OBJECTIVE: To describe a technique for carpal panarthrodesis using a medially applied dynamic compression plate (DCP) and to evaluate outcome. STUDY DESIGN: Retrospective study. ANIMALS: Nine dogs with 10 carpal joint injuries. METHODS: Medical records of dogs that had carpal panarthrodesis by medial application of a DCP were reviewed. Signalment, cause and type of injury, preoperative treatment, operative technique, and postoperative clinical and radiographic outcome were retrieved. RESULTS: Screw loosening in the metacarpal bones required surgical revision in 3 dogs. The plate was removed because of lick dermatitis in another dog. No complications were observed in 6 arthrodeses. All dogs were subsequently sound. CONCLUSIONS: Medial application of a DCP is a reliable, reproducible method for carpal panarthrodesis. The load on the edge of the plate provides an increased area moment of inertia of the plate, enhancing it resistance to bending forces. Because of the valgus standing position of the canine carpus, dynamic compression is achieved. The DCP is secured to 1-3 metacarpal bones and loosening of the implant and bone fractures are expected to occur less frequently. CLINICAL RELEVANCE: Medial plating for carpal panarthrodesis is a valid alternative method that can be used in dogs with injuries to the carpus for which panarthrodesis of the carpal joint is indicated.  相似文献

Fracture of the medial femoral condyle in three dogs is described. This injury resulted in lateral (varus) instability, which was difficult to differentiate from a lateral collateral ligament tear. Radiographs and surgical exploration were necessary to differentiate the two conditions. A medial femoral epicondyle osteotomy was used in two cases to expose the fracture. This exposure technique is advantageous because it allows early return to functional use of the injured limb by eliminating postoperative splintage.  相似文献

The clinical features and treatment of eight cats with medial patellar luxation are described. In six cases the condition was bilateral. The luxation was surgically treated in 10 instances and four joints were treated conservatively. Patellar function following surgery was good in 90 per cent of the cases while the results of the conservatively managed joints were disappointing.  相似文献

This case report describes the investigation and treatment of an adult horse with purulent, malodorous exudate originating from a large intermandibular mass. Microbial culture of fluid aspirated from a draining tract yielded Peptostreptococcus and Prevotella species which are usually considered to originate from the oral cavity. Computed tomography (CT) was used to aid in the diagnosis and to allow careful presurgical planning, and the horse underwent successful debridement of the abscess cavity under general anaesthesia. Post-operatively, the horse developed trismus (reduced opening of the jaws) but repeat CT identified that the surgery site was healing appropriately. It was recommended to continue feeding the horse hay to act as physiotherapy and the masticatory action of the horse subsequently returned to normal.  相似文献

In four dogs with forelimb lameness a bony exostosis had developed near the caudal edge of the superficial digital flexor muscle on the medial epicondyle of the humerus. The aetiology possibly involves a traumatic enthesopathy. Surgical resection of the exostoses resolved the lameness.  相似文献

The purpose of this study was to identify the angiosome of the medial saphenous artery and vein and to evaluate the use of this cutaneous angiosome as a free skin flap in the dog. In phase 1 of this study, selective angiography of the medial saphenous artery performed in six canine cadavers showed that the skin covering the entire medial femorotibial area, the distal half of the caudal head of the sartorius muscle, and the gracilis muscle were perfused by the medial saphenous artery. In phase 2, a medial saphenous fasciocutaneous island flap was raised and sutured back to the skin edges of the donor wound in three dogs. One hundred percent survival of all of the flaps occurred. In phase 3, a medial saphenous fasciocutaneous microvascular free flap was transferred to a wound that was created over the dorsal metacarpal (n = 3) or metatarsal region (n = 3). The mean length ± SD of the medial saphenous vascular pedicle was 80 ± 13 mm (n = 5); the mean diameter ± SD of the medial saphenous artery was 2.8 ± 0.2 mm (n = 5) and the mean diameter ± SD of the medial saphenous vein was 4.2 ± 0.2 mm (n = 5). One hundred percent of all flaps survived (n = 6). Selective angiography of the distal cranial tibial artery (metatarsal wounds, n = 3) and the median artery (metacarpal wound, n = 3) was performed 3 weeks after surgery. All of the vascular anastomoses were patent and neovascularization of the wound beds was present. This free flap was found to be acceptable for cosmetic reconstruction of wounds located on the distal extremity.  相似文献

Unilateral medial patellar luxation was diagnosed in 10, and bilateral medial patellar luxation in six, large and giant-breed dogs (22 stifles). Lameness occurred in five dogs after trauma or surgery, and 11 dogs had no known predisposing history. The mean age at presentation was 25 months, and the mean time from initial onset of clinical signs to diagnosis was 13 weeks. All traumatic or iatrogenic luxations (five dogs) were unilateral. Luxations presumed to be congenital were unilateral in five dogs and bilateral in six. The grades of medial patellar luxation were I (1 stifle), II (11 stifles), III (9 stifles), and IV (1 stifle). Preoperative function was good (1 dog), fair (9 dogs), and poor (6 dogs). Surgical correction was performed in dogs with grades II, III, and IV luxations (21 stifles). Complications included one wound dehiscence and trochlear wedge migration, one pin loosening, and one persistent lameness caused by lymphoplasmacytic synovitis. Long-term follow-up was available in 13 dogs (18 stifles). Function was judged by owners to be excellent in seven dogs, good in five dogs, and poor in one dog. Surgical treatment of grades II and III luxations yielded good (8 stifles) and excellent (9 stifles) results, while one grade IV luxation had a poor long-term outcome.  相似文献

