5 mm cortical screws with washers were inserted in lag fashion, under radiographic guidance (skyline projection). In one case, an additional 6.5 mm cancellous screw was used. Both fractures healed without evidence of degenerative joint disease. However, the first horse's wound dehisced, then healed by second intention uneventfully. The second horse was put in a sling in order to prevent lying down. Both horses were able to be used for their intended purpose. In these clinical cases, sagittal and medial parasagittal patellar fractures were treated successfully by open reduction and internal fixation. Post‐operative management in a sling was used successfully in the second case to prevent wound dehiscence.  相似文献

Three immature cats with growth abnormalities of the distal radius secondary to trauma were presented between July 2001 and July 2003. Initial injuries included a Salter-Harris type II fracture of the distal radial physis in one cat, fractures of the styloid process of the ulna in two cats, and metacarpal fractures in one cat. The cats were between 3.5 and 4.5 months of age at the time of injury. Damage to the distal radial physis resulted in a variable degree of shortening of the radius in all cats. Asymmetrical closure caused widening of the lateral aspect of the radiocarpal joint space in two cats. Elbow incongruity was not observed. One cat was treated with osteotomies of the radius and ulna and realignment of the radius, stabilised with external skeletal fixation. Another was treated by ulnar ostectomy alone. Radius and ulna osteotomies and distraction osteogenesis of the radius were performed in the third cat. Clinical outcome was satisfactory in all three cases.  相似文献

A mature female domestic shorthaired cat with obstipation as a result of pelvic canal narrowing due to a pelvic fracture malunion was treated by triple pelvic osteotomy to widen the canal. The right hemipelvis was moved laterally and a modified bone plate was applied to the right ilium. A mild lick dermatitis occurred over the dorsum of the hind paw seven days postoperatively but resolved without treatment. The cat made an uneventful recovery without recurrence of the obstipation.  相似文献

Five cats with fracture of the talar neck were treated using external skeletal fixation (ESF). In four of the cases, ESF was the only fixation technique used and this was applied in a closed manner coupled with traction. In the remaining cat, ESF was used in combination with a lag screw. All the fractures eventually healed, demonstrating that ESF provided a simple and effective method of treatment for these cases.  相似文献

A condition characterised by osteochondral fragmentation of the distal aspect of the patella in 15 horses is described. The problem was unilateral in six horses and bilateral in nine. There were eight Quarterhorses, three Thoroughbreds, two American Saddlebreds, one American Paint and one Warmblood-Thoroughbred cross. A previous medial patellar desmotomy had been performed on 12 of the 15 horses. The condition manifested as hindlimb lameness and stiffness ranging from mild to severe. There was fibrous thickening in the stifle area in the 12 cases with a previous medial patellar desmotomy, and synovial effusion in seven of 12 cases. Synovial effusion was present in two of the three cases in which a previous medial patellar desmotomy was not performed. The radiographic changes included bony fragmentation, spurring (with or without an associated subchondral defect), subchondral roughening and subchondral lysis of the distal aspect of the patella. All horses were treated with arthroscopic surgery. The lesions at arthroscopy varied from flaking, fissuring, undermining or fragmentation of the articular cartilage to fragmentation or lysis of the bone at the distal aspect of the patella. The subchondral bone was involved in all cases that had a previous medial patellar desmotomy. Of the 12 horses that had a previous medial patellar desmotomy, eight became sound at their intended use, one was sold in training without problems, one is in early training without problems, one never improved and one is in convalescence. Of the three that did not have a patellar desmotomy, two performed their intended use well but one was unsatisfactory.  相似文献

A survey of 108 bone fractures in the cat was made over a 1 year period in a city practice in south west England. Seventy-three per cent involved the pelvic limb. The three bones most commonly affected were femur (38%), pelvis/sacrum (22%) and mandible (16%). Forty-five cases were the result of road accidents and 75% of cats were 2 years old or less.
Treatment of femoral shaft fractures by intramedullary pinning gave good results. The majority of proximal epiphyseal separations and femoral neck fractures responded well to conservative treatment. 'Cage rest' was used successfully to treat severely comminuted femoral shaft fractures. All cases of pelvic fracture were treated conservatively (up to 3 weeks 'cage rest'). Separation of the mandibular symphysis and fracture of the horizontal ramus healed readily. Repair of tibial (10%) and humeral (5%) fractures was without complications.  相似文献

