An 8-month-old castrated male Saint Bernard was evaluated for bilateral hind limb lameness. Lameness was ascribed to bilateral metatarsal rotational abnormalities on the basis of the physical examination and radiographic evaluations. Staged, bilateral deformity correction and tarsometatarsal arthrodeses were performed using circular external skeletal fixators. The dog's gait improved following surgery and the fixators were removed 3 (left hind paw) and 4 (right hind paw) months following surgery. Twenty months after the initial surgery, the owner reported that the dog was walking well without apparent lameness. Previous reports suggest that metatarsal rotation is untreatable; however, our results suggest that surgical correction of this deformity can substantially improve limb function in dogs affected with metatarsal rotation.  相似文献

OBJECTIVE: To report the biomechanical properties of 3 external skeletal fixation (ESF) devices for use in large ruminants. STUDY DESIGN: In vitro biomechanical testing of ESF constructs. SAMPLE POPULATION: Adult buffalo (weighing, 250-350 kg) tibiae (n=27). METHODS: ESF constructs (bilateral linear fixator [BLF], 4-ring circular external fixator [CEF], and hybrid fixator [HF]) were made using mild (low carbon) steel implants plated with nickel and cadaveric buffalo tibiae. After ESF application, a 1 cm mid-diaphyseal gap was created. Constructs were loaded to failure, on a materials testing machine, in axial compression (n=5/ESF type) and craniocaudal bending (n=3/ESF type). In addition, 3 CEF constructs were tested in intact tibiae under craniocaudal bending. RESULT: In compression, HF was the strongest and most rigid construct; yield load was significantly higher for HF than for BLF or CEF. Under bending, both CEF and HF had similar strength and modulus of elasticity. Strength for BLF was higher than CEF and HF, whereas the reverse was true for modulus of elasticity. OBJECTIVE: To report clinical outcome and complications after pancarpal arthrodesis using circular external skeletal fixation (CESF) in dogs. STUDY DESIGN: Retrospective study. ANIMALS: Dogs (n=9) with carpal injury. METHODS: Medical records including radiographs (10 limbs) with underlying severe carpal injury that had pancarpal arthrodesis with CESF were reviewed. Short-term follow-up (>6 months) was obtained by phone interview of owners. RESULTS: Mean weight was 24.5 kg (range, 10.1-69.5 kg). Mean duration until CESF removal was 110.6 days (range, 72-149 days). Complications in all dogs were mild postoperative edema of the distal aspect of the limb and initial mild serous discharge form exit points of CSEF wires. Follow-up (mean, 328 days; range, 190-541 days) was available for 8 dogs. All owners reported limb function and cosmesis as being excellent, and perceived that their animals were pain free. Perception of fixator care difficulty, as reported by owners, varied significantly based on previous splint management experience; owners who had experienced splint management for >1 month found fixator care equal to or easier than splint management. CONCLUSION: Pancarpal arthrodesis with a CESF produces a favorable clinical outcome with high owner acceptance. CLINICAL RELEVANCE: Pancarpal arthrodesis by use of CESF is a clinically applicable technique and may offer advantages over internal fixation with respect to postoperative complications.  相似文献

OBJECTIVE: To compare anatomic reduction and the biomechanical properties of a circular external skeletal fixator (CESF) construct to pin and tension band wire (PTBW) fixation for the stabilization of olecranon osteotomies in dogs. STUDY DESIGN: Cadaveric study. ANIMALS: Forelimbs from 12 skeletally mature mixed-breed dogs, weighing 23 to 28 kg. METHODS: An olecranon osteotomy was stabilized with either a CESF construct or PTBW fixation. A single distractive load to failure was applied to each specimen through the triceps tendon. Osteotomy reduction and biomechanical properties were compared between fixation groups. RESULTS: Reduction was not significantly different (gap: P =.171; malalignment: P =.558) between fixation groups. Osteotomies stabilized with the CESF had greater stiffness (P <.0001) and maximum load sustained (P <.0001) compared to PTBW fixation. There was no significant difference for yield load (P =.318) or for load at 1 mm of axial displacement (P =.997) between fixation groups. Failure of fixation occurred by bending of the intramedullary Steinmann pin and the fixation wires in the CESF specimens and by untwisting of the tension band wire knot with pullout and bending of the Kirschner wires in the PTBW specimens. CONCLUSIONS: Specimens stabilized with the CESF construct had similar reduction and yield load, greater stiffness and maximum load sustained, and less elastic deformation than specimens stabilized with PTBW fixation. CLINICAL RELEVANCE: The CESF construct may provide a biomechanically favorable alternative to PTBW fixation for stabilization of olecranon osteotomies in dogs, and its application warrants clinical investigation.  相似文献

