The ability of duplex Doppler ultrasonography to assist with the diagnosis of urinary tract obstruction was investigated in a study of 5 dogs with surgically induced, unilateral ureteral obstruction. The resistive index (RI) of obstructed kidneys was compared to that of controls and to the contralateral unobstructed kidneys. The RI was also evaluated following relief of obstruction. On the basis of an RI measurement ≥0.70 indicating obstruction, a sensitivity of 73% and a specificity of 77% was determined for the diagnosis of obstruction with Doppler ultrasonography. Although mean RI was elevated in obstructed kidneys compared to controls, it was concluded that a high false-negative rate (27%) limits the clinical usefulness of Doppler ultrasonography for the detection of urinary obstruction in dogs. The RI difference between obstructed and nonobstructed kidneys was also evaluated within individual animals, but the magnitude of difference between kidneys did not significantly improve the detection rate for obstruction.  相似文献

Some studies have shown that relative to baseline, the renal resistive index (RI) remains unchanged in nonobstructed kidneys and increases in obstructed kidneys after administration of furosemide. To our knowledge, the effect of mannitol administration on the renal RI of dogs has not been reported. We evaluated the renal RI in 16 kidneys in 8 young adult dogs after administration of mannitol. The mean RI decreased significantly from baseline (P < .01). Additionally, left complete ureteral obstruction wasinduced in 5 dogs. Evaluation by Doppler ultrasonography was performed for 5 days. On the 5th day, Doppler examination was repeated at 30 and 60 minutes after administration of mannitol to obstructed dogs. After induction of left ureteral obstruction, the RI of the left kidney increased significantly over 5 consecutive days. Administration of mannitol decreased the RI in the nonobstructed contralateral kidneys, and thus the RI difference between obstructed and nonobstructed kidneys was increased above normal (P < .001). In conclusion, administration of mannitol may be useful as another diuretic agent to identify unilateral ureteral obstruction on Doppler sonographic examination.  相似文献

Renal diseases are common in older cats. Decreased renal blood flow may be the first sign of dysfunction and can be evaluated by Doppler ultrasound. But previous studies suggest that the resistive index (RI) has a low sensitivity for detecting renal disease. Doppler waveforms of renal and intrarenal arteries demonstrate decreased blood flow before there are any changes in the RI. The purpose of this study was to evaluate the normal Doppler flowmetrics parameters of renal arteries (RAs), interlobar arteries (IAs) and abdominal aorta (AO) in adult healthy, Persian cats. Twenty-five Persian cats (13 females and 12 males with mean age of 30 months and an age range 12-60 months) with normal clinical examinations and biochemical tests and normal systemic blood pressure were given B-mode ultrasonographies in order to exclude all nephropathies, including polycystic kidney disease. All measurements were performed on both kidneys. Both kidneys (n=50) were examined by color mapping of the renal vasculature. Pulsed Doppler was used to examine both RAs, the IAs at cranial, middle and caudal sites, and the AO. The RI was calculated for all of the vessels. Early systolic acceleration (ESA) of RA and IA was obtained with Doppler spectral analysis. Furthermore, the ratio indices between RA/AO, and IA/RA velocities were calculated. The mean values of peak systolic velocity (PSV) and the diameter for AO were 53.17±13.46 cm/s and 0.38±0.08 cm, respectively. The mean RA diameter for all 50 kidneys was 0.15±0.02 cm. Considering the velocimetric values in both RAs, the mean PSV and RI that were obtained were 41.17±9.40 cm/s and 0.54±0.07. The RA had a mean ESA of 1.12±1.14 m/s(2) and the calculated upper limit of the reference value was 3.40 m/s(2). The mean renal-aortic ratio was 0.828±0.296. The IA showed PSV and RI values of 32.16±9.33 cm/s and 0.52±0.06, respectively. The mean ESA of all IAs was 0.73±0.61 m/s(2). The calculated upper limit of the reference value was 2.0m/s(2). The mean renal-interlobar artery ratio was 1.45±0.57. The RI values obtained in this study were similar to values reported in the literature. Some conditions that lead to a decrease in compliance and to an increase in vascular resistance can affect the Doppler spectral waveforms without changes in RI. To our knowledge, there are no studies that were directed toward to the normal ESA values of the renal vasculature in Persian cats. This study introduced a new ratio between the PSV of the RA and the IA. This index was developed based on the well-known effects of Doppler on the detection of stenosis, regardless of the cause. Further studies are necessary to verify the hemodynamic behavior of this index under pathological conditions in cats as well as the effect of aging, nephropathies and systemic pressure on Doppler velocimetric parameters.  相似文献