A 5-year-old Birman cat presented with paraparesis associated with a fracture of the third thoracic vertebra and was managed conservatively. Voluntary function was regained over the next 6 months, but the cat was referred 4 years after the trauma because of recurrence of hindlimb paresis and ataxia. Magnetic resonance imaging (MRI) showed a spinal arachnoid cyst at the level of the fourth thoracic vertebra, which was treated surgically by dorsal laminectomy and durectomy, and hindlimb function subsequently recovered. The cat re-presented 4 years later for recurrent hindlimb paresis. Myelography and computed tomography–myelography (CT-M) showed dorsolateral pooling of cerebrospinal fluid (CSF) at the previous laminectomy site. The neurological signs after the second surgery improved, but not as much as after the first surgery and the cat remained ataxic with moderate paresis. Seven months later repeat CT-M revealed an atrophic spinal cord, but negligible pooling of CSF at the previous site of the arachnoid cyst. The cat could walk, but was being treated for self-induced trauma of the left hindlimb that was thought to be related to paraesthesia. The neurological signs gradually deteriorated over 3 months and the cat was euthanased.  相似文献

This case report describes the treatment of recurrent medial patellar luxation associated with varus deformity of the distal femur by distal femoral ostectomy stabilised using a distal femoral plate. Four dogs (five affected limbs) were included in the study. All cases had received previous surgery for medial patellar luxation but remained significantly lame. All were treated by femoral ostectomy to correct distal femoral varus deformity with application of a distal femoral bone plate. All cases improved following surgery. An excellent outcome was recorded in four of five cases. Lameness persisted in one case despite satisfactory patellar stability as a result of concurrent cranial cruciate ligament deficiency. One case suffered implant failure. Femoral ostectomy is an effective treatment for medial patellar luxation associated with femoral varus deformity. The distal femoral plate provides a convenient method by which to stabilise the ostectomy. Care should be exercised when the 2 mm distal femoral plate is applied.  相似文献

Three cases of fibroepithelial hyperplasia (FEH) of the mammary gland in the cat are reported. A one year old female cat had a distinct enlargement of the middle mammary glands, one on each side, 5 days after the first estrus. One week later the cat was treated with medroxyprogesterone acetate (Depo-Promone). The affected glands, along with the remaining glands, increased further in size. A five year old female cat was treated with Proligeston (Covinan) for the suppression of estrus. Two weeks later fibroepithelial hyperplasia occurred in two glands, one with a well demarcated ulceration. A seven months old male cat was treated with delmadinon acetate (Tarden) because of urine spraying. Two months later he had enlargement of all mammary glands. All three cats were treated with the progesterone antagonist Aglépristone (Alizine). Within 5 to 11 weeks the mammary glands had regressed to normal.  相似文献

A 16-year-old spayed female Siamese cat was presented with a history of paraparesis and obstipation. The cat's impaired function was attributed to malalignment of the L7-S1 spine (cauda equina syndrome) and to traumatic fracture of the left femoral neck, with severe degenerative arthritis. Laminectomy resolved the cauda equina problem, and the hip joint was not treated.  相似文献