OBJECTIVE: To determine relative effects of ring diameter and wire tension on axial biomechanical properties of 4-ring circular external skeletal fixator constructs. SAMPLE POPULATION: 4-ring circular external skeletal fixator constructs and artificial bone models. PROCEDURE: 4-ring constructs were assembled, using 50-, 66-, 84-, or 118-mm-diameter rings. Two 1.6-mm-diameter fixation wires were attached to opposing surfaces of each ring at intersection angles of 90 degrees and placed through a gap-fracture bone model. Three examples of each construct were loaded in axial compression at 7 N/s to a maximum load of 400 N at each of 4 wire tensions (0, 30, 60, and 90 kg). Response variables were determined from resulting load-displacement curves (construct stiffness, load at 1 mm of displacement, displacement at 400 N). RESULTS: Ring diameter and wire tension had a significant effect on all response variables and had a significant interaction for construct stiffness and displacement at 400 N. Significant differences within all response variables were seen among all 4 ring diameters and all 4 wire tensions. As ring diameter increased, effect of increasing wire tension on gap stiffness and gap displacement at 400 N decreased. Ring diameter had a greater effect than wire tension on all response variables. CONCLUSIONS AND CLINICAL RELEVANCE: Although effects of wire tension decrease as ring diameter increases, placing tension on wires in larger ring constructs is important because these constructs are inherently less stiff. The differential contribution of ring diameter, wire tension, and their interactions must be considered when using circular external skeletal fixators.  相似文献

The biomechanical contribution of the interfragmentary Kirschner wire as a component of composite fixation for acetabular fracture repair was subjectively and objectively evaluated. Acetabular osteotomies were repaired using the screw/wire/polymethylmethacrylate (SWP) composite fixation with or without one of three configurations of Kirschner wire in 32 hemipelves obtained from 16 dogs. Reduction, assessed objectively and subjectively, was unaffected by Kirschner wire placement. Hemipelves repaired with Kirschner wire(s) were subjectively more stable prior to application of polymethylmethacrylate when manually assessed in multiple planes. Consistent incremental increases in stiffness, yield load, and maximum load sustained that were observed during biomechanical testing were not significant, with the exception that hemipelves repaired with two Kirschner wires had significantly greater yield loads than hemipelves repaired without Kirschner wires. The subjective results of this study support the use of at least one interfragmentary Kirschner wire to maintain reduction prior to polymethylmethacrylate application; however, fracture configuration and location may dictate the number and pattern of interfragmentary Kirschner wires used to maintain reduction prior to application of the polymethylmethacrylate. The objective results and observations made during biomechanical testing suggest that use of one or more interfragmentary Kirschner wires may enhance stability after polymethylmethacrylate application.  相似文献

Knowledge and experience in the proper use of IM pins, K-wires, and orthopedic wire is a valuable asset to the veterinarian's ability to successfully repair a variety of long bone fractures. Most long bone fractures are amenable to repair with this form of fixation. When the principles of application are violated or the implants are used when contraindicated, complications often occur. A four-ring circular external skeletal fixation device was evaluated for transcarpal fixation of compound fractures in two calves. Case 1 was an eight-month-old female Holstein-Friesian x indigenous breed calf weighing 72 kg, which had a Salter-Harris type II fracture at the distal metaphysis of the right radius/ulna with an open contaminated wound on the medial aspect of the carpus. Case 2 was an 18-month-old crossbred Haryana heifer weighing 105 kg, which had a comminuted fracture at the proximal end of its left metacarpus with severe soft tissue trauma and an open wound on the medial aspect. In both cases, the fractures were repaired with four-ring circular fixators by fixing the proximal two rings in the distal radius/ulna and the distal two rings in the metacarpus. Postoperatively, both calves were treated with analgesic and anti-inflammatory drugs and antibiotic, and the pin-bone interfaces and the open wound were cleaned regularly with povidone-iodine solution. In both animals weight bearing was good to excellent in the immediate postoperative period. The fixator was well maintained and tolerated by the animals until the fractures healed after 45 to 60 days. The movement of the carpal joint was slightly affected when the fixator was removed on day 60, but a follow-up examination after one year showed that both calves had normal functional usage of the limbs.  相似文献