The objectives of this study were to determine some morphological characteristics of kidneys and resistive index (RI) changes in renal interlobar artery using B-Mode and color coded doppler sonography in lambs with urolithiasis. The study was performed in 14 male lambs (8 patients, 6 healthy) aged between 2-2.5 months in the same herd. The kidneys had a mean length of 55.6 +/- 1.6 mm, a width of 31.8 +/- 1.2 mm and a parenchymal width of 10.6 +/- 0.6 mm in healthy lambs. These values were 95.4 +/- 3.6., 52.4 +/- 1.3 and 8.75 +/- 0.5 mm, respectively in lambs with urolithiasis. These differences between both groups were significant. Furthermore, the mean resistive index values in lambs with urolithiasis (0.81 +/- 0.017 m/sn) were significantly higher than controls (0.62 +/- 0.013 m/sn). It was concluded that B-Mode and color coded doppler sonography might provide useful information for detection of changes in kidneys of lambs with urolithiasis like hydronephrosis and renal swelling as well as elevated resistance in the renal interlobar artery.  相似文献

The purpose of this study was to determine changes in resistive index (RI) and renal size that occur in normal feline kidney autografts in the immediate post-transplant period. Qualitative changes in renal parenchymal echogenicity and pelvic dilation were also noted, although this was not a major focus of the study. Fourteen domestic short-haired cats underwent bilateral nephrectomies followed by an autograft of the right kidney to either the aorta or external iliac artery. Due to post-operative complications, only nine of these cats completed the study. Ultrasonographic determination of the RI and renal size was made before transplantation and on days 1, 4, 7, 10, and 13 following transplantation. No significant changes in renal RI were identified during the study. Renal size increased significantly, peaking at 63% above baseline on day 7, and stayed elevated at 50% above baseline on day 13. There was no evidence of rejection or other abnormalities found on histologic evaluation of the transplanted kidneys. It was concluded that the RI of normal feline renal autografts does not significantly change in the immediate post-operative period. However, significant renal enlargement, likely due to a combination of post-operative edema and hypertrophy, does occur. This study provides baseline information for comparison with feline allografts in the immediate post-transplant period.  相似文献

Doppler ultrasound is a non-invasive technique that can be used to estimate vascular resistance by calculation of resistive index (RI) and pulsatility index (PI). Liver disease may increase renal RI and PI, and in humans with liver disease the indices are monitored to attain prognostic information. Systemic hypertension has been found in dogs with hepatic disease and is also related to increased renal vascular resistance in humans. The aim of this study was to examine renal vascular resistance increases in dogs with hepatic disease and to ascertain whether these may be related to blood pressure increases and biochemical parameters. Twenty dogs with hepatic disease were evaluated. The mean renal RI, PI, and systolic blood pressure were significantly higher than in normal animals. A positive correlation was found between the indices and alkaline phosphatase but not with systolic blood pressure. It is concluded that renal vascular resistance may increase in dogs with hepatic disease and in this study was above the limit value in 50% of the animals.  相似文献