Clinical cases of pelvic disease in the cat were surveyed over a 5 year period and were classified into proximal femoral epiphyseal separation and femoral neck fracture (19-6%), anterodorsal hip luxation (19-6%), pelvic fracture (34%), sacroiliac separation (16%), sacral and coccygeal fracture separation (7-2%) and neoplasia (3-1%).
Treatment of proximal femoral epiphyseal separation and femoral neck fracture by excision arthroplasty gave a more rapid return to soundness compared with conservative treatment. The majority of hip luxations responded well to external reduction and restricted exercise for 2-4 days. All cases of pelvic fract. ure were treated conservatively (2-14 days of 'cage-rest') and showed marked improvement within 2 weeks. Radiographic and clinical assessment of these revealed that the fractured bones eventually stabilized in their displaced positions. Sacral and coccygeal fracture separations required careful evaluation for bladder function and some cases needed tail amputation. Certain complications associated with conservative treatment of pelvic injuries in cats have been considered although in no case did these prove a serious clinical problem. Two cases of giant cell sarcoma and one of spindle cell sarcoma, of periosteal origin, were included in the survey.  相似文献

Five cats with Y-T fractures of the humeral condyle were reviewed. Breeds presented included domestic shorthair (four cats) and Maine Coon (one cat). Age ranged from two to 16 years. All the cats were neutered males. A road traffic accident was suspected in all cases. Four of the fractures were severely comminuted and one fracture had four fragments. The fractures were repaired via combined medial and lateral approaches. Fixation of the epicondylar ridges was performed using buttress plates in four cases and neutralisation plates in one case. The intracondylar fracture was stabilised using a 2.7 mm lag screw in four cases and a 3.5 mm lag screw in the other. A corticocancellous bone graft was applied in two cases. The intracondylar fracture was accurately reduced in all cases. Complete radiographic healing was documented in two cases 6 and 11 weeks following surgery. Failure of the fixation occurred in the most severely comminuted fracture five days postoperatively. Surgical revision was not performed, and the limb was amputated. Three cats were free of lameness and had resumed outdoor activities at follow-up (five to eight months after surgery). Marked lameness due to loss of elbow movement was observed in the other case.  相似文献

Methods of treating fractures of the olecranon in the horse are reviewed. A further 4 cases are recorded in which the fracture was treated by open reduction and fixation with a heavy duty Venable's plate placed on the caudal aspect of the ulna. Three cases had an oblique fracture through the semilunar notch; this was complicated in one by a fracture of the lateral half of the radial head which resulted in subluxation of the radio-humeral joint. The fracture of the radial head was treated by open reduction and fixation with a lag screw. The fourth case had an oblique fracture of the olecranon just proximal to the semilunar notch, complicated by a fracture of the anconeal process which was removed. The fractures healed within 10 weeks and the plates were removed in all 4 cases. During follow-up periods ranging from 10 to 26 months, all 4 animals became sound.  相似文献

A 6-month-old male-castrated domestic shorthair cat was presented shortly after being bitten on the neck by a large breed dog. On presentation the cat was non-ambulatory tetraparetic with preserved deep pain perception. Plain radiographs of the neck did not show any abnormalities. Computed tomography was performed and showed a right-sided depressed fracture of the dorsal lamina of the atlas. A partial dorsal laminectomy was performed to alleviate the spinal cord compression. The cat made a full recovery within 12 weeks of surgery.  相似文献

Tracheal narrowing is described in two cats. In both cases, inspiratory radiographs demonstrated tracheal narrowing just cranial to the thoracic inlet; no narrowing was seen on expiratory radiographs. Magnetic resonance imaging revealed suspected nasal tumours in both cases, but no abnormalities were identified in the region of the narrowed trachea. Nasal biopsy confirmed intranasal lymphoma in one cat and nasal adenocarcinoma in the other. The former cat was treated with chemotherapy. The owner of the latter cat declined further treatment. The tracheal narrowing disappeared after the initiation of chemotherapy in the cat with intranasal lymphoma and it is suggested that the narrowing might have been associated with the nasal tumour. A careful evaluation of the airway, especially cranial to the narrowing, is recommended in cases of tracheal narrowing in cats.  相似文献

Physeal fracture at the proximal olecranon has been rarely reported in the cat. Bilateral involvement in this case resulted in the cat adopting a ‘kangaroo-like’ posture with both elbows flexed. Careful clinical examination and radiography were essential for a diagnosis. The fractures were successfully treated using the wire tension band principle.  相似文献