OBJECTIVE: To evaluate treatment of antebrachial growth deformities (AGD) with a lengthening procedure using a circular external skeletal fixation (CESF) system and to determine prognostic factors. STUDY DESIGN: Prospective clinical study. ANIMALS: Thirty-four dogs with unilateral AGD. METHODS: Length deficits, angular and rotational deformities, elbow incongruity (EI), osteoarthritis (OA) of the elbow and carpal joint, function, and cosmesis were determined before and after a CESF lengthening procedure. RESULTS: On admission, EI (21 dogs; 62%), OA of the elbow joint (17 dogs; 50%), carpal OA (12 dogs; 35%), and concomitant elbow and carpal OA (5 dogs; 7%) were common findings. Treatment significantly improved function (normal, 20 dogs; 60%) and cosmesis (normal, 22 dogs; 65%). Angular and rotational deformities were almost completely corrected with small remaining length deficits. Elbow and carpal OA increased significantly during the follow-up period. Significant correlations were demonstrated between initial elbow OA and final function (R=0.42, P=.02), initial function and final function (R=0.41, P=.02), and initial ulnar and radial deficit and final cosmesis (R=0.58, P=.0001 and R=0.45, P=.008). CONCLUSIONS: Treatment of AGD by a CESF lengthening procedure was successful despite small remaining length deficits. Initial elbow OA, function, and ulnar and radial length deficits are prognostic factors in the treatment of AGD. CLINICAL RELEVANCE: Initial elbow OA and initial function are prognostic factors in predicting the functional outcome of treatment of AGD with a CESF lengthening procedure in the dog.  相似文献

The purpose of this study was to investigate the effectiveness of treatment of long bone defects and extremity shortness in 5 dogs using circular external skeletal fixator. In one case a bone necrosis and osteomyelitis shortened radius by 7.0 cm; however in others extremity shortening between 3.0 and 6.0 cm affected radius-ulna or tibia. 2 or 3 rings of 80.0-100.0 mm diameters, and 3 rods were used as a circular external skeletal fixator. The distractions at the rate of 0.5 mm X 2 times/day were performed following a latent period of 3 days after the corticotomy. Unifocal internal lengthening for case 1 and unifocal external lengthening for the others were performed. Distraction was performed between 30 and 70 days (mean 50 days). It was noted that the circular external skeletal fixator was well-tolerated in all cases. The first usages of limbs in all cases were on postoperative day between 8 and 43 (mean 22 days). The radiographic follow-up examinations revealed a complete consolidation on postoperative day between 35 and 75 (mean 55.6 days). The fixators were removed on the postoperative day between 45 and 85 (mean 65 days). The treatment was successful in restoring the appropriate length of the limbs. After removing the fixator, functional and cosmetic results were graded as excellent in 3 cases, good in one case and fair in one case.  相似文献

ESF is useful in treatment of open fractures and delayed union and nonunion fractures. The fixation pins can be placed away from the fracture site to avoid disruption of blood supply. The appliance is well tolerated and inexpensive, and many of the parts can be reused. Adjustment of the splint is possible to permit correction of stabilization for optimal results. Gradual reduction of frame rigidity improves the quality of healing by increasing the stress on the bone. As stiffness of the healing fracture increases, return of limb function occurs. Early fixator removal reduces patient morbidity and permits a return to normal activity.  相似文献