OBJECTIVE: To determine scintigraphic, sonographic, and histologic changes associated with renal autotransplantation in cats. ANIMALS: 7 adult specific-pathogen-free cats: 5 males, 2 females, 1 to 9 years old. PROCEDURE: Renal autotransplantation was performed by moving a kidney (5 left, 2 right) to the left iliac fossa. Before and at multiple times after surgery, for a total of 28 days, cats were evaluated by B-mode and Doppler ultrasonography, scintigraphy, and renal biopsy. RESULTS: By 24 hours after surgery, a significant decrease (42%) in mean glomerular filtration rate (GFR) and an increase in mean renal size (81% increase in cross-sectional area) were evident in the transplanted kidney, compared with preoperative values. By postsurgery day 28, reduction in GFR was 23%. Significant changes in renal blood flow velocity were identified in both kidneys. Consistent changes in resistive index or pulsatility index for either kidney could not be identified. When all postoperative histologic data were combined, the histologic score, indicating degree and numbers of abnormalities detected, for the transplanted kidney was significantly higher than that for the control kidney. CONCLUSIONS: Significant changes in renal function, size, and histologic abnormalities develop secondary to acute tubular necrosis in cats after uncomplicated renal autotransplantation. CLINICAL RELEVANCE: Evaluation of renal size and function may be of benefit for clinical evaluation of feline renal transplant patients, whereas measurement of the resistive index may be of little clinical value.  相似文献

The haemodynamics of the canine left renal artery (LRA) and interlobar artery (ILA) were evaluated in eleven fasted, healthy, conscious beagles with severe acute (haematocrit [Hct] 16%), moderate chronic (Hct 26%) and mild chronic (Hct 34%) normovolaemic anaemia using Doppler ultrasound. Heart rate, peak systolic velocity (PSV), end diastolic velocity (EDV), time-averaged mean velocity (TAVmean), pulsatility index (PI) and resistive index (RI) were recorded. Doppler values in the dogs following the induction of anaemia states were compared with corresponding values in the same dogs prior to the induction of anaemia. Left renal artery mean PSV, mean PI and mean RI were significantly higher and the mean EDV was significantly lower in severe acute anaemia. No significant change was seen in mean values of the same parameters in moderate or mild chronic anaemia. There was no significant change in TAVmean of the LRA or mean PI and mean RI of the ILA in any grade of anaemia. Acute, severe normovolaemic anaemia significantly altered LRA Doppler parameters in resting dogs without influencing those of the ILA. Moderate or mild chronic anaemia had no effect on any renal Doppler parameter.  相似文献

The feasibility and reproducibility of obtaining the pulsed-wave Doppler measurements of resistive index (RI) and pulsatility index (PI) were investigated in intrarenal arteries of normal, nonsedated cats, and cats anesthetized with isoflurane. In addition, relative renal function and relative renal blood flow were evaluated using quantitative renal scintigraphy. The percentage of injected dose uptake, time to peak activity, and two indices of renal blood flow (K/A ratio and flow index) obtained during the first pass of 99mTc-MAG3, were determined for both awake and anesthetized cats. Results indicate that measuring RI and PI in nonsedated cats is readily accomplished and that the results are reproducible within an animal. Mean RI and PI values in the awake cats were 0.55 and 0.8, respectively. Significant differences between the awake and anesthetized cats were found for all pulsed-wave Doppler and quantitative renal scintigraphic measurements evaluated.  相似文献

OBJECTIVE: To evaluate the use of color Doppler imaging (CDI) for determining the resistive index (RI) of the medial long posterior ciliary artery (mLPCA) in clinically normal conscious dogs. ANIMALS: 18 (10 sexually intact males, 8 sexually intact females) dogs between 1 and 5 years old. PROCEDURE: Color Doppler ultrasonography was performed on both eyes with dogs in a sitting position. Each eye was imaged from the region dorsal to the zygomatic arch with the transducer positioned in a horizontal plane. The mLPCA was localized, and RI was calculated from velocities obtained for 3 similar Doppler waveforms. To determine the reproducibility of CDI-derived RI, measurements were repeated twice at a 10-day interval. RESULTS: Mean (+/- SD) RI of the mLPCA was 0.68 +/- 0.07 (95% confidence interval, 0.65 to 0.70; n = 36 eyes). Resistive index did not significantly differ between right and left eyes or male and female dogs. In addition, body weight was not correlated with RI. Repeated measurements of RI did not yield significantly different results (interclass correlation coefficient, 0.8297). CONCLUSION AND CLINICAL RELEVANCE: Color Doppler imaging appears to be a valid technique for determination of RI of the mLPCA in conscious dogs. This technique may be useful for investigating the pathophysiologic processes of many ocular and orbital vascular disorders in dogs.  相似文献