Objective— To evaluate potential iatrogenic medial meniscal (MM) damage during tibial plateau leveling osteotomy (TPLO) and to establish a safe zone (SZ) for hypodermic needle (HN) identification of the medial aspect of the stifle joint.
Study Design— Prospective cohort.
Animals— Cadaveric canine stifles (n=40).
Methods— HN (20 or 25 G) were inserted through the medial collateral ligament (MCL) of the femorotibial joint and through the SZ insertion points. The medial meniscus was inspected for iatrogenic damage. Statistical comparison of MM damage caused by different needle sizes and insertion sites was performed using Fisher's exact test with significance at P <.05.
Results— Twenty-gauge group: 65% of stifles had minor MM damage with MCL insertion compared with 35% of stifles with SZ insertion ( P =.0049). Severe MM damage occurred in 25% of stifles with MCL insertion compared with 0% of stifles with SZ insertion ( P =.0014). Twenty-five-gauge group: 85% of stifles had minor MM damage with MCL insertion compared with 30% after SZ insertion ( P =.0011); however, no severe MM injury was noted.
Conclusions— HN insertion though the MCL can produce iatrogenic damage to the MM. Use of a 25 G HN and SZ site for insertion reduced the frequency and severity of MM damage.
Clinical Relevance— HN insertion into the medial aspect of the femorotibial joint during TPLO can cause gross iatrogenic MM damage, which may contribute to the incidence and misdiagnosis of latent MM injuries after TPLO.  相似文献

Fifteen longitudinal fractures involving the medial condyle of the third metatarsal bone were diagnosed in racing Thoroughbreds and Standardbreds. Twelve were repaired surgically with lag screws placed through stab incisions. Two of the horses suffered catastrophic fracture of the third metatarsal bone during recovery from anesthesia, and 3 more sustained complete fractures within 4 days of the repair. Of 3 horses with fractures treated without surgery, 2 healed without complication. Preoperative radiography did not demonstrate a nonlongitudinal fracture component in any of the horses that sustained complete fractures. The complete fractures were uniformly "Y" shaped. Horses in which the fractures healed had a good prognosis for return to racing.  相似文献

A method of assessment of cases of medial luxation of the patella for deciding the type of surgical intervention is described. The results of treatment of the cases after such an assessment indicate that, if there is no gross distortion and no deviation of the tibial tuberosity, simple lateral capsular overlap gives a satisfactory return of function, even in cases of irreducible patellar luxation.
Résumé. Les auteurs décrivent une méthode propre à évaluer les luxations médiales de la rotule en vue de déterminer la nature de l'intervention chirurgicale à pratiquer. Les résultats thérapeu-tiques obtenus sur la base de cette évaluation portent à conclure que, sauf grave déformation ou déviation de la tubérosité du tibia, le simple chevauchement capsulaire latéral permet de rétablir un état fonctionnel satisfaisant, měme dans les cas de luxation rotulaire irréductible.
Zusammenfassung. Es wird eine Methode zur klinischen Beurteilung von Fällen einer medialen Luxation der Patella zwecks Entscheidung über die Art des erforderlichen chirurgischen Eingriffs beschrieben. Die Behandlungsergebnisse in den Fällen nach einer solchen Beurteilung zeigen, dass beim Fehlen einer makroskopischen Verformung und einer Abweichung der Tuberositas tibiae die einfache laterale Kapselüberlappung eine zufriedenstellende Rückkehr der Funktion selbst in Fällen nicht reduzierbarer patellarer Luxation ergibt.  相似文献

Medial patellar desmotomy was performed on one (treated) hindlimb, and a sham operation on the other (control) hindlimb, of 12 normal horses. The horses were examined for lameness before medial patellar desmotomy and at months 1, 2, and 3. The femoropatellar joints were examined radiographically and arthroscopically at month 3. All horses were lame on the treated limb at months 1, 2, or 3. Radiographic changes, including fragment formation at the distal aspect of the patella and bone production at the attachment of the middle patellar ligament on the patella, were present in 11 horses at month 3. Articular cartilage fibrillation or detachment was seen arthroscopically on the patellas of all treated limbs. In six horses, loosely attached fragments of bone and cartilage were removed arthroscopically from the distal aspect of the patella of the treated limb. The control limbs were clinically, radiographically, and arthroscopically normal throughout the study. Medial patellar desmotomy results in pathologic changes in the articular cartilage of the patella and adjacent soft tissues. Use of this surgical procedure should be reserved for persistent and confirmed cases of upward fixation of the patella.  相似文献