The study was undertaken to evaluate the feasibility of a simple, inexpensive model of circular external fixator (CEF) for use in large ruminants. A simple model of CEF frames consisting of four full rings (13-19 cm diameter, 4 cm wide and 4 mm thick with 18-24 holes) connected by threaded rods (8 mm diameter, 10-15 cm long) and nuts was developed using mild (low carbon) steel and were nickel-plated. In the first phase of the study, three male cow calves were utilized to study the feasibility of application of the fixators in the metatarsus, tibia and radius, in reference of adaptation and tolerance by animals. In the second phase, the fixators were tested in osteotomized bones. Six bull calves of 1.5-2 years of age weighing about 200-250 kg were utilized for this purpose. After preparing the area for aseptic surgery, under xylazine (at 0.1 mg/kg, i.m.)-ketamine (i.v. till effect) general anaesthesia, the test bone (metatarsus, radius and tibia in two animals each) was approached through the medial surface and an osteotomy was created with a saw and chisel at the mid-diaphysis. The pre-constructed 4-ring CEF was mounted on the limb around the test bone in such a way that it formed a cylinder with the axis of the limb at the centre. Each ring was then fixed to the bone with a pair of beaded wires (316 SS) of 3.5 mm diameter. During the post-operative period, the animals were observed for any change in behaviour, tolerance of the fixators, the weight bearing on the test limb, the status of the fixator, and the level of reduction of the osteotomy, alignment and healing at different intervals. The fixation of CEF was easier in the metatarsus and radius than in the tibia. The inner ring diameters found adequate for metatarsus, radius and tibia were 13-15 cm, 15-17 cm and 17-19 cm, respectively. The fixators applied to different bones were well-tolerated, and the animals could lay down, stand and walk freely with the fixator without any problems. All the animals showed good weight bearing in the immediate post-operative period. Weight bearing, however, slightly reduced in the inflammatory period and showed improvement after about 4 weeks. Wire tract infection was seen in all the animals, which was more on proximal wire tracts on lateral aspect of limbs. Circular fixators were well-tolerated by all the animals, and there was no breaking/bending of connecting rods, slotted bolts or the rings at any stage of observation. Slight bowing of some wires was noticed in cases of tibia and radius (in phase II) and bending of some wires was noticed from first week in two animals one each in radius and tibia. Bone fragments were well maintained until healing occurred (appearance of bridging periosteal callus) at 60-70 days. The functional results were good in all but one case of tibia, where a mal-union was recorded. From this study, it can be concluded that CEF are well-tolerated by large ruminants and can be used to treat long bone fractures. The fixator assembly using mild steel is sufficiently strong to maintain fracture fixation and is inexpensive. The simple design of the CEF appears to be sufficient to treat simple diaphyseal fractures of the metatarsus and radius. Improvement in the technique of fixation and design-like coupling of linear and circular fixator components into hybrid constructs may help to provide greater rigidity in treating tibial and open long bone fractures.  相似文献

The influence on sperm morphology of different methods for preparation of semen and of storage in a fixative solution was examined in 27 beef bulls subjected to a regular breeding health examination. Sperm head morphology under light microscopy did not differ between smears of fresh semen stained with carbol-fuchsin-eosin (Williams staining) or Nigrosin-Eosin. Nor was there any difference between samples stained immediately after collection and those stained after 1 month of storage at + 4 degrees C in buffered formal-saline solution. Formol-saline fixed spermatozoa examined in wet preparations under phase contrast microscopy had a higher prevalence of acrosome defects and cytoplasmic droplets than stained smears of fresh semen under light microscopy. One month of storage in formol-saline did not affect the prevalence of acrosome defects or cytoplasmic droplets. There was no influence of fixation method (wet or dry), staining, examination technique, or storage time on midpiece or sperm tail morphology. The affinity of spermatozoa to eosin at staining with Nigrosin-Eosin ("live and dead count") did not differ between fresh semen and spermatozoa that had been stored in formol-saline for 1 month. It is concluded that bull semen can be stored for at least 1 month at + 4 degrees C in buffered formal-saline without major changes in sperm morphology. Furthermore, examination of wet preparations of fixed spermatozoa under phase contrast microscope is likely to yield the most accurate results for morphological characteristics like acrosome morphology and cytoplasmic droplets.  相似文献