Ultrasonographic examination is a commonly employed technique for postoperative renal allograft evaluation after transplantation. Allograft size and resistive index (RI) are two objective ultrasonographic measures that may help establish a diagnosis and direct postoperative management for grafts with suboptimal function but their diagnostic efficacy has not been evaluated in clinical veterinary patients. Results of 69 feline renal transplant ultrasonographic examinations and RI determinations were studied. Based on clinical parameters at the time of the ultrasonographic examination, patients were grouped into six clinical/functional categories including evaluations of clinically normal grafts, delayed graft function, ureteral obstruction, uroabdomen, graft thrombosis, and rejection. RI, graft size (length, cross-sectional area, and volume), cyclosporine A whole blood trough concentration, Doppler blood pressure, creatinine concentration, and days from transplantation were compared between these categories and associations with each other were examined. RI was of little value in differentiating among the clinical categories with the exception of graft thrombosis. Graft volume and time from transplantation were significantly greater in grafts with signs of rejection and ureteral obstruction compared to clinically normal ultrasound examinations. Graft volume, cross-sectional area and length were generally associated. Cyclosporine A blood concentrations was associated with RI in both the pooled data and in the delayed graft function category. These results indicate RI should be used only as part of a larger clinical picture and in light of other factors including cyclosporine A concentration and the timing of the study relative to the implantation surgery for the diagnosis of postoperative transplantation complications. Graft volume may provide a more sensitive, albeit, nonspecific, indicator of allograft dysfunction.  相似文献

The renal resistive index (RI) value of 0.73 has been proposed as the upper limit in normal adult dogs. In humans, changes in RI with age are associated with plasma renin activity. There are relatively few equivalent reference data for dogs. We obtained reference RI data from 22 clinically healthy dogs <4 months of age and 33 healthy dogs between 4 months and 7 years of age. An association between the RI and plasma renin activity was investigated. The mean RI in the older dogs was 0.65 ± 0.05 vs. 0.75 ± 0.05 in dogs <4 months of age. The mean plasma renin activity in the older dogs was 1.18 ± 1.03 vs. 4.23 ± 3.09 ng/ml/h in dogs <4 months of age. There was a weak linear relationship between the RI and plasma renin activity (r2=0.280, P<0.01) in dogs <4 months of age. Also in these younger dogs, RI was negatively correlated with age (r2=0.682, P<0.01). The RI was higher in dogs <4 months of age than in older dogs. Therefore, the mean renal RI is slightly higher in young dogs than reported for an older population and interpretation of the RI must include an assessment of patient age.  相似文献

Colour-coded duplex sonography was used to examine the testes of 42 dogs whose testes were normal on clinical examination. With colour Doppler, the blood flow in the supratesticular region could be displayed clearly in 70 per cent of the cases, independent of the dog's age, bodyweight, pulse rate and the volume of its testes. The marginal artery could be displayed clearly in 56 per cent of the cases, and the clarity was correlated with bodyweight (P<0.001), testicular volume (P<0.001), and pulse rate (P<0.001). The intratesticular blood flow could not be discerned in 42 per cent of the cases. With pulsed Doppler, the mean resistive index (RI) in the supratesticular region was 0.57, and the mean pulsatility index (PI) was 1.00. In the marginal artery, the mean RI was 0.49, and mean PI was 0.78. In the intratesticular arteries, the mean RI was 0.47, and the mean PI was 0.75. At all three sites both indices were independent of age, bodyweight, pulse rate and testicular volume.  相似文献

Renal failure is an important cause of morbidity and mortality in feline patients. In human patients, intrarenal blood flow values obtained with duplex Doppler ultrasonography have been used to aid in the diagnosis and prognosis of renal failure. The purpose of this report is to present values for resistance to intrarenal blood flow in normal cats, derived by duplex Doppler ultrasonography. Ten mature clinically healthy conditioned domestic shorthaired cats were screened for normal renal anatomy and function by CBC, serum biochemical profile, urinalysis, urine protein/creatinine ratio, endogenous creatinine clearance, and renal B-mode ultrasonography; and for normal systemic blood pressure with Doppler pressure cuff measurement. Subsequently, the cats were sedated with ketamine hydrochloride, and values for mean intrarenal (arcuate artery) resistance to blood flow were determined by duplex Doppler ultrasonography, and expressed as the Pourcelot resistive index, a ratio of systolic to diastolic flow commonly used in duplex Doppler sonography in human patients. After duplex Doppler sonographic examination, histological normalcy of the kidneys was verified by evaluation of ultrasound-guided percutaneous renal biopsy. Based on the obtained resistive indices, 95% confidence intervals for normal values were calculated as follows: left kidney, 0.52 to 0.60; right kidney, 0.55 to 0.63. Resistive index (RI) and pulsatility index (PI) are indirect measurements of blood flow resistance that may be used to evaluate vascular changes in renal and ophthalmologic diseases. To our knowledge, no reports are available describing values for renal and ocular PI index in the unsedated dog and ocular RI and PI indices in the unsedated cat. The purpose of this study was to measure normal values for both intrarenal and ocular RI and PI within the same subject in unsedated clinically normal dogs and cats. Twenty-seven dogs and 10 cats were considered healthy by means of physical examination, CBC, biochemical profile, urinalysis, and ultrasonography. Systolic blood pressure was measured by Doppler ultrasonography. Intrarenal and ocular arteries were scanned by pulsed Doppler ultrasonography to calculate RI and PI. No significant differences were noted between the values obtained for the right vs. the left kidney and eye. The upper values of these indices were calculated as mean+2 standard deviations resulting in 0.72 and 1.52 for dog renal RI and PI; 0.7 and 1.29 for cat renal RI and PI; 0.76 and 1.68 for dog ocular RI and PI; and 0.72 and 1.02 for cat ocular RI and PI.  相似文献

Renal ischemia as a course of renal transplantation is a common cause of renal dysfunction as renal failure. The purpose of this study was to investigate the influence of ascorbic acid on blood urea nitrogen (BUN), creatinine (Cr) and resistive index (RI) for dog models with renal ischemia-reperfusion (I/R) injury. Renal ischemia was induced on 6 Beagle dogs. The left kidney was exposed to normothermic ischemia for a short period at 30 min followed by reperfusion. On the blood Cr level and RI, there was no significant difference comparing both groups. 14 days after I/R injury a significant reduction on the blood BUN level was observed in the vehicle group (34.06 mg/dl) compared to that of ischemia induced treated group (10.3mg/dl) (p < 0.05). In conclusion, administration of ascorbic acid for renal ischemic-reperfusion injury had influence on blood BUN level, but it was not revealed the influence on blood Cr and RI.  相似文献

The effect of low dose dopamine on the excretory urographic image quality and contrast media-induced nephropathy in normal dogs (experiment 1) and the dogs with decreased renal function (experiment 2) were assessed. In experiment 2, decreased renal function was induced by gentamicin overdose. In each experiment, animals were divided into 3 groups. In group 1, only contrast medium (iohexol) was administered. In group 2, contrast medium plus intravenous fluid (0.9% saline) were administered. And in group 3, contrast medium plus intravenous fluid and low dose dopamine were administered. Investigated parameters included intrarenal resistive index (RI), serum BUN and creatinine concentrations, contrast medium elimination time and radiographic image quality. In experiment 1, RI of group I increased at 80 min after contrast medium administration (p<0.05), but RI of group 3 decreased at 48 and 72 hr (p<0.05). Serum BUN concentration of group 1 was higher than that of group 2 and 3 (p<0.05); in radiographic examination, contrast medium elimination time decreased in group 2 and 3, but image quality of group 2 was inferior to that of group 3. In experiment 2, image quality of group 3 only provided adequate visualization of renal structures. The formula of contrast medium plus low dose dopamine was found to provide good nephrogram and pyelogram image quality without supplemental contrast medium, and to protect renal tubules from prolonged exposure to concentrated contrast medium.  相似文献

The purpose of this study was to measure splanchnic blood flow during digestion in unsedated dogs by using duplex Doppler sonography. The study population consisted of 12 healthy dogs. Blood flow in the cranial mesenteric artery, the celiac artery, and the aorta was measured before a test meal and at 20, 60, and 90 minutes after eating. The following measurements were made or calculated: vessel diameter, peak systolic velocity, end diastolic velocity, mean velocity, resistive index, pulsatility index, and flow volume. There was a significant postprandial decrease in the resistive and pulsatility indices in both the cranial mesenteric (preprandial RI = 0.867, postprandial RI = 0.796, preprandial PI = 3.033, postprandial PI = 2.173) and the celiac (preprandial RI = 0.854, postprandial RI = 0.769, preprandial PI = 2.639, postprandial PI = 1.930) arteries. In both vessels the end diastolic velocity, the mean velocity, and the flow volume increased significantly postprandially. These changes occurred significantly earlier in the celiac artery than in the cranial mesenteric artery. The findings most likely correspond to postprandial splanchnic vasodilation. Doppler ultrasound provide a good methode of detecting changes in postprandial splanchnic blood flow in the dog.  相似文献

We assessed the ability of the resistive index (RI) and pulsatility index (PI) to allow differentiation between normal, reactive, and neoplastic lymph nodes. Forty-seven medial iliac and 54 mesenteric lymph nodes from 83 dogs were evaluated sonographically. A cytologic sample was obtained in each dog that allowed categorization into one of the categories defined above. We found a significant difference in the RI and PI in nonneoplastic vs. neoplastic medial iliac and mesenteric lymph nodes. Values higher than 0.67 for the RI and 1.02 for the PI in medial iliac lymph nodes and higher than 0.76 for the RI and 1.23 for the PI in mesenteric lymph nodes had a high sensitivity and specificity for differentiating benign from neoplastic lymph nodes.  相似文献

The common hepatic artery in 10 healthy adult beagles, 20 healthy pups (13 Deerhounds and seven Irish Wolfhounds), and seven dogs with either a congenital or acquired hepatic disorder was examined by duplex-Doppler ultrasonography. Peak systolic velocity (Vs), resistive index (RI), pulse rate, angle of insonation, and sample volume depth were recorded. Measurements in Beagles were made after a 24 h fast and repeated 2 h after feeding. The mean Vs and RI in fasted Beagles were 1.5 m/s (range 1.1-2.3 m/s) and 0.68 (range 0.62-0.74), respectively. Postprandially, there were no significant differences in any of these variables. Mean Vs and RI were lower in pups: 1.0 m/s (range 0.8-1.3 m/s) and 0.59 (range 0.46-0.65), respectively (P < .001). Two dogs with congenital arterioportal fistula had higher mean Vs (P < .001) and lower mean RI (P < .05) than normal pups. No significant differences were detected between normal Beagles, three dogs with portal vein thrombosis, and two dogs with acquired hepatic insufficiency. Differences in measured hepatic blood flow in dogs of different age or breed will complicate diagnostic use of hepatic arterial Doppler measurements.  相似文献
